Lung cancer is the leading cause of cancer mortality in the developed world. Diffuse fibrosing interstitial lung disease (ILD) consist of a heterogeneous group that includes idiopathic pulmonary fibrosis (IPF). Diffuse ILD is a risk factor for the development of lung cancer which on its own is associated with an increased risk of morbidity and mortality. Despite common mechanisms between fibrogenesis and carcinogenesis, the underlying pathogenesis of lung cancer and fibrosis overlap is poorly understood. The clinical management of these patients remains a medical challenge requiring a multidisciplinary approach, particularly in view of the risk of acute exacerbation of fibrosing ILD following most lung cancer treatments, leading to a considerable negative outcome on overall prognosis.Le carcinome bronchopulmonaire (CBP) est la première cause de mortalité par cancer dans les pays développés. Les pneumopathies interstitielles diffuses fibrosantes (PIDf) sont un groupe hétérogène comprenant, entre autres, la fibrose pulmonaire idiopathique. Les PIDf sont un facteur de risque de développement du CBP et sont associées à un risque accru de morbi-mortalité. Malgré des mécanismes communs entre la fibrogenèse et la carcinogenèse, la pathogenèse sous-jacente de l’association CBP et PIDf est mal comprise. La gestion clinique de ces patients est un défi médical nécessitant une prise en charge multidisciplinaire, en particulier devant les risques d’exacerbation aiguë de la PIDf grevant le pronostic.

Idiopathic pulmonary fibrosis (IPF) is a lethal respiratory disease characterized by the excessive deposition of extracellular matrix in the alveolar zones. The bronchiolar epithelium has been implicated in the development of this disease and is capable of secreting IgA into the airway lumen thanks to its expression of the polymeric immunoglobulin receptor. Several elements indicate a dysregulation of this system, such as raised serum IgA levels in IPF patients and the pro-fibrotic effect of IgA on several key cell types. Our work aims at studying the underlying mechanisms so as to better understand the role of IgA mucosal immunity in this disease.

CT scan is a key exam in idiopathic pulmonary fibrosis diagnosis defining the UIP pattern according to the last international guidelines published in 2018. However, it is a radiating exam which solely provide morphological information. Lung MRI is a developing technic which could be non-inferior to CT scan to describe UIP patterns and which would be able to provide functional data. We conducted a monocentric prospective study at CHU Bordeaux from 2016 to 2020 including 60 subjects (18 UIP, 7 possible UIP, 15 stage IV sarcoidosis and 20 patients with disease without reticulations or healthy subjects). Each patient performed a CT scan with millimetric slices, and a lung MRI with UTE VIBE sequence before and 15 minutes after gadolinium injection 1 BLADE sequence. Diagnostic performances of lung MRI for each of the 4 diagnostics criteria forming UIP pattern such as sub pleural and basal localization of lesions, reticulations, honeycombing and absence of excluding lesion had sensitivity and specificity better than 65% and 75%, respectively. Lung MRI was non inferior to CT scan to describe UIP pattern lesions and provided functional information which would be interesting in the future to sort different types of patients and to evaluate treatment answers.; Le scanner thoracique est un examen clé dans le diagnostic de la fibrose pulmonaire idiopathique définissant le pattern de PIC selon les recommandations internationales publiées en 2018. Cependant, le scanner thoracique est un examen irradiant qui ne fournit que des données morphologiques. L'IRM thoracique morphologique en développement pourrait être non inférieur au scanner pour la description des aspects de PIC et fournir des données fonctionnelles. Nous avons mené une étude monocentrique prospective au CHU Bordeaux de 2016- 2020 portant sur 60 sujets (18 PIC certaines, 7 PIC possibles avec preuve anatomo-pathologique, 15 sarcoïdoses stade IV, et 20 patients ayant une pathologie respiratoire sans réticulation ou des sujets sains). Une TDM en coupes fines de 1 mm sans injection et une IRM thoracique (avec séquence UTE VIBE morphologique avant et 15 minutes après l’injection de gadolinium et une séquence BLADE) ont été réalisées chez tous les sujets. Les performances diagnostiques de l'IRM thoracique pour chacun des 4 critères constituant l'aspect de PIC certaine : prédominance sous pleurale basale des lésions, réticulations, rayon de miel et absence de signe incompatible avec une PIC, étaient jugées bonnes avec une sensibilité et une spécificité supérieures à 65 et 75% respectivement. Il existait une corrélation inverse entre la CVF et T2-HSV et le rehaussement tardif chez les patients avec un PIC certaine. L’IRM thoracique était non inférieure à la TDM pour décrire les lésions radiologiques élémentaires de l'aspect de PIC et a fourni des données fonctionnelles pouvant permettre d’identifier des profils de patients (exacerbateurs, progresseurs rapides, répondeurs au traitement).

