The treatment and management of heart failure (HF) are constantly evolving. The latest guidelines recommend the use of SGLT2 inhibitors (SGLT2i) as an integral part to treating HF with reduced ejection fraction (40%). However, given that the patients included in these trials do not reflect the heterogeneity of the health of many elderly patients, we recommend basing the therapeutic decision on the patient's state of frailty. If a SGLT2i treatment at a standard dose (10 mg 1x/day) is recommended for robust patients, we suggest initiating treatment at 5 mg 1x/day for vulnerable patients, and then after 1 month increasing the dose to 10 mg 1x/day. Finally, for dependent patients, we recommend therapeutic abstention in the absence of sufficient scientific evidence.La prise en charge de l’insuffisance cardiaque (IC) est en constante évolution. Les dernières recommandations préconisent l’utilisation des inhibiteurs du SGLT2 (iSGLT2) pour le traitement de l’IC à fraction d’éjection réduite (40%). Cependant, les populations des études ne reflètent pas l’hétérogénéité de la population âgée en termes de santé et nous proposons de baser la décision thérapeutique selon la Clinical Frailty Scale : si, pour les patients robustes, un traitement par iSGLT2 à dose standard (10 mg 1 x/jour) est préconisé, nous proposons, pour les patients vulnérables, d’initier le traitement à 5 mg 1 x/jour, puis d’augmenter à 10 mg 1 x/jour après 1 mois. Finalement, pour les patients dépendants, nous recommandons une abstention thérapeutique en l’absence d’évidences scientifiques suffisantes.

Diabetes and pancreatic cancer have an intricate relationship where each is a risk factor for developing the other. In case of type 2 diabetes, there is an increased probability of developing pancreatic cancer. Similarly, the onset of diabetes often precedes the diagnosis of pancreatic cancer. Since hyperglycemia is secondary to tumor involvement of the exocrine pancreas, diabetes is considered pancreatogenic. In the current classification, it is part of the entities belonging to type 3c diabetes. The pathophysiology is specific, characterized by a high glycemic variability and a tendency to weight loss. Early identification of inaugural type 3c diabetes would reduce diagnostic delays and could optimize oncologic management. In the absence of specific markers, the challenge for the clinician is indisputable.Diabète et cancer du pancréas entretiennent une relation intriquée où chacun constitue un risque de développer l’autre. En cas de diabète de type 2, le risque de cancer augmente. La survenue d’un diabète précède aussi souvent le diagnostic de cancer du pancréas. L’hyperglycémie étant secondaire à l’atteinte tumorale du pancréas exocrine, ce diabète est considéré comme pancréatogénique. Dans la dénomination actuelle, il fait partie des entités appartenant au diabète de type 3c. La pathophysiologie est spécifique, caractérisée par une forte variabilité glycémique et une tendance à la perte de poids. Distinguer précocement les diabètes inauguraux de type 3c permettrait de réduire les délais diagnostiques et d’optimiser la prise en charge oncologique. En l’absence de marqueurs spécifiques, le challenge est indiscutable pour le clinicien.

Gliflozins (sodium-glucose cotransporter type 2 inhibitors or SGLT2is) reduced hospitalisations for heart failure in all large prospective cardiovascular outcome trials performed in patients with type 2 diabetes at risk of cardiovascular disease. This protective effect was confirmed in two dedicated trials that specifically targeted patients with heart failure and reduced left ventricular ejection fraction (DAPA-HF with dapagliflozin and EMPEROR-reduced with empagliflozin) and in two trials in patients with preserved left ventricular ejection fraction (EMPEROR-preserved with empagliflozin and DELIVER with dapagliflozin), independently of the presence of diabetes. These favourable results contribute to give a privileged position to SGLT2is in recent international guidelines produced by diabetologists and cardiologists.Les gliflozines (inhibiteurs des cotransporteurs sodium-glucose de type 2 (iSGLT2)) réduisent les hospitalisations pour insuffisance cardiaque (IC) dans tous les grands essais prospectifs chez les patients avec diabète de type 2 à risque cardiovasculaire. Les effets ont été confirmés dans 2 essais ciblant spécifiquement les patients avec IC à fraction d’éjection réduite (DAPA-HF avec la dapagliflozine et EMPEROR-Reduced avec l’empagliflozine) et dans 2 essais chez des patients à fraction d’éjection préservée (EMPEROR-Preserved avec l’empagliflozine et DELIVER avec la dapagliflozine), qu’ils soient diabétiques ou non. Ces résultats favorables ont donné aux iSGLT2 une place privilégiée chez les patients avec IC dans les dernières recommandations internationales de diabétologie et de cardiologie.

Finerenone is a new mineralocorticoid receptor antagonist with a different structure, volume of distribution and half-life compared to spironolactone. This drug has been tested in two large, randomized trials including diabetic patients with chronic kidney disease (in terms of glomerular filtration rate and albuminuria) and already treated by renin-angiotensin system blockade. Results are positive on hard renal- and cardiac endpoints. Risk of hyperkalaemia is higher than with placebo but is considered as acceptable. An open question that will be tested in further studies is the role of finerenone in the context of a treatment by gliflozins, drugs that also showed cardiorenal protection.La finérénone est un antagoniste non stéroïdien du récepteur des minéralocorticoïdes avec une structure, un volume de distribution et une demi-vie différents de la spironolactone. Cette molécule a récemment été testée dans deux grands essais randomisés et contrôlés chez des patients avec une néphropathie diabétique avérée (en termes de débit de filtration glomérulaire et d’albuminurie) et un blocage optimal du système rénine-angiotensine-aldostérone (SRAA). Les résultats attestent d’une néphroprotection et d’une cardioprotection conférées par cette molécule, en addition aux bloqueurs du SRAA, sur des critères de jugement durs. Le risque d’hyperkaliémie était supérieur au placebo, mais acceptable. Une question ouverte reste celle de la place de cette molécule par rapport aux gliflozines, ayant aussi prouvé une protection cardiorénale.

