2. The Perennial Wisdom of St. Thomas Aquinas and the Great Books Tradition

In this article I argue for the pedagogical complementarity of the perennial wisdom of St. Thomas and Mortimer Adler’s dialectical method of the Great Books, where the Great Books highlight the ministerial function of the imagination to the will and intellect in the order of learning. Characterized by communal inquiry, the thought of St. Thomas and the Great Books are shown to be well matched instruments of the special Providence by which we direct one another to our proper end. A review of key Thomistic dispositions of teaching and learning, the nature of authentic conversation, and various objections and replies to the Great Books method of education and its alliance with the thought of St. Thomas focus the analysis. Several points of contact enrich the task of liberal learning. The Great Books are seen to supply students of St. Thomas with the spontaneous play of associations, rapprochements and comparisons as they strive to apply texts to the practices of virtue and truth seeking, while the perennial wisdom directs the students’ personal exegesis through the rigor of philosophical principles, logic, and distinctions.

The role of divine power in Aquinas’s spiritual doctrine has often been neglected in favor of a focus on the primacy of charity, the controlling virtue of spiritual progress. The tendency among some thinkers (e.g. Polkinghorne) to juxtapose divine love and power stems from the stress on divine immanence at the cost of divine transcendence, and from an evolutionary (vs. classical) view of God with its ‘kenotic’ theodicy. A study of the ways in which divine power grounds and directs the spiritual life highlights the robust role that metaphysics plays in spiritual ascent for Aquinas, and offers a philosophical entry point to his doctrine. Themes in his doctrine of the spiritual life incorporate Platonic transcendent causal plenitude and Aristotelian causal axioms and motifs of growth and unity. From the side of theology, divine power is analyzed through several lenses, including power through weakness in Christ, the sin of Lucifer against the gift of being in contrast to the counsel of obedience, and the role of Christ’s human nature in the Church. Taken together, these themes combine to characterize divine power as redemptive medicine, as opposed to a distant, arbitrary force, and to reveal the ways in which Aquinas applies metaphysical insights to the supernatural order.

5. Modernism and the Growing Catholic Identity Problem: Thomistic Reflections and Solutions

Philosophical forces gathered in late nineteenth and early twentieth century Catholic Modernism have crystallized into theological views which permeate the antinomian atmosphere in the Church today, resulting in an ongoing Catholic identity problem, both within the Church and in relation to the world. In place of the perennial philosophy and its contemplative ideal, many now welcome the incoherence of broad philosophical and theological pluralism, while pastoral practice is infused with the fruits of pragmatism and the rhetoric of false dichotomies (justice/mercy, intellectual/pastoral, tradition/living faith, speculative truth/charity, for example). To reverse this anti-intellectual course, rehabilitation of Aquinas’s positions on the primacy of the speculative order and contemplative charism, his integration of natural, revealed and mystical wisdoms, and his sense of objective worship, is needed. A brief account of the robust role of philosophy in the Church’s mission and of Gilson’s nuanced position on the encounter of Thomism and Modernism supports this assertion.

Space and spatiality play a pivotal role in Michel Foucault's thinking that is also marked by a continuous ambivalence towards psychoanalysis. Both topics have their roots in his earliest and still not very well known writings. By examining those early texts aimed at the nascent structural psychoanalysis of Jacques Lacan, the author demonstrates that Foucault's philosophical preference for space is deeply entangled with his ambiguous relationship to psychoanalysis. Going beyond such reevaluation of Foucault's early works, the paper finally shows how this positioning influenced the later developments of his philosophy.

9. Migration intra-vésicale d'une bandelette sous urétrale type TVT: à propos d'un cas et revue de la littérature

Les techniques de soutènement sous urétral par bandelette synthétique constituent le gold standard actuel pour la prise en charge de l'incontinence urinaire d'effort. Nous rapportant le cas d'une patiente de 60 ans, ayant comme antécédents une incontinence urinaire d'effort traitée par une bandelette sous urétrale type TVT (Tension free Vaginal Tape) en 2008. La cystoscopie per opératoire était normale. Six ans plus tard la patiente a présenté des infections urinaires à répétition, à Escherichia Coli, rebelles à toute antibiothérapie adaptée. Une échographie a été réalisée ayant objectivé un calcul intra vésical faisant 2.5cm de grand axe, par ailleurs le résidu post mictionnel était nul. La fibroscopie a objectivé une bandelette en intra-vésical avec une grosse calcification autour. Notre patiente a bénéficié d'un traitement conservateur qui consisté à une section endoscopique de la portion intra vésicale de la bandelette avec préservation de la portion sous urétrale, par ailleurs le calcul a été fragmenté par le lithoclaste. Sur le plan fonctionnel, notre patiente est toujours continente.

