4. Using educational networks to promote internationalization experiences in physiotherapy education

Abstract

Introduction and Objectives: The Academic Network of Health Sciences of Lusofonia (RACS - racslusofonia.org) was created in 2016, with the mission of promoting training and scientific cooperation in health sciences, between higher education institutions and research centers in different countries and Portuguese speaking communities. Within the Physiotherapy Education Group of RACS we develop a project with a set of activities, developed along the academic path of the physiotherapy students, which allow them to be exposed progressively to the international context. Methods: During the 2021/22 academic year, 10 institutions involving 103 Physiotherapy students from 6 different countries (Portugal, Brazil, Mozambique, Cabo Verde, Angola, and São Tomé e Principe) participate in this project. Students are asked, over a period of 2 months, to carry out at least 2 online meetings within an international group to discuss topics related to the reality of education and the practice of Physiotherapists. As a result, students submit an individual reflection on the most relevant points that were identified in this sharing process. In each of the participating institutions, there was a teacher responsible for preparing, supporting, and evaluating the students' reflections. Results: To assess the impact and satisfaction of the students with activity, an online questionnaire (10 questions) was used, with several dimensions (preparation, performance, and global opinion) where a response rate of 83% was obtained (n=86). Regarding the answers, 93% considered that the objectives of the activity were clear and attainable and that the support materials were adequate (83%). Regarding meetings with international peers, 70% said that communication was easy during the meetings and that the proposed objectives were achieved (91%). Concerning the impact of the activity, 94% considered that the activity contributes to a better understanding of the international context of teaching and practice of physiotherapists, stressing that it should continue to be carried out within the scope of the physiotherapy course (89%). Conclusion(s): The overall reflexion from the students and teachers involved was very positive, reinforcing the importance of this activity and identifying areas of improvement related to student recruitment and support throughout this process. Based on the results, it seems important for us to carry out this project as it facilitates students’ early exposure to international contexts, which may facilitate the development of skills that allow them to play a more active and global professional role. info:eu-repo/semantics/publishedVersion

Abstract

International audience; AimsDilated cardiomyopathy (DCM) is a complex disease where genetics interplay with extrinsic factors. This study aims to compare the phenotype, management, and outcome of familial DCM (FDCM) and non‐familial (sporadic) DCM (SDCM) across Europe.Methods and resultsPatients with DCM that were enrolled in the prospective ESC EORP Cardiomyopathy & Myocarditis Registry were included. Baseline characteristics, genetic testing, genetic yield, and outcome were analysed comparing FDCM and SDCM; 1260 adult patients were studied (238 FDCM, 707 SDCM, and 315 not disclosed). Patients with FDCM were younger (P < 0.01), had less severe disease phenotype at presentation (P < 0.02), more favourable baseline cardiovascular risk profiles (P ≤ 0.007), and less medication use (P ≤ 0.042). Outcome at 1 year was similar and predicted by NYHA class (HR 0.45; 95% CI [0.25–0.81]) and LVEF per % decrease (HR 1.05; 95% CI [1.02–1.08]. Throughout Europe, patients with FDCM received more genetic testing (47% vs. 8%, P < 0.01) and had higher genetic yield (55% vs. 22%, P < 0.01).ConclusionsWe observed that FDCM and SDCM have significant differences at baseline but similar short‐term prognosis. Whether modification of associated cardiovascular risk factors provide opportunities for treatment remains to be investigated. Our results also show a prevalent role of genetics in FDCM and a non‐marginal yield in SDCM although genetic testing is largely neglected in SDCM. Limited genetic testing and heterogeneity in panels provides a scaffold for improvement of guideline adherence.

