The article discusses the impact of exclusion from work to the quality of psychiatry user's lives. The paper describes the process experienced by the majority of users after retirement, which in many cases ends with a wish for paid work. The exclusion of psychiatry users is sanctioned by early and extensive retiring and by legislation that only allows minimal work for retired persons, in Slovenia. Exclusion has a negative impact upon various aspects of mental functioning, resulting in poverty. It also prevents users from participation in leisure time activities accessible to people. This area has been all but neglected until now. A number of cases have been presented that show how users were granted paid work elsewhere. The paper concludes with the author's proposals to solve the problem in Slovenia.

Abstract: This paper was initially presented during the scientific workshop of the French society for child and adolescent psychiatry and allied professions (SFPEADA), devoted to psychomotricity and its current bonds with child and adolescent psychiatry (Paris, September 29, 2005). The authors are a child and adolescent psychiatrist, using daily psychoanalytical psychotherapies in a universitary public hospital and two psychomotricians, who work in a child and adolescent public department, in a private activity and in a specialized institute (therapeutic, educational and teaching institute or ITEP). They present here the results of their clinical practices, exchanges and thoughts about a complex and discussed subject, which touches in more than one way with the bases of our clinical and therapeutic practices with children and adolescents: how do we choose the differential indications between psychotherapies and psychomotor therapies? Abstract: The following paper aims at finding and analyzing the objective conditions and the subjective processes which lead people to choose studying psychiatry. The authors have used the testimonies of 29 French psychiatrists who had been invited by the review Psychiatrie Internationale and who had been asked the question: “Why have you chosen to study psychiatry?” These texts were compiled and published in 1993 and have undergone content analysis. The results show that in order to explain this choice of studies, it is necessary to consider objective factors (social background, level of achievement during internship, etc.), contingent elements (encounters with professors and/or patients), and more subjective processes which make the choice of this medical specialization an answer to each person''s problems. Furthermore, the specificities of this original corpus show the importance of socio-historical conditions under which the choice of studies is done and invite us to reconsider the issue through the current context. 10. Mental illness in Bwindi, Uganda: Understanding stakeholder perceptions of benefits and barriers to developing a community-based mental health programme

Background: Mental illness has been increasingly recognised as a source of morbidity in low- and middle-income countries and significant treatment gaps exist worldwide. Studies have demonstrated the effectiveness of task sharing through community-based treatment models for addressing international mental health issues. Aim: This paper aims to evaluate the perceptions of a wide range of mental health stakeholders in a Ugandan community regarding the benefits and barriers to developing a community-based mental health programme. Setting: Bwindi Community Hospital (BCH) in south-west Uganda provides services through a team of community health workers to people in the Kanungu District. Methods: Thematic analysis of 13 semi-structured interviews and 6 focus group discussions involving 54 community members and 13 mental health stakeholders within the BCH catchment area. Results: Stakeholders perceived benefits to a community-based compared to a hospital-based programme, including improved patient care, lower costs to patients and improved community understanding of mental illness. They also cited barriers including cost, insufficient workforce and a lack of community readiness.Conclusions: Stakeholders express interest in developing community-based mental health programmes, as they feel that it will address mental health needs in the community and improve community awareness of mental illness. However, they also report that cost is a significant barrier to programme development that will have to be addressed prior to being able to successfully establish such programming. Additionally, many community members expressed unique sociocultural beliefs regarding the nature of mental illness and those suffering from a psychiatric disease.

This paper analyzes the clinical files from two inmates in the General Insane Asylum La Castañeda (Mexico City) during the Revolution (1910-1920). It exposes three aspects: First, the methodological problem that this kind of files present. Secondly, it analyzes the social relations and the cultural context of both cases. Third, it exposes the value of this kind of clinical files for the history of psychiatry and culture.

Background. This article presents a literature review based on the following question: According to service providers and clients, is the approach adopted by service providers in psychiatry, centred on client needs? Purpose. To compare the perceptions of clients and service providers regarding the use of a client-centred approach and the identification of the intervention needs in psychiatry. Methods. Medline, Psyclnfo and CINAHL databases were consulted; eight studies were selected. Results. Service providers and clients agree to the fact that the client-centred approach is notpriorized in psychiatry. Different reasons are given by both parties. There is little agreement between clients and service providers regarding the intervention needs. Many clients needs seem to be unsatisfied (interpersonal relationships, independent life skills, productive activities, coping with illness, health care). Practice Implications. Occupational therapists must adopt a client-centred approach to their interventions by evaluating the global needs of the clients, by developing services to encourage clients to engage in social or productive activities and by consulting the clients and their families in order to evaluate the quality of the services. Within the framework of a political economy approach, the paper analyses the social construction of both markets and illnesses in the field of mental health, linked with the recent debates associated to the publication of the latter classification of mental disorders (DSM-5). Illustrated by the example of attention deficit and hyperactivity disorder (ADHD), this way of analysing the political work carried out by different actors (pharmaceutical firms, parent of patient associations, the school system, the state, the wealth system, etc.) discerns their respective roles and, in particular, enables research to understand considerable differences between the French and US cases. More than just enlightening ethical questions in the field of mental health, studying the production of scientific discourse about a specific illness identifies the economy of research financing and of scientific publication that drives actor interactions and interdependencies.
