Lateral ankle sprains are a commonly incurred injury in sports. They have a high recurrence rate and can lead to the development of persistent injury associated symptoms. We performed a quantitative synthesis of published case reports documenting the kinematics of acute lateral ankle sprains and episodes of 'giving-way' of the ankle joint to provide a comprehensive description of the mechanisms. A systematic literature search was conducted to screen records within MEDLINE® and EMBASE®. Additional strategies included manual search of specific journals, as well as contacting researchers in relevant communities to retrieve unpublished data. Twenty-four cases were included in the quantitative synthesis, 11 from individual case reports and 13 from four separate case series. Two authors independently reviewed all the articles and extracted ankle joint kinematic data. Excessive ankle inversion was the most pronounced kinematic pattern observed across all included cases, with a mean peak inversion angle of 67.5° (range 2.0 to 142) and a mean peak inversion velocity of 974°/s (range 468 to 1752). This was followed by internal rotation and plantar flexion, respectively. A homogeneous linear function revealed a mean inversion velocity across all cases of 337°/s (range 117 to 1400; R2 = 0.78; p < 0.0001).

Background: Injury risk prediction is an emerging field in which more research is needed to recognize the best practices for accurate injury risk assessment. Important issues related to predictive machine learning need to be considered, for example, to avoid overinterpreting the observed prediction performance. Purpose: To carefully investigate the predictive potential of multiple predictive machine learning methods on a large set of risk factor data for anterior cruciate ligament (ACL) injury; the proposed approach takes into account the effect of chance and random variations in prediction performance. Study Design: Case-control study; Level of evidence, 3. Methods: The authors used 3-dimensional motion analysis and physical data collected from 791 female elite handball and soccer players. Four common classifiers were used to predict ACL injuries (n = 60). Area under the receiver operating characteristic curve (AUC-ROC) averaged across 100 cross-validation runs (mean AUC-ROC) was used as a performance metric. Results were confirmed with repeated permutation tests (paired Wilcoxon signed-rank-test; P < .05). Additionally, the effect of the most common class imbalance handling techniques was evaluated. Results: For the best classifier (linear support vector machine), the mean AUC-ROC was 0.63. Regardless of the classifier, the results were significantly better than chance, confirming the predictive ability of the data and methods used. AUC-ROC values varied substantially across repetitions and methods (0.51-0.69). Class imbalance handling did not improve the results. Conclusion: The authors’ approach and data showed statistically significant predictive ability, indicating that there exists information in this prospective data set that may be valuable for understanding injury causation. However, the predictive ability remained low from the perspective of clinical assessment, suggesting that included variables cannot be used for ACL prediction in practice.

The main objectives of the present study were to describe the injury incidence and to analyze the anthropometric and physical characteristics of players from three high-level women’s football teams. The present study involved 54 female football players (21.9 ± 4.9 years old) from three different teams competing in the Spanish Reto Iberdrola-Segunda División PRO league. A battery of tests was carried out to determine the anthropometric and physical performance characteristics of the players along with an injury incidence record during a full competitive season. The obtained results showed that there was a high incidence of injury, as 38% of the players suffered some type of injury during the season (range 1–5, 1.75 ± 1.02 injuries per player). Injuries occurred in both matches and during training at a similar percentage (48.6 vs. 51.4%), and the majority of the registered episodes were graded as moderate or severe injury types (60%). Players suffering from an injury accumulated a total of 1587 chronological days off work due to injury during the season, with a recurrence rate of 55%. Considering the high incidence of injury, and the injury burden and the reinjure rate observed in this research, it seems necessary to apply the most efficient prevention and recovery measures possible in these female football teams. These descriptive data could serve athletic trainers and medical staff of female football teams to better understand their own screening procedure-derived data.