Abstract

Aims: Exercise-derived parameters, specifically peak exercise oxygen uptake (peak VO 2 ) and minute ventilation/carbon dioxide relationship slope (VE/VCO 2 slope), have a pivotal prognostic value in heart failure (HF). It is unknown how the prognostic threshold of peak VO 2 and VE/VCO 2 slope has changed over the last 20 years in parallel with HF prognosis improvement. Methods and results: Data from 6083 HF patients (81% male, age 61 ± 13 years), enrolled in the MECKI score database between 1993 and 2015, were retrospectively analysed. By enrolment year, four groups were generated: group 1 1993–2000 (n = 440), group 2 2001–2005 (n = 1288), group 3 2006–2010 (n = 2368), and group 4 2011–2015 (n = 1987). We compared the 10-year survival of groups and analysed how the overall risk (cardiovascular death, urgent heart transplantation, or left ventricular assist device implantation) changed over time according to peak VO 2 and VE/VCO 2 slope and to major clinical and therapeutic variables. At 10 years, a progressively higher survival from group 1 to group 3 was observed, with no further improvement afterwards. A 20% risk for peak VO 2 15 mL/min/kg (95% confidence interval 16–13), 9 (11–8), 4 (4–2) and 5 (7–4) was observed in group 1, 2, 3, and 4, respectively, while the VE/VCO 2 slope value for a 20% risk was 32 (37–29), 47 (51–43), 59 (64–55), and 57 (63–52), respectively. Conclusions: Heart failure prognosis improved over time up to 2010 in a HF population followed by experienced centres. The peak VO 2 and VE/VCO 2 slope cut-offs identifying a definite risk progressively decreased and increased over time, respectively. The prognostic threshold of peak VO 2 and VE/VCO 2 slope must be updated whenever HF prognosis improves.

Abstract

The ability of sorghum to maintain a high level of biomass production in stressful environments makes it an interesting crop for animal feed as fodder, and for biomaterials and bioenergy production. Sorghum hybrids with improved agronomical behavior and high biomass quality are being developed for the European market. However the development of such hybrids requires a lot of field testing and biomass analyses. The current marker assisted breeding strategies offer the possibilities to better understand the genetic architecture of biomass quality and accelerate the creation of improved varieties. The G2MARS design developed in the frame of the “Biomass For the Future” project is gathering the know-how of both private companies and publics institutes to deliver new genetic marker and improved germplasm for the European biomass market. Seven biparental connected populations were developed in both genetic pools (3 in the male, 3 in the female and one male-female populations). These populations involved elite commercial cultivars and high biomass quality sorghum landraces. A total of 964 hybrids from these 7 populations were characterized in 2016 and 2017 in 2 locations. Agronomical traits and biomass quality related traits were measured. A QTL detection taking advantage of the connected design was performed with MCQTL and allowed the detection of 17 and 6 QTL respectively in the male and female pools. For each QTL the parental alleles were ranked allowing to define 2 target molecular ideotypes. The crossing strategy aiming to combine the most efficiently the favorable alleles was defined using Optimas. The G2MARS project offered the opportunity to take advantage of the specific skills of both private and public partners and develop a joint breeding action do develop hybrids suitable to fit the expectations of the biomass value chains. IN addition, this design improved the understanding of the genetic architecture of biomass quality traits that was mobilized to optimize the recombination strategy. Abstract