Postprandial hyperglycaemia (PPH) may sometimes be relegated to the background in the treatment of diabetic patients, while its control seems important if not essential to reach an adequate overall glycaemic control. PPH is correlated with glycated haemoglobin and diabetic complications. It is also identified as a cardiovascular risk factor. PPH's monitoring and adequate control are not only a therapeutic goal for diabetes itself but also for reducing associated adverse outcomes other than diabetic complications. PPH is related to the quality of life of patients. Continuous glucose monitoring allows a better appraisal of PPH. The use of new insulin formulations as ultra-fast insulins seems to be the better way to manage post-prandial blood glucose peaks.L’hyperglycémie postprandiale (HGPP) peut parfois passer au second plan lors de la prise en charge globale du patient diabétique. Néanmoins, sa maîtrise est indispensable pour obtenir un équilibre glycémique global optimal. L’HGPP est corrélée à l’hémoglobine glyquée ainsi qu’aux complications du diabète. Elle semble également jouer un rôle par rapport au risque cardiovasculaire. Outre son intérêt dans la prise en charge thérapeutique du diabète proprement dit, l’HGPP doit être maîtrisée en raison de son lien avec d’autres conséquences médicales et de son impact négatif sur la qualité de vie des patients. Le monitoring continu du glucose permet une meilleure appréciation de l’HGPP. Les nouvelles insulines ultra-rapides paraissent être les molécules les plus adéquates pour réduire, au mieux et au plus vite, l’HGPP.

Diseases related to fatty liver, independently of alcohol consumption («non-alcoholic fatty liver disease» or NAFLD), are increasing because of the epidemics of obesity and type 2 diabetes. These disorders reflect a continuum that comprises isolated steatosis, steatohepatitis (NASH) and fibrosis, with, at the end, an increased risk of cirrhosis and hepatocarcinoma. It has been recently proposed to replace the term NAFLD by MAFLD, i.e. «Metabolic (dysfunction) Associated Fatty Liver Disease», which better reflects the pathogenesis of the disease. Inflammation plays a crucial role in the aggravation of the disorder and profoundly influences the prognostic evolution. This article illustrates the natural history of this underestimated metabolic disorder, recall the diagnostic criteria used in clinical practice, emphasizes the deleterious role of inflammation and discusses some therapeutic perspectives.: Les maladies liées à un «foie gras», indépendamment de la consommation d’alcool («non-alcoholic fatty liver disease» ou NAFLD), sont en augmentation en raison de l’«épidémie» d’obésité et de diabète de type 2. Il s’agit d’un continuum comprenant la stéatose isolée, la stéatohépatite (NASH) et la fibrose avec, in fine, un risque accru de cirrhose et d’hépatocarcinome. Il a été proposé récemment de remplacer le terme NAFLD par MAFLD pour «Metabolic (dysfunction) Associated Fatty Liver Disease», ce qui reflète mieux la pathogénie de la maladie. L’inflammation joue un rôle clé dans l’aggravation du trouble et conditionne l’évolution pronostique. Cet article retrace l’histoire naturelle de cette pathologie métabolique sous-estimée, rappelle les critères diagnostiques utilisés en clinique, précise le rôle délétère de l’inflammation et conclut par quelques perspectives thérapeutiques.

Type 2 diabetes is characterized by chronic hyperglycaemia in a context of insulin resistance and ?-cell dysfunction. A chronic low-grade inflammation is observed in obesity and has been associated with the development of metabolic disorders. The molecular mechanisms underlying this inflammation are not fully understood. Production of interleukin-1? by macrophages infiltrating insulin-sensitive tissues and pancreatic islets plays a major role in the pathogenesis of type 2 diabetes. This pro-inflammatory cytokine is produced through the activation of the NLRP3 inflammasome in response to danger signals that accumulate during obesity, including saturated fatty acids. The composition of the intestinal microbiota differs in obese subjects compared with lean individuals, particularly in response to high saturated fat diet. These modifications could trigger a chronic low-grade inflammation and promote the emergence of type 2 diabetes. The microbiota could therefore constitutes a therapeutic target in the prevention and management of metabolic abnormalities associated with obesity.: Le diabète de type 2 est caractérisé par une hyperglycémie chronique survenant dans un contexte de résistance à l’insuline et de déficit de sécrétion d’insuline par les cellules ? du pancréas. Un état inflammatoire chronique à bas bruit est observé dans l’obésité et est associé au développement d’anomalies métaboliques. Les mécanismes moléculaires à l’origine de cette inflammation ne sont pas encore bien compris. La production d’interleukine-1? par les macrophages infiltrant les tissus cibles de l’insuline et les îlots pancréatiques joue un rôle important dans la pathogénie du diabète de type 2. Cette cytokine pro-inflammatoire est produite via l’activation de l’inflammasome NLRP3 en réponse à divers signaux de danger s’accumulant avec l’obésité, dont les acides gras saturés. La composition du microbiote intestinal diffère chez les sujets obèses par rapport aux individus minces, notamment en réponse à une alimentation riche en graisses saturées. Ces modifications pourraient entraîner le déclenchement d’un état inflammatoire chronique et favoriser l’émergence d’un diabète de type 2. Le microbiote pourrait, dès lors, constituer une cible thérapeutique dans la prévention et la prise en charge des anomalies métaboliques associées à l’obésité.

