3. Factors associated with respiratory morbidity in the first year of life

Objectives: To investigate the association between socioeconomic and nutritional factors with respiratory morbidity in the first year of life in different regions of Brazil. Methodology: A nested case-control study within a randomized field trial was conducted in three capital cities (Porto Alegre, Manaus, and Salvador), representing different macro-regions of the country. Cases were defined as children with a reported previous diagnosis of asthma, bronchiolitis, or pneumonia. Corresponding controls were matched by age and sex in a 2:1 ratio, selected consecutively from the original cohort, resulting in a sample of 222 children. Bivariate analyses were performed to assess the association between sociodemographic and nutritional variables with respiratory morbidity outcomes, calculating odds ratios (OR) and their respective confidence intervals (95% CI). Values of p < 0.05 were considered significant. Potential confounding factors were adjusted through multivariate analysis (logistic regression). Results: Maternal smoking and breastfeeding for less than six months showed a significant association and increased risk of respiratory disease (OR=2.12 and 2.05, respectively). Children born in the Southern region of Brazil also demonstrated a higher association and risk of respiratory morbidity. The consumption of ultra-processed foods did not show a significant association or increased risk of respiratory disease. Conclusions: Maternal smoking, breastfeeding for less than six months, and being born in the Southern region of Brazil are risk factors for the development of respiratory morbidity in the first year of life. The consumption of ultra-processed foods does not appear to pose a risk, but it was prevalent in more than 80% of the population, limiting its discriminatory power of analysis.

5. Placebo response in trials with patients with anxiety, obsessive-compulsive and stress disorders across the lifespan: a three-level meta-analysis

Question Randomised controlled trials assessing treatments for anxiety, obsessive-compulsive and stress-related disorders often present high placebo response rates in placebo groups. Understanding the placebo response is essential in accurately estimating the benefits of pharmacological agents; nevertheless, no studies have evaluated the placebo response across these disorders using a lifespan approach.Study selection and analysis We searched MEDLINE, PsycINFO, Embase, Cochrane, websites of regulatory agencies and international registers from inception to 9 September 2022. The primary outcome was the aggregate measure of internalising symptoms of participants in the placebo arms of randomised controlled trials designed to assess the efficacy of selective serotonin reuptake inhibitors (SSRIs) and serotonin and norepinephrine reuptake inhibitors (SNRIs) in individuals diagnosed with anxiety, obsessive-compulsive or stress-related disorders. The secondary outcomes were placebo response and remission rates. Data were analysed through a three-level meta-analysis.Findings We analysed 366 outcome measures from 135 studies (n=12 583). We found a large overall placebo response (standardised mean difference (SMD)=−1.11, 95% CI −1.22 to −1.00). The average response and remission rates in placebo groups were 37% and 24%, respectively. Larger placebo response was associated with a diagnosis of generalised anxiety disorder and post-traumatic stress disorder, when compared with panic, social anxiety and obsessive-compulsive disorder (SMD range, 0.40–0.49), and with absence of a placebo lead-in period (SMD=0.44, 95% CI 0.10 to 0.78). No significant differences were found in placebo response across age groups. We found substantial heterogeneity and moderate risk of bias.Conclusions Placebo response is substantial in SSRI and SNRI trials for anxiety, obsessive-compulsive and stress-related disorders. Clinicians and researchers should accurately interpret the benefits of pharmacological agents in contrast to placebo response.PROSPERO registration number CRD42017069090.

O presente artigo discute características das franquias de mídia, prática cada vez mais relevante para o cinema contemporâneo. Como principal objetivo, buscamos compreender como se caracterizam as franquias cinematográficas de maior sucesso dos mercados global e brasileiro, respectivamente, o “Universo Cinematográfico da Marvel” e “Minha Mãe É Uma Peça”. Para este objetivo, analisamos dados de bilheteria em salas de cinema como forma de identificar as franquias de sucesso nos dois mercados. O artigo apresenta conceitos de franquia, contextualiza a sua relação com o cinema e apresenta uma análise baseada em Jenkins (2009) acerca dos objetos de estudo. Os principais resultados evidenciaram que as franquias global e brasileira apresentam diferentes patamares de complexidade de estrutura, marcadas, respectivamente, pela construção de um universo ficcional expansivo e pela centralização em um personagem. Porém, ambas demonstraram que o formato apresenta grande potencial para atingir sucesso de público e bilheteria, tanto no mercado global quanto no brasileiro, e filmes que se afastam de uma franquia têm mais dificuldade em conquistar estas posições.

