Noninvasive monitoring of kidney elimination of engineered nanoparticles at high temporal and spatial resolution will not only significantly advance our fundamental understandings of nephrology on the nanoscale, but also aid in the early detection of kidney disease, which affects more than 10 % of the worldwide population. Taking advantage of strong NIR absorption of the well‐defined Au25(SG)18 nanocluster, photoacoustic (PA) imaging was used to visualize its transport in situ through the aorta to the renal parenchyma and its subsequent filtration into the renal pelvis at a temporal resolution down to 1 s. High temporal and spatial resolution imaging of Au25(SG)18 kidney elimination allowed the accurate quantification of the glomerular filtration rate (GFR) of individual kidneys in normal and pathological conditions, broadening the biomedical applications of engineered nanoparticles in preclinical kidney research. Background: Paricalcitol, a selective activator of the vitamin D receptor (VDR), influences calcium and phosphorus homeostasis and bone metabolism. Whether paricalcitol reduces cardiovascular risk and protects renal function remains unclear. To systematically evaluate this in patients with chronic kidney disease (CKD), we conducted a meta-analysis of published randomized controlled trials (RCTs).Methods: We searched MEDLINE, Embase, the Cochrane Library, and reference lists for RCTs comparing paricalcitol with placebo in stage 2-5 CKD (including pre-dialysis and renal replacement patients). The Cochrane quality assessment method was used to evaluate study quality. Results were summarized as risk ratios (RRs) for dichotomous outcomes or mean differences (MD) for continuous outcomes.Results: We included 21 studies comprising 1894 patients. Compared to placebo, paricalcitol reduced the risk of cardiovascular events (RR 0.55; 95% CI 0.35-0.87; p = 0.01), but the RR of hypercalcemia associated with paricalcitol was 6.50 (95% CI 3.21-13.15; p < 0.00001). Paricalcitol cannot significantly change systolic blood pressure and cardiac structure. Although proteinuria reduction was achieved more frequently with paricalcitol (RR 1.51; 95% CI 1.25-1.82; p < 0.0001), it did not significantly reduce proteinuria level compared to placebo. Paricalcitol could not protect renal function to delay CKD progression, since it reduced the glomerular filtration rate (MD −3.15; 95% CI −4.35-−1.96; p < 0.0001) and elevated serum creatinine (MD 0.93; 95% CI 0.10-0.68; p = 0.008).Conclusion: Paricalcitol reduces the risk of cardiovascular events in CKD patients but increases the risk of hypercalcemia and cannot improve cardiac structure. Determination of clearance of endogenous creatinine using its plasma and urinary concentration (standard clearance) or Cockroft and Gault equation (estimated clearance) is commonly performed for assessment of glomerular filtration rate. Although evaluation of renal function in this way is useful, it is biased with an error resulting from secretion of creatinine in tubules. This error can be reduced by determining the clearance after administration of cimetidine. This study enrolled 40 patients who were divided into two groups- the first group consisted of 12 healthy subjects and the second group joined healthy subjects and patients with chronic renal failure. The second group was divided into three subgroups in relation to creatinine serum concentration (intervals: creatinine < 1 mg/dL - 9 subjects, creatinine 1-2 mg/dL - 7 subjects, creatinine > 2 mg/dL - 10 subjects). Estimated and standard creatinine clearance, clearance after cimetidine administration and inulin clearance were assessed. Analysis of mean percentage differences between clearance value after cimetidine administration and estimated or standard clearance shows increasing contribution of tubular creatinine secretion with increase in plasma creatinine concentration. This study suggests that determination of creatinine clearance after cimetidine administration can be used in precise evaluation of renal function. [ABSTRACT FROM AUTHOR]

Techniques have been developed for collection of urine in embryonic and newly hatched chickens for the purpose of studying the development of renal function. The reliability of EDTA-Cr as a substitute for inulin-C in the determination of GFR was studied. Since inulin and EDTA-Cr clearances in the hatched chicken averaged 1.61±0.23 (S.E.) ml/min per kg body weight and 1.58±0.27 ml/min per kg body weight, respectively, EDTA-Cr clearance was considered a suitable measure of GFR. GFR increased significantly in the first few days after hatching. Filtration rate was 0.068±0.008 (S.E.) ml/min per g kidney weight in the embryo and increased to 0.148±0.008 ml/min per g shortly after hatching. By nine days after hatching GFR had risen to 0.290±0.015 ml/min per g, a value comparable to that reported for the adult. Clearances of sodium, potassium, chloride and total osmolyte also increased with age. When these clearances were corrected for changing glomerular filtration rates the embryonic chicks were found to excrete a greater percentage of the filtered load.