Often underdiagnosed and sometimes falsely considered as a type 2 diabetes, type 3c diabetes (mostly known as pancreatogenic diabetes) is composed by a heterogenous group of illnesses characterized by diabetes mellitus associated with exocrine pancreatic insufficiency. The clinical, physiopathological and treatment features differ from type 2 diabetes, and they are important to keep in mind when diabetes is diagnosed, to avoid potential complications. The aim of this article is to review the principal etiologies of this often-unrecognized pathology.Souvent sous-diagnostiqué et parfois considéré à tort comme un diabète de type 2, le diabète de type 3c, plus connu sous le nom de diabète pancréatoprive, est composé d’un groupe hétérogène de maladies caractérisées par un diabète sucré lié à une insuffisance pancréatique exocrine. La présentation clinique, la physiopathologie et la prise en charge de ces pathologies diffèrent sensiblement du diabète de type 2, et il convient de s’en rappeler au moment du diagnostic d’un diabète sucré afin d’éviter de potentielles complications. Le but de cet article est de faire le point sur les principales étiologies de cette pathologie peu connue.

Treatment combining long-acting and short-acting insulins is essential for people with type 1 diabetes, but may become also compulsory in other forms of diabetes in case of insulinopenia. The purpose of short-acting insulins is to mimic physiological insulin secretion in response to carbohydrate intake at meals. There is a delay between the injection and its action, sometimes limiting their use and effectiveness. Ultra-rapid insulins have been developed to more closely approximate the expected insulin response to a meal, through faster absorption. They do not improve diabetes control but allow more flexibility with mealtime injections. These new analogues are also an attractive alternative for use in insulin pumps.Un traitement combinant insulines lente et rapide est essentiel pour les personnes avec un diabète de type 1, mais peut le devenir dans d’autres formes de diabète en cas d’insulinopénie. Le but des insulines rapides est de mimer la sécrétion physiologique d’insuline en réponse à la prise de glucides aux repas. Il y a un délai entre l’injection et son action, limitant parfois leur usage et leur efficacité. Des insulines ultrarapides ont été développées pour se rapprocher davantage de la réponse insulinique attendue à un repas, grâce à une absorption plus rapide. Elles n’améliorent pas le contrôle du diabète mais permettent plus de flexibilité avec les injections aux repas. Ces nouveaux analogues sont également une alternative intéressante pour une utilisation dans les pompes à insuline.

Since their launch, sodium-glucose cotransporter type 2 inhibitors (SGLT2is) were suspected to be associated with various adverse events. They contributed to delay, as in France, or to restrict the use of this new pharmacological class in clinical practice, despite remarkable results reported in large cardiovascular or renal clinical trials. This article is devoted to three major adverse events that were imputed to SGLT2is : lower-limb extremity amputations, euglycaemic ketoacidosis and acute kidney injuries. In contrast to pharmacovigilance reports that raised suspicion, analysis of all data from the literature, either placebo-controlled trials or retrospective observational cohort studies, led to rather reassuring conclusions. The incidence of amputations does not appear to be increased while cases of acute kidney injury are reduced instead of increased as suspected earlier. Ketoacidosis events are almost doubled with SGLT2is versus comparators, yet their incidence remains extremely low among patients with type 2 diabetes. Of note, this potentially severe complication contributes to the denial of marketing authorization and reimbursement of SGLT2is in the population with type 1 diabetes.Depuis leur mise sur le marché, les inhibiteurs des cotransporteurs sodium-glucose de type 2 (iSGLT2) ont été incriminés dans diverses manifestations indésirables. Celles-ci ont contribué à retarder, comme en France, ou à limiter la prescription de cette nouvelle classe pharmacologique en pratique clinique, malgré les résultats remarquables rapportés dans de grands essais à visée cardiovasculaire ou rénale. Cet article fait le point sur trois effets secondaires délétères importants imputés aux iSGLT2 : les amputations des extrémités des membres inférieurs, les acidocétoses dites euglycémiques et les insuffisances rénales aiguës. Malgré des données de pharmacovigilance qui avaient soulevé la suspicion, l’analyse de l’ensemble des données de la littérature, que ce soit les essais prospectifs contrôlés versus placebo ou les études observationnelles rétrospectives de cohorte versus des comparateurs actifs, aboutit à des conclusions assez rassurantes. Les amputations ne semblent pas être augmentées tandis que les cas d’insuffisance rénale aiguë sont plutôt en diminution au lieu de présenter une incidence accrue. Les cas d’acidocétose sont environ doublés sous iSGLT2 par rapport aux comparateurs, mais leur incidence reste extrêmement basse chez les patients diabétiques de type 2. Rappelons, néanmoins, que c’est cette complication potentiellement grave qui a entraîné le refus d’autorisation de mise sur le marché et du remboursement des iSGLT2 dans la population diabétique de type 1.

