Calculating polyenviromic risk/protective scores provides an updated perspective in research on the environmental causes of psychiatric disorders. We aimed to compare environmental risk and protective factors in patients with psychosis or a mood disorder (PSYCH+MOOD) and those with an anxiety disorder (ANX).Methods: We administered the internationally accepted questionnaire from the EUropean Network of National Schizophrenia Networks Studying Gene-Environment Interactions (EU-GEI) study, enriched with mood and anxiety disorder-relevant measures, to patients at two large university hospitals in the Czech Republic.Results: Ninety-four PSYCH+MOOD patients (average age 42.5 years; 46 males) and 52 ANX patients (average age 47.2 years; 17 males) participated. Neither polyenviromic risk score nor polyenviromic protective score differed significantly between PSYCH+MOOD and ANX groups (p = 0.149; p = 0.466, respectively).Conclusion: Scientific validity of the polyenviromic risk/protective score construct must still be demonstrated in large psychiatric samples, ideally in prospective studies. Nevertheless, researchers have already started to investigate environmental factors in the etiology of mental disorders in their complexity, similarly to polygenic risk scores.Keywords: anxiety disorders, environmental factors, mood disorders, schizophrenia, stress

PURPOSE OF THE STUDY To determine the optimal strategy for tranexamic acid (TXA) administration in diabetic patients, smokers and obese patients (BMI > 30 kg/m2) undergoing primary total knee arthroplasty (TKA). MATERIAL AND METHODS The total of 400 consecutive patients indicated for primary TKA were randomised into 4 basic groups with different TXA administration regimens. Group 1 (IV1) had a single intravenous dose (15 mg TXA/kg) applied prior to skin incision. Group 2 (IV2) got two intravenous doses (15 mg TXA/kg): one prior to skin incision and one subsequently 6 hours after the first dose. Group 3 (TOP) had 2 g TXA in 50 ml of saline irrigated topically at the end of the surgery. Group 4 (COMB) combined IV1 and TOP regimens. We monitored the amount of total blood loss (TBL), haemoglobin drop, use of blood transfusions (BTs), and complications in each patient. Follow-up period was one year postoperatively. RESULTS In the group of diabetic patients (n = 87; 21.7%) the lowest TBL was observed in the order: IV1, IV2 > COMB > TOP. In the obese patients (BMI > 30 kg/m²; n = 242; 60.5%), TBL was significantly lower in the intravenous regimens (IV1: p = 0.002; IV2: p = 0.005, respectively) than in the TOP regimen. In the smoking patients (n = 30; 7.5%), TBLs were significantly lower in the order: IV1 > IV2 > COMB > TOP. DISCUSSION Individualised approach to prevention and therapy is a recent trend, also because comorbidities significantly affect the result of the intervention. In the case of diabetes, obesity and smoking, there is a proven link to early post-operative infections, mainly due to poorer innate immunity. It is conceivable, though, that the occurrence of infectious complications is also contributed to by larger hematomas or hemarthroses which are largely preventable. CONCLUSIONS In the diabetic and obese patients (BMI > 30kg/m2), the combined topical/intravenous TXA application and two intravenous doses of TXA interventions were shown to be the most effective. However, no evidence of superiority of any of the TXA administration routes was obtained in the smokers. ABSTRACT Objective To establish the prevalence of risk factors for aortic dissection, such as bicuspid aortic valve, aortic coarctation and ascending aorta dilatation, in women with low-level 45,X/46, XX mosaicism undergoing an in-vitro fertilization ( IVF) procedure. Methods The study group comprised 25 women with low-level 45,X/46, XX mosaicism (ranging from 3.3% to 10.0%) who were referred to two reproductive medicine units between 2009 and 2013 because of infertility and who underwent subsequent karyotyping. In accordance with the recommendation of the Practice Committee of the American Society for Reproductive Medicine for patients with Turner syndrome ( TS), prior to the IVF procedure, all women underwent careful cardiovascular screening for congenital heart disease and thoracic aorta dilatation, including standard cardiac examination, echocardiography and non-contrast cardiac magnetic resonance imaging. Aortic size index ( ASI, diameter of the ascending aorta normalized to body surface area) and the prevalence of coarctation of the aorta and of bicuspid aortic valve were compared with findings previously reported in women with TS and the general population. Results Bicuspid aortic valve without any stenosis or regurgitation was found in one woman in the study group with low-level 45,X/46, XX mosaicism, a statistically significantly lower prevalence of bicuspid aortic valve than that reported in women with TS. Aortic coarctation was not identified in any individual. The ASI was below the 95th percentile in all cases and the mean value was significantly lower than the mean reference values for both the general population and women with TS. Conclusion Compared with the general population, the prevalence of risk factors for aortic dissection was not found to be higher in women with low-level 45,X/46, XX mosaicism without any noticeable features except infertility. Czechia lost more than 3,000,000 inhabitants as a result of the WW II. Germans displaced from the borderland formed the largest part. The newcomers after 1945 were of a different character – without any relation to their new settlements. This population formed a special social milieu familiar with the socialist way of thinking and that of a suppressed middle class. The consequences of it are seen in demographic, economic, environmental and social areas. After 1989, the factories in the borderland were mostly closed down, armies left the territory, people were not prepared to start their own businesses. Large-scale landscape protection formed a new barrier. Tourism is not able to substitute for the decrease in employment. The hope in cross-border collaboration has been overestimated.