Genetic studies of familial forms of interstitial lung disease (ILD) have led to the discovery of telomere-related gene (TRG) mutations (TERT, TERC, RTEL1, PARN, DKC1, TINF2, NAF1, NOP10, NHP2, ACD, ZCCH8) in approximately 30% of familial ILD forms. ILD patients with TRG mutation are also subject to extra-pulmonary (immune-hematological, hepatic and/or mucosal-cutaneous) manifestations. TRG mutations may be associated not only with idiopathic pulmonary fibrosis (IPF), but also with non-IPF ILDs, including idiopathic and secondary ILDs, such as hypersensitivity pneumonitis (HP). The presence of TRG mutation may also be associated with an accelerated decline of forced vital capacity (FVC) or poorer prognosis after lung transplantation, notwithstanding which, usual ILD treatments may be proposed. Lastly, patients and their relatives are called upon to reduce their exposure to environmental lung toxicity, and are likely to derive benefit from specific genetic counseling and pre-symptomatic genetic testing.

La meilleure méthode de dépistage est le CT-scan à haute résolution. Le traitement de l’ILD repose sur les glucocorticoïdes, les immunosuppresseurs et les antifibrotiques, ainsi que sur des mesures non médicamenteuses. Nous détaillerons l’algorithme de dépistage et de traitement de l’ILD associée à la sclérodermie systémique, car il est le mieux défini. Nous discuterons également de celui de l’ILD associée à la polyarthrite rhumatoïde, maladie qui est très fréquente et donc d’intérêt majeur.

Pulmonary fibrosis is a pathological entity still too little understood today, burdened with significant morbidity and mortality. Idiopathic pulmonary fibrosis is a complex diagnostic disease requiring a multidisciplinary approach and in some cases the performance of a lung biopsy. In addition, the early identification of the pathology remains the key in order to preserve lung function as much as possible. In this context and in view of the diagnostic difficulty, it seems essential to identify new biomarkers to help with the differential diagnosis, the evaluation of the prognosis and the response to treatment. In addition, the evolution of the pathology remaining inexorable despite anti-fibrotic treatments, it appears critical to be able to identify new potential therapeutic routes.La fibrose pulmonaire est une entité pathologique de nos jours encore trop méconnue, grevée d’une morbi-mortalité importante. La fibrose pulmonaire idiopathique est une maladie de diagnostic complexe nécessitant une approche pluridisciplinaire et, dans certains cas, la réalisation d’une biopsie pulmonaire. De plus, l’identification précoce de la pathologie reste la clé afin de préserver au maximum la fonction pulmonaire. Dans ce contexte et devant la difficulté diagnostique, il semble primordial de pouvoir identifier de nouveaux biomarqueurs permettant d’apporter une aide au diagnostic différentiel, à l’évaluation du pronostic et à la réponse au traitement. De plus, l’évolution de la pathologie restant inexorable en dépit de traitements anti-fibrotiques, il apparaît comme critique de pouvoir identifier de nouvelles voies thérapeutiques potentielles.

We present data on prognostic factors in a Tunisian cohort of people with Idiopathic pulmonary fibrosis.Idiopathic pulmonary fibrosis (IPF) has a poor prognosis, with a median survival in patients with the condition of only 3 to 5 years. Previous studies have identified a number of prognostic factors in this chronic pulmonary disease.We conducted a retrospective study, including patients with idiopathic pulmonary fibrosis (IPF) who were diagnosed at the Pneumology Department of the University Hospital Fattouma-Bourguiba, Monastir, between 1991 and 2014. The aim of this study was to compare clinical, radiological, pulmonary functional predictors of survival in IPF in a Tunisian cohort with those of previous studies.This study included 126 patients. Their mean age was 66 years, with a male predominance (68.3%). Respiratory function tests revealed a restrictive ventilatory deficit in 72.6% of cases. The median survival of our study population was 22.5 months [6.7-49.5]. In univariate analysis, factors associated with a poor prognosis were: lower baseline values of TLC, FCV and DLco, level of dyspnea assessed by mMRC scale, hypoxemia at diagnosis, the degree of desaturation during exercise, a higher annual decline of FVC and DLco, acute respiratory distress and also the GAP score. In multivariate analysis, independent prognostic factors were: baseline DLco, level of dyspnea, desaturation at exertion and the annual decline of the DLco.Lower baseline DLco, the level of dyspnea, desaturation on exercise, and annual decline in DLco are all associated with a poor prognosis in IPF.

