The orbital septum, a well-known structure since a long time ago, is a controversy object anyway. Its physiological, anatomical and pathological importance is undeniable. However, it will be difficult not to consider it as a part of the whole structure which organizes the general system of the soft tissues of the face, in which the orbital frame is one of the pivots.

Aging brings about anatomical changes that are undesirable. Senescence is consequently one of the most powerful stimuli for aesthetic surgery. The face, responsible for the myriad expressions of human experience, is constantly subject to exposure and also suffers from the perennial action of environmental factors and gravity. Frequently, the face is the main focus of anxiety in individuals who have attained a certain age, presenting skin flaccidity and loss of tone, marked lines of expression and fat deposits. Expectations of both the patient and the surgeon, regarding facial aesthetic surgery, have increased considerably over the last two decades with the advent of newer procedures and equipment. While experience allows the surgeon a better selection of techniques, at the same time he must be knowledgeable in details of different surgical approaches and variations to attain the best result for each individual case. The anatomy of the aging face should be examined carefully for a personalized treatment plan. Currently, a satisfactory result of an aesthetic facial procedure is obtained when signs of an operation are undetectable and no anatomical landmarks have been altered. The bond created between the patient and surgeon will be frustrated if there remain signs that reveal the "secret" that surgery was performed.

Objective: The retro vesical septum (DENONVILLIERS) can be dissected and separated of the prostate and the bladder. The neurovascular bundles bound laterally this septum and it must be removed during radial prostatectomy. The aim of this study was to know if this septum is crossed by nerves., Material and Methods: Dissection of cadaver--histological study of recto-vesical fascia., Results: Nerves are present coming from the neurovascular bundle to the septum and end in the prostate., Conclusion: These anatomical observations conduct to recommend to remove systematically the recto-vesical septum during radical prostatectomy for cancer.

Passage through the skin of an abutment anchored on an implant fixed on the underlying one constitutes a Permanent Percutaneous Connection of which the principal applications are stabilization of maxillofacial prosthesis (MFP) or bone anchored hearing aid (BAHA) support. The long-term outcome of a PPC depends on a variety of different parameters. We analyzed the influence of the most important factor: osseointegration.

Areolar tissue is a loose conjunctive tissue, located between the fascia temporalis superficialis and the temporal aponeurosis. It can be used in several circumstances during otologic surgery. First, in stapes surgery, it can be used as an interposition between the piston and the platinotomy. Its thickness has no equivalent in comparison with other temporal tissues, explaining in part its usefulness in this indication. In perilymphatic fistula surgery, the areolar tissue is especially recommended for round window and oval window grafting because of its spontaneous adhesive capacity. It can be useful in myringoplasty, to reenforce an atrophic tympanic membrane or to close a small perforation. Also areolar tissue can be used as a flap or as a free graft to reconstruction of the external acoustic meatus. It's a good conjunctive support, which can be covered by skin graft. Finally areolar tissue is an interesting and useful material for otologic surgery. Its qualities are complementary to those of the temporal aponeurosis.

The authors report their experience in the use of the reversed fascio-subcutaneous flap of the leg in 11 cases for covering soft tissue defects of the distal third of the leg, the malloli and the posterior heel. An anatomical study has been performed on fifteen fresh cadavers (30 legs), in order to precise the indications and limits in the dissection of the flap pedicle. The authors report six necrosis (2 superficial, 4 total) of the distal part of the flap. Despite these complications, they consider that this flap is an interesting procedure due to its advantages: the respect to anatomical structures, an easy dissection, a supple pedicle and an aesthetic aspect of the donor area.

The author describes a new, simple technique for the treatment of idiopathic hydrocele according to a "fenestration" procedure designed to correct the imbalance of intravaginal hydrostatic exchanges. The operation consists of initially hemming separately the deep fibrous layer and the parietal leaflet of the tunica vaginalis to form the margins of the "window", which is then applied by its fibrous layer to the dartos, continuous with the subcutaneous connective tissue, a site of rich lymphatic drainage. In a series of 108 patients treated by this technique since September 1982, with a follow-up of 6 years, the anatomical and functional results were excellent in 98 cases (90.7%), moderate in 6 cases (5.5%) and poor in 4 cases (3.7%). Compared to classical techniques, the fenestrated opening of the tunica vaginalis with preservation of the tunica vaginalis and limited opening of the serosa, preserves the testicular support and the protective role of the serosa on the testis. Lastly, suture of the margins of the window to the dartos, by facilitating deep lymphatic drainage towards the superficial connective tissue, reconstitutes a physiological equilibrium in the constant exchanges of intravaginal serous fluids. Experience has demonstrated the anatomical and functional efficacy and reliability of this extremely simple procedure, which is essentially indicated in adult patients with idiopathic hydrocele and a healthy tunica vaginalis.

The fibrosis of the hypodermis in the lower part of the leg is a sign of venous stasis. But it appears in various forms. One is diffuse, circular, atrophic sclerosis, in a patch, occurring frequently in old post-thrombotic syndromes, another is sclerous hypodermititis in plaque form, submalleolar, well circumscribed and without aetiopathogenic links with deep venous thrombosis. This latter form is the only one to respond favorably to surgical resection.

A new technique of functional laryngectomy using sub-perichondral curettage of some or all of the soft parts of the larynx is described. This method has an embryological and anatomical basis. It is supported by a score of cases which have had a satisfactory outcome, all the patients operated on having been decannulated. Histopathological research was continued with a view to confirming the function of the perichondrium and the cartilage as a barrier to neoplastic invasion.