Few prospective studies have investigated hip and pelvic control as a risk factor for lower extremity (LE) injuries. The purpose of this study was to investigate whether deficits in hip and lumbopelvic control during standing knee lift test are associated with increased risk of acute knee and LE injuries in youth team sports. At baseline, 258 basketball and floorball players (aged 12‒21 y.) participated in a standing knee lift test using 3‐dimensional motion analysis. Two trials per leg were recorded from each participant. Peak sagittal plane pelvic tilt and frontal plane pelvic drop/hike were measured. Both continuous and categorical variables were analysed. New non‐contact LE injuries, and match and training exposure, were recorded for 12 months. Seventy acute LE injuries were registered. Of these, 17 were knee injuries (eight ACL ruptures) and 35 ankle injuries. Risk factor analyses showed that increased contralateral pelvic hike was significantly associated with knee injury risk when using categorical variable (HR for high versus low group 4.07; 95% CI 1.32–12.6). Furthermore, significant association was found between high lateral pelvic hike angles and ACL injury risk in female players (HR for high versus low group 9.10; 95% CI 1.10–75.2). Poor combined sensitivity and specificity of the test was observed. In conclusion, increased contralateral pelvic hike is associated with non‐contact knee injury risk among young team sport players as well as non‐contact ACL injuries among female players. More research to determine the role of pelvic control as a risk factor for knee injuries is needed. peerReviewed

OBJECTIVE To determine the agreement between 2-dimensional video analysis and subjective visual assessment by a physiotherapist in evaluating young athletes' knee control, and to determine the intrarater reliability and inter-rater reliability of the single-leg squat test. DESIGN Frontal plane knee control was assessed by a physiotherapist on a 3-point scale. Frontal plane projection angles were calculated from video images. To determine the intrarater reliability, a physiotherapist reassessed 60 subjects' performances from a video. For the inter-rater reliability, 20 subjects were assessed by both the physiotherapist and a nonexperienced tester. The study continued for 3 test years. SETTING Research institute. PARTICIPANTS Three hundred and seventy-eight floorball, basketball, ice hockey, and volleyball players. ASSESSMENT OF VARIABLES Knee control was assessed to be good, reduced, or poor. MAIN OUTCOME MEASURES Agreement between the video analysis and subjectively assessed frontal plane knee control. Intrarater reliability and inter-rater reliability. RESULTS There were statistically significant differences in the mean frontal plane knee angles between subjects rated as having "good," "reduced," or "poor" knee control. Intrarater reliability was fair for the assessments in the first year, moderate (dominant leg) and good (nondominant leg) for the second year, and very good (dominant leg) and good (nondominant leg) for the third year. Inter-rater reliability was fair/poor. CONCLUSIONS This study suggests that by using the subjective assessment of the single-leg squat task, it is possible to detect differences in frontal plane knee control in young team sport athletes. The assessment can be considered to be reliable for clinical use when performed by an experienced tester.

Mausehund, L, Skard, AE, and Krosshaug, T. Muscle activation in unilateral barbell exercises: Implications for strength training and rehabilitation. J Strength Cond Res 33(7S): S85-S94, 2019-The purpose of the present investigation was to assess lower-body muscle activity and hamstrings-to-quadriceps (HQ) activation ratios during performance of the split squat (SS), single-leg squat (SLS), and rear foot elevated split squat (RFESS), while using the same relative load and performing the exercises to muscular failure. Eleven healthy, moderately strength-trained subjects performed a 6-8 repetition maximum set of each exercise while electromyographic (EMG) activity of the vastus lateralis, biceps femoris, gluteus maximus, and gluteus medius was recorded. The results show that there were no significant differences in EMG peak activity of the gluteus maximus and vastus lateralis between any of the exercises. Gluteus medius activation was significantly (p ≤ 0.05) higher during the SLS (81.9% maximum voluntary isometric contraction [MVIC]), compared with the RFESS (54.9% MVIC) and SS (46.2% MVIC). The RFESS elicited higher (p ≤ 0.05) biceps femoris activity (76.1% MVIC) than the SS (62.3% MVIC), as well as higher (p ≤ 0.05) HQ activation ratios (0.83) than the SS (0.69) and SLS (0.63). During the SLS and the SS, HQ activation ratios increased significantly in the course of the repetition maximum set. In conclusion, although absolute loading differs between exercises, similar training stimuli of the gluteus maximus and quadriceps femoris can be expected for all exercises. The SLS is likely to induce the greatest improvements in gluteus medius strength, whereas the RFESS should be preferred if high hamstring coactivation is desired. To improve validity in EMG studies, strength training exercises should be performed close to failure while using the same relative loading.

