Itä-Suomen yliopiston Sosiaali- ja terveydenhuollon tietohallinnon maisterikoulutusohjelman opetus perustuu Kansainvälisen lääketieteellisen tietotekniikan ja tiedonhallinnan yhdistyksen IMIA:n (International Medical Informatics Association) koulutussuosituksiin. Koulutusohjelma on akkreditoitu vuosina 2012 ja 2018, ja se vastaa kansallisten strategioiden esittämiin haasteisiin, jotka liittyvät muun muassa kansalaisen ja ammattilaisen tiedonhallinnan osaamiseen, tiedolla johtamiseen sekä tiedonhallinnan ja tietojärjestelmien tutkimiseen. Tämän artikkelin tarkoituksena on kuvata sosiaali- ja terveydenhuollon tietohallinnon koulutusohjelmasta valmistuneiden pro gradu -tutkielmien ja väitöskirjojen keskeisiä tutkimuskohteita sosiaali- ja terveydenhuollon tiedonhallinnan paradigman viitekehyksessä vuosina 2002–2018. Tavoitteena on kuvata paradigman tutkimuskohteiden jakautumista, jota tietoa voidaan hyödyntää sekä oppiaineen opintojaksojen kehittämisessä että tulevien opinnäytteiden ohjauksessa. Tutkimusaineisto koostui maisteriohjelmasta valmistuneista pro gradu-tutkielmista (N=158) vuosilta 2002–2018 sekä väitöskirjoista (N=14) vuosilta 2011–2018. Aineisto analysoitiin kuvailevin tilastollisin menetelmin kuvaamalla aineiston jakautumista frekvenssein ja prosenttiosuuksin. Koulutusohjelmasta valmistuneet pro gradu -tutkielmat kohdistuivat pääasiassa toimintaprosessien tiedon hallinnan organisointiin ja ohjaukseen (n=55) kuten prosessien mallinnukseen tai arviointiin sekä tieto- ja viestintätekniikan käyttöön (n=48) kuten käyttöönottojen arviointiin tai sähköiseen asiointiin. Vastaavasti väitöskirjat (N=14) kohdistuivat erityisesti toimintaprosessien tiedon hallinnan organisointiin ja ohjaukseen (n=5) sekä tietorakenteisiin ja -malleihin (n=5) kuten tietosisältöjen määrittämiseen tai terminologian käyttöön. Prosenttiosuuksina tarkasteltaessa molemmissa opinnäytetöissä korostui toimintaprosessien tiedon hallinnan organisointi ja ohjaus (36 % ja 35 %) sekä tiedonhallinnan osaaminen ja tiedolla johtaminen (21 % ja 22 %). Tulokset linkittyvät kansallisen Sote-tieto hyötykäyttöön -strategian visioihin ja tuottavat tarpeellista tutkimusta tiedonhallinnasta. Jatkossa opinnäytteitä tulee kohdistaa enemmän myös tiedonhallinnan osaamisen tutkimiseen., The recommendations of the International Medical Informatics Association (IMIA) on education in biomedical and health informatics have been used as the basis for the master’s in health and human services informatics program at the University of Eastern Finland. Accredited in 2012 and 2018 by IMIA, the program responds to current challenges by focusing on informatics knowledge and skills among citizens and professionals, along with knowledge management and research and development of information management and systems as described in Finnish eHealth strategies. The purpose of this paper was to describe research focused on defining the health and human services informatics paradigm adopted in health and human services informatics programs. The aim was to describe the distribution in the research areas covered in master’s and doctoral theses and to use that information in the development of courses and theses guidance. The research data consisted of master’s theses (N=158) from the years 2002–2018 and doctoral theses (N=14) from the years 2011–2018. The data were analyzed using descriptive statistics to show distribution with frequenses and percentages. The master’s theses focused mostly on the steering and organizing of information management in work processes (n=55) such as modelling or assessing processes and the use of information and communication technology (n=48) such as assessing information systems’ implementations or digital services. The doctoral theses focused mostly on the steering and organizing of information management in work processes (n=5) and data models and structures (n=5) such as describing data elements or terminology usage. By percentages, both master’s and doctoral theses highlighted research on steering and organizing of information management in work processes (36% and 35%, respectively) and knowledge management and informatics competencies (21% and 22%, respectively). The study findings are connected to the visions of national eHealth and eSocial strategies to support citizens’ well-being and service renewal. In the future, both master’s and doctoral theses should also be directed to knowledge management and informatics competencies among citizens and professionals.