O objetivo deste estudo é revisar, através de uma busca sistemática, a aderência ao tratamento nos pacientes com transtornos psiquiátricos. É discutida a influência dos aspectos psicossociais, dos transtornos de humor e ansiedade, da psicose e do insight na aderência ao tratamento. Este estudo permite identificar quais fatores, sintomas e transtornos psiquiátricos estão relacionados com pobre aderência.

14. Sharing without laws: an exploration of social practices and ad hoc labeling standards in online movie piracy

This paper discusses self-labelling standards as sharing mediators in pirated versions of movies available online. Piracy has existed since the beginning of the film industry, challenging established rules and regulations. The dynamics of digital movie piracy often try to meet viewers' expectations while ignoring any regional and premiere date restrictions. Movie piracy organises its abundant offer by generating a self-regulatory repertoire of labeling standards. In this paper we propose an exploration of social practices related to the ad hoc labeling standards as regulators of a presumed user experience in unofficial versions of the movie Captain America: The Winter Soldier. Lessons for sharing economy regulation, especially in contexts where chaotic social relationships are involved, are identified and discussed.

OBJETIVO: Estimar a prevalência dos subtipos do HIV-1 e analisar fatores associados. MÉTODOS: Foi realizado um estudo transversal com amostra de conveniência de 80 pacientes adultos HIV-positivos atendidos em serviço especializado em DST/Aids em Canoas, RS, no período de julho de 2008 a janeiro de 2009. A determinação dos subtipos do HIV foi realizada por amplificação de fragmento do genoma viral pela reação em cadeia da polimerase seguida do seqüenciamento dos fragmentos amplificados. Variáveis sociodemográficas, clínicas e comportamentais foram coletadas em questionário estruturado. Foi realizada análise estatística univariada utilizando os testes de qui-quadrado e t de Student. RESULTADOS: Foi observada uma prevalência maior do subtipo C (43,8%; IC 95%: 32,9;54,6), seguida pelo CRF31_BC (35,0%; IC 95%: 24,6;45,5) e subtipos B (18,8%; IC 95%: 10,2;27,3) e F (2,4%; IC 95%: 0;5,9). Outros subtipos de HIV-1 não foram observados. Pacientes infectados com CRF31_BC apresentaram diagnóstico mais recente do que os pacientes infectados com o subtipo B (p < 0,05). Observou-se também maior freqüência de co-infecção com outros vírus (hepatites B e C e T-linfotrópicos humanos) nos indivíduos portadores do CRF31_BC do que nos demais subtipos. Com relação aos aspectos sociodemográficos, não foram observadas diferenças na distribuição dos subtipos e formas recombinantes quanto ao sexo e práticas sexuais. CONCLUSÕES: Os resultados obtidos indicam uma freqüência maior do subtipo C e do CRF31_BC nesse centro urbano do sul do Brasil, com possíveis vias de transmissão diferentes.

