Abstract

Abstract Background The objectives of this study were to determine the minimum incidence of penile cancer in the poorest Brazilian state, and to describe the epidemiologic and clinical characteristics of patients diagnosed with the disease. Methods A retrospective study of 392 patients diagnosed with penile cancer in the three most important referral center in the state was conducted during 2004–2014. Results The age-standardized incidence was 6.15 per 100,000 and the crude annual incidence was 1.18 per 100,000. More than half (61.1%) of the tumors were histological grades 2 and 3, and 66.4% of tumors were classified as at least stage T2. The average age of patients was 58.6 ± 15.7 years (range, 18 to 103 years), with 20.8% of patients ≤40 years of age at diagnosis. The vast majority underwent penectomy (93%). Only 41.8% underwent lymphadenectomy, 58 patients (14.8%) received chemotherapy, and 54 patients (13.8%) received radiotherapy. Stage 3/4 and vascular invasion were statically significant at disease-free survival analysis. Conclusion The state of Maranhão has the highest incidence of penile cancer in Brazil and globally. Tumors are locally advanced and at the time of diagnosis, and there is a high frequency among young individuals. Patients have a low socioeconomic status, making it difficult to complete treatment and receive appropriate follow-up.

Abstract

Infertility affects 15% of couples in reproductive age worldwide. In women in particular, infertility can be caused by various abnormalities, with polycystic ovary syndrome (PCOS) being the most common. Currently, there are many assisted reproductive techniques (ART) available to combat the burden of infertility. However, positive results are not guaranteed. The administration of inositol has been shown to increase positive reproductive outcomes in women undergoing ART. Here we present a series of clinical cases in which women with a history of infertility and previously failed ART, supplemented with a specific 3.6:1 MYO:DCI ratio, antioxidants, vitamins, and minerals for a period of 1 to 3 months before undergoing in vitro fertilization (IVF). In this series of case reports, we provide preliminary evidence that supplementation with a specific 3.6:1 MYO to DCI ratio, as well as antioxidants, vitamins, and minerals may contribute positively to female fertility in women undergoing IVF, with a history of primary or secondary infertility and previously failed ART. Abstract

Context. Invasive feral cats strongly threaten native fauna on islands worldwide. The impact of feral cat populations is assumed to be higher in the presence of introduced rodents and may also vary according to an island’s rodent species assemblage. Aims. This study assessed feral cat impacts on island biodiversity in French Polynesian islands harbouring differing assemblages of rat species, by investigating their abundance and trophic ecology. Methods. We focused on the following six human-inhabited islands of three archipelagos in French Polynesia (South Pacific) with differing rat species assemblages: Tahiti and Moorea, Ua Huka and Tahuata, Rurutu and Rimatara. We studied (1) cat abundance, by setting up camera traps during 15 consecutive days to provide an abundance index, the general index (GI) or mean number of virtual captures per camera per occasion, and (2) cat diet, by performing macroscopic analyses of scat samples to determine the frequency of occurrence (FO) of prey categories. Key results. Our study showed previously unreported patterns of cat-abundance index for the Society archipelago, with a GI of 0.30 ± 0.12 in Tahiti and 0.02 ± 0.02 in Moorea; for the Marquesas, with a GI of 0.75 ± 0.20 in Ua Huka and 0.20 ± 0.06 in Tahuata; and for the Austral, with a GI of 0.06 ± 0.04 in Rurutu and 0.19 ± 0.05 in Rimatara. Feral cats are shown to prey strongly on introduced rodents (FO = 91.3%), arthropods (37.8%), squamates (18.6%) and birds (13.3%) in our study sites. FO of birds are particularly high in Rimatara (31.9%) and Ua Huka (16.8%). Conclusions. We demonstrated that feral cats represent a serious threat to biodiversity in French Polynesian islands, with 15 species preyed on, including eight endemic birds, four of them being considered threatened by the IUCN Red List. Surprisingly, relative abundances of feral cats were higher both on islands harbouring only one rat species (Ua Huka, with only Rattus exulans) and on islands free of black rats. This finding raises questions regarding the ‘hyperpredation’ hypothesis for multi-invaded island ecosystems. Implications. This study on islands with differing assemblages of introduced rodents demonstrated the need for invasive predator studies in multi-invaded ecosystems, so as to improve bird conservation and guide management strategies and site prioritisation. Abstract

ABSTRACT OBJECTIVE To identify and analyze factors associated with preventable child deaths. METHODS This analytical cross-sectional study had preventable child mortality as dependent variable. From a population of 34,284 live births, we have selected a systematic sample of 4,402 children who did not die compared to 272 children who died from preventable causes during the period studied. The independent variables were analyzed in four hierarchical blocks: sociodemographic factors, the characteristics of the mother, prenatal and delivery care, and health conditions of the patient and neonatal care. We performed a descriptive statistical analysis and estimated multiple hierarchical logistic regression models. RESULTS Approximatelly 35.3% of the deaths could have been prevented with the early diagnosis and treatment of diseases during pregnancy and 26.8% of them could have been prevented with better care conditions for pregnant women. CONCLUSIONS The following characteristics of the mother are determinant for the higher mortality of children before the first year of life: living in neighborhoods with an average family income lower than four minimum wages, being aged ≤ 19 years, having one or more alive children, having a child with low APGAR level at the fifth minute of life, and having a child with low birth weight.