AIMS: Cardiomyopathies are a heterogeneous group of disorders associated with premature death due to ventricular arrhythmia or heart failure. The purpose of this study was to examine the characteristics of patients enrolled in the pilot phase of the EURObservational Research Programme (EORP) cardiomyopathy registry. METHODS AND RESULTS: Between 1 December 2012 and 30 November 2013, four cardiomyopathy phenotypes were studied: hypertrophic cardiomyopathy (HCM), dilated cardiomyopathy (DCM), arrhythmogenic right ventricular cardiomyopathy (ARVC), and restrictive cardiomyopathy (RCM). Twenty-seven centres in 12 countries participated; 1115 patients were enrolled. The commonest cardiomyopathy was HCM (n = 681), followed by DCM (n = 346), ARVC (n = 59), and RCM (n = 29); 423 patients (46.4% of those reported) had familial disease; and 56 (5.0%) had rare disease phenocopies. Median age at enrolment and diagnosis was 54 [interquartile range (IQR), 42-64] and 46 years (IQR, 32-58), respectively; fewer patients with ARVC and more with RCM were diagnosed in the upper age quartile (P < 0.0001). There was a male predominance for all cardiomyopathies except RCM (P = 0.0023). Most patients were in New York Heart Association functional class I (n = 813) at enrolment; 139 (12.5%) reported syncope, most frequently in ARVC (P = 0.0009). Five hundred and seven (45.5%) patients underwent cardiac magnetic resonance imaging, 117 (10.6%) endomyocardial biopsy, and 462 (41.4%) genetic testing with a causative mutation reported in 236 individuals (51.1%). 1026 patients (92.0%) were receiving drug therapy; 316 (28.3%) had received an implantable cardioverter defibrillator (highest proportion in ARVC, P < 0.0001). CONCLUSION: This pilot study shows that services for patients with cardiomyopathy are complex, requiring access to a large range of invasive and non-invasive investigations and involvement of multidisciplinary teams. Treatment regimens are equally multifaceted and show that patients are likely to need long-term follow-up in close liaison with expert centres.

43. Accuracy of computer-aided oral implant surgery: a clinical and radiographic study.

Abstract

Purpose: Computer-aided oral implant surgery offers several advantages over the traditional approach. The purpose of this study was to evaluate the in vivo accuracy of computer-aided, template-guided oral implant surgery by comparing the three-dimensional positions of planned and placed implants. Materials and Methods: Oral implant therapy was performed in two treatment centers on eligible patients using computerized tomography (CT)-based software planning and computer-aided design/computer-assisted manufacture stereolithographic templates. A second CT scan was obtained after surgery. Preoperative and postoperative CT images were compared (planned vs actual implant positions), and the accuracy of this type of image-guided therapy was assessed. Results: Twenty-five adult patients were included in this retrospective study; 17 (11 partially and eight fully edentulous arches) were treated in center 1, and eight (six partially and two fully edentulous arches) in center 2. Of the 104 implants inserted with the computer-aided method, 100 integrated, giving a cumulative survival rate of 96% (mean follow-up, 36 months). There were no major surgical complications. With regard to accuracy, 89 implants were available for comparison; mean lateral deviations at the coronal and apical ends of the implants were 1.4 mm and 1.6 mm, respectively. Mean depth deviation was 1.1 mm and mean angular deviation was 7.9 degrees. There was a statistically significant correlation in the accuracy of any implants placed with the same guide. There was no difference in accuracy data from the two private centers; nor could a learning curve be demonstrated. Conclusions: Based upon this clinical study of 25 patients, the following observations were made: (1) computer-aided oral implant surgery used in two treatment centers provided a high likelihood (96%) of implant survival, and (2) deviations from planned implant positions existed in the coronal and apical portions of the implants as well as with implant angulation. Median age at enrolment and diagnosis was 54 [interquartile range (IQR), 42-64] and 46 years (IQR, 32-58), respectively; fewer patients with ARVC and more with RCM were diagnosed in the upper age quartile (P, 0.0001). There was a male predominance for all cardiomyopathies except RCM (P = 0.0023). Most patients were in New York Heart Association functional class I (n = 813) at enrolment; 139 (12.5%) reported syncope, most frequently in ARVC (P = 0.0009). Five hundred and seven (45.5%) patients underwent cardiac magnetic resonance imaging, 117 (10.6%) endomyocardial biopsy, and 462 (41.4%) genetic testing with a causative mutation reported in 236 individuals (51.1%). 1026 patients (92.0%) were receiving drug therapy; 316 (28.3%) had received an implantable cardioverter defibrillator (highest proportion in ARVC, P, 0.0001). Conclusion: This pilot study shows that services for patients with cardiomyopathy are complex, requiring access to a large range of invasive and non-invasive investigations and involvement of multidisciplinary teams. Treatment regimens are equally multifaceted and show that patients are likely to need long-term follow-up in close liaison with expert centres. © The Author 2015.