Objective: To estimate the prevalence of vitamin A deficiency (VAD) in children and associated risk factors. Design: Analysis of data from a cross-sectional multicentre study performed in the primary care units of the municipalities from January to June 2015. The children's legal guardians answered a socio-economic questionnaire, and the children's blood samples were obtained by venipuncture. Plasma retinol was determined by HPLC. Plasma retinol values of <0·70 μmol/l were considered VDA. Poisson multiple regression with robust variance was used. Values of P < 0·05 were considered significant. The data were analysed in the SPSS software, 21.0. Setting: Forty-eight poorest municipalities in the South Region of Brazil. Participants: Children (n 1503) aged 12–59 months. Results: The prevalence of VAD in the sample was 1·9 % (95 % CI (0·5, 6·8)). The following risk factors were associated with the outcome in the final explanatory model: family received Bolsa Familia program benefits (PR = 3·19; 95 % CI (1·69, 6·02)), child was not being breastfed (PR = 5·22; 95 % CI (1·68, 16·18)) and stunting (PR = 4·75; 95 % CI (2·10, 10·73)). Conclusions: VAD did not represent a public health problem for children living in socio-economically vulnerable municipalities in the South Region of Brazil, suggesting a new panorama of this nutritional deficiency even in regions of low socio-economic conditions in these three states. Thus, in view of the current nutritional transition scenario, it is necessary to continuously monitor and improve public policies related to vitamin A supplementation in the country. Few studies have investigated the simultaneous effect of individual and contextual factors on the occurrences of anemia. This study aims to evaluate the variability of children’s hemoglobin levels from municipalities in social vulnerability and its association with factors of individual and municipal nature. This is a cross-sectional, multi-center study, with children data (12-59 months) collected from 48 municipalities of the Southern region of Brazil, that were included in the Brazil Without Poverty Plan. Individuals’ data were collected using a structured questionnaire, and secondary and ecological data of children’s municipalities were collected via national surveys and institutional websites. The outcome was defined as the hemoglobin level obtained by HemoCue. A multilevel analysis was performed using Generalized Linear Models for Location Scale and Shape using R, with a 5% significance level. A total of 1,501 children were evaluated. The mean hemoglobin level was 12.8g/dL (95%CI: 12.7-12.8), with significant variability between municipalities. Lower values of hemoglobin were observed in children who lived in municipalities with a higher urbanization rate and a lower number of Community Health Agents, in relation to the reference categories. At the individual level, lower hemoglobin values were identified for children under 24 months, not enrolled at daycares, who were beneficiaries of the conditional cash transfer program and diagnosed with underweight. The results shed light on important factors at the municipal and the individual levels that were associated to the hemoglobin levels of children living in municipalities in social vulnerability. Resumo: Poucos estudos investigaram o efeito simultâneo dos fatores individuais e contextuais sobre a ocorrência da anemia. O estudo procura avaliar a variabilidade dos níveis de hemoglobina em crianças de municípios com vulnerabilidade social e a associação com fatores individuais e municipais. Foi realizado um estudo transversal com dados de crianças de idade pré-escolar (12-59 meses) de um estudo multicêntrico em 48 municípios do Sul do Brasil, incluídos no Plano Brasil Sem Miséria. Os dados dos indivíduos foram coletados com um questionário estruturado, e os dados secundários e ecológicos dos municípios das crianças foram obtidos através de inquéritos nacionais e websites institucionais. O desfecho foi definido como o nível de hemoglobina, obtido com o sistema HemoCue. Foi realizada análise multinível usando modelos lineares generalizados para posição, escala e forma, no R, com nível de 5% de significância . Foram avaliadas 1.501 crianças. O nível médio de hemoglobina foi 12,8g/dL (IC95%: 12,7-12,8), com variabilidade significativa entre os municípios. Níveis de hemoglobina mais baixos foram observados nas crianças em municípios com taxas de urbanização mais altas e menor número de agentes comunitários de saúde, comparado com as categorias de referência. Em nível individual, níveis de hemoglobina mais baixos foram identificados em crianças abaixo de 24 meses, não matriculadas em creches, beneficiárias do Programa Bolsa Família e diagnosticadas com baixo peso. Os resultados destacam fatores importantes nos níveis municipal e individual que estão associados com os níveis de hemoglobina em crianças de municípios com vulnerabilidade social. El objetivo de este estudio fue evaluar la variabilidad de los niveles de hemoglobina en niños socialmente vulnerables en municipios del sur de Brasil y su asociación con factores en el nivel individual y municipal. Se trata de un estudio trasversal con datos de niños (12-59 meses), procedentes de un estudio multicéntrico, realizado en 48 municipios de la región sur de Brasil, incluidos en el Plan Brasil sin Pobreza. Se recogieron los datos de los participantes, usando un cuestionario estructurado, así como datos secundarios y ecológicos de los municipios de los niños, a través de encuestas nacionales y sitios web institucionales. El resultado se definió como el nivel de hemoglobina obtenido por HemoCue. Se realizó un análisis multinivel, usando modelos lineales generalizados para la escala de localización y forma usando R, con un nivel de un 5% de significancia. Un total de 1.501 niños fueron evaluados. La media de nivel de hemoglobina fue 12,8g/dL (95%CI: 12,7-12,8), con una significativa variabilidad entre municipios. Los valores más bajos de hemoglobina se observaron en niños que vivían en municipios con unas tasas más altas de urbanización, y un número de agentes de salud comunitario más bajo, en relación con las categorías de referencia. En el nivel individual, los valores de hemoglobina más bajos fueron identificados en niños con menos de 24 meses, no matriculados en guarderías, beneficiarios de ayudas económicas, enmarcadas en programas de ayuda económica, y diagnosticados como con bajo peso. Los resultados aclararon importantes factores en el nivel municipal e individual que estaban asociados a los niveles de hemoglobina de niños residentes en municipios, así como vulnerables socialmente.