With the publication of this special issue, Le foucaldien continues its experiment of updating the thought of Michel Foucault. Can historical discourse analyses be carried out with the aid of computers? In order to examine this question, we compare Franco Moretti's Distant Reading with Foucault's archaeological method. Despite their common origins in the French Annales School, the two approaches differ fundamentally. While Moretti interprets literary data by means of social history, Foucault seeks the immanent meaning of discourses. Our preliminary conclusion: digital archaeology appears to founder on the operationalization of the complex concept of the statement (énoncé).

This paper evaluates Franco Moretti's 'distant reading' from the perspective of Foucauldian discourse analysis and basic principles of computer science. After a short account of the essential features of both Moretti's and Foucault's approaches, the author assesses their combined potential for automation or algorithmization. This is accomplished by focusing on the decompositional and operational aspects of Foucauldian archeology, which seems to provide (at least in principle) a way to realize or implement Moretti's idea of an automated distant reading. The technical feasibility of this theoretical solution is eventually criticized in the light of the constraints and limitations of computation compared to 'human interfaces'.

This special issue collects the papers and talks delivered at the international conference "Was heißt: Foucault historisieren?" (What is: Historicizing Foucault?) at the University of Zurich on March 19–21, 2015. The collection attempts both to historicize Foucault and to historicize Foucault. That is, the focus not only lies on the historical contexts of Foucault's thought (e.g., cold war, cybernetics, Maoism, structuralism), but also on the question if we should start to forget Foucault (as Baudrillard already suggested in a polemic from 1977). Do Foucault's concepts, thirty years after his death, still enable us to understand our contemporary world? Or do we have to adjust, to update the Foucauldian toolbox?

Nietzschean genealogy is undoubtedly a central tool in Foucault's 'tool-box' of concepts. Accordingly, Foucault reflects upon his reference to Nietzsche in two famous essays ("Nietzsche, Freud, Marx" and "Nietzsche, Genealogy, History"). While his 1975/76 lectures at the Collège de France are known for an introduction or first draft of 'biopolitics', they also provide a history of the provenance of genealogical thinking. This history or 'genealogy' is in many ways congruent with important findings of current research on Nietzsche. The paper thus attempts to combine these two sources in order to foster a deeper understanding of Foucault's Nietzsche reception and to uncover problematic aspects of this legacy.

The uniqueness of Foucault's work bears not least on a rare combination of meticulous historical research and philosophical abstraction. However, this combination—exemplified by the notion of the 'historical a priori'—also induces a fundamental tension that seems impossible to resolve. By tracing Foucault's 'historical a priori' back to Husserl, the paper shows that the concept remained unwillingly bound to this phenomenological legacy. In a comparison with American Pragmatism, drawing on its major similitudes with and differences to Foucault's thought, the author finally hints at a possible remedy for these methodological contradictions.