Abstract

Simple Summary: The term wasting is a clinical name describing a physical condition characterized by growth retardation, usually of multifactorial origin. The present study describes an apparently new condition of pigs characterized clinically by wasting and pathologically by vacuolization of the brain. During 2016–2018, animals from eight farms were weaned in good body condition, and after 1–2 weeks, they started losing weight. To investigate potential causes of this condition, apparently sick and healthy pigs from each herd of eight affected farms were studied by means of histopathology, transmission electron microscopy and detection of usual infectious agents. Histopathologically, the most consistent lesion was neuropil vacuolization of the prosencephalon, mainly located in the thalamic nuclei and in the transition between the white and grey matter of the neocortex. Electron microscopy of some of these sick animals showed preserved axons, with dilated myelin sheaths (interpreted as edema of myelin sheath). The literature suggests this lesion type is linked to congenital or metabolic (toxic/deficiency) scenarios. The present case was probably a vitamin/mineral deficiency since supplementation with nutritional complexes solved the problem. The term wasting refers to a clinical sign used to describe a physical condition characterized by growth retardation, usually of multifactorial origin. The objective of the present study was to describe for the first time a pathological process characterized by forebrain neuropil vacuolization in pigs showing wasting without conspicuous neurological signs. To characterize the lesions pathologically, affected and non-affected pigs from eight of these farms were investigated. Histologically, the most consistent lesion was neuropil vacuolization of the prosencephalon, mainly located in the thalamic nuclei and in the transition between the white and grey matter of the neocortex (40/56 in sick and 4/30 in healthy pigs). In the most severe cases, the vacuolation also involved the midbrain, cerebellar nuclei and, to a lesser extent, the medulla oblongata. Vacuolization of the forebrain was associated with pigs experiencing marked emaciation and growth retardation. Although the specific cause of the present case remained unknown, the preventive use of multivitamin and mineral complexes in drinking water ameliorated the condition, strongly suggesting a metabolic origin of the observed condition. Resumo: Para estudar os fatores anatomofisiológicos que interferem na qualidade de complexos cumulus-oócitos (CCOs) bovinos, foram obtidas 396 ovários após abate de 198 fêmeas Bos indicus em frigorífico. Os ovários foram separados por categorias, sendo distribuídos em nulípara vs multípara e com progesterona (P4 - presença de corpo lúteo em um dos ovários) vs sem progesterona (NP4 - ausência de corpo lúteo). Todos os folículos foram mensurados e categorizados em pequenos (9mm). Em seguida todos os folículos foram puncionados e os CCOs recuperados e avaliados morfologicamente. Não houve diferença na taxa de recuperação nem na qualidade dos CCOs de fêmeas nulíparas vs multíparas. O percentual de CCOs desnudos/degenerados foi maior no grupo NP4 e os CCOs expandidos foram superiores no grupo P4. A taxa de recuperação e o percentual de CCOs selecionados para PIV (graus I e II) foram similares nos grupos P4 vs NP4. Folículos pequenos apresentam menor taxa de recuperação em comparação aos de tamanho médio e grande, porém o percentual de CCOs de grau I foi superior em folículos pequenos e médios. Diante dos resultados aqui encontrados conclui-se que a categoria da doadora e a progesterona não influenciaram a qualidade de CCOs selecionados para PIV e que folículos menores apresentam de CCOs de melhor qualidade.

Abstract

Several epidemiological studies have reported the relation between chromium exposure (used in different industrial processes) and cancer risk. Evidence indicates that the hexavalent form is mutagenic and carcinogenic. Chemoprevention has emerged as a good strategy for reducing the risk from exposure to heavy metals. There is evidence that some tetrapyrrols such as protoporphyrin IX (PP-IX), a porphyrin without a metal center and which is a precursor of hemoglobin and cytochrome, acts as an antioxidant modulating the induction of antioxidant enzymes. The present study was performed to evaluate their antimutagenic potential of PP-IX against genetic damage induced by chromium trioxide (CrO3). The wing spot test was used. Groups of 48 h-old larvae were pretreated for 24 h with 0, 0.69, 6.9, or 69 mM of PP-IX, after which groups of larvae were fed 0.025–2.5 mM CrO3 solution in Drosophila instant medium. The results indicated that the lower PP-IX concentration (0.69 mM) significantly reduced the genetic damage induced by all CrO3 concentrations tested. In contrast, 6.9 and 69 mM only inhibited the damage induced by CrO3 2.5 mM. Absence of an inhibitor effect of PP-IX against 20 Gy gamma rays suggested that this porphyrin acted primarily by forming complexes with chromium at low doses, inactivating its genotoxic action rather than capturing or inactivating the reactive oxygen species generated by the chromium.