Background: Absorption rates of the phosphate-buffered insulin analogs aspart, lispro, and glulisine prevail over that of regular human insulin. The aim of this prospective observational open-label controlled study was to compare the effects of aspart and human regular insulin resulting from their sequential long-lasting routine administration in small preprandial boluses to individuals with type 2 diabetes acccording to identical algorithms. Methods: Fifty-seven individuals with type 2 diabetes 64.0 +/- 1.29 (mean +/- SE) years old with diabetes' duration of 12.4 +/- 1.06 years, treated with human regular insulin for 5.2 +/- 0.44 years, and a serum C-peptide level of 1.1 +/- 0.10 nmol/L were enrolled into the study. Following two checkups performed in the course of the 364 +/- 17.9-day baseline period, human regular insulin was replaced with aspart in equivalent boluses, and two checkups in the course of 330 +/- 11.1-day sequential period were performed. The control group consisted of 17 individuals with type 2 diabetes 68.4 +/- 2.36 years old with diabetes' duration of 9.9 +/- 1.57 years, treated with insulin for 4.2 +/- 0.57 years, and a C-peptide level of 1.1 +/- 0.11 nmol/L. Data were analyzed using the statistical program SPSS version 10.1. (SPSS, Inc., Chicago, IL). Results: Following the switch from human regular insulin to aspart, hemoglobin A1c (HbA1c) decreased from 8.4 +/- 0.23% at baseline to 7.9 +/- 0.17% (P = 0.031), and thereafter to 7.5 +/- 0.20% (P < 0.001), while plasma glucose concentrations in 10-point profiles, daily insulin dose (37.1 +/- 1.39 IU/day), body mass index (BMI) (30.5 +/- 0.82 kg/m(2)), and frequency of hypo- and hyperglycemic episodes did not change (P > 0.05). Patientsquote satisfaction was good. No adverse events were recorded. In the control group, no significant change of baseline HbA1c (8.4 +/- 0.54%), insulin dose (33.1 +/- 3.17 IU/day), and BMI (32.1 +/- 1.12 kg/m(2)) was found. Tumors of the oral cavity are relatively common malignancies with a poor prognosis. Use of both prognostic and predictive biomarkers in saliva represents one approach to improve the management of patients with these cancers. The aim of the present study was to measure salivary neopterin concentrations before and after surgery in patients with oropharyngeal cancer to assess potential use as a biomarker in clinical practice. Salivary neopterin concentration was determined before and after surgery in 16 patients treated for oropharyngeal cancer and compared with 15 healthy volunteers. Salivary neopterin concentrations were significantly higher in patients before surgery compared to the healthy controls. After the surgery, neopterin concentrations decreased significantly, and concentrations in patients and controls were comparable. A significant correlation was observed between the sum of the Community Periodontal Index of Treatment Needs values and salivary neopterin concentrations. In conclusion, tumor removal results in significant decrease of salivary neopterin concentrations. The use of salivary neopterin in diagnosis is compounded by an association with periodontal disease.

The paper analyses the position of small towns in the Czech settlement system. It deals with the definition of small towns, their geographical positions, demographic characteristics and functions in the national settlement system. A typology of small towns aimed at individual pillars of their sustainability is one of the results of the paper. The article discusses the position of small towns as part of the urban world and their position as a part of the countryside. It concludes that small towns are functionally important as rural centres. However, differences between urban and rural seem to be less important than differences among individual types of the Czech countryside (suburban, intermediate, inner periphery, borderland).