This research aimed to verify whether the number of airplanes run by surfers in the 2016 Word Surf League (WSL) men's season and their grades were related to their final classification in the events and season. Were viewed 3475 waves from 49 surfers and selected for analysis 275 waves. Spearman's correlation test, one-way ANOVA, and t-test, with p?0.05, were used for statistical analysis. The air proved to be an effective maneuver in isolated clashes, helped to reach the finals and decide events, but did not prove to be the main determinant of the season's final standings. The average of the marks attributed to the reverse 360 ??aerial was significantly higher than the notes attributed to the reverse aerial, however, the variables the position of the surfer in front or back to wave and whether or not to grab the board edge did not present significant differences. Resumo Esta pesquisa teve como objetivo verificar se a quantidade de aéreos executados pelos surfistas na temporada masculina da Word Surf League (WSL) de 2016 e suas respectivas notas tiveram relação com a classificação final deles nos eventos e na temporada. Foram visualizadas 3475 ondas de 49 surfistas e selecionadas para analise 275 ondas. Utilizou-se o teste de correlação de Spearman, ANOVA um fator e o teste t, com p?0.05, para análise estatística. O aéreo se mostrou uma manobra eficaz em confrontos isolados, ajudou a chegar às finais e a decidir eventos, mas não se mostrou ser o principal determinante para a classificação final da temporada. A média das notas atribuídas ao aéreo reverse 360 foi significativamente maior que as notas atribuídas ao aéreo reverse, no entanto, as variáveis posições do surfista de frente ou de costas para onda e agarrar ou não borda da prancha, não apresentaram diferenças significativas.

Objective: To evaluate the epidemiology of traumatic injuries of the upper limbs treated at a university hospital and identify the causes, types of injuries, and risk factors. Methods: A prospective study was performed with accidents resulting in trauma categorized into three groups: domestic, occupational, or transportation-related. A questionnaire containing information about the patient and the accident was administered. Lesion characteristics were evaluated according to the injured area, the type of injury, and the management strategy adopted for each case. Results: A total of 613 patients were evaluated. The most frequent accidents were domestic (66.6%), predominantly involving men (67.9%) with a mean age of 31 years. Wrist lesions prevailed in transportation-related (31.1%) and domestic (29.6%) accidents, and in accidents involving fingers at work (54.2%). Closed fractures were more frequent and conservative treatment was indicated most often. Serious injuries were associated with finger accidents (39.4%). There was a correlation between the level of education and the type of accident. Conclusion: Traumatic injuries of the upper limbs were more frequent in domestic accidents and in male patients. Closed fractures were the most common type of fracture and were usually treated conservatively. Serious injuries were more often related to finger trauma. Education level influenced domestic, transportation-related, and occupational accidents. Level of Evidence IV, Case Series. RESUMO Objetivo: Realizar a epidemiología das lesões traumáticas dos membros superiores, atendidas em Hospital Universitário, identificando sua origem e tipos de lesões, determinando fatores de riscos. Métodos: É um estudo prospectivo onde os acidentes foram caracterizados em três grupos: doméstico, trabalho ou transporte. Um questionário contendo informações pessoais e do acidente foi aplicado. Avaliaram-se as características da lesão referente ao seguimento afetado, tipo de lesão e conduta adotada para cada caso. Resultados: Avaliou-se 613 pacientes; os acidentes mais frequentes foram domésticos (66,6%), predominando o sexo masculino (67,9%), com média de idade 31 anos. As lesões em punhos prevaleceram nos acidentes de transporte (31,1%) e domésticos (29,6%). Nos acidentes de trabalho, lesões de dedos (54,2%). A fratura fechada foi mais recorrente e o tratamento conservador o mais indicado. As lesões graves relacionaram-se à acidentes em dedos (39,4%). Houve correlação entre nível de escolaridade e tipos de acidentes. Conclusão: Lesões traumáticas dos membros superiores são mais incidentes em acidentes domésticos e em pacientes do sexo masculino. Neonatal mortality still remains a complex challenge to be addressed. In Brazil, 60% of neonatal deaths occur among preterm infants with a gestational age of 32 weeks or less (≤32w). The aim of this study was to evaluate the factors involved in the high mortality rates among newborns with a gestational age ≤32w in a socioeconomically developed southern city in Brazil. Approximately one-third of the individuals infected with human immunodeficiency virus type 1 (HIV-1) are co-infected with hepatitis C virus (HCV). Co-infected patients have an increased risk for developing end-stage liver diseases. Variants upstream of the IFNL3 gene have been associated with spontaneous and treatment-induced clearance of HCV infection. Recently, a novel polymorphism was discovered, denoted IFNL4 ΔG > TT (rs368234815), which seems to be a better predictor of spontaneous clearance than the IFNL4 rs12979860 polymorphism. We aimed to determine the prevalence of the IFNL4 ΔG > TT variants and to evaluate the association with spontaneous clearance of HCV infection in Brazilian HIV-1 patients. The IFNL4 ΔG > TT genotypes were analyzed by polymerase chain reaction followed by restriction digestion in 138 HIV-1 positive patients who had an anti-HCV positive result. Spontaneous clearance of HCV was observed in 34 individuals (24.6%). IFNL4 genotype distribution was significantly different between individuals who had spontaneous clearance and chronic HCV patients (p=0.002). The probability of spontaneous clearance of HCV infection for patients with the IFNL4 TT/TT genotype was 3.6 times higher than for patients carrying the IFNL4 ΔG allele (OR=3.63, 95% CI:1.51-8.89, p=0.001). The IFNL4 ΔG > TT polymorphism seems to be better than IFNL4 rs12979860 to predict spontaneous clearance of the HCV in Brazilian HIV-1 positive patients.