Le colloque international intitulé L'artisanat antique en milieu urbain de Gaule romaine et des régions voisines s'est tenu à Autun, du 20 au 22 septembre 2007, à l'initiative de Pascale Chardron-Picault. Cet événement s'inscrivait certes dans la prolongation de plusieurs rencontres et monographies qui se sont efforcées, ces dernières années, de faire le point sur l'artisanat durant la période romaine, mais il apportait quelque chose de plus. Il se déroulait dans le chef-lieu de la cité des Éduens, et cela a assurément contribué à sa coloration spécifique. La qualité et la diversité des productions artisanales d'Autun-Augustodunum apparaissent en effet aujourd'hui avec toujours davantage d'évidence. Malgré cela, une minorité des 33 études réunies dans ces Actes est consacrée à Autun et à la Bourgogne antique, l'ambition du colloque étant de toucher toute la Gaule, et même l'Italie et les provinces occidentales de l'Empire. Le volume s'organise autour de trois thèmes : - Le premier, Les espaces urbains réservés à l'artisanat, regroupe des contributions portant sur la nature des métiers pratiqués dans les villes et leur évolution, sur leur localisation dans des quartiers réservés ou au contraire sur leur imbrication avec d'autres activités dans les zones commerçantes. - Le second, Vie, statut et savoir-faire des artisans, apporte notamment un éclairage renouvelé sur l'homme-artisan, son statut, son apprentissage, sa reconnaissance sociale…, dans une visée ample d'archéologie et d'histoire sociale, d'histoire économique, d'histoire des savoirs. - Le troisième, La transformation des matériaux, rassemble des synthèses sur la place de ces activités, la connaissance des techniques et des savoir-faire, grâce à de nouvelles découvertes et surtout de nouvelles analyses. La rencontre a ainsi permis de faire progresser la connaissance sur une série de questions majeures : l'insertion des ateliers dans les villes et les liens entre production et urbanisme, l'aménagement des ateliers et des boutiques, les chaînes opératoires, l'organisation par corps de métiers, les circuits économiques, les évolutions et les changements techniques et de mode. La qualité et la variété des auteurs, historiens et spécialistes des techniques, archéologues, archéomètres, archéozoologues, épigraphistes, historiens de l'art, l'équilibre entre les synthèses, les études d'un métier ou d'un site et l'approche historique ont permis de dépasser la diversité des approches pour étudier sous toutes ses facettes « l'artisan… héros secret » de l'histoire ancienne, pour paraphraser une formule célèbre de P. Vidal-Naquet.

Parmi les objets de tabletterie, les os d’équidés sont nettement plus représentés que parmi la faune non travaillée. Pour quelle raison les artisans ont-ils préféré les os de ces animaux qui jouaient un rôle particulier dans le monde gallo-romain et qui, de ce fait, étaient difficiles à se procurer ? Dans cet article, nous émettons l’hypothèse que la taille des os a pu constituer un critère de sélection pour les tabletiers romains. Among artifacts bones of equids are much better represented than they are among food remains. Why did Roman craftsmen prefer to work bones of equids which played a special role in the culture of Roman Gaule and therefore were hard to provide? In this article the size of equid bones is proposed as a selection criterium. Unter den Knochenartefakten sind Equidenknochen deutlich besser vertreten als unter den Speiseabfallen. Wieso haben die römischen Handwerker bevorzugt Knochen von Equiden verarbeitet, die eine besondere Rolle in der gallo-römischen Kultur spielten und deshalb schwierig zu beschaffen waren? In diesem Artikel wird die Grösse der Pferdeknochen als Auswahlkriterium vorgeschlagen. (Traduction S. Deschler-Erb)

Introduction: The French population is aging and home care is a major issue. The general practitioner is at the center of this care but his role is poorly characterize. Objective: Conduct an inventory of the feeling of the general practitioner and his place in the establishment of home health care for elderly people. Material and Method: Qualitative research was conducted through individual semi-structured interviews and groups with thirteen general practitioners from Savoie and Haute-Savoie. Results: The place of the general practitioner appears most often as essential. Screening for loss of autonomy and coordination between the evaluations centres, the patient’s relatives and the caregivers themselves are the two most cited roles. Then comes the information on dependency and how to supplement, the prevention of loss of autonomy, the prescription of help and the care of the caregiver. The actual establishment of interventions at home by the doctor remains limited. Concerning them affect, the doctors deplore a time-consuming and insufficiently paid management with sometimes an administrative overload. They also consider that some of them have gaps in their training, and ignore the local social support network. They sometimes face the lack of available help and the communication difficulties between the different actors. Conclusion : Finally, there are as many practices as doctors because their role is poorly defined and they must adapt to the existing local network but also to each of their patients. Nevertheless, the doctors remain voluntary to take part in this care, even if at present it is rather as a duty that they do it and that many improvements are to be envisaged.; Introduction : La population française vieillit et le maintien à domicile se pose comme un enjeu majeur. Le médecin généraliste est au centre de cette prise en charge mais son rôle est mal défini. Objectif : Faire un état des lieux sur le ressenti du médecin généraliste et sa place dans la mise en place des aides pour le maintien à domicile des personnes âgées. Matériels et Méthodes : Recherche qualitative par entretiens semi-dirigés auprès de treize médecins généralistes de Savoie et Haute-Savoie. Résultats : La place du médecin généraliste apparaît souvent comme essentielle. Le dépistage des pertes d’autonomie et la coordination entre les structures d’évaluation, l’entourage et les aides proprement dites sont les deux rôles les plus cités. Ensuite viennent l’information sur la dépendance et les moyens d’y suppléer, la prévention des pertes d’autonomie, la prescription d’aide et la prise en charge de l’aidant. Concernant le ressenti, les médecins déplorent une prise en charge chronophage et insuffisamment rémunérée avec quelquefois une surcharge administrative. Ils considèrent, pour certains, avoir des lacunes dans leur formation, et méconnaissent le réseau d’aide local. Ils se heurtent au manque d’aide disponible et aux difficultés de communication entre les différents acteurs. Conclusion : Il existe finalement autant de pratiques que de médecins car leur rôle est mal délimité et ceux-ci doivent s’adapter au réseau local existant mais également à chacun de leur patient. Malgré tout, les médecins restent volontaires pour participer à cette prise en charge, même si actuellement c’est plutôt par devoir qu’ils le font et que de nombreuses améliorations sont à envisager.