This work evaluated the contribution of the thrombin generation assay in cell culture models, in a mouse model of APS, and in patients with endothelial dysfunction leading to a hypercoagulable state. Antiphospholipid antibodies cause a decrease in ePCR, inducing resistance to activated protein C, found in our different models and patients. Treatment with hydroxychloroquine in the experimental model of APS showed a protective effect of this treatment on endothelial function, restoring vasorelaxation and normalizing thrombin generation after injection of purified antiphospholipid antibodies. A hypercoagulable state, associated with the development of pulmonary embolism, has been demonstrated in idiopathic pulmonary fibrosis. Circulating endothelial progenitors from patients with idiopathic pulmonary fibrosis induce a hypercoagulable state associated with hypoexpression of thrombomodulin. These results open a therapeutic avenue. Currently, clinical trials are testing thrombomodulin as a potential therapeutic. Recently, a pandemic by Sars-CoV-2, is associated with a hypercoagulable state, with the development of multiple thrombosis, found during autopsies of patients. We have demonstrated the value of the thrombin generation test as a prognostic factor during Sars-CoV-2 infection, predicting the hospitalization of patients in intensive care. The thrombin generation test seems to be an interesting tool during endothelial damage inducing hypercoagulability in cross-sectional models, and suggests possible therapeutic avenues, such as the modulation of the glycocalyx in APS., Ce travail a permis d’évaluer l’apport du test de génération de thrombine dans des modèles de cultures cellulaires, dans un modèle murin de SAPL, et chez des patients atteints d’une dysfonction endothéliale entrainant un état d’hypercoagulabilité. Les anticorps antiphospholipides entrainent une diminution de l’ePCR, induisant résistance à la protéine C activée, retrouvée dans nos différents modèles et patients. Le traitement par l'hydroxychloroquine dans le modèle expérimental de SAPL, a montré un effet protecteur de ce traitement sur la fonction endothéliale, en restaurant la vasorelaxation et une normalisation de la génération de thrombine après injection d'anticorps antiphospholipides purifiés. Il a été démontré un état hypercoagulable, associé au développement d’embolie pulmonaire, au cours de la fibrose pulmonaire idiopathique. Les progéniteurs endothéliaux circulants de patients atteints de fibrose pulmonaire idiopathique, induisent un état d’hypercoagulabilité, associé à une hypoexpression de thrombomoduline. Ces résultats ouvrent une piste thérapeutique. Actuellement des essais cliniques testent la thrombomoduline comme thérapeutique potentielle. Récemment, une pandémie par Sars-CoV-2, est associée à un état hypercoagulable, avec le développement de thromboses multiples, retrouvées lors d’autopsies de patients. Nous avons démontré l’intérêt du test de génération de thrombine comme facteur pronostique au cours de l’infection à Sars-CoV-2, prédisant l’hospitalisation en réanimation des patients. Le test de génération de thrombine semble être un outil intéressant au cours de l’atteinte endothéliale induisant une hypercoagulabilité dans des modèles transversaux, et laisse entrevoir de possibles pistes thérapeutiques, comme la modulation du glycocalyx dans le SAPL.

EPIDemio study is a multicenter, prospective and observational study. The objective is to estimate the prevalence and incidence of fibrosing interstitial lung diseases (ILDs) in the department of Haute Garonne (31) in France. Fifty-five pulmonologists from the Toulouse university hospital and 8 private establishments participated in this study. Two hundred and fifty-six cases of fibrosing ILDs were reported (gross overall prevalence: 22.8/100,000 and estimated 30.1/100,000. Idiopathic ILDs represent 55.8% of fibrosing ILDs ahead of systemic disease-related ILDs (24.6%) and ILDs associated with environmental exposure (13.3%). Idiopathic pulmonary fibrosis (IPF) represents 35.9% of fibrosing ILDs, which corresponds to a minimal prevalence of 8.2/100,000 and an estimated prevalence of 11.2/100,000. This study confirms epidemiological data collected in France and Europe.