31. Use of superficial peroneal nerve graft for treating peripheral nerve injuries

OBJECTIVE: To evaluate the clinical results from treating chronic peripheral nerve injuries using the superficial peroneal nerve as a graft donor source. METHODS: This was a study on eleven patients with peripheral nerve injuries in the upper limbs that were treated with grafts from the sensitive branch of the superficial peroneal nerve. The mean time interval between the dates of the injury and surgery was 93 days. The ulnar nerve was injured in eight cases and the median nerve in six. There were three cases of injury to both nerves. In the surgery, a longitudinal incision was made on the anterolateral face of the ankle, thus viewing the superficial peroneal nerve, which was located anteriorly to the extensor digitorum longus muscle. Proximally, the deep fascia between the extensor digitorum longus and the peroneal longus muscles was dissected. Next, the motor branch of the short peroneal muscle (one of the branches of the superficial peroneal nerve) was identified. The proximal limit of the sensitive branch was found at this point. RESULTS: The average space between the nerve stumps was 3.8 cm. The average length of the grafts was 16.44 cm. The number of segments used was two to four cables. In evaluating the recovery of sensitivity, 27.2% evolved to S2+, 54.5% to S3 and 18.1% to S3+. Regarding motor recovery, 72.7% presented grade 4 and 27.2% grade 3. There was no motor deficit in the donor area. A sensitive deficit in the lateral dorsal region of the ankle and the dorsal region of the foot was observed. None of the patients presented complaints in relation to walking. CONCLUSIONS: Use of the superficial peroneal nerve as a graft source for treating peripheral nerve injuries is safe and provides good clinical results similar to those from other nerve graft sources. RESUMO OBJETIVO: Avaliar resultados clínicos do tratamento das lesões crônicas de nervos periféricos com o nervo fibular superficial como fonte doadora de enxerto. MÉTODOS: Estudo de 11 pacientes com lesões de nervos periféricos nos membros superiores tratados com enxerto do ramo sensitivo do nervo fibular superficial, com intervalo médio de 93 dias entre a data de registro da lesão e a cirurgia. Foram observadas lesões do nervo ulnar em oito pacientes e do nervo mediano em seis. Em três ambos os nervos foram lesados. Na cirurgia faz-se incisão longitudinal na face anterolateral no tornozelo, visualiza-se o nervo fibular superficial, situado anteriormente ao músculo extensor longo dos artelhos. Proximalmente disseca-se a fáscia profunda entre os músculos extensor longo dos artelhos e o fibular longo. A seguir, identifica-se o ramo motor do músculo fibular curto, um dos ramos do nervo fibular superficial. O limite proximal do ramo sensitivo encontra-se nesse ponto. RESULTADOS: A média do espaço entre os cotos nervosos foi de 3,8 cm, comprimento médio dos enxertos de 16,44 cm, número de segmentos usados de dois a quatro cabos. Na avaliação da recuperação da sensibilidade, 27,2% evoluíram para S2+, 54,5% para S3 e 18,1% para S3+. Quanto à recuperação motora, 72,7% apresentavam grau 4 e 27,2%, grau 3. Não houve déficit motor da área doadora, observou-se déficit sensitivo na região dorso lateral do tornozelo e dorsal do pé. Nenhum paciente apresentou queixas à deambulação. CONCLUSÕES: O uso do nervo fibular superficial no tratamento das lesões de nervos periféricos como fonte de enxerto é seguro e proporciona resultados clínicos semelhantes a outras fontes de enxerto de nervos.