45. Effect of orégano oil supplementation in diets formed using either crude soybean oil or acidulated soybean oil soapstock as source of energy on the growth performance parameters of broilers

Abstract

The type of oil used in the broiler diet is mainly based on its price; in the case of soybean oil (CSO), acidulated soybean oil soapstock (ASS) is cheaper. The aim of the present study was to evaluate the effect of oregano oil dietary supplementation when the diet contains either CSO or ASS as source of energy. 320 broilers Ross 308 were assigned to four groups: (1) CSO containing diet without oregano supplementation, (2) ASS containing diet without oregano supplementation, (3) CSO containing diet with oregano supplementation (100 mg/kg), and (4) ASS containing diet with oregano supplementation (100 mg/kg). The growth performance parameters were weekly recorded during the experiment. Data were analysed with a completely randomised design with 2 X 2 factorial arrangement and the mortality rates were adjusted using the arc-sine transformation to fulfill the analysis requirements. The results showed no effect of oregano oil (containing 30.7 and 9.7 % of thymol and carvacrol, respectively) on feed intake, weight gain and feed conversion rate. A significant interaction of energy source by oregano oil supplementation was found in mortality rates (P < 0.05); mortality was higher when the diet did not contain oregano oil and was formed using ASS. It can be concluded that the addition of 100 mg of oregano oil per kg of feed did not have a positive effect on the growth performance parameters of broilers when the diet contained either crude soybean oil or acidulated soybean oil soapstock as sources of energy. El tipo de aceite utilizado en la alimentación de los pollos de engorda se basa principalmente en su precio; en el caso del aceite de soya crudo (CSO) el aceite acidificado de soya (ASS) es más económico. El objetivo del estudio fue evaluar el efecto del aceite de orégano adicionado en la dieta cuando contiene CSO o ASS como fuente de energía. 320 pollos de engorda Ross fueron asignados a cuatro grupos: (1) CSO dieta sin adición de aceite de orégano, (2) ASS dieta sin adición de aceite de orégano, (3) CSO dieta adicionada con aceite de orégano (100 mg/kg), y (4) ASS dieta adicionada con aceite de orégano (100 mg/kg). Se registraron los parámetros de rendimiento productivo y los datos fueron analizados con un diseño completamente al azar con arreglo factorial 2 X 2 y los índices de mortalidad fueron ajustados utilizando la transformación arco-seno para cumpliar las bases para su análisis. Los resultados no mostraron efecto del aceite de orégano (su contenido 30,7% y 9,7% de timol y carvacrol, respectivamente) en consumo de alimento, ganancia de peso e índice de conversión alimenticia. Hubo interacción significativa del recurso de energía y la adición del aceite de orégano a la dieta en los índices de mortalidad (P < 0,05); la mortalidad fue mayor cuando la dieta no contenía aceite de orégano y fue elaborada con ASS. Se concluye que la adición de 100 mg de aceite de orégano por kg de alimento no tuvo un efecto positivo sobre los parámetros de rendimiento productivo de los pollos de engorda cuando la dieta es formulada con aceite de soya crudo o aceite acidificado de soya como fuente de energía.

Abstract

Factors that compete to establish heart failure (HF) are not completely known. In the last years the several technological improvements allowed us to deeply study the molecular and genetic aspects of this complex syndrome. This new approach to HF based on molecular biology new discoveries shows us more clearly the pathophysiological bases of this disease, and a future scenery where the genetics may be useful in the clinical practice, as screening of high risk populations, as well as in the diagnosis and therapy of underlying myocardial diseases. The purpose of this review was to analyse the molecular, genetic and epigenetic factors of HF. We described the molecular anatomy of the sarcomere and the pathogenesis of the heart muscle diseases, abandoning the previous monogenic theory for the concept of a polygenic disease. Different actors play a role to cause the illness by themselves, modifying the expression of the disease and, eventually, the prognosis of the patient.