Idiopathic pulmonary fibrosis (IPF) is a fibrotic lung disease characterized by an accumulation of connective tissue in the lungs leading to fatal restrictive respiratory failure. It appears to be the consequence of alveolar epithelial cells (AEC) dysfunction and fibroblast aberrant activation in response to repeated micro-injuries of the alveolar epithelium. In this pathophysiological mechanism, several molecular and cellular events are involved and actively participate in the loss of the epithelial phenotype. Among these events, endoplasmic reticulum (ER) stress seems to play a major role. In addition, a high prevalence of obstructive sleep apnea syndrome (OSA) has been reported in IPF patients. OSA is characterized by nocturnal intermittent hypoxia (IH) due to repeated upper airway obstructive events. The impact of OSA on the pathogenesis of IPF remains poorly understood.In the first study, we showed in a murine model of bleomycin-induced lung fibrosis exposed to IH an increase of pulmonary fibrosis severity through an exacerbation of lung inflammation and tissue damage. In the second study, we showed that ER stress, a pathogenetic element common to pulmonary fibrosis and IH, could be at least in part responsible for this aggravating effect of IH. We have also shown that other cellular events such as cellular senescence could be involved.During this PhD, we have demonstrated that IH induce an aggravating effects on pulmonary fibrogenesis at histological and molecular level and that ER stress and an induction of UPR pathways could be a crucial element. This work thus contributes to a better understanding of the impact of OSA in IPF patients.; La fibrose pulmonaire idiopathique (FPI) est une pneumopathie fibrosante, caractérisée par l’accumulation de tissu conjonctif dans les poumons conduisant à une insuffisance respiratoire restrictive d’évolution fatale. Elle semble être la conséquence d’un dysfonctionnement de cellules épithéliales alvéolaires (CEA) et d’une activation aberrante des fibroblastes suite à des micro-agressions répétées de l’épithélium. Dans ce processus, un ensemble d’événements moléculaires et cellulaires sont mis en jeu et participent activement à la perte du phénotype épithélial. Parmi ces évènements, le stress réticulum endoplasmique (RE) semble jouer un rôle majeur et central. Par ailleurs, une forte prévalence du syndrome d’apnées obstructives du sommeil (SAOS) a été rapportée chez les patients FPI. Le SAOS se caractérise par une hypoxie intermittente (HI) nocturne due aux évènements obstructifs répétés des voies aériennes supérieures. L’impact du SAOS sur la pathogenèse de la FPI reste encore mal connu. Dans une première étude, nous avons montré dans un modèle murin de fibrose induite par la bléomycine et exposé à l’HI, une aggravation de la sévérité de la fibrose pulmonaire mise en évidence par une exacerbation de l’inflammation et des lésions tissulaires. Dans une seconde étude, nous avons montré que le stress du RE, élément pathogénique communs à la fibrose pulmonaire et à l’HI, pouvait être au moins en partie responsable de cet effet aggravant. Nous avons également mis en évidence que d’autres évènements cellulaires tels que la senescence pourraient être impliqués. Au cours de ce doctorat nous avons mis en évidence que l’HI avait des effets aggravants sur la fibrogènése pulmonaire au niveau tissulaire et moléculaire et que le stress du RE et l’induction des voies de l’UPR pourrait constituer un élément central critique. Ce travail contribue ainsi à la meilleure compréhension de l’impact du SAOS chez les patients FPI.

The main objective of this work was to investigate a possible link between lung density, small pulmonary vessels, and pulmonary hypertension (PH) in patients with progressive fibrosing interstitial lung disease (PF-ILD).The study focused on patients with PF-ILD, all of whom underwent right cardiac catheterization and chest computed tomography prior to lung transplantation. Computed tomography scans were analyzed quantitatively for density and pulmonary vascularity. The relationship between computed tomography features and PH was investigated.Fifty-one patients with usual interstitial pneumonia (UIP) damage on lung explant were included. mPAP was positively correlated with lung mass (r=0.36, P=0.03) and lung volume (r=0.43, P=0.007). Patients with severe PH had more voxels lower than -856 Hounsfield Units (HU) (+16%, P=0.02), fewer voxels greater than -700 HU (-20%, P=0.03), and a higher lung volume (+1.57L, P=0.007) compared to patients without PH. No correlation was found between vascularization and HTP.Patients with PF-ILD and severe PH have lower lung density than patients with moderate or without PH.

