Summary: Study Purpose: For ten years the market for resorbable fillers has seen a large increase. A CE mark is sufficient for its placing on the market, they do not require AMM because they are not considered drugs. The Macrolane® is a hyaluronic acid NASHA gel-based technology, available on the French market since 2007 as filler used in all areas of the body except face and breasts. It is highly crosslinked, which slows its resorption, leaving in place long-term waste. At end of 2008, Macrolane® received a CE mark for breast augmentation. The aim of this paper is to review current scientific knowledge on the Macrolane™ and list the many uncertainties regarding its recent breast indication. Materials and methods: We reviewed the PubMed literature and study levels of evidence on Macrolane®. All AFFSAPS communication and correspondence with the SOFCPRE on hyaluronic acids and particularly Macrolane™ were collected. Results: At the moment there is no scientific study of high level of evidence which has studied the effects of Macrolane® on breast parenchyma in terms of carcinogenesis or the disruption of radiological monitoring of the breast. The subglandular method of injection remains complex and uncertain especially about the risk of wrong passage. Conclusions: The Macrolane® is proposed as an alternative less invasive than breast implants. Nevertheless the lack of scientific data on this product led to its non approval by the Food and Drug Administration in the United States. Currently there remains too much uncertainty on this filler for reasonable use plebiscite. It would therefore be preferable to return to the manufacturer the burden of proof of Macrolane® safety and security and limit its use in clinical trials yet. [ABSTRACT FROM AUTHOR]

OBJECTIVE AND DATA-GATHERING: We reviewed experimental litterature about kidney adaptation after nephrectomy in mammals. KNOWLEDGE SYNTHESIS: Renal mass increases after nephrectomy thanks to two components, one is immediately due to the rise of glomulary capillary vascular flow, other is linked to cellular modifications with hyperplasia stage which precedes hypertrophy stage. After nephrectomy, young animals show higher renal adaptability than adults. Similarly, the sex influences the remnant kidney parenchyma volume, the increase of glomerular filtration, the hyperplasia's intensity or length, the hypertrophy's metabolic pathways and the glomerular and tubular cells' injury. Introduction: Primary malignant melanoma of the larynx is a very rare entity. There are less than sixty cases described in the literature. Through both cases, we describe and discuss the clinical management of melanoma of the larynx., Clinical Cases: Two men aged 80 and 61 years were supported for MPL. The tumor was revealed by persistent dysphonia for the first. For the second, it was a second melanoma of laryngeal location. In both cases it was performed a total laryngectomy extended to the first tracheal rings with bilateral lymphadenectomy associated with adjuvant radiotherapy., Discussion: Excision margins with oncological safety are necessary to except a curative surgery. Still controversial, the adjuvant radiotherapy seems to provide a benefit in terms of local control. The place of targeted therapies remains unclear. However, no prognostic factor is currently clearly identified for this disease., Conclusion: Rare tumor, the MPL is associated with a very poor prognosis. Resective surgery followed by adjuvant radiotherapy is for us the treatment of choice.

The transsexualism or gender dysphoria is a pathology during which an individual does not recognize himself in his sexual identity and wishes to change it: in that it must be differentiated from the sexual ambiguities (hermaphrodism, pseudohermaphroditism) in which the sexual phenotype is not clearly established. In France the number of transsexuals is estimated at approximately 50,000 people. Since 2009 the transsexualism is not any more considered as a mental illness, it remains regarded as a long term illness. The objective of this article is to present the recent evolutions concerning the management of transsexual patients seeking feminization.

Submental skin and fat excess are common with aging. Real cutaneous crop in pronounced forms, may be felt with difficulty by patients, which leads them to consult. The proposal of a conventional face-lift is not always suitable or accepted by a predominantly male population. Therefore, an anterior medial cervicoplasty with elliptical excision of excess skin and a medial platysmaplasty will be an interesting alternative. The authors offer a new description of this technique by simplifying, compared to the previously described techniques. Two clinical cases are reported, with both types of recommended incisions. This procedure is performed under local anesthesia. The results obtained by this technique were considered good to excellent by all operated patients in our unit. The final scar that may appear significant and sometimes unacceptable for some surgeons remains in fact very inconspicuous. The anterior medial cervicoplasty is an easy technique that allows responding gracefully to a frequent request, with good cosmetic results and few complications., (Copyright © 2011 Elsevier Masson SAS. Freon gas is a halogenated derivative widely used in refrigeration and air conditioning. It is maintained at a temperature below -41°C and its contact with skin may cause very serious burns. This is usually an accident at work and the burns affect the hands of patients first. Unfortunately, early clinical presentation is often reassuring and does not reflect the actual depth of the injury. Few cases of this injury are reported and no treatment protocol is established at this time. We present two cases of frostbite by freon gas, initially evaluated at a stage of superficial burns and evolved spontaneously in a few days to full thickness burns necessitating surgical treatment by excision and skin grafting. This evolution in two phases has never been described and could help to better understand the pathophysiology of this frostbite and the possibilities of management., (Copyright © 2012 Elsevier Masson SAS. Objective: To present our learning curve in diagnostic and interventional sialendoscopy for obstructive salivary diseases., Materials and Methods: Monocentric descriptive retrospective study from March 2009 to July 2011. Clinical and demographic data were collected. We are particularly interested in arising technical issues, the use of combined approach, operative time, functional improvement as well as parameter changes over time., Results: 92 operations were performed to explore 101 glands (63 parotid glands against 38 submandibular). We found 39.6% of stones and as many stenosis. The rate of complete stone removal was 65% and dilation was effective in 75% of stenosis. The median of the visual analog scale for pain was 1/10 and functional improvement was effective in 77%. The removal of the gland did not exceed 3.3%. No major complication was noted. Since the initiation of this activity, the median operative time was steady while procedures were more complex, with increased interventional sialendoscopy procedure often requiring combined approach. In about 25% of cases, we have been faced with technical issues. These have evolved over time: initially failure to enter the papilla, difficulty of removing large stones today., Conclusion: The learning curve in sialendoscopy allows rapid empowerment and acquisition of expertise in security. Mastery of this technique allows for innovative approaches, complementary to conventional procedure, without compromising neither the operative time nor the functional benefit.

Benign tumors, in-situ carcinomas and small carcinomas of the anterior floor mouth or of the internal surface of the cheek may require resection of salivary duct papilla i.e. papilla of submandibular duct or Stensen's duct. If a lymph node dissection is not required, excision of submandibular or parotid duct which papilla is interested in the resection can be avoided using a surgical technique not reported. The authors present two cases illustrating a simple surgical proceeding of salivary duct resection/transposition. The present method can be used to keep a functional principal salivary gland with an optimal oncologic result.