The relative risk of developing MetS is higher in patients with severe mental illness (SMI) than in the general population. Similarly, the risk of developing obesity or type 2 diabetes (T2DM) is also higher in patients with SMI. GLP-1 receptor agonists, such as liraglutide, have been shown to be effective in the treatment of T2DM and, more recently, in obesity or overweight associated with at least one metabolic disease. Their psychiatric adverse effect profiles seem to be reassuring, thus not represent a limitation for prescribing in psy chiatry. We aimed to explore the therapeutic usefulness of liraglutide in patients with psychiatric disorders associated with somatic comorbidities such as obesity, T2DM or MetS.Le risque relatif de développer un syndrome métabolique (SMet) est plus élevé chez les patients connus pour une maladie psychiatrique sévère (MPS) que dans la population générale. De même, le risque de développer une obésité ou un diabète de type 2 (DT2) est également plus important chez les patients souffrant de MPS. Les analogues du GLP-1 (Glucagon-Like Peptide 1), tels que le liraglutide, ont fait leurs preuves pour le traitement du DT2 et, plus récemment, de l’obésité ou de la surcharge pondérale associée à une maladie métabolique. Leurs profils d’effets indésirables sur la santé mentale semblent rassurants, ne représentant ainsi pas de limitation à leur prescription en psychiatrie. Nous questionnons ici l’intérêt du liraglutide chez les patients souffrant de troubles psychiques associés à des comorbidités somatiques telles que l’obésité, le DT2 ou le SMet.

We aimed at assessing the association between demographical and clinical data and the intrahospital mortality in older patients with COVID-19 in Belgium.Descriptive, retrospective study of consecutive patients admitted to Brugmann university hospital, Brussels (Belgium) due to COVID-19 (Mars-September-2020).Patients aged ≥ 70 years admitted to acute care with a positive PCR-RT test, or a highly indicative computed tomography scan.Patients transferred to another institution during hospitalization.All-cause intrahospital mortality. Demographic, clinical data, presence of comordibidties and comprehensive geriatric assessment were collected. Adjusted and unadjusted logistic regression were performed.From the 226 eligible patients, 160 (82.7 ± 6.5-year-old; 57.5 % females) met inclusion criteria, from which 67 (42 %) died during hospital stay. The adjusted logistic regression showed an association between intrahospital mortality and increasing age [OR = 1.09 per every year increase (95 % CI 1.02-1.16); p0.001], type 2 diabetes [OR = 2.75 ( 1.17-6.46); p = 0.021], and acute respiratory distress syndrome (ARDS) [OR = 8.67 ( 3.48-21.61); p0.01].A higher positive association between intrahospital mortality and increasing age, type 2 diabetes, and ARDS was found. The prognosis value of the comprehensive geriatric assessment in older people with COVID-19 in Belgium requires further studies.Les études sur l’impact de la pandémie en Belgique sont rares. L’objectif est d’évaluer l’association de l’âge et des comorbidités à la mortalité intra-hospitalière de toutes causes chez les patients âgés ? 70 ans avec COVID-19 dans un hôpital universitaire à Bruxelles, Belgique. Méthodes : Etude rétrospective descriptive des patients admis au CHU Brugmann pour cause de maladie COVID-19 (mars-septembre 2020). Critères d’inclusion : âge ≥ 70 ans admis avec PCR-RT positif ou haute probabilité d’infection au CT-scan thoracique. Critères d’exclusion : transfert dans un autre hôpital. Critère de jugement : toute cause de mortalité intra-hospitalière. Variables collectées : démographiques, cliniques et gériatriques [Katz, Lawton, MMSE, MNA, MNA-SF]. Une régression logistique non ajustée et ajustée a été réalisée. Résultats : Parmi les 226 patients éligibles, 160 (82,7 ± 6,5 ans; 57,5 % femmes) ont rempli les critères d’inclusion. Au total, 67 (42 %) sont décédés durant l’hospitalisation. La régression logistique a montré une association augmentée entre la mortalité et l’âge [odds ratio ou OR = 1,09/année en plus (IC 95 % 1,02-1,16) ; p0,001], le diabète de type 2 [OR = 2,75 (1,17-6,46); p = 0,021], l’ARDS [OR = 8,67 (3,48-21,61); p0,01].L’âge, le diabète de type 2 et le syndrome de détresse respiratoire aiguë (ARDS) sont associés à une augmentation de la mortalité chez les patients âgés hospitalisés avec COVID-19 en Belgique.

Most physicians do not know, or do not remember, the name of phlorizin. Hence this molecule has a major historical importance because it was the precursor of gliflozins, a new class of oral antidiabetic drugs with recent therapeutic perspectives beyond diabetes. This article recalls the history of phlorizin: its discovery in the 19th century by De Koninck and Stas, the demonstration of its ability to induce glucosuria and reduce hyperglycaemia by von Mering, its use to demonstrate the concept of glucose toxicity by the team of DeFronzo and finally the development of selective (phlorizin being not selective) sodium-glucose cotransporter type 2 inhibitors (gliflozins) which block glucose reabsorption in renal tubules. Gliflozins have increasing therapeutic indications, not only in type 2 diabetes, but also in cardiology and nephrology among non-diabetic people with heart failure or renal insufficiency.La plupart des médecins ne connaissent pas, ou ne se souviennent plus, de la phlorizine. Pourtant, cette molécule a une grande importance historique car elle a été le précurseur des gliflozines, une nouvelle classe d’antidiabétiques oraux ouvrant maintenant de nouvelles perspectives thérapeutiques au-delà du diabète. Cet article retrace l’histoire de la phlorizine : sa découverte au 19ème siècle par De Koninck et Stas, la démonstration de l’induction d’une glucosurie abaissant la glycémie par von Mering, son utilisation pour conceptualiser la notion de glucotoxicité par l’équipe de DeFronzo et, enfin, le développement d’inhibiteurs sélectifs (la phlorizine étant non sélective) des cotransporteurs sodium-glucose de type 2 (SGLT2, gliflozines),dans les tubules rénaux, bloquant la réabsorption du glucose. Les gliflozines ont, maintenant, des indications thérapeutiques de plus en plus larges, non seulement dans le diabète de type 2, mais aussi en cardiologie et en néphrologie chez des personnes non diabétiques avec insuffisance cardiaque ou insuffisance rénale.