Interstitial lung disease is a frequent complication of systemic sclerosis and has now become the leading cause of death in this disorder. It mainly occurs during the first five years after the diagnosis of systemic sclerosis. Various risk factors are associated with the occurrence of interstitial lung disease, including the presence of anti-topoisomerase I antibodies (Scl-70) and the diffuse cutaneous form of systemic sclerosis. The most common radio-pathological presentation is nonspecific interstitial pneumonia, followed by usual interstitial pneumonia. The classical immunosuppressive treatment of systemic sclerosis-associated interstitial lung disease is evolving, as recent studies suggest a beneficial effect of biological agents such as rituximab and tocilizumab, and antifibrotic drugs such as nintedanib.La pneumopathie interstitielle est une complication fréquente de la sclérodermie, dont elle est devenue ces dernières années la première cause de mortalité. Elle survient principalement durant les cinq premières années suivant le diagnostic de sclérodermie. Divers facteurs de risque sont associés à sa survenue, dont la présence d’anticorps anti-topoisomérase I (Scl-70) et la forme cutanée diffuse de sclérodermie. La présentation radio-pathologique la plus fréquente est la pneumopathie interstitielle non spécifique, suivie de la pneumopathie interstitielle commune. Le traitement immunosuppresseur classique de la pneumopathie interstitielle associée à la sclérodermie est en évolution, des études récentes suggérant l’efficacité de traitements biologiques comme le rituximab et le tocilizumab, et de médicaments antifibrotiques comme le nintédanib.

Background: idiopathic pulmonary fibrosis (IPF) is the most frequent pulmonary interstitial and fibrosing disease in the elderly, with a poor prognosis. Recently, structural alterations of the lung vasculature have been pointed out in the peripheral lung of IPF patients, although their functional modifications remains unknown. Imaging tools enabling to assess the dynamics of the lung small vessels involvement in IPF are needed. Purpose: the main endpoint was to demonstrate that the feasibility of quantifying the lung first-pass peripheral perfusion in IPF and to correlate it to severity indexes. Secondary endpoint was to compare both first-pass and late perfusion between IPF and controls. Methods: the retrospective pilot study was held between February 2017 and December 2018. All participants had to perform thoracic-MRI at 1.5 T, chest-CT and pulmonary function tests (PFT). Both first-pass and late perfusions were assessed after gadolinium chelate injection, with calculation of the normalized peripheral pulmonary blood flow (PBF) and T1-weighted late enhancement (ΔT1w), respectively. IPF severity was assessed using the Composite Physiological Index (CPI) and Transfer Lung Carbon Monoxide (TLCO) at PFT. Correlations were assessed using Spearman’s test, comparisons using Mann-Whitney test, and reproducibility using intraclass correlation coefficients (ICC). Results: 15 FPI and 17 controls were included. PBF significantly correlated to CPI (rho=0.56; p=0.03) and TLCO% (rho=-0.58; p=0.02). Une hypothèse physiopathologique est une atteinte vasculaire, avec une augmentation hétérogène de la densité capillaire pulmonaire. Objectif : l’objectif principal de cette étude est de déterminer s’il existe une corrélation entre le débit sanguin pulmonaire (PBF) au sein du poumon périphérique, et un index de sévérité de la maladie, l’index composite physiologique (CPI) chez des patients FPI. Une comparaison du PBF et du rehaussement tardif a ensuite été réalisée entre des patients FPI et une population contrôle. Méthodes : FPI et contrôles ont réalisé des imageries par résonance magnétique (IRM) thoraciques avec séquences de perfusion pulmonaire de premier passage, des séquences T1 réalisées sans et après injection, une tomodensitométrie pulmonaire et des épreuves fonctionnelles respiratoires. Une analyse semi-quantitative a été réalisée sur des cartographies de PBF, avec normalisation du signal sur celui de l’aorte ascendante. Un pourcentage de rehaussement a été calculé à partir des séquences T1. Les corrélations ont été calculées avec le test de Spearman. Résultats : 15 FPI et 17 contrôles correspondaient aux critères de l’étude. Le PBF moyen normalisé était corrélé avec le CPI (rho=0.56 ; p=0.03) et la TLCO (rho=-0.58 ; p=0.02). Le PBF normalisé était significativement supérieur au sein du poumon périphérique des FPI par rapport aux contrôles (p=0.02) de même que le rehaussement tardif (p=0.01). Le PBF des zones de réticulation et de rayon de miel était significativement supérieur en comparaison du poumon périphérique des sujets contrôles. Les mesures réalisées entre les deux séries étaient reproductibles. Conclusion : le débit sanguin pulmonaire périphérique est mesurable de façon reproductible et est corrélé avec la sévérité de la FPI.