Sosiaali- ja terveysministeriön laatiman Sote-tieto hyötykäyttöön -strategian tarkoituksena on mahdollistaa, että terveydenhuollon ammattilaisella on käytössään ajantasainen potilastieto ja työtä tukevat tietojärjestelmät. Tietojen vaihto yli organisaatiorajojen toteutetaan valtakunnallisen Potilastiedon arkiston (Kanta-arkiston) avulla. Arkistoon on tallennettu tietoja lähes kaikista suomalaisista ja sen käyttö lisääntyy terveydenhuollon ammattilaisten keskuudessa koko ajan. Tutkimuksen tarkoituksena oli selvittää Kanta-arkiston käyttöä työterveyshuollossa. Sähköinen kysely lähetettiin työterveyslääkäreille, työterveyshoitajille, työfysioterapeuteille ja työterveyspsykologeille kesällä 2017 ja siihen vastasi yhteensä 359 henkilöä. Tulokset osoittavat, että Kanta-arkistoa pidetään hyvänä ideana, mutta sitä käytetään vielä vähän. Vastaajat kokevat, että tietojen haku on hidasta, tarvittavan tiedon löytäminen hankalaa ja potilastietojen tallentumisessa Kanta-arkistoon on ajoittain viivettä. Lisäksi Kanta-arkistossa näkyvät potilaskäyntien päivämäärät eivät aina välttämättä vastaa käynnin todellista ajankohtaa. Työterveyshuollon ammattilaiset uskovat muiden ammattilaisten hyödyntävän työterveyshuollossa syntyneitä tietoja, vaikka kokevatkin, että Kanta-arkiston avulla kokonaiskuvan saaminen potilaan hoidon tilanteesta ei onnistu helposti. Tietojen ajantasainen kirjaaminen ja niiden tallentuminen Kanta-arkistoon mahdollistaa potilaalle parhaan mahdollisen hoidon, kun ammattilaiset saavat tarvittaessa esille potilaan koko terveys- ja sairaushistorian. Vaikka Kanta-arkiston käyttö on vielä osin hankalaa ja tietojen löytämisessä on vaikeutta, voidaan kuitenkin sanoa sen täyttävän tarkoituksensa ja hyödyttävän työterveyshuollon ammattilaisen työtä. Tulevaisuudessa tilanne tulee paranemaan, kun Kanta-arkistoon toteutettavan tiedonhallintapalvelun kautta saadaan ammattilaisille käyttöön kooste potilasta koskevista keskeisistä terveystiedoista., The purpose of the eHealth and eSocial Strategy 2020 by the Finnish Ministry of Social Affairs and Health is to provide health care professionals with access to up-to-date patient data and information systems that support their work. Patient data are exchanged between organizations using the national Patient Data Repository (Kanta repository). The repository is increasingly used by the health care professionals as well as citizens. Presently, patient data of nearly every Finn are stored in it. The purpose of this study was to determine the use of the Kanta repository in Occupational Health Services using an electronic questionnaire in summer 2017. The survey targeted occupational health physicians, nurses, physiotherapists, and psychologists. There was a total of 359 respondents. The results indicate that the Kanta repository is positively viewed but not used extensively. Respondents say that patient data retrieval is slow, the required data cannot be found with certainty and occasionally, there are some delays with saving patient data in the Kanta repository. Furthermore, the dates of patient visits showed in the Kanta repository do not always correspond to the actual dates of the visits. Occupational health professionals believe that other professionals utilize patient data generated in occupational health services. However, they feel that getting an overall status of a patient’s care is not easy with the Kanta repository. Documenting and archiving patient data without delay in the Kanta repository provides patients with the best care possible, as professionals can access the patient’s entire medical history if necessary. Although using the Kanta repository is still somewhat cumbersome and there are difficulties with finding patient data, it fulfils its purpose and supports the occupational health professionals’ work. In the future, the situation will hopefully improve. The national patient data management service of the Kanta repository will start providing professionals with the summaries of patient’s key structural health information.