OBJETIVO: Estimar a prevalência dos subtipos do HIV-1 e analisar fatores associados. MÉTODOS: Foi realizado um estudo transversal com amostra de conveniência de 80 pacientes adultos HIV-positivos atendidos em serviço especializado em DST/Aids em Canoas, RS, no período de julho de 2008 a janeiro de 2009. A determinação dos subtipos do HIV foi realizada por amplificação de fragmento do genoma viral pela reação em cadeia da polimerase seguida do seqüenciamento dos fragmentos amplificados. Variáveis sociodemográficas, clínicas e comportamentais foram coletadas em questionário estruturado. Foi realizada análise estatística univariada utilizando os testes de qui-quadrado e t de Student. RESULTADOS: Foi observada uma prevalência maior do subtipo C (43,8%; IC 95%: 32,9;54,6), seguida pelo CRF31_BC (35,0%; IC 95%: 24,6;45,5) e subtipos B (18,8%; IC 95%: 10,2;27,3) e F (2,4%; IC 95%: 0;5,9). Outros subtipos de HIV-1 não foram observados. Pacientes infectados com CRF31_BC apresentaram diagnóstico mais recente do que os pacientes infectados com o subtipo B (p < 0,05). Observou-se também maior freqüência de co-infecção com outros vírus (hepatites B e C e T-linfotrópicos humanos) nos indivíduos portadores do CRF31_BC do que nos demais subtipos. Com relação aos aspectos sociodemográficos, não foram observadas diferenças na distribuição dos subtipos e formas recombinantes quanto ao sexo e práticas sexuais. CONCLUSÕES: Os resultados obtidos indicam uma freqüência maior do subtipo C e do CRF31_BC nesse centro urbano do sul do Brasil, com possíveis vias de transmissão diferentes. Determination of HIV subtypes was performed with the amplification of viral genome fragment, using polymerase chain reaction, followed by sequencing of the amplified fragments. Sociodemographic, clinical and behavioral variables were collected in a structured questionnaire. Univariate statistical analysis was performed, using chi-square test and Student's t-test. MÉTODOS: Foi realizado um estudo transversal com amostra de conveniência de 80 pacientes adultos HIV-positivos atendidos em serviço especializado em DST/Aids em Canoas, RS, no período de julho de 2008 a janeiro de 2009. A determinação dos subtipos do HIV foi realizada por amplificação de fragmento do genoma viral pela reação em cadeia da polimerase seguida do seqüenciamento dos fragmentos amplificados. Variáveis sociodemográficas, clínicas e comportamentais foram coletadas em questionário estruturado. Foi realizada análise estatística univariada utilizando os testes de qui-quadrado e t de Student. RESULTADOS: Foi observada uma prevalência maior do subtipo C (43,8%; IC 95%: 32,9;54,6), seguida pelo CRF31_BC (35,0%; IC 95%: 24,6;45,5) e subtipos B (18,8%; IC 95%: 10,2;27,3) e F (2,4%; IC 95%: 0;5,9). Outros subtipos de HIV-1 não foram observados. Pacientes infectados com CRF31_BC apresentaram diagnóstico mais recente do que os pacientes infectados com o subtipo B (p < 0,05). Observou-se também maior freqüência de co-infecção com outros vírus (hepatites B e C e T-linfotrópicos humanos) nos indivíduos portadores do CRF31_BC do que nos demais subtipos. Com relação aos aspectos sociodemográficos, não foram observadas diferenças na distribuição dos subtipos e formas recombinantes quanto ao sexo e práticas sexuais. CONCLUSÕES: Os resultados obtidos indicam uma freqüência maior do subtipo C e do CRF31_BC nesse centro urbano do sul do Brasil, com possíveis vias de transmissão diferentes.

