Since it is quite difficult to commonly use isoelectric focusing (IEF) of proteins in polyacrylamide gel for biological diagnosis, we have developed a method based on IEF in agarose gel, to split proteins from sera and cerebrospinal fluid (CSF). A prefocalisation at low voltage (250 V) is made on a custom thin gel of agarose (0.5 mm) containing some carrier ampholytes (pH 5-9). After deposition of biological samples, the gel is run at 500 V, thereafter at 1200 V. After focusing, the gel is fixed before being coloured by a simplified silver staining technique. In order to demonstrate the good resolution of the immunoglobulines (Ig) in the pH gradient, a transfer on a nitrocellulose membrane followed by an immunofixation was carried out from unstained gels after IEF. This separation on agarose gel shows several advantages, ie its speed (3H total), its lack of toxicity, its sensibility and its reproductibility. It is specially well suited for the diagnosis of diseases characterised by oligoclonal or monoclonal Ig, particularly those found in the CSF during neurologic diseases like multiple sclerosis. Several examples of focused sera and CSF are reviewed in the paper.

The demonstration and characterization of monoclonal immunoglobulins in biological fluids are usually performed using electrophoresis coupled to antigen-antibody reactions. This paper briefly discusses the most commonly used techniques. Small, often multiple serum monoclonal immunoglobulins, mostly unrelated to malignant proliferations, are frequent when using recent high resolution methods. Their pathological significance should take into account a number of physiological, clinical and biological data.

The aim of this paper was to review the action of the receptors for the Fc portion of Ig on the polyclonal activation of human B lymphocytes. The IgG Fc fragments were shown to be digested by monocytes into peptides which triggered B-cell differentiation and the release of a T-cell replacing factor termed (Fc)TRF. In the presence of PWM and monocytes, unbound aggregated IgG suppressed B-cell differentiation into Ig-synthesizing cells. This suppression was not isotype-specific. After having bound aggregated IgG, T cells were able to display an isotype-restricted suppression of B-cell differentiation induced by PWM. Finally, soluble Fc gamma R released from T or B lymphocytes or neutrophils were found to inhibit both the synthesis of IgG and the maturation of B cells into IgM or Ig A-secreting cells.

The paper reports the case of a 14 year-old boy, born at full term with intra-uterine growth retardation (1,640 g) who presented with short stature, generalized eczema and recurrent infections. During the first years of life, hypogammaglobulinemia with antibody deficiency occurred. The in vitro T lymphocyte function was normal. The infections have become less severe. The plasma levels of IgA, IgG, IgM and IgE are normal. There is hyperimmunoglobulinemia D. The deficiency of antibodies against most of the tested antigens persists. T and B lymphocyte counts are normal. The in vitro lymphocyte proliferation with Concanavalin A, Phytohemagglutinin, Pokeweed mitogen and Nocardia is normal. The OKT4+ and OKT8+ cell counts are normal. The ratio "helper T cell/suppressive T cell activity" appears to be abnormal. A very prolonged maturation delay, possibly associated with fetal hypotrophy may be hypothesized.

In this paper, the authors show that the cellulose acetate immunofixation technic allows from a certain dilution of patient serum to point out only the monoclonal character of the immunoglobulins.

Human monoclonal antibodies will be essential in medicine. They are valuable tools for biological diagnosis and therapeutics. Our model, human monoclonal antibodies directed against the Rhesus D antigen can be used for the determination of the Rhesus D phenotype and for the suppression of Rh(D) immunisation in women. These new products require new procedures of preparation, new regulations for the quality controls, which will be discussed in this paper.

Abstract: The French quality control is organized by the French Health Products Safety Agency. In 2005, the immuno-hæmatology testing control included the screening of an anti KEL 1 antibody. 17 out of 2639 laboratories (0,64%) answered ‘negative screening’. All laboratories received a questionnaire in order to understand the failure. In this paper the authors present the detailed laboratories'' responses and failure explanations.
