Abstract: Haemophilia causes principally bleedings in muscles and joints. Therapy is purely palliative, based on injections of antihemophilic factor. Muscular hematomas represent the most precocious complication and generally happen after a trauma. Though they scarcely threaten the life of the patients, they often may generate an important functional disability if not treated quickly, and vigorously. Treatment should associate injections of the defective clotting factor to physiotherapy and contractions of the involved muscle. When hematomas involve lower limbs, standing up and walking should not be authorized before the end of the flessum or recovering of the statu quo ante. [Copyright &y& Elsevier]

L’hémothorax spontané est une complication rare de l’hémophilie. Ce fait est rarement décrit dans la littérature. Nous rapportons le cas d’un grand enfant âgé de 15 ans suivi dans notre service pour hémophilie A et qui consulte en Octobre 2008 pour gêne respiratoire sans notion de traumatisme et pâleur cutanéo-muqueuse. L’examen clinique trouve une matité pulmonaire gauche, des signes de lutte respiratoire et un bon état hémodynamique. La radiographie du thorax objective une opacité de l’hémithorax gauche refoulant le cœur, l’échographie thoracique évoque un aspect en faveur d’un hémothorax gauche de grande abondance. Le malade a été mis sous traitement conservateur à base de facteur antihémophilique sans drainage. L évolution clinico-radiologique est favorable au bout de quatre semaines. La survenue d’un hémothorax spontané est exceptionnelle au cours de l’hémophilie. Son traitement peut consister en un traitement conservateur sans recours au drainage. [ABSTRACT FROM AUTHOR]