Abstract Approximately one-third of the individuals infected with human immunodeficiency virus type 1 (HIV-1) are co-infected with hepatitis C virus (HCV). Co-infected patients have an increased risk for developing end-stage liver diseases. Variants upstream of the IFNL3 gene have been associated with spontaneous and treatment-induced clearance of HCV infection. Recently, a novel polymorphism was discovered, denoted IFNL4 ΔG > TT (rs368234815), which seems to be a better predictor of spontaneous clearance than the IFNL4 rs12979860 polymorphism. We aimed to determine the prevalence of the IFNL4 ΔG > TT variants and to evaluate the association with spontaneous clearance of HCV infection in Brazilian HIV-1 patients. The IFNL4 ΔG > TT genotypes were analyzed by polymerase chain reaction followed by restriction digestion in 138 HIV-1 positive patients who had an anti-HCV positive result. Spontaneous clearance of HCV was observed in 34 individuals (24.6%). IFNL4 genotype distribution was significantly different between individuals who had spontaneous clearance and chronic HCV patients (p=0.002). The probability of spontaneous clearance of HCV infection for patients with the IFNL4 TT/TT genotype was 3.6 times higher than for patients carrying the IFNL4 ΔG allele (OR=3.63, 95% CI:1.51-8.89, p=0.001). The IFNL4 ΔG > TT polymorphism seems to be better than IFNL4 rs12979860 to predict spontaneous clearance of the HCV in Brazilian HIV-1 positive patients.