Diabetes is a rapidly evolving discipline ; numerous new molecules are available and recommendations regarding treatment change. However, these rapid changes are sometimes difficult to follow for general practitioners. Metformin remains the cornerstone of type 2 diabetes treatment after lifestyle modifications, which should always be encouraged before medications. Currently, the best classes to add after metformin are SGLT2 inhibitors and GLP-1 receptor agonists, as these molecules showed some cardiovascular and renal beneficial effects in dedicated studies. The aim of this article is to guide the general practitioner in choosing the most suitable pharmacological treatment for each patient, in light of the novelties in the field of diabetes that appeared during the year 2021.La diabétologie est une discipline qui évolue rapidement, de nombreuses molécules apparaissent sur le marché et les recommandations de traitement changent. Il est donc souvent difficile pour le médecin de premier recours d’être à jour. Après les mesures du style de vie qu’il faut bien sûr privilégier avant toute approche médicamenteuse, la metformine reste le pilier pharmacologique du traitement. Les recommandations actuelles préconisent d’instaurer un inhibiteur du SGLT2 ou un agoniste du récepteur du GLP-1 après la metformine car ces molécules ont montré des effets bénéfiques cardiovasculaires et rénaux dans des études. Le but de ce bref article est d’aider le médecin de premier recours à choisir le traitement pharmacologique le plus adapté à chaque patient·e, à la lumière des nouveautés 2021 dans le domaine du diabète.

Both renin-angiotensin-aldosterone system inhibitors (RAASi) and sodium-glucose cotransporter type 2 inhibitors (SGLT2i, gliflozins) reduce the risk of heart failure and of progressing towards end-stage renal disease, especially in patients with type 2 diabetes (T2D). Positive results reported in patients with T2D have been confirmed in people without diabetes. These two pharmacological classes now occupy a privileged place in international guidelines, in diabetology, cardiology and nephrology. The present article describes similarities and differences between these two types of medications. It emphasizes the importance of combining both approaches in order to optimize the cardiovascular and renal prognosis, while maintaining a good safety profile.Les inhibiteurs du système rénine-angiotensine-aldostérone (iSRAA) et les inhibiteurs des cotransporteurs sodium-glucose de type 2 (iSGLT2, gliflozines) réduisent le risque d’insuffisance cardiaque et de progression vers l’insuffisance rénale terminale, notamment chez les patients avec un diabète de type 2 (DT2). Les effets positifs rapportés chez les patients DT2 ont été confirmés chez les personnes non diabétiques. Ces deux classes pharmacologiques occupent maintenant une place de choix dans les recommandations internationales, en diabétologie, en cardiologie et en néphrologie. Cet article fait le point sur les similitudes et les différences entre ces deux familles médicamenteuses. Oral semaglutide (Rybelsus®) is a co-formulation of semaglutide, a glucagon-like peptide-1 (GLP-1 RA) receptor agonist, with an absorption enhancer, sodium N- (8- [2- hydroxybenzoyl] amino) caprylate (SNAC), which facilitates the absorption of semaglutide across the gastric epithelium in a concentration dependent manner. The safety and efficacy of oral semaglutide were assessed in the PIONEER clinical trial programme, which included 9543 patients with type 2 diabetes (T2DM). Across a range of different T2DM patients receiving different background medications, oral semaglutide provides more effective glycaemic control than common oral glucose-lowering therapies, associated with a clinically relevant reduction in body weight, including in patients with more advanced T2DM on insulin treatment. The tolerability profile for oral semaglutide was consistent with the other GLP-1 RAs. Cardiovascular (CV) safety of oral semaglutide was noninferior to placebo in CV high-risk patients. For a better biological management of diabetes for patients with hemoglobinopathy. Glycated hemoglobin, HbA1c, as a cumulative retrospective marker of glycemic control in diabetic patients could be faulty in patients with hemoglobinopathy. Abnormal hemoglobin could introduce bias analytically during HbA1c measure but also due to its physiopathology by altering the lifespan of red blood cells and/or the normal glycation process. The aim of this communication is to raise awareness of physicians on some important elements for an appropriate interpretation of HbA1c levels so that these patients are better diagnosed and balanced. This review enlightens the practitioner on the HbA1c assay methods (separatives) to prioritize. It provides him critical situations where inconsistent HbA1c results should lead him to suspect hemoglobinopathy, what to do in terms of additional investigations and follow-up. It also offers a pragmatic solution for relevant personalized follow-up. Clinico-biological collaboration will help determine the HbA1c target to be reached.Quelle prise en charge biologique du diabète pour les patients ayant une hémoglobinose ? L’hémoglobine glyquée (HbA1c) en tant que marqueur rétrospectif cumulatif de l’équilibre glycémique chez le patient diabétique est mise en défaut chez ceux ayant une hémoglobinose. Cette hémoglobine anormale entraîne des biais analytiques à cause des méthodes de dosage mais également par sa physiopathologie en perturbant la durée de vie des hématies et le processus normal de glycation. Cette mise au point a pour objectif de sensibiliser les médecins sur une interprétation adéquate des taux d’HbA1c afin que ces patients soient mieux diagnostiqués et équilibrés, et de les éclairer sur les méthodes de dosage d’HbA1c à privilégier (séparatives). Elle attire leur attention sur des situations où des résultats incohérents d’HbA1c doivent faire suspecter une hémoglobinose et la conduite à tenir en termes d’explorations complémentaires et de suivi. Elle propose également une solution pragmatique pour un suivi pertinent personnalisé. Une collaboration clinico-biologique permettra de déterminer la cible d’HbA1c à atteindre.