Important changes have been proposed by expert groups for the management of mild asthma and chronic obstructive pulmonary disease (COPD): for safety reasons, short-acting beta-2 agonists (SABA) are no longer recommended as « reliever therapy » in mild asthma, and should be replaced by symptom-driven inhaled corticosteroids, alone or combined with a beta-2 agonist ; for COPD patients recommendations as to use of inhaled corticosteroids have been redefined. New therapeutic options for idiopathic pulmonary fibrosis are being evaluated: recombinant human pentraxin 2 may become a new therapeutic option among the existing specific treatments (pirfenidone, nintedanib). These novelties are discussed in this review of the recent medical literature.D’importantes modifications ont été proposées par les sociétés savantes dans la prise en charge des sujets asthmatiques légers et des sujets souffrant de bronchopneumopathie chronique obstructive (BPCO) : abandon des bêta-2 agonistes à action courte (BAAC) en traitement de secours pour l’asthme pour des raisons de sécurité, en privilégiant le rôle des corticostéroïdes inhalés seuls ou associés à un bêta-2 mimétique chez des sujets symptomatiques, et redéfinition de la place des corticostéroïdes inhalés lors de BPCO. Dans la fibrose pulmonaire idiopathique, la pentraxine-2 humaine recombinante se profile comme une nouvelle option thérapeutique possible, venant compléter la gamme des traitements spécifiques (pirfénidone, nintédanib). Ces nouveautés sont discutées dans cette revue de la littérature récente.

Idiopathic pulmonary fibrosis (IPF) is a progressive and fatal disease without therapeutic options. The development of new therapeutic strategies for the disease is needed. IPF is characterized by myofibroblast accumulation and collagen overproduction. Transforming growth factor-β1 (TGF-β1) is a key cytokine activating these pathological processes. Heat shock proteins (HSPs) are crucial regulators and promote TGF-β1 activity. Among them, HSP27 is overexpressed in animal models and in the lung of patients with IPF. HSP27 activates pro-fibrotic mechanisms and therefore may represents a potential target. Strategies aiming to inhibit HSP27 might pave the way towards new treatment options for patients with IPF.

D'importantes modifications ont été proposées par les sociétés savantes dans la prise en charge des sujets asthmatiques légers et des sujets souffrant de bronchopneumopathie chronique obstructive (BPCO) : abandon des bêta-2 agonistes à action courte (BAAC) en traitement de secours pour l'asthme pour des raisons de sécurité, en privilégiant le rôle des corticostéroïdes inhalés seuls ou associés à un bêta-2 mimétique chez des sujets symptomatiques, et redéfinition de la place des corticostéroïdes inhalés lors de BPCO. Dans la fibrose pulmonaire idiopathique, la pentraxine-2 humaine recombinante se profile comme une nouvelle option thérapeutique possible, venant compléter la gamme des traitements spécifiques (pirfénidone, nintédanib). Ces nouveautés sont discutées dans cette revue de la littérature récente.