Teknologia mahdollistaa kansalaisten hoidon koordinoinnin eri palveluntuottajien välillä kansallisesti yli hallintorajojen. Myös rajat ylittävää hoitoa voidaan toteuttaa. Sähköinen arkistopalvelu parantaa asiakkaan hoitoa koskevan tiedon saantia sekä mahdollistaa kansalaisen osallisuuden itseään koskevaan päätöksentekoon. Ammattiin valmistuva sairaanhoitaja saa koulutuksensa aikana eHealth perustiedot ja -taidot. Tämän lisäksi sairaanhoitajat tarvitsevat monimuotoista koulutusta koko työuransa ajan. Teknologian kehittymisen myötä sairaanhoitajan työssä korostuu tieto- ja viestintätekniset taidot sekä interaktiivisen online viestintä- sekä vuorovaikutustaidot. Sairaanhoitajan työ muuttuu kohti potilaan omahoidon tukemista. Uusina rooleina ovat mentorointi, valmentaminen ja potilaan kumppanina oleminen. Sairaanhoitajien työolobarometrin (2016) mukaan suositus kuuden päivän vuotuisesta täydennyskoulutuksesta toteutui vain osin, sillä vain 30 prosenttia vastaajista ilmoitti käyneensä täydennyskoulutuksessa vähintään viisi päivää. Furthermore, the technology enables the cross-border care. The Finnish national archiving system supports citizen’s own initiative and decision-making. Nurse learns basic eHealth skills and knowledges during her/his studies. However, the while working the nurse needs to update technology- know-how continuously. New technology bring new demands for nurse competences like information technology skills, diversified interpersonal skills, online-based communication and interaction skills. Today the nurse expertise is challenges more towards self-care of smart patients. The new roles of the nurse are mentoring, coaching and companionship with the patient. According to the recent Nurse barometer related to working conditions, only 30 percent of the nurses use their justified benefit, five-day’s continuing training. The article describes nurse education and training from novice to expert, and the challenges nurses face.

ODA (Omat digiajan hyvinvointipalvelut) ja Virtuaalisairaala 2.0 -hankkeet kehittävät Suomeen uusia kansalaisille suunnattuja sähköisiä omahoitopalveluita. Sähköiset palvelut muuttavat terveysammattilaisten työtä ja aiheuttavat usein myös vastustusta. Uusien palveluiden onnistunut käyttöönotto niiden suunnittelusta ja kokeilusta vakiintuneeseen käyttöön asti vaatii, että muutokset tuodaan organisaatioon suunnitellusti. Tässä tutkimuksessa tunnistettiin käyttöönoton hyviä käytäntöjä tutkimuskirjallisuuden sekä ODA ja Virtuaalisairaala 2.0 -hankkeiden kokemusten perusteella sekä arvioitiin käytäntöjen toteutumista näissä hankkeissa. ODA-hankkeesta kartoitettiin kyselyllä siihen osallistuvien 14 sosiaali- ja terveysalan organisaation esimiesten ja johtajien (n=478) odotuksia Omaolo-palvelukokonaisuuden käyttöönotosta. Virtuaalisairaala 2.0 -hankkeen käyttöönottokokemuksia kerättiin haastattelemalla neljää Terveyskylän eri taloprojektin projektipäällikköä tai -koordinaattoria. Tulosten perusteella kokemukset käyttöönoton onnistumista tukevista käytännöistä ovat hyvin samansuuntaisia suomalaisissa hankkeissa sekä kansainvälisissä tutkimuksissa. Hyviksi käytännöiksi nousivat esimerkiksi selkeän vision ja tavoitteiden kommunikointi, johdon tuki, tehokas tiedottaminen palvelun käyttöönotosta ja sen hyödyistä sekä ammattilaisten ja asiantuntijoiden osallistuminen suunnitteluun. ODA-kyselyn tulosten mukaan käyttöönoton hyvät käytännöt eivät olleet kovin laajassa käytössä suomalaisissa sote-organisaatioissa. Tutkimuksessa tunnistetut hyvät käytännöt antavat vinkkejä käyttöönottotiimeille sekä johtajille ja esimiehille siitä, miten käyttöönoton voi toteuttaa suunnitelmallisemmin ja samalla tukea käyttöönoton onnistumista. Esimiehet ovat tärkeässä roolissa uusien sähköisten palveluiden organisaationlaajuisessa käyttöönotossa ja tunnistettuja käytäntöjä on tarkoitus