The relationships between sodium-glucose cotransporter type 2 inhibitors (SGLT2is) and kidney are complex: reduction of the glucose-lowering effect with the decline of glomerular filtration rate (GFR), biphasic effects on GFR along with treatment duration, low risk of acute kidney injury, rapid reduction of albuminuria, and, most importantly in clinical practice, a remarkable nephroprotection, with a decrease of the risk of end-stage renal disease or death from renal cause. This article describes the renal outcomes in the main clinical trials and analyses the impact of treatment with SGLT2is on renal prognosis according to different patient baseline characteristics. A nephroprotection is observed whatever the severity (even if GFR45 ml/min/1,73 m²) and type (with or without albuminuria, with or without diabetes) of renal disease.Les relations entre inhibiteurs des cotransporteurs sodium-glucose de type 2 (iSGLT2) et rein sont complexes: réduction de l’effet antihyperglycémiant avec la diminution du débit de filtration glomérulaire (DFG), effets biphasiques sur le DFG dans le décours du traitement, risque faible d’insuffisance rénale aiguë, diminution rapide de l’albuminurie et, surtout, remarquable néphroprotection à terme, avec une diminution du risque d’insuffisance rénale terminale ou des décès d’origine rénale. Cet article décrit les résultats rénaux dans les grands essais cliniques et analyse l’impact du traitement par iSGLT2 en fonction des caractéristiques des patients à l’inclusion. Une néphroprotection est observée quels que soient la sévérité (DFG45 ml/min/1,73 m²) et le type (albuminurique ou non, diabétique ou non) de la maladie rénale.

People with diabetes are considered to have an increased cardiovascular risk. Patients with type 1 diabetes (T1D) generally have a cardiovascular risk profile that is different from those with type 2 diabetes. For this reason, we wanted to assess whether a population of T1D designed to be at very high cardiovascular risk achieved the strict goals recommended by the European Society of Cardiology. This is a descriptive cross-sectional analysis of a cohort of patients with T1D for at least 20 years followed at the University Hospital of Liege and considered to be at very high cardiovascular risk. We then discuss the relevance of strict targets in such patients by comparing them to different scientific societies. Finally, we briefly discuss the potential mechanisms by which T1D present an increased cardiovascular risk.Les personnes diabétiques sont considérées comme ayant un risque cardiovasculaire accru. Les patients diabétiques de type 1 (DT1) ont un profil de risque cardiovasculaire souvent différent de celui des diabétiques de type 2. Nous avons évalué si une population de patients DT1, dits « à très haut risque cardiovasculaire », atteignait les objectifs stricts recommandés par la Société européenne de cardiologie. Il s’agit d’une analyse transversale descriptive d’une cohorte de patients avec au moins 20 ans de DT1, suivis au CHU de Liège et considérés comme à très haut risque cardiovasculaire. Nous discutons de la pertinence de tels objectifs chez de tels patients, en les comparant à ceux de différentes sociétés savantes. Nous abordons brièvement les mécanismes potentiels à l’origine, dans ce groupe, d’un risque cardiovasculaire accru.