Background/aimPoor frontal plane knee control can manifest as increased dynamic knee valgus during athletic tasks. The purpose of this study was to investigate the association between frontal plane knee control and the risk of acute lower extremity injuries. In addition, we wanted to study if the single-leg squat (SLS) test can be used as a screening tool to identify athletes with an increased injury risk.MethodsA total of 306 basketball and floorball players participated in the baseline SLS test and a 12-month injury registration follow-up. Acute lower extremity time-loss injuries were registered. Frontal plane knee projection angles (FPKPA) during the SLS were calculated using a two-dimensional video analysis.ResultsAthletes displaying a high FPKPA were 2.7 times more likely to sustain a lower extremity injury (adjusted OR 2.67, 95% CI 1.23 to 5.83) and 2.4 times more likely to sustain an ankle injury (OR 2.37, 95% CI 1.13 to 4.98). There was no statistically significant association between FPKPA and knee injury (OR 1.49, 95% CI 0.56 to 3.98). The receiver operating characteristic curve analyses indicated poor combined sensitivity and specificity when FPKPA was used as a screening test for lower extremity injuries (area under the curve of 0.59) and ankle injuries (area under the curve of 0.58).ConclusionsAthletes displaying a large FPKPA in the SLS test had an elevated risk of acute lower extremity and ankle injuries. However, the SLS test is not sensitive and specific enough to be used as a screening tool for future injury risk.

The strengths of interpersonal dyads formed by the attacker and defender in one-on-one situations are crucial for performance in team ball sports such as soccer. The purpose of this study was to analyze the kinematics of one-on-one defensive movements in soccer competitions, and determine the relationships between lower limb kinematics and the center of mass translation during cutting actions. Six defensive scenes in which a player was responding to an offender’s dribble attack were selected for analysis. To reconstruct the three-dimensional kinematics of the players, we used a photogrammetric model-based image-matching technique. The hip and knee kinematics were calculated from the matched skeleton model. In addition, the center of mass height was expressed as a ratio of each participant’s body height. The relationships between the center of mass height and the kinematics were determined by the Pearson’s product-moment correlation coefficient. The normalized center of mass height at initial contact was correlated with the vertical center of mass displacement (r = 0.832, p = 0.040) and hip flexion angle at initial contact (r = −0.823, p = 0.044). This suggests that the lower center of mass at initial contact is an important factor to reduce the downwards vertical center of mass translation during defensive cutting actions, and that this is executed primarily through hip flexion. It is therefore recommended that players land with an adequately flexed hip at initial contact during one-on-one cutting actions to minimize the vertical center of mass excursion.

AB Purpose: This study aimed to prospectively investigate the association between isolated and functional lower extremity muscle strength and the risk for noncontact anterior cruciate ligament (ACL) injury in Norwegian female elite handball and football players. Methods: From 2007 through 2015, premier league players participated in strength testing and were prospectively followed for ACL injury risk. At baseline, we recorded player demographics, playing and ACL injury history, and measured peak concentric isokinetic quadriceps and hamstring torques (60[degrees][middle dot]s-1), hamstring-to-quadriceps ratio, isometric hip abduction strength, and one-repetition maximum in a seated leg press. We followed a predefined statistical protocol where we generated five separate logistic regression models, one for each of the proposed strength risk factors and adjusted for confounding factors. New ACL injury was the outcome, using the leg as the unit of analysis. Results: A total of 57 (6.6%) of 867 players (age = 21 +/- 4 yr, height = 170 +/- 6 cm, body mass = 66 +/- 8 kg) suffered from a noncontact ACL injury after baseline testing (1.8 +/- 1.8 yr). The OR of sustaining a new injury among those with an ACL injury history was 3.1 (95% confidence interval = 1.6-6.1). None of the five strength variables selected were statistically associated with an increased risk of ACL rupture when adjusted for sport, dominant leg, ACL injury history, and height. Conclusion: Peak lower extremity strength was not associated with an increased ACL injury risk among female elite handball and football players. Hence, peak strength, as measured in the present study, cannot be used to screen elite female athletes to predict injury risk. (C) 2016 American College of Sports Medicine