hyödyntää esimiesten koulutuksen kehittämisessä., The ODA (Self-Treatment and Digital Value Services) and Virtuaalisairaala 2.0 projects develop new eHealth services for citizens in Finland. eHealth services change health professionals’ work and professionals often resist change. The successful implementation of eHealth services from planning and piloting to the organizational adoption requires that the changes are systematically incorporated within organizational practices. In this study, we reviewed the existing literature and collected experiences of the ODA and Virtuaalisairaala 2.0 projects to identify good implementation practices and understand their use. First, we collected survey-based data from supervisors and leaders who worked in the 14 health care organizations participating in the ODA project. The supervisors and leaders were asked about their expectations about the implementation of the ODA services. Second, we interviewed four project managers or coordinators about their experiences of implementing services developed in the Virtuaalisairaala 2.0 project. The results suggest that the experiences of the implementing practices supporting successful eHealth service adoption are similar in the Finnish eHealth projects and in international studies. For example, the identified good practices were communicating clear visions and goals for service adoption, leadership support, informing about service implementation and its benefits, and user participation in planning. According the ODA survey results, good implantation practices were not very widely used in Finnish health organizations. The research results give advice for implementing teams, leaders and supervisors how to support systematic implementation leading to successful outcomes. Supervisors have an important role in institutionalization of eHealth services and the identified implementation practices will be used in developing training materials for them.

The number of eHealth services supporting patients’ self-care and self-management is increasing. Patients are expected to participate more actively in their care, but nevertheless chronically ill patients use eHealth services less frequently than healthy citizens. The healthcare personnel’s motivation and guidance could increase willingness and capability of patients to use eHealth services. This article reports a study, which investigates what kind of support patients receive and how patients wish the healthcare personnel to support them in using eHealth services as well as what kind of eHealth services patients want to use in future. The study was conducted in Finland. A total of 397 patients representing three patient organizations for chronic illnesses (heart disease, diabetes or cancer) answered to the online questionnaire. Most of the respondents had used at least one eHealth service (92%) and the most common purpose was to seek information. eHealth services were mostly used on the patients’ own initiative. Only less than half of the patients (47%) had received information about eHealth services from the healthcare personnel. Even less participants had been encouraged (30%) or advised (21%) how to use these services by the healthcare personnel. The study shows that patients wish for a more active manner from the healthcare personnel in informing about the availability of eHealth services as well as providing guidance and encouragement in using the services. The wishes about future eHealth services were rather modest. Respondents wished for a more versatile digital booking of medical appointments and novel, easy-to-use communication channels with the personnel. The results suggest that it is not only sufficient to increase the number of eHealth services, but also the active patients need more information about eHealth services as well as guidance in their use.