Nintedanib (NTD) is an antifibrotic drug that decreases the decline of the respiratory function of patients suffering from idiopathic pulmonary fibrosis (IPF). NTD exerts antifibrotic effects by inhibiting cellular activity of various tyrosine kinase proteins. It also prevents the activity of the human recombinant CSF-1R protein that is expressed by myeloid cells, especially macrophages. Resident cells or différentiated from circulating monocytes, the macrophages contribute to IPF by secreting several proinflammatory and profibrotic cytokines. In this context, the first objective of my thesis was to determine, if by inhibiting CSF-1R activity, NTD could alter the functions of human macrophages differentiated from blood monocytes (MoDM) by CSF-1 (also called M-CSF). Our results demonstrate that NTD blocks the phosphorylation and the activity of CSF-1R in CSF-1-activated MoDM at very low concentrations that are similar to those inhibiting the human recombinant protein. By preventing CSF-1R activity, NTD reduces cell adhesion and expression of the profibrotic chemokine CCL2. Moreover, by blocking the activity of CSF1R and that of other molecular targets, NTD modulates the polarization of CSF-1-activated MoDM. It reduces the production of various interleukins and chemokines by M1 MoDM and prevents the membrane expression of M2 markers. Our results notably demonstrate that NTD significantly inhibits the expression of CXCL13, a prognostic biomarker of IPF. This chemokine is expressed in pulmonary tissues but its cellular origin is unclear. The second objective of my thesis was thus to characterize CXCL13 expression in pulmonary macrophages. Our results demonstrate that CXCL13 is expressed by alveolar macrophages in patients with IPF. In vitro, CXCL13 expression in activated alveolar macrophages and MoDM is controlled by TNF-α and IL-10 through the activation of the NF-κB and JAK/STAT pathways, respectively. Moreover, we demonstrate that seric concentrations of TNF-α, IL-10 and CXCL13 are correlated in patients with IPF. Diffuse interstitial lung disease (ILD) is one of the most frequent extra-articular manifestation of rheumatoid arthritis (RA) and is an important factor of morbidity and mortality. However, the physiopathological mechanisms underlying RA-associated ILD remain poorly understood, and disease management is difficult in the absence of effective treatments and international guidelines. The recent identification of genetic variants and mutations similar to those observed in idiopathic pulmonary fibrosis (IPF), a disease affecting exclusively the lung, provides new insights into the understanding of RA-associated ILD. Furthermore, new antifibrotic drugs approved for the treatment of IPF, including pirfenidone and nintedanib, could also prove to be effective for RA-associated ILD. Studies are ongoing to confirm this hypothesis.La pneumopathie interstitielle diffuse compte parmi les manifestations extra-articulaires les plus fréquentes de la polyarthrite rhumatoïde (PR), et elle est un facteur important de morbidité et de mortalité. Elle reste toutefois mal comprise du point de vue physiopathologique et sa prise en charge est difficile, faute de traitements efficaces et de directives internationales. L’identification récente de variants génétiques et de mutations similaires à ceux observés dans la fibrose pulmonaire idiopathique (FPI), une maladie touchant exclusivement le poumon, offre de nouvelles perspectives de compréhension de la pneumopathie interstitielle associée à la PR. Par ailleurs, les nouveaux traitements antifibrotiques disponibles pour la FPI (pirfénidone et nintédanib) pourraient s’avérer aussi utiles dans la pneumopathie interstitielle associée à la PR. Des études sont en cours pour le déterminer.

Anti-PD1 immunotherapies have become an essential treatment for bronchial cancer. According to published studies, PD1 and PD-L1 inhibitors have a better toxicity profile than chemotherapy. Nevertheless, some immune related toxicities can be potentially severe, such as induced interstitial lung disease (ILD). Currently, ILD patients are excluded from clinical trials using immunotherapy in lung cancer. IPF is the most frequent and severe form of ILD. Lung cancer represents a major complication of this disease and to date few data exist on the safety of immunotherapy in this context. We report 3 cases of IPF with lung cancer treated by nivolumab. All had a clinically mild to moderate IPF. The patients had received at least one line of chemotherapy before nivolumab and had progressive, metastatic lung cancer. Two patients experienced rapid cancer progression without immune toxicities. The third had a partial response but developed grade III immune colitis that led to discontinuation of the treatment. None developed lung toxicity or worsening of IPF on CT during follow-up, and death was always related to progression of the cancer. In our series of three patients with IPF, nivolumab was well tolerated with regard to their pulmonary condition. As inflammation and autoimmunity are probably marginal mechanisms in the pathogenesis of IPF, we do not believe that the presence of IPF should definitely disqualify potential candidates for treatment with nivolumab. Decisions should be taken, case-by-case, in selected patients without severe IPF and with no evidence of autoimmunity. In view of the epidemiology of lung cancer in IPF and the critical role of immunotherapy in the management of lung cancer, studies of prospective cohorts are urgently needed in this population.

In 2015, the International Society for Heart and Lung Transplantation (ISHLT) published a consensus document for the selection of lung transplant candidates. In the absence of recent French recommendations, this guideline is useful in order to send lung transplant candidates to the transplantation centers and to list them for lung transplantation at the right time.The main indications for lung transplantation in adults are COPD and emphysema, idiopathic pulmonary fibrosis and interstitial diseases, cystic fibrosis and pulmonary arterial hypertension (PAH). The specific indications for each underlying disease as well as the general contraindications have been reviewed in 2015 by the ISHLT. For cystic fibrosis, the main factors are forced expiratory volume in one second, 6-MWD, PAH and clinical deterioration characterized by increased frequency of exacerbations; for emphysema progressive disease, the BODE score, hypercapnia and FEV1; for PAH progressive disease or the need of specific intravenous therapy and NYHA classification. Finally, the diagnosis of fibrosing interstitial lung disease is usually a sufficient indication for lung transplantation assessment.These new recommendations, close to French practices, help clinicians to find the right time for referral of patients to transplantation centers. This is crucial for the prognosis of lung transplantation.