The tougher current competitive environment, the decreasing trend in public subsidies and the new consumer expectations of sports activities are forcing the business models of Olympic sports federations to evolve.Split apart between the need to deliver successful performances at world events, notably the Olympic games, in order to project an attractive image thanks to the media coverage and that of the of promoting the sport for everyone in order to increase membership, Olympic federations seek new competitive leavers.The ambition of this research has been to study the way in which they integrate competitive positioning marketing initiatives in their development strategy.How to increase members’ loyalty? How to attract more spectators and fans? How to assert the Federation brand worth for their partners and the media? We have analysed how Olympic federations, very different from each other, apply classic marketing models differently.This has lead us to deem the sports federations as brands in their own right, in relation to what they aim to build as an emotional relationship (members, spectators, partners) which goes beyond mere usage of the product.But, the ethics of what should be an Association and the mind set of being a benevolent institution, ingrained in the federations DNA to varying degrees but still very prevalent amongst their elected executive, is a hindrance to the building of a genuine Federation brand.This is why we have proposed, based on three different types of federations, different development models using classical marketing stages.Caught in-between a utilitarian ethic (and therefore of a commercial nature) and the ethics of an association (closer to the ethics of sport), the federations have no other alternative than to embrace a business attitude, opening their organisations to a marketing approach, economic viability and managerial efficiency whilst taking into account the different specific requirements of their sport on the marketing matrix.The planning of the Paris Olympic Games in 2024 welcomes a passionate period where one of the stakes will be to measure its impact over the development model of the federations., Un contexte concurrentiel exacerbé sur les marchés du sport, une baisse tendancielle des aides publiques et de ouvelles attentes des consommateurs obligent les fédérations sportives olympiques à faire évoluer leurs modèles économiques. Tiraillées entre la nécessité d’être performantes en résultats sportifs lors des grands rendez-vous planétaires couverts par les médias, notamment les Jeux Olympiques, et celle de la promotion du sport pour tous afin d’augmenter leur nombre de licenciés, les fédérations olympiques sont à la recherche de nouveaux leviers de développement et de compétitivité.L’ambition de cette thèse est d’analyser la façon dont elles sont en mesure d’adopter une démarche marketing au service de leurs stratégies de développement dans un environnement concurrentiel. Comment positionner la fédération ? Comment fidéliser les licenciés et en faire des fans ? Comment attirer des prospects et notamment de nouveaux spectateurs ? Comment valoriser l’image fédérale auprès des partenaires et des médias ?Sur la base d’une typologie des fédérations olympiques, nous avons analysé comment chaque type de fédération mobilise le modèle classique du marketing commercial de façon différenciée. Cela nous a amené à les évaluer face à la nécessité de se transformer en véritables marques pour être en mesure de construire une relation affective et émotionnelle avec les consommateurs (pratiquants licenciés, spectateurs, partenaires) qui dépasse le simple usage du produit. Mais, l’héritage d’une éthique associative et bénévole plus ou moins prégnantes chez les élus, peut apparaître comme un obstacle à une politique de marque fédérale.C’est pourquoi nous proposons des modèles de développement différenciés selon les types de fédérations, déclinant et adaptant les étapes classiques du plan marketing en fonction des spécificités de chacun. Coincées entre une éthique utilitariste (moteur de la réussite sur le marché) par nature commerciale et une éthique associative (qui fonde leur légitimité) centrée sur les différentes modalités de pratique, les fédérations n’ont pas d’autres choix que celui du compromis en s’engageant à la fois vers une logique managériale d’entreprise (en s’ouvrant à l’expertise marketing et à la rentabilité économique) et en préservant leur spécificité culturelle qui est leur socle de positionnement. L’apport de la thèse se trouve dans cette modélisation différenciée car toutes les fédérations n’ont pas les mêmes capacités de s’ouvrir à la concurrence commerciale.L’organisation des Jeux Olympiques à Paris en 2024 ouvre une période passionnante à la fois sur le plan sportif et économique dont un des enjeux sera sans doute aussi de mesurer son impact sur le modèle de développement des fédérations sportives olympiques françaises.

