Over the past few years, an interprofessional team of lecturers in Geneva has developed courses entitled « adolescent health and development ». These courses are open to students in medicine, nursing and nutrition, as well as to other interested health professionals. Using vignettes, students who participated in the courses present their thoughts on the use of the HEADSSS interview guide in the diagnostic approach with adolescents. These optional courses provide an opportunity to explore infrequently taught domains of the curriculum in a creative way. Ensuring the continuity and quality of home health care services as well as coordinating their work with that of other caregivers, such is the role of the nurse in the provision of services.The nurse's role has developed over recent years, in order to meet the needs of society and the health care economy. The role played by nurses as home health care service providers is clearly that of caring for and supporting the patient. They work in close partnership with community professionals.

Objectives: This study aimed to describe the professional practices in pediatric transfusion to assess the accuracy of transfusion guidelines in children., Methods: The study retrospectively analyzed the characteristics of all the pediatric transfusions prescribed in the Clermont-Ferrand (France) university hospital center over 1 year and determined whether they conformed to the national guidelines., Results: One thousand six hundred and seven blood products were delivered to 233 children (806 red cell units, 670 platelet units, and 131 plasma units), accounting for 5.3% of the center's whole blood products. Transfusions were mainly prescribed by the oncohematology unit (68.2%), the intensive care unit (15.4%), and for surgery (10.2%). Ten adverse events were reported in eight patients (0.6% of transfusions). The prescription conformed to the national guidelines in 35.9%, 41.6%, and 80.9% of the red blood cell, platelet, and plasma unit transfusions, respectively. Nonconformity was mainly due to abusive irradiation and "cytomegalovirus seronegative" specifications., Conclusion: Malignancies, intensive care, and surgery are the main indications for transfusion in children. Substantial discrepancy between recommendations and actual practices was observed. This illustrates the variability of risk evaluation. Meeting adolescents' health needs in primary care implies knowledge of developmental, psycho-social and physical specificities of this age group. Primary care physicians need appropriate communication skills and knowledge of specific adolescent health issues such as the role of families, the need for confidential services and networking with schools, social services and other key individuals in young people's lives. Primary care physicians rarely receive training in adolescent health and are usually interested in practical screening or counselling strategies. HEADSSS is a useful mnemonic to guide the psychosocial interview of an adolescent. It can be used in daily practice to help the physician to focus not only on "risk taking", but also on the personal strengths and protective factors that allow young people to develop harmoniously.

Cyst or intestinal duplications can arise anywhere along the gut, however those located in the rectum are very rare and only a few dozen cases have been reported. The reason that induced us to report this patient is double: first to present a new case of rectal duplication diagnosed at 45 days old that had a normal barium enema previously, second to confirm once more that the muscular complex of the rectum can be cut in the posterior middle line without any damage to the rectal continence function, as Peña's surgical approach for anorectal atresias. The case reported correspond to a newborn weighing 2,850 grs who had exomphalos of 5 cm. Wide with an integral sac and was operated by primary closure. When he was 10 days old, and because he had some intestinal disturbances compatible with malrotation, a barium enema was done that was normal. He was discharged and returned 30 days later because of striped feces and constipation. Rectal examination showed a retrorectal tumor located at left posterolateral space. Ultrasound showed a cystic mass and barium enema displayed a narrowed rectum channel. First we did a Wangesteen colostomy. Ten days later, by a sagittal posterior approach cutting the Levator and Muscular Complex of the rectum in the middle line and without opening the lumen, a tumor like a nut, sharing its muscular coat with the rectum, was excised. The Muscular Complex and the Levator were repaired with the aid of the electrostimulator. After the 7 day postoperation we made some rectal dilatations and closure of colostomy at 21 day.(ABSTRACT TRUNCATED AT 250 WORDS)

This research studies family communications during the critical period of change provoked by the puberty of one of the children. Two groups of families are compared: one group of 15 "functional" families (no psychological consultation has been requested) and one group of 15 "dysfunctional" families (a psychological consultation has been requested for one of the adolescents). The micro-analysis of the verbal communications reveals a few significant differences between the two groups of families. The results point to structural differences between the family systems.

This study analyzes communication in families with one or more adolescent children, thus undergoing a critical period of transition. Two groups are compared: a group of 11 "functional" families (no child referral for psychological problems) and a group of 11 "dysfunctional" families (one adolescent child referral for psychological problems). The analysis of relationships between verbal and non-verbal communication indicates that more inter-channel congruence is achieved in "functional" families; this holds for individual as well as for inter-partners interactions. The results of the previously published separate analysis of verbal and non-verbal communication are briefly resumed. The findings of the study as a whole are interpreted in terms of the General Systems Theory with regard to the function of "parental framing" of the child's development.

Experimental studies have been made in rabbits in which an operative gap in the nerve was bridged either with a fresh nerve graft taken from the opposite side or with a degenerate graft, the nerve on the opposite side having been divided 15 days earlier. Technical details of the method are described. An electromyographic study demonstrated better results with the predegenerate graft and histological examination showed a better maturation of the axons.

Castrated rats submitted to androgenic stimulation present hyperseborrhea with an increase in foamy cells associated with an increase in cutaneous lipids and a change in the number of epidermal cell layers. 13-cis-retinoic acid and aromatic retinoid (Ro 10-9359) were given orally during 17 days (10 mg/kg) to castrated rats which had received a testosterone implant. Cyproterone acetate was chosen as the antiandrogenic drug; it was given at doses of 10 mg/kg i.m. once every other day. As opposed to cyproterone acetate, the two substances studied had no effect on the seminal vesicle and ventral prostate. 13-cis-retinoic acid and retinoic aromatic acid increased the total number of epidermal layers: In the case of 13-cis-retinoic acid, the increase involves especially the nonnucleated cell layers, whereas with retinoid aromatic acid it involves the nucleated cell layer. The number of sebaceous glands is not changed by either products, on the other hand, the number of foamy cells is considerably decreased and the weight of cutaneous lipids is decreased as well.

We report a case of a boy of 13 who was born with muscular hypotony which has tended to improve over the course of the years. We was examined for the first time when he was 4 years old and a diagnosis was made of "congenital myopathy". Examination showed curved feet, gothie palate, severe laxity of the ligaments, and spider fingers which evoked Marfan's syndrome. The present myopathy is concentrated in the shoulder girdle and the E.M.G. showed "myogenic" tracings. Biopsy of the deltoid showed the presence of large numbers of rods in all muscle fibers stained with Engel's modified trichromic stain, localized mainly under the sarcolemma. Histochemical staining showed no evidence of differences in the fibers, all of which had the same enzymatic activity. A diagnosis was made of nemaline myopathy. As we were able to follow clinical progress in this case for nine consecutive years we could confirm that not only was the disease non-progressive but that it was regressing in this patient.