Poor knee alignment is associated with increased loading of the joints, ligaments and tendons, and may increase the risk of injury. The study purpose was to compare differences in knee kinematics between basketball and floorball players during a vertical drop jump (VDJ) task. We wanted to investigate whether basketball players, whose sport includes frequent jump-landings, exhibited better knee control compared with floorball players, whose sport involves less jumping. Complete data was obtained from 173 basketball and 141 floorball players. Peak knee valgus and flexion angles during the VDJ were analyzed by 3D motion analysis.Larger knee valgus angles were observed among basketball players (- 3.2°, 95%CI -4.5 to - 2.0) compared with floorball players (- 0.9°, 95%CI -2.3 to 0.6) (P=0.022). Basketball players landed with a decreased peak knee flexion angle (83.1°, 95%CI 81.4 to 84.8) compared with floorball players (86.5°, 95%CI 84.6 to 88.4) (P=0.016). There were no significant differences in height, weight or BMI between basketball and floorball players. Female athletes exhibited significantly greater valgus angles than males. This study revealed that proper knee control during jump-landing does not seem to develop in young athletes simply by playing the sport, despite the fact that jump-landings occur frequently in practice and games.

Background: Few prospective studies have investigated the biomechanical risk factors of anterior cruciate ligament (ACL) injury. Purpose: To investigate the relationship between biomechanical characteristics of vertical drop jump (VDJ) performance and the risk of ACL injury in young female basketball and floorball players. Study Design: Cohort study; Level of evidence, 3. Methods: At baseline, a total of 171 female basketball and floorball players (age range, 12-21 years) participated in a VDJ test using 3-dimensional motion analysis. The following biomechanical variables were analyzed: (1) knee valgus angle at initial contact (IC), (2) peak knee abduction moment, (3) knee flexion angle at IC, (4) peak knee flexion angle, (5) peak vertical ground-reaction force (vGRF), and (6) medial knee displacement. All new ACL injuries, as well as match and training exposure, were then recorded for 1 to 3 years. Cox regression models were used to calculate hazard ratios (HRs) and 95% CIs. Results: Fifteen new ACL injuries occurred during the study period (0.2 injuries/1000 player-hours). Of the 6 factors considered, lower peak knee flexion angle (HR for each 10° increase in knee flexion angle, 0.55; 95% CI, 0.34-0.88) and higher peak vGRF (HR for each 100-N increase in vGRF, 1.26; 95% CI, 1.09-1.45) were the only factors associated with increased risk of ACL injury. A receiver operating characteristic (ROC) curve analysis showed an area under the curve of 0.6 for peak knee flexion and 0.7 for vGRF, indicating a failed-to-fair combined sensitivity and specificity of the test. Conclusions: Stiff landings, with less knee flexion and greater vGRF, in a VDJ test were associated with increased risk of ACL injury among young female basketball and floorball players. However, although 2 factors (decreased peak knee flexion and increased vGRF) had significant associations with ACL injury risk, the ROC curve analyses revealed that these variables cannot be used for screening of athletes.

Background Current knowledge on anterior cruciate ligament (ACL) injury mechanisms in male football players is limited. Aim To describe ACL injury mechanisms in male professional football players using systematic video analysis. Methods We assessed videos from 39 complete ACL tears recorded via prospective professional football injury surveillance between 2001 and 2011. Five analysts independently reviewed all videos to estimate the time of initial foot contact with the ground and the time of ACL tear. We then analysed all videos according to a structured format describing the injury circumstances and lower limb joint biomechanics. Results Twenty-five injuries were non-contact, eight indirect contact and six direct contact injuries. We identified three main categories of non-contact and indirect contact injury situations: (1) pressing (n=11), (2) re-gaining balance after kicking (n=5) and (3) landing after heading (n=5). The fourth main injury situation was direct contact with the injured leg or knee (n=6). Knee valgus was frequently seen in the main categories of non-contact and indirect contact playing situations (n=11), but a dynamic valgus collapse was infrequent (n=3). This was in contrast to the tackling-induced direct contact situations where a knee valgus collapse occurred in all cases (n=3). Conclusions Eighty-five per cent of the ACL injuries in male professional football players resulted from non-contact or indirect contact mechanisms. The most common playing situation leading to injury was pressing followed by kicking and heading. Knee valgus was frequently seen regardless of the playing situation, but a dynamic valgus collapse was rare. Background Little epidemiological information exists on overuse injuries in elite road cyclists. Anecdotal reports indicate anterior knee pain and lower back pain may be common problems. Purpose This study was conducted to register overuse injuries among professional road cyclists with special focus on anterior knee and lower back pain. Study Design Descriptive epidemiology study. Methods We attended training camps of 7 professional teams and interviewed 109 of 116 cyclists (94%) on overuse injuries they had experienced in the previous 12 months. Injuries that required attention from medical personnel or involved time loss from cycling were registered. Additional information on anterior knee pain and lower back pain was collected using specific questionnaires. Results A total of 94 injuries were registered; 45% were in the lower back and 23% in the knee. Twenty-three time-loss injuries were registered—57% in the knee, 22% in the lower back, and 13% in the lower leg. Fifty-eight percent of all cyclists had experienced lower back pain in the previous 12 months, and 41% of all cyclists had sought medical attention for it. Thirty-six percent had experienced anterior knee pain and 19% had sought medical attention for it. Few cyclists had missed competitions because of pain in the lower back (6%) or anterior knee (9%). Conclusion Lower back pain and anterior knee pain were the most prevalent overuse injuries, with knee injuries most likely to cause time loss and lower back pain causing the highest rates of functional impairment and medical attention. Clinical Relevance Future efforts to prevent overuse injuries in competitive cyclists should focus on lower back pain and anterior knee pain.