Terveydenhuollon sähköisten palveluiden määrä on lisääntynyt merkittävästi. Potilaiden odotetaan osallistuvan aktiivisemmin hoitoonsa, mutta erityisesti kroonisesti sairaat käyttävät sähköisiä palveluita terveitä harvemmin. Kansainvälisten tutkimusten mukaan hoitohenkilökunnan motivointi ja tuki vaikuttavat merkittävästi potilaiden halukkuuteen ja kyvykkyyteen käyttää terveydenhuollon sähköisiä palveluita. Tässä artikkelissa raportoidaan tuloksia verkkokyselystä, jossa tutkittiin, millaista tukea ja ohjausta kroonisesti sairaat potilaat saavat ja toisaalta toivovat hoitohenkilökunnalta terveydenhuollon sähköisten palveluiden käyttöön. Kyselyssä potilaita pyydettiin arvioimaan, miten hoitohenkilökunta on ohjannut heitä sähköisten palveluiden käyttöön kertomalla palveluista, kannustamalla käyttämään niitä ja neuvomalla niiden käyttöä. Lisäksi kyselyllä selvitettiin, millaisia sähköisiä palveluja potilaat haluavat tulevaisuudessa käyttää. Kyselyyn vastasi 397 sydän-, diabetes- ja syöpäpotilasta. Suurin osa kyselyyn vastanneista potilaista oli käyttänyt vähintään yhtä terveydenhuollon sähköistä palvelua (92 %) ja eniten näitä palveluja käytettiin tiedon etsimiseen. Palvelujen käyttö perustui suurimmaksi osaksi potilaiden omaan aloitteeseen ja hoitohenkilökunnan rooli potilaiden motivoinnissa ja ohjauksessa jäi pieneksi. Alle puolet vastaajista (47 %) raportoi hoitohenkilökunnan kertoneen heille terveydenhuollon sähköisistä palveluista. Palvelujen käyttöön hoitohenkilökunta oli kannustanut noin kolmannesta (30 %) tai opastanut vielä harvempaa (21 %). Alle kolmannes vastaajista koki, ettei tarvitse apua tai tukea terveydenhuollon sähköisten palvelujen käyttöön, mutta monet toivoivat hoitohenkilökunnalta aktiivisempaa otetta palveluista tiedottamisessa sekä palveluiden käytön neuvomisessa ja kannustamisessa. Tulevaisuudessa potilaat ovat halukkaita käyttämään erityisesti käytännöllistä sähköistä ajanvarausta sekä uusia, helppoja yhteydenpitokanavia hoitohenkilökunnan kanssa. Suurin osa suomalaisista on 2000-luvun aikana siirtynyt käyttämään internetiä tiedonvälitykseen ja palveluiden hankintaan. Nuoret aikuiset ovat aktiivisin internetiä ja sähköpostia käyttävä ikäryhmä. Internet on hyvin suosittu terveysaiheisen tiedon lähde. Terveydenhuollon toiminnot, joissa tietoliikenneverkkoja hyödynnetään osana potilaiden hoitoa, muodostavat oman lääketieteen alan, telelääketieteen. Yhtenä uutena telelääketieteen osa-alueena, 2000-luvun sukupolvena , on sähköisen viestinnän ja internetpalveluiden soveltaminen terveydenhuollossa. Väitöskirjatutkimuksessa selvitettiin potilaan ja lääkärin välisen sähköpostiviestinnän ja yleisellä tasolla terveysneuvontaa antavan verkkoneuvontapalvelun käyttöä osana yliopisto-opiskelijoiden terveydenhuoltoa. Ylioppilaiden terveydenhoitosäätiö (YTHS) on valtakunnallinen organisaatio, joka tuottaa terveydenhuollon palveluita yliopistojen ja tiede- ja taidekorkeakoulujen opiskelijoille. YTHS on jo vuodesta 1993 tarjonnut opiskelijoille myös sähköisiä terveyspalveluita. Tutkimuksen aineisto koottiin kahdesta kyselytutkimuksesta ja rekisteritutkimuksesta. Vuonna 2003 selvitettiin lomakekyselyllä YTHS:n lääkärien sähköpostin käyttöä potilas-lääkärisuhteessa sekä lääkärien asenteita sähköiseen viestintään. Vuonna 2004 toteutettiin Suomessa laaja valtakunnallinen yliopisto-opiskelijoiden terveyttä ja terveyskäyttäytymistä selvittävä kyselytutkimus (Yliopisto-opiskelijoiden terveystutkimus 2004), jonka aineiston avulla selvitettiin opiskelijoiden suhtautumista sähköisiin terveyspalveluihin sekä niiden käyttöä ja käyttöön liittyviä taustamuuttujia. Rekisteritutkimuksen aineisto koottiin vuosien 2001 2007 aikana YTHS:n sairauskertomusjärjestelmään tallennetuista vastaanotto-, puhelin- ja sähköpostikontakteista. Lisäksi aineistoa koottiin verkkoneuvontapalveluun tallennetuista kysymyksistä ja YTHS:n opiskelija-määrätilastoista. Lääkärit suhtautuivat myönteisesti sähköpostin käyttöön potilastyössä, mutta arvioivat vain 2 % vastaanottokäyneistä olevan korvattavissa sähköpostikontaktilla. Lääkäreillä