Au niveau master, cette ségrégation a des conséquences significatives sur l'insertion professionnelle des femmes et leurs conditions d'emploi, donnant à l'orientation universitaire un poids déterminant dans la re/production des inégalités de genre. Les femmes, pourtant plus nombreuses à être diplômées, s'insèrent plus difficilement que les hommes sur le marché du travail. S'appuyant sur une collecte annuelle et exhaustive, l'enquête Insertion professionnelle des diplômés de l'université permet de produire des analyses de données au niveau des disciplines fines du diplôme de master. Les étudiantes sont majoritaires parmi les disciplines donnant accès à des métiers du secteur public, associatif et social où les conditions d'emploi sont plus dégradées que dans les autres secteurs. La ségrégation disciplinaire explique à elle-seule la moitié des écarts de genre pour l'accès aux emplois stables et plus d'un quart des écarts d'accès aux emplois de catégorie cadre. Si les inégalités d'accès aux emplois les plus rémunérateurs ne trouvent pas d'explication dans les modélisations par discipline, les écarts de répartition selon le genre permettent de poser un regard différent sur l'influence de l'orientation dans les trajectoires d'insertion professionnelle. E n 2017, une étude publiée par l'Insee annonçait que pour la première fois, les femmes avaient atteint la même part d'emploi de catégorie cadre que les hommes, trois ans après leur sortie d'études. Ce constat, dû pour partie à la sur-diplomation des femmes aux niveaux d'études les plus élevés, cache la persistance d'inégalités socio-économiques sur le marché du travail au détriment des femmes [Di Paola, DuPray et alii, 2017].

Recent developments in HIV-specific CTL quantification (staining with MHC-epitopic peptide tetramers and IFNγ production) have shed new light on human immunodeficiency virus (HIV)-specific cytotoxic T lymphocytes (CTL). One week after infection, anti-HIV CTLs quickly appear, with a weak specificity spectrum which broaden afterwards. During the asymptomatic stage, an equilibrum between host and virus exists, characterized by vigourous CTL responses (frequencies: 10-2 to 10-3 CD8 T cells) directed against all HIV proteins and poor HIV replication. The ratio anti-HIV CTL : CD4 infected cells is 10:1, thus in favour of CTL action. When HIV viral load rises up, Aids stage is reached due to either viral escape or immune system exhaustion. Highly active antiretroviral therapies permit some immune reconstitution while effector CTLs decrease. Even if it is controled. HIV is still present in « viral reservoir ».