The basis of ACL-injury prevention is the understanding of the injury mechanism. Therefore a new approach was developed and validated that enables the calculation of knee and ankle joint moments during the injury. Detailed analysis of ACL-injury situations was performed to detect the kinematics as input data for a simplified 3D-human body model. An inverse-dynamics approach was used to realize the movement. The model was driven by Net-Muscle-Torque-Motors that calculate 3-D ankle and knee joint moments. Although there are some limitations that have to be considered this approach has the potential to generate a better understanding of injury mechanisms.

Obtained results significantly extend our perception about cross education phenomenon, or phenomenon of contralateral strength training effects. Contralateral training effects are relevant and these results could provide the implementation potential in many areas, especially in the field of physical medicine, rehabilitation, prevention of falls and injuries. The results of this study show that unilateral concentric strengths training of lower extremities improve balance of ipsilateral leg for 35 %, and contralateral for more than 15 %. The results of this study are in accordance with the study from 2011. [9]. These results suggest that contralateral training with unilateral hip isokinetic exercise increases one-legged standing balance of the contralateral limb. In our research were trained muscles that surround the joints relevant for one-legged standing balance. For example, one of important postural strategies in balance preservation is so-called Strategy of ankle joint [5]. According to this Strategy, human body is rotating around ankle joint by principle of the reverse pendulum, therefore ; it stands to a reason, that training of strengths muscles that surround ankle joints significantly improves stability and upright standing balance. The results of t-test for dependent samples showed that there is no statistically significant difference in BALANCE test of dominant and undominant leg between groups in Pre testing. For this reason, statistically significant changes in the ipsilateral and contralateral single leg balance between Pre and Post testing (Table 1.) attributed to the effects of training program. Subjects. 30 young, healthy, and physically active women (age: 21 yrs., mass: 60 kg, height: 67 cm, percent body fat: 26%) were randomly allocated to one of two groups: 1) control group (n=15), 2) experimental group (n=15). Training program. The training program consisted of unilateral concentric contractions of knee extensors and flexors, and ankle dorsi and plantar flexors and it was performed on isokinetic dynamometer (Biodex System 3, Biodex Corporation, Shirley, NY). The training velocity was 60°/s for knee, and 30°/s for ankle. Over the 4-week program the training volume increased progressively with the increase in sets number, starting first with two (first training), then three (from second to fifth training) and finally with four sets (from 6th to 12th training). Testing procedure. Each participant was tested on two occasions: 1) prior to commencement of training (Pre), and 2) one week after the completion of the 4-week training period (Post). The testing procedure included one-legged standing balance measurement, followed by leg strength (i.e. muscle function) measurement. One - legged standing balance. Single-limb postural stability was assessed on a Biodex Stability System, level 5 (Biodex, Shirley, New York, USA). BSS was used in a numerous of studies before (1, 13, 15, 16, 9). The measures of dynamic balance included overall stability index scores (OSI). The OSI represents the variance of foot-platform displacement in degrees from a level position, in all motions during a test. A high stability score indicates poor balance. Data analyses. Pre and Post training Means and SDs were calculated for ipsilateral (trained) and contralateral (untrained) one – legged standing balance in both group. T-test for independent (EXP – CON) and dependent (Pre-Post) samples was set at p < 0, 01.