oli keskimäärin 9 sähköpostikontaktia viikossa, ja sähköpostikontaktien määrä oli keskimäärin viidesosa vastaanottokäyntien määrästä. Vastaanotto- ja puhelinkontaktit kirjattiin useimmiten sähköiseen sairauskertomusjärjestelmään, mutta suurin osa sähköpostikontakteista jäi kirjaamatta. YTHS:n lääkärien sähköposti- ja puhelinkontaktien määrä kasvoi selvästi vuosien 2001 2007 aikana. Sähköiseen sairauskertomukseen tallennettujen puhelin- ja sähköpostikontaktien yhteenlaskettu määrä vuonna 2007 oli 33 % kaikista potilaskontakteista. Jos tuloksia tarkastellaan huomioimalla sairaus-kertomukseen todennäköisesti tallentumatta jäävät kontaktit, oli sähköposti- ja puhelinkontaktien osuus vuonna 2007 kaikista potilaskontakteista 44 %. Opiskelijat suhtautuivat myönteisesti sähköisiin terveyspalveluihin: suurin osa haluaisi korvata joitakin nykyisistä palveluista sähköisillä. 12 % opiskelijoista oli käyttänyt sähköistä terveysneuvontaa. YTHS:n palveluita käyttäneistä opiskelijoista 19 % oli ollut sähköpostilla yhteydessä heitä hoitaneeseen terveydenhuollon ammattilaiseen. Naisista ja pääkaupunkiseudulla opiskelevista suurempi osa kuin muista opiskelijoista käytti sähköisiä terveyspalveluita. Verkkoneuvontapalvelun käyttäjien arvio omasta terveydentilasta ei poikennut muiden opiskelijoiden arviosta, mutta heistä suurempi osa kuin muista opiskelijoista oli käyttänyt YTHS:n lääkäripalveluita. Verkkoneuvonnan kysymysten ja vastausten määrässä ei tapahtunut merkittäviä muutoksia vuosien 2003 2006 aikana. Väitöskirjan tulokset osoittavat, että asioita, joita opiskeluterveydenhuollossa ennen hoidettiin vastaanotolla, hoidetaan nykyään puhelimessa ja sähköisesti. Opiskelijat ja lääkärit suhtautuvat sähköisiin palveluihin pääosin myönteisesti. Sähköpostikontakteilla voidaan lääkärien arvion mukaan korvata kokonaan vain hyvin pieni osa vastaanottokäynneistä, mutta ne voisivat korvata viidenneksen puhelinkontakteista. Yleisellä tasolla terveysneuvontaa tarjoavan verkkoneuvontapalvelun käyttö liittyy vastaanottopalveluiden käyttöön; sähköinen neuvonta voi toimia joko vastaanotolle hakeutumisen kynnystä madaltavana tai vastaanottotapahtumaa täydentävänä palveluna. Koska nykyiset lääkärin työn kirjaamismenetelmät eivät suoraan tue sähköisten kontaktien kirjaamista, jää merkittävä osa lääkärin työtä ja potilaiden hoitoon liittyvää tietoa kirjaamatta. Terveydenhuollossa tarvitaan sellaisia teknisiä ratkaisuja, joissa potilaan ja terveydenhuollon väliset puhelinkontaktit ja sähköisen viestinnän kontaktit voidaan liittää mahdollisimman yksinkertaisesti suoraan sairauskertomukseen. Ne parantaisivat osaltaan potilasturvallisuutta ja kaikkien osapuolten oikeusturvaa sekä helpottaisivat lääkärin työtä. The Internet is increasingly used by citizens for personal communication, as an information source and service channel. The Internet is particularly popular as a source of health information. The health care sector has been using telecommunications for nearly four decades now, and the first scientific articles in the field of telemedicine were published in the 1970s. Applications of electronic communication and networks in health care have led to the emergence in the 2000s of a completely new field, a new generation of telemedicine. The term eHealth , widely used in medical and health sciences literature, refers to the use of modern information and communication technologies to meet the needs of citizens, health care professionals and providers of health services. This study explores the use of email for patient communication and the use of a web-based health advice service among Finnish university students, together with a survey of the attitudes of students and doctors regarding eHealth applications in student health care. Finnish Student Health Services (FSHS) is nationwide organisation providing primary health care services for Finnish university students. FSHS services include health promotion, preventive health care, health care and medical treatment provided by general practitioners and specialists, mental health