Metabolomics is a new fast groing field with the aim of studyind all the small molecules produced (under different conditions and times in their life cycles) in a biological system known as metabolites. Applying the metabolomics to the cancer's problematic will be resumed in the identification of metabolic perturbation linked to oncological pathology. The metabolomics is the result of coupling analytical techniques such as mass spectrometry or nuclear magnetic resonance spectroscopy to specific statistical methods in order to identify metabolic information able to caracterize the being state of the biological system. The high resolution magic angle spinning (HRMAS) NMR is the technique we choose for studying tumour as it deals great advantages (easiness in sample preparation, the inter et intra laboratory reproducibility and the moderate cost per sample). The principal study carried out within the framework of this thesis related to the cerebral tumours. Almost one hundred tumours were analyzed and their metabolome were characterized. The analysis carried out was focused on the problem of the “ malignancy grading ” of the cerebral tumours, mainly on the gliomas. Thus, a correlation was found between metabolic profile of the gliomas (glioblastomas, oligodendrogliomas) and the clinical evolution of the related patients. The use of metabolomics for tumour studying seems promising today on its diagnosis and prognosis capabilities. Moreover, the study of metabolic profile, should, in the near futur, allow a better comprehension of the metabolic disturbances related to pathology, and potentially, lead to the installation of individualized treatments perfectly adapted to each patient. Nevertheless, a stage of validation for those observations is necessary. In order to achieve this goal, the CARMeN project, started in november 2007, consisting in the installation of the first clinical HRMAS NMR spectrometer, was done. The main aim of this project is to extend our experience to all the fields of the cancerology where, a dissociation between the histopathologic diagnosis and the patient outcome, or a discordance between the therapeutic response for a sub‐type of tumour exist., La métabolomique est un outil récent, aujourd'hui en plein essor, qui se définit comme étant l'étude de l'ensemble des petites molécules ; les métabolites, produites par un organisme sous différentes conditions. Son application à la problématique du cancer consiste en l'étude des perturbations métaboliques liées au processus oncologique. La métabolomique mêle diverses techniques d'analyses telles que la spectroscopie de masse et la spectroscopie de résonance magnétique nucléaire (RMN), couplées à des méthodes statistiques évoluées afin d'extraire les informations métaboliques pertinentes permettant de caractériser les systèmes biologiques. La spectrométrie RMN haute résolution en rotation à l'angle magique (HRMAS) est la technique que nous avons choisie pour l'étude du métabolome des tumeurs car elle regroupe un nombre important d'avantages (simplicité de préparation des échantillons, la reproductibilité intra et inter laboratoire et le cout relativement modeste de fonctionnement). L'étude principale réalisée dans le cadre de cette thèse a porté sur les tumeurs cérébrales. Une centaine de tumeurs ont ainsi été analysé et leur métabolome caractérisé. L'analyse effectuée s'est focalisé sur la problématique du « grading de malignité » des tumeurs cérébrales, principalement sur les gliomes. Ainsi une forte corrélation a été trouvé entre les profiles métabolique des gliomes (glioblastome, oligodendrogliome) et l'évolution clinique des patients. L'approche métabolomique pour l'étude des tumeurs, d'un point de vue diagnostic et pronostic, semble aujourd'hui prometteuse. De plus l'étude des profiles métaboliques, devrait, à terme, permettre une meilleure compréhension des perturbations métaboliques lié à la pathologie, et potentiellement, conduire à la mise en place de traitements individualisés parfaitement adaptés à chaque patient. Néanmoins, une étape de validation de ces premières observations est aujourd'hui indispensable. C'est pourquoi ces travaux ont accompagné l'installation du premier spectromètre RMN HRMAS dans le milieu clinique en novembre 2007 dans le cadre du projet CARMeN. L'objectif général de ce projet est, maintenant, d'étendre l'expérience acquise durant cette thèse à tous les domaines de la cancérologie où il existe, soit une dissociation entre le diagnostic histopathologique de la tumeur et le devenir du patient, soit une discordance à la réponse thérapeutique pour un sous‐type de tumeur.

The Cathedral Hill at Basle has been densely occupied for some 2000 years, so that the archaeological exploration of its earliest use is difficult. Furthermore, the initial discovery of the Iron Age occupation of the site is relatvely recent, dating only from 1971. Since then, knowledge of Iron Age Basle has grown considerably, thanks to successive archaeological interventions. In particular, the analysis of the results of large-scale excavations on the Cathedral Hill now allows an overview of the Late La Tène occupation. It is also now possible to clarify the nature of the «Cathedral Hill horizon »., La colline de la cathédrale à Bâle est occupée sans discontinuité depuis plus de 2000 ans, ce qui rend les investigations archéologiques systématiques fort complexes, du moins pour les périodes anciennes. Les données dont nous disposons pour l’occupation celtique de la Colline de la cathédrale sont relativement récentes, puisqu’elles ne remontent qu’à 1971. Depuis, des données inédites sont venues se greffer sur ce bilan à chaque nouvelle intervention archéologique. Plus particulièrement, les importantes tranchées creusées à travers la colline ont permis de se faire une idée globale de l’occupation à la Tène finale. The aim of this study was to evaluate for the interest of realistic mannequin-based simulations as a tool to assess the knowledge of emergency medicine physicians in the field of difficult tracheal intubation.Prospective.Emergency physicians.Twenty-four emergency physicians were invited entering the study. The first step of the study consisted of an initial assessment of their knowledge in the field of difficult tracheal intubation. Then theoretical lectures on the tools and techniques of difficult tracheal intubation were given, followed by standard mannequin-based driven workshops. The second step was conducted six weeks later. Each physician's knowledge was re-evaluated and their ability to manage two difficult airway scenarios simulated on the AirMan simulator (Laerdal was assessed.Only one physician could not complete the program. Half of them worked at the University Hospital (UH) with half of them for less than three years. Lectures and standard mannequin-based driven workshops significantly improved physician's theoretical knowledge. Practical performance during difficult airway management scenarios was poor.We have demonstrated that theoretical lectures and standard mannequin-based driven workshops improved overall theoretical knowledge but did not translated to practical skill during of realistic mannequin-based simulations. Realistic mannequin-based simulations teaching programs in the field of difficult tracheal intubation should be considered.

To assess the perioperative evolution of the nutritional status of head and neck surgical patients.Prospective, descriptive case series.Fifty-four patients candidates for total or partial laryngectomy for malignancy of the neck tract without a past of neck surgery. The nutritional status of all patients hospitalized for total pharyngolaryngectomy, total or partial laryngectomy was assessed by 1) clinical parameters including weight (W), weight variation (WV, percentage of loss), Body Mass Index (BMI), triceps skin fold measurement (T), midarm circumference (M), and 2) biological parameters such as serum albumin (SA), transthyretin (TTR), lymphocytes (Ly). These parameters were noted at the time of diagnostic laryngoscopy (T1), the day before surgery (T2), and 10 days afterward (T3) when patients were authorized to eat normally. All patients had enteral nutrition (EN) support (35 kcal/kg/day) starting at D1 and for 10 days afterward. Only complete data per patient were analyzed.24 patients were excluded. During the 21 days [7-53] preoperative period (T1-T2), WV was (6.6% [-8,1-+20.0] [T2] vs. 4.7% [-12,9-+20.0] [T1], p0.05) without difference in T and M. In the postoperative period (T2-T3), all parameters worsened, except T, with: WV (8.2% [-8,1-+20.0] [T3], p0.05 vs T2), M (27.4 cm [20.0-37.0] [T3] vs 28.3 cm [20.5-39.0] [T2], p0.05) et TTR (0.21 mg/l [0.09-0.36] [T3] vs. 0.27 mg/l [0.08-0.45] [T2], p0.05). BMI was 22.9 [15.2-36.7] (T1) vs 22.9 [15.2-35.3] (T2), NS and 22.1 [15.0-34.9] (T3), p0.05 vs (T2).The nutritional status in malignancy head and neck surgical patients seems to be best assessed by loss weight. It worsened mainly during the postoperative period even if a well-conducted EN was performed as defined by the French consensus conference.

Breast-like carcinomas of the vulva is a rare finding. Only 11 cases are reported in the literature. This article reports a painful tumor of the left vulvar labia in a 62 year-old woman. Excisional biopsy showed an infiltrating adenocarcinoma, histologically similar to a breast tumor with positive hormonal receptors. The origin of this tumor, ectopic mammary tissue or specific adnexal genito-anal gland, is discussed. From data of the literature, we offer guidelines for diagnosis, treatment and origin of this rare tumor.

This contribution discusses implications of the lockdown on working conditions working relationship for employees. It uses data from first surveys on the French lockdown. This unique period has generated a large economic shock that led to significant changes on the employees’ activity and the working conditions and institutions (the labour-wage nexus). In some cases, employers desire to maintain the activity in spite of the sanitary context and the necessity to preserve the health of workers. Usually, social differences by gender are overlooked, for instance, caring for children, also being confined at home, is not foreseen. In other cases, new ways of working appear such as the development of remote working and short-time working to tackle mobility restrictions. Even if the continuities and the changes we observe had not been planned, they may announce a new labour-wage nexus in the long run.

Zidovudine (ZDV) was the most widely used anti-HIV drug between 1987 and 1995, and, as already reported, transmission of ZDV-resistant viruses occurs. Several mutations of the reverse transcriptase gene have been identified; one of them affects the 215 codon and is associated with a high degree of resistance. We have determined, using selective PCR, the prevalence of transmission of 215 mutant isolates in 134 patients with primary HIV infection (PHI) and have identified 8 patients with 215 mutant virus between 1989 and 1995 in Switzerland. Mutant resistant viruses have been isolated from patients treated with most antiviral drugs. A systematic search for mutant viruses may provide useful information for the adaptation of treatment strategies.