Sacubitril/valsartan, a first-in-class angiotensin receptor-neprilysin inhibitor (ARNI), is indicated for the treatment of heart failure with reduced ejection fraction. It has demonstrated benefits in terms of cardiovascular morbidity and mortality reduction in this population. Recently, this drug association has also been shown to improve glycemic control and insulin sensitivity in patients with obesity and/or type 2 diabetes. Furthermore, some studies suggest a protective role of this new drug class in diabetic nephropathy. Glucagon-like peptide-1 receptor agonists are the antidiabetic agents that are associated with the greatest weight loss beyond a marked reduction in glycated haemoglobin. Weight loss results from a reduction in appetite through a predominant central effect combined with a peripheral effect. Liraglutide and semaglutide are developed for the treatment of obesity, independently of type 2 diabetes. Three approaches may be considered to potentiate weight loss: an increase of the drug dosage because of the demonstration of a dose-response, an add-on therapy with a sodium-glucose cotransporter type 2 inhibitor as this agent exerts a complementary action through urinary calorie loss (glucosuria) or a combination of the effects of two incretin hormones (GLP-1 and GIP), as the potent dual agonist tirzepatide currently in development.Les agonistes des récepteurs du glucagon-like peptide-1 (GLP-1) sont les agents antidiabétiques qui, outre une baisse importante du taux d’hémoglobine glyquée, offrent la perte pondérale la plus marquée, en augmentant la satiété par un effet central, prédominant et périphérique. Masked arterial hypertension (MHTN) is a recently described entity that is associated with the same cardiovascular risk as permanent hypertension. Its prevalence is more frequent in patients with diabetes. The objective of this study is to assess the value of systematic screening for MHTN by 24-hour blood pressure monitoring in a population of type 2 diabetic patients by estimating its prevalence and looking for predictive factors of MHTN in this population.Through a prospective study, we recruited normotensive type 2 diabetics for clinical measurement, in whom we systematically searched for MHTN by performing an ambulatory blood pressure measurement (ABPM). The diagnosis of MHTN is established if: mean daytime BP ≥ 135/85 mmHg and / or, mean nighttime BP ≥ 120/70 mmHg and / or, mean 24 hour BP ≥ 130/80 mmHg. We then compared the two populations of MHTN (G1) and normotensive (G2) on clinical and laboratory parameters and we assessed end-organ damage in order to identify the predictive factors of MHTN.We recruited 53 patients whose mean age was 55.3 ± 8.4 years (range 35-72 years) with a female predominance (53%). The duration of diabetes was on average 8.7 ± 3.9 years with extremes between 2 and 17 years. The average BMI of our patients was 28.2 ± 5.3 Kg/m2. Overweight was found in almost half of our patients (47.2%). Obesity was found in 32.1% of cases. Metabolic syndrome was found in 64.2% of patients. In our study, the prevalence of HTAM in type 2 diabetics was 64%. We also found that MHTN was more often nocturnal (58.5%) and occurred mainly in non-dipper patients. Left ventricular hypertrophy, microalbuminuria and arterial stiffness evidenced by pulse pressure greater than 60mmHg were more common in the MHTN group. For the predictive factors of MHTN, we were able to collect in univariate analysis the following factors: duration of diabetes, fasting blood sugar, weight and microalbuminuria. In multivariate analysis, the predictive factors that emerged in our study are poor glycemic control (HbA1c ≥7%), high BMI and duration of diabetes.MHTN should be sought in diabetics because it allows a better assessment of the cardiovascular risk, in particular by identifying end-organ damage.

SGLT2 inhibitors are more and more prescribed in the treatment of type 2 diabetes. Their cardioprotective and nephroprotective effects make them particularly attractive. However, it is important to be aware of their potential side effects. This article aims to summarize the actual evidence regarding the most studied ones. Euglycemic ketoacidosis, genital infections and hypovolemia have been well described. Evidence is less robust regarding the increased risk of amputations, fractures, urinary tract infection or acute kidney disease. This article also aims to summarize some key messages to discuss with patients, in order to prevent the occurrence of euglycemic ketoacidosis and hypovolemia.Les iSGLT2 (inhibiteurs du sodium-glucose de type 2) sont prescrits de plus en plus précocement chez nos patients diabétiques de type 2. Leur cardio et néphroprotection ne font plus de doute, les rendant particulièrement attractifs. Cependant, ces traitements ne sont pas dénués de risque. Cet article propose une mise au point sur l’évidence actuelle concernant les effets indésirables les plus étudiés. L’augmentation du risque d’acidocétose euglycémique, d’infections génitales ou encore d’hypovolémie a été bien démontrée. La littérature est plus hétérogène concernant l’augmentation du risque d’amputations, de fractures, d’infections du tractus urinaire ou encore d’insuffisance rénale aiguë. Cet article reprend également les messages clés à transmettre aux patients afin de prévenir la survenue d’une acidocétose euglycémique ou d’un état hypovolémique.

New systemic cancer therapies are increasingly oriented towards specific signaling pathways involved in carcinogenesis. However, these new treatments may lead to disorders of glycemic homeostasis ranging from glucose intolerance, diabetes or the occurrence of severe acute hyperglycemic syndrome due to blockade of certain pathways common to glucose metabolism. This article discusses the estimated frequency of new-onset diabetes, the pathophysiological mechanisms as well as the diagnostic, therapeutic, monitoring and prognostic management of glycemic dysfunction in patients treated with these novel systemic cancer therapies.Les nouvelles thérapies du cancer sont de plus en plus orientées contre des voies de signalisation spécifiques à la carcinogenèse. Cependant, ces nouveaux traitements peuvent mener à des troubles de l’homéostasie glucidique, allant d’une intolérance au glucose au diabète insulinorequérant, avec une potentielle décompensation aiguë, en raison du blocage de certaines voies communes à l’homéostasie glucidique. Cet article discute de la fréquence estimée de la survenue du diabète, des mécanismes physiopathologiques ainsi que de la prise en charge diagnostique et thérapeutique, de la surveillance et du pronostic de la dysfonction glycémique chez les patients traités par ces nouvelles thérapies contre le cancer.