care, and oral health care. Since 1993, the services have also included eHealth services. Material for this study was collected from three separate data sources: from two cross-sectional studies and from a register study. In 2003, the use of email by FSHS physicians (n=74), their attitudes towards using email for patient communication and the percentage of email contacts missing from documentation in electronic patient records were studied. The survey, conducted using a postal questionnaire, had a response rate of 70%. A study to assess how students used eHealth and what they thought about it and to describe users of a web-based health advice service was undertaken in 2004 as a part of the national postal survey of Finnish undergraduate students. The population size was 101,805 and the study sample size 5,030. The response rate was 63%. The material of register study was collected from FSHS electronic patient record data regarding patient visits, phone calls and email contacts from year 2001 to 2007. The annual count of students entitled to use FSHS services and ! the count data of the questions and answers of the FSHS web-based health advice service from 2003 to 2006 were also used in the register study. The majority of FSHS physicians had a positive attitude towards using email for patient communication. They estimated that 2% of patient visits could have been replaced by email consultation. FSHS physicians reported an average of 9 email contacts per week and a mean percentage of 20% for email/visit . The patient visits and phone calls which FSHS physicians participating in the survey registered during the study period were mainly documented in the electronic patient record, but a major percentage of the email contacts was not documented in the electronic patient record. The electronic patient record supports and reminds doctors in documentation patient visits and contacts by phone as part of the patient data. There is no similar system in place for email contacts. The number of email and phone contacts of FSHS physicians increased noticeably between 2001 and 2007. The total number of phone and email contacts entered in the electronic patient records amounted to 33% of all patient contacts in 2007. If we consider the contacts not entered in the electronic patient records, this figure goes up to 43% of all patient contacts. Students generally had a positive attitude towards eHealth; many of them claimed that they were willing to substitute eHealth for some of the traditional forms of health services. 19% of the students had used email to communicate with health care professionals in the student health services, and 12% had used the web-based health advice service of the FSHS. Women tended to use eHealth more actively, as did students living in the Helsinki area. The use of the FSHS web-based health advice service was not associated with self-reported health status, but the users of the web service reported a higher numbers of visits to an FSHS doctor than the non-users. The number of questions sent and answered in the FSHS web-based health advice service did not vary remarkably during the study period 2003-2006. The findings in this dissertation demonstrate that in student health care, many of the things that used to be handled during a visit to the physician are now dealt with on the phone and electronically. Because the present systems for recording a physician s work do not directly support the entering of electronic contacts, a substantial amount of information about the physician s work and the patient s care remains unrecorded. The health care sector needs technical solutions for merging phone contacts and electronic contacts directly and conveniently into the patient record. Such solutions could significantly improve patient security, the effectiveness of the physicians work, and the legal protection of all parties involved. Health care professionals should work with IT professionals to develop electronic services that complement existing services and support and improve good patient care.