The aim of this study was to analyse the follow-up of patients with a history of endotracheal intubation and/or tracheotomy because of respiratory distress, surgical necessity or long-term resuscitation. Twenty adults were followed up in a ENT facility for laryngeal or tracheal stenosis, of which 7 cases were diagnosed during 1994. This study shows that, in patients with a history of intubation, or intubation followed by tracheotomy, the stenosis developed within two months after resuscitation with a favourable outcome. After tracheotomy only, the stenosis developed later (2 months or more) with more recurrences. The diagnosis of stenosis was made on gradual or acute dyspnea. While respiratory resuscitation methods are essential and often life-saving, they are not without complications. Laryngeal and tracheal stenosis could be largely prevented by more careful techniques of endotracheal intubation and tracheotomy. Periodic endoscopic airway surveillance is useful to detect stenosis even when there is no clinical symptom.

Systemically administered carbonic anhydrase inhibitors have been in clinical use since the 1950's. Their use and side effects have been reviewed extensively. Our review summarizes the development of topically administered carbonic anhydrase inhibitors in patients with glaucoma. Moreover, it documents clinical trials leading to the present choice of dorzolamide (MK-507), which is now being reviewed for worldwide approval for marketing by 1995.

45. [Reconstruction with free jejunal graft after pharyngo-esophageal resection for cancer: functional results and survival]

57 patients with cancer of the hypopharynx underwent 59 pharyngo-esophageal reconstructions with a free jejunal graft after total removal of a tumor combined with resection of the circumferential tissues. 18 patients had been previously irradiated. Median resumption of oral intake was 16 days; 40 out of 50 patients (98%) followed for more than 2 years regained normal eating habits. The cumulative 5-year survival rate was 44%. The free jejunal graft should be advocated as a safe and reliable procedure for hypopharyngeal cancer.

International audience; Dysregulation of free radical metabolism has been supposed to be involved in schizophrenia etiopathogeny. Recently, Wang et al. showed a red blood cell super oxide dismutase increase in positive schizophrenia (Crow's type I), but neither in negative schizophrenia (Crow's type II) nor in controls. The study included 28 in-patients suffering from acute positive psychosis who were compared with 15 controls. We confirmed the results of Wang. We found a significantly red blood cell Super oxide dismutase increase in positive psychosis, in comparison to negative psychosis and controls (p = 0.0001). This SOD increase was in relationship with the degree of clinical psychomotor excitement. After 21 days of neuroleptic treatment, SOD activity decreased and reached standard values. These results support the hypothesis of striking relationships between catecholaminergic hyper-metabolism and SOD increase, in positive psychosis. These could account for psychotic positive symptoms improvement with neuroleptic treatment, which blocks dopamine pathways.

International audience; Serum levels of haloperidol and reduced haloperidol as well as the reduced haloperidol/haloperidol ratios were determined in nine acute schizophrenics on oral haloperidol medication and correlated over 21 days with psycho pathology and extra-pyramidal symptom scores. We have investigated red blood cells haloperidol reductase activity in the group of patients. Significant correlations were found between haloperidol plasma levels and positive sub scale for each patient (r = 0.86 and p < 0.01; r = 0.70 and p < 0.05). We found a correlation between red blood cells reductase activity and the improvement of the psychotic anxiety scale (r = 0.64/and p < 0.05; r = 0.67 and p < 0.05), but not with reduced haloperidol/haloperidol ratios in plasma. The knowledge of reductase activity could predict the treatment response in acute schizophrenic patients. We suggest that the reported inter individual and inter ethnic differences in haloperidol and reduced haloperidol and in clinical response and adverse effects may be a reflection of genetic control of the two oxidative pathways mediated by cytochrome P450 isozyme and/or the reductase pathway mediated by haloperidol reductase in individual subject.

48. [Morphological characterization of cell lines of human uveal melanoma]

Several cell lines have been derived from an ocular melanoma obtained from an enucleated patient. Three cell types are observed during the time in culture of all the cell lines under study. Two of them have epithelial and spindle shape respectively. A third cell type, having a spheroidal shape, is formed from spindle cells and may be transformed into epithelial cells upon re-seeding. Further experiments showed that the same cell may change of shape following the cycle: spheroidal--epithelial--fusiform--spheroidal. Scanning microscopy shows the coexistence of the three cell shapes in the same culture and the presence of several filaments and processes protruding at the surface of the cells. Transmission electron microscopy shows that the cell lines, in general, contain melanosomes empty or fairly pigmented and several filaments and microtubules. The presence of melanin may be stimulated by seeding of melanoma cells over a "feeder layer" of fibroblasts.