Home fortification with multiple micronutrient powder (MNP) is effective in the prevention of anemia in young children. However, the impact on their vitamin A status remains controversial. This study aimed to evaluate the effect of MNP on vitamin A status in young Brazilian children. A multicenter pragmatic, controlled trial was carried out in primary health centers in four Brazilian cities. In the beginning of the study, the control group (CG) consisted of children 11-14 months old ( n = 395) attending in routine pediatric health care. In parallel, the intervention group (IG) was composed of children 6-8 months old ( n = 399), in the same health centers, who followed the intervention with MNP for 2-3 months. The analysis of the effect of MNP on vitamin A status was performed by comparing the IG with the CG after a 4- to 6-month follow-up when IG children had reached the age of the controls. The prevalence of vitamin A deficiency (VAD; serum retinol <0.70 μmol/L) in the CG was 16.2%, while in the IG was 7.5%-a 55% reduction in the VAD [prevalence ratio (95% confidence interval) = 0.45 (0.28; 0.72)]. This reduction was also significant when stratifying the study centers by coverage of the Brazilian Vitamin A Supplementation Program. The adjusted mean of vitamin A serum concentrations improved in the IG compared with CG children, with a shift to the right in the vitamin A distribution. Objetivo: Determinar as tendências da mortalidade infantil de 1995 a 1999, segundo a escolaridade materna, medidas em base geográfica, em Porto Alegre, Brasil. Métodos: Estudo baseado em dados secundários de um banco de dados municipal, criado em 1994. Todos os nascidos vivos (119.170 nascimentos) e óbitos infantis (1.934 óbitos) foram considerados. Foram definidas cinco diferentes áreas geográficas segundo os quintis de percentagem de escolaridade materna baixa (menos de seis anos de estudo): alta, médio-alta, média, média-baixa e baixa escolaridade. Foi usado o teste do qui-quadrado para tendências de comparação das taxas entre as áreas. Foi calculada a razão de incidências pela regressão de Poisson para identificar excesso de mortalidade infantil nas áreas mais pobres, em comparação com as mais ricas. HIV/human T cell lymphotropic virus (HTLV) coinfection has a large range of prevalence in the different risk groups and geographic regions of the world. Most of the HTLV-infected people live in geographic areas where the virus is endemic, as it happens in Brazil. The aim of this study was to identify HTLV prevalence and risk factors in HIV-positive patients. A cross-sectional study was conducted with 580 HIV-positive patients (mean age of 40.6 years and 45.0% men) from a specialized HIV/AIDS diagnosis and treatment center in Southern Brazil. Sociodemographic data, HIV risk factors, and HTLV-1/2 antibodies were collected. HTLV proviral DNA was detected by polymerase chain reaction (PCR). A multivariate analysis was performed to identify risk factors for HTLV infection. HTLV antibodies were detected in 29 (5.0%) and HTLV provirus in 17 (2.9%) patients. HTLV-1 was identified in 11 (64.7%) patients and HTLV-2 in 6 (35.3%) patients. No significant differences were observed between mono and coinfected patients in clinical characteristics regarding HIV/AIDS (time since HIV diagnosis, HIV viral load, lymphocytes CD4
+ count, and use of highly active antiretroviral therapy). Blood transfusion history was significantly associated with HIV/HTLV coinfection ( p=0.039). Alcohol abuse was more prevalent in HTLV-positive (47.1%) than in HIV mono-infected patients (20.4%; p=0.008). Tattooing was the only risk factor independently associated with HIV/HTLV coinfection ( p=0.035). Acute phenylalanine and tyrosine depletion (APTD) studies have been used to assess the role of the cathecholaminergic system in various aspects of human behaviour. In this study we conducted a randomized, double-blind, controlled and cross-over comparison to evaluate the effects of APTD on memory, attention and mood in normal subjects. Twelve healthy male volunteers were included in this study. The subjects ingested a nutritionally balanced mixture (B) or a similar mixture deficient in phenylalanine and tyrosine (PT-). Before and 5 h after ingestion of the drink, volunteers underwent tests on mood, memory and attention. Results of the memory tests showed that PT- mixture impaired word recall as measured in Rey's test (p = 0.016). The assessment of changes in mood showed that the balanced mixture improved scores of as alertness (VAMS factor I, p = 0.037) and the PT- mixture induces an opposite effect, increased scores of anxiety (Profiles of Mental State composed-anxious dimension, p = 0.022). These results suggest that tyrosine plasma levels and cathecholamines may be important factors in regulating mood and memory. [ABSTRACT FROM AUTHOR]

This article seeks to understand how the Cultural Analytics’ methodological approach and computational tools help interpret large image datasets. A set of 87 730 images of 389 Olympic athletes was collected from Instagram and analyzed, featuring a timespan from September 2011 to November 2020. The image set was structured and organized using computer vision processing combined with interactive visualization tools (Google Vision, PixPlot, Image Network Plotter). The analysis, mixing quantitative and qualitative methods, identified patterns represented as image clusters. Regular personal computers served as the hardware platform. Approximately 60 % of the athletes’ posts were related to non-sports topics, highlighting common characteristics of the visual culture disseminated on Instagram, such as selfies, lifestyle, leisure, travel, and food. Images of sports content, considered a central aspect of the research, had a lower frequency of publications featuring topics such as competitions, training, exercises, and sports practices in general. Beyond this result, the study offers a possible technical framework for similar researchers using large image datasets.

Este texto questiona como Plataformas de Inteligência Artificial Generativa (IAGs) podem colaborar com a pré-visualização de cenas, parte do processo de criação de uma obra audiovisual. Para isto, selecionamos uma cena do roteiro do filme Titanic (1997) como fonte dos comandos para a geração de imagens. Utilizamos uma metodologia exploratória, revisamos a literatura compilando conceitos básicos sobre Inteligência Artificial Generativa, testamos as plataformas Stable Diffusion, Bing Image Creator e Genmo.AI e comparamos as imagens geradas com os segmentos do filme original e discutimos suas aproximações e diferenças. Como resultados, destacamos que as ferramentas forneceram resultados consistentes com os segmentos de roteiro, embora nem sempre alinhadas com o que o filme apresentou, o que indica um processo de tomada de decisões pelas IAGs, o que as coloca como ferramentas relevantes para a pré-visualização de um roteiro, embora não abstraiam a presença humana na criação das cenas.