Idiopathic pulmonary fibrosis (IPF) is a rare disorder of unknown origin, which is associated with a high mortality and whose incidence has been increasing for several years. Nowadays there are two anti-fibrotic therapies (pirfenidone - nintedanib) known to reduce significantly the decline in respiratory function tests of patients suffering from this condition. The only curative therapeutic option remains the pulmonary transplantation whose accessibility remains limited. Pulmonary rehabilitation is also central in the treatment of patients. A major challenge for patients remains early and aggressive management to reduce as early as possible the evolution towards severe pulmonary fibrosis.La fibrose pulmonaire idiopathique (FPI) est une maladie rare d’étiopathogénie encore mal connue et d’incidence croissante depuis plusieurs années. La mise récente sur le marché de deux traitements anti-fibrotiques (pirfénidone – nintédanib) a permis de réduire, de manière significative, le déclin de la fonction respiratoire des patients souffrant de cette pathologie. La seule option thérapeutique à visée curative est la transplantation pulmonaire ont l’accessibilité reste limitée. La revalidation pulmonaire est, quant à elle, également centrale dans la prise en charge. L’enjeu majeur pour ces patients est une prise en charge précoce et agressive afin de limiter l’évolution de la fibrose pulmonaire.

We chose, for this update on novelties in treatments for pulmonary disorders in 2016, to focus on advances in 3 pulmonary disorders : chronic obstructive pulmonary disease (COPD), asthma and IPF. For COPD, the contribution of inhaled glucocorticosteroids and roflumilast are revisited and the benefits obtained through endoscopic volume reduction procedures are clarified. In asthma and IPF, new molecules offer new hopes : a bettre control of exacerbations in asthma and a stabilisation or slowing of disease progression in IPF.Nous avons choisi, pour cette mise au point sur les nouveautés en pneumologie en 2016, de commenter les acquisitions thérapeutiques les plus récentes dans 3 pathologies pulmonaires : la bronchopneumopathie chronique obstructive (BPCO), l’asthme, et la fibrose pulmonaire idiopathique (FPI). Dans la BPCO, la place des corticostéroïdes inhalés et celle du roflumilast sont revisitées et les bénéfices de la réduction de volume par voie endoscopique interventionnelle se précisent. Dans l’asthme et dans la FPI, de nouvelles molécules offrent de nouveaux espoirs : meilleur contrôle des exacerbations dans l’asthme et stabilisation ou ralentissement du déclin fonctionnel dans la FPI.

New techniques of DNA sequences allow to discover genetics mutations involved in familial pulmonary fibrosis. Among them, the PARN (Poly[A]-specific ribonuclease) mutation. Herein, we report the case of one patient who has pulmonary fibrosis with PARN mutation and the experience of our patient care.

Idiopathic interstitial pneumonias comprise 8 clinicopathological entities, most of them with a chronic course and various prognosis. Idiopathic pulmonary fibrosis is the most frequent and most severe of these. Computed tomography has an important role for its diagnosis. It can identify the corresponding pathological pattern of usual interstitial pneumonia in about 50 percent of cases. It can suggest differential diagnosis in other cases, most frequently fibrosing nonspecific interstitial pneumonia and chronic hypersensitivity pneumonitis. Imaging features should be integrated to clinical and available pathologic data during multidisciplinary team meetings involving physicians with a good knowledge of interstitial diseases. Some cases may be unclassifiable, but these could later be reclassified as new data may occur or imaging features may change. Surgical lung biopsy is being less frequently performed and an emerging less invasive technique, lung cryobiopsy, is under evaluation. Pleuroparenchymal fibroelastosis is a distinct entity only recently described, with uncertain prevalence and prognosis that seems being quite often associated to another pattern of interstitial pneumonia.

Idiopathic pulmonary fibrosis usually develops after 60 years of age, especially in men, leading to progressive exercise dyspnea. Median survival is about 3 years after diagnosis. This disease emerged in the middle of the last century and is linked to smoking. Mutations of genes encoding surfactant proteins and the telomerase complex may occasionally be present. International diagnostic criteria based on histopathology and computed tomography (CT) define the diagnosis as definite, probable or possible. Recent treatments (pirfenidone and nintedanib) have proven beneficial. Therapeutic advances warrant earlier diagnosis, based on Velcro crackles on pulmonary auscultation or interstitial opacities on CT screening for lung cancer.

Idiopathic pulmonary fibrosis (IPF) is one of the multiple pathologies included in the large family of diffuse interstitial parenchymal lung diseases (IPD). The latter represent a large group of about 200 different diseases, most of which are orphan diseases. Recently, some new therapeutic options