Insulin therapy, often initiated after hygiene and dietary measures and non-insulin antidiabetics, is part of the treatment of patients with type 2 diabetes. Fear of injections or hypoglycemia often delays its implementation. However, its introduction is recommended in cases of poorly balanced diabetes despite a well-controlled therapeutic escalation but also in cases of acute imbalance. Introduction of insulin therapy requires patient education and close monitoring by the healthcare team. Type of insulin and its titration reduce the incidence of hypoglycemia in patients at risk. The determination of the fasting glycemic target - relative to HbA1c - for the titration of insulin is important to define for an optimal benefit (prevention of secondary complications)/risk (hypoglycemia, weight gain) balance.L’insulinothérapie, souvent mise en place après les mesures hygiéno-diététiques et les antidiabétiques non insuliniques, fait partie du traitement des patients diabétiques de type 2. La peur des injections ou des hypoglycémies retarde souvent sa mise en place. Cependant, son introduction est recommandée en cas de diabète mal équilibré malgré une escalade thérapeutique bien conduite mais aussi en cas de déséquilibre aigu. L’insulinothérapie implique un enseignement au patient et un suivi rapproché par l’équipe soignante. Le type d’insuline et sa titration progressive permettent de réduire l’incidence des hypoglycémies chez les patients à risque. La détermination de la cible glycémique pour la titration de l’insuline est importante à définir pour une balance bénéfice (prévention des complications secondaires)/risque (hypoglycémie, prise pondérale) optimale.

MODY diabetes, for Maturity Onset Diabetes of the Young, is a form of monogenic diabetes characterized by a typical onset before the age of 25 years, the lack of autoimmunity against the b cells of the pancreas, a preserved β cells function and an autosomal dominant mode of inheritance. This type of diabetes constitutes 2 to 5% of all cases of diabetes but remains often undiagnosed. Nearly 15 MODY subtypes have been identified to date. The 3 most common subtypes are caused by mutations in the genes encoding glucokinase, HNF1a and HNF4a, and account for approximately 80% of all MODY cases. Carrying out a genetic test can thus make it possible to make the diagnosis of MODY diabetes and to set up an appropriate treatment. The transition from hospital to ambulatory care is a high-risk period for patients with diabetes mellitus and is a challenge for health care professionals. Various interprofessional collaborative interventions have shown a positive impact on continuity of care at discharge. Communication and transmission of information between the hospital and ambulatory settings as well as coordination between healthcare professionals are key points to explore and to improve to ensure optimal continuity of care.La transition entre l’hôpital et l’ambulatoire est une période à risque pour les patients avec un diabète sucré et un enjeu pour les professionnels de la santé. Différentes interventions en collaboration interprofessionnelle ont montré un impact positif sur la continuité des soins à la sortie de l’hôpital. La communication et la transmission d’informations entre les milieux hospitalier et ambulatoire ainsi qu’une coordination entre les professionnels de la santé sont des points clés à explorer et à continuer d’améliorer pour garantir une continuité des soins optimale.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors are a class of drugs which offer cardiovascular (CV) and renal benefits. They are currently indicated as first-line treatments of type 2 diabetes mellitus (T2DM) in patients with CV disease, high CV risk, renal disease, or heart failure with reduced ejection fraction (HFrEF). Two randomized clinical trials have shown the benefits of dapagliflozin and empagliflozin in patients with HFrEF, regardless of the presence of T2DM. Despite an overall favorable safety profile, attention has to be paid to adverse events, such as an increased risk of euglycemic diabetic ketoacidosis and genital mycotic infections. We present an up-to-date narrative literature review of the physiological mechanisms of action, current indications, and side effects of SGLT2 inhibitors.Les inhibiteurs du cotransporteur sodium-glucose de type 2 (iSGLT2) sont une classe d’antidiabétiques oraux ayant de nombreux bénéfices cardiovasculaires (CV) et rénaux. Ils sont indiqués chez les patients avec un diabète de type 2 (DT2) et une maladie CV, un risque CV élevé, une insuffisance rénale chronique ou une insuffisance cardiaque à fraction d’éjection réduite (ICFER). Des essais cliniques randomisés ont montré les bénéfices de la dapagliflozine et de l’empagliflozine chez les patients avec une ICFER, avec ou sans DT2. Malgré un profil de sécurité favorable, il convient de connaître les effets indésirables éventuels, tels que l’acidocétose euglycémique et les infections génito-urinaires. Nous présentons une revue narrative de la littérature à jour portant sur les mécanismes d’action, indications et effets secondaires des iSGLT2.

To determine the hospital frequency of diabetic patients with a rapid decline in their renal function, to look for the associated factors.Descriptive and analytical cross-sectional study carried out over 12 months (May 1, 2019 to April 31, 2020). Were included all patients aged 18 and over, having achieved at least 3 creatinine during the previous 2 years but spaced at least 6 months apart and having an eGFR calculated from their last creatinine greater than 30 mL/min by the formula of CDK-EPI. We evaluated the eGFR by the CDK-EPI formula using the calculator developed by the Poitiers University Hospital and the Inserm unit of the Francophone Diabetes Society.A total of 80 patients medical files were retained. The rapid decline in renal function was found in 28 patients, either a frequency of 35%. The sex ratio M/F was 1.5. The mean age was 62.93 years (range 18-85 years). Hypertension was the most common comorbidity (92.5%). The very high cardiovascular risk was predominant in 82.5% of cases. The very high renal risk was found in 20 patients, either 25%. Univariate and multivariate analysis showed that the rapid decline in renal function was associated with very high cardiovascular risk (P=0.037) and glomerular filtration rate (P˂0.001).this study showed a high hospital frequency of the rapid decline in renal function in Togo (35%). Our results have identified the very high cardiovascular risk and glomerular filtration rate as risk factors. The originality of our study was the demonstration of the high proportion of very high cardiovascular risk (82.5%) and very high renal risk (25%) in the evaluation of renal and cardiovascular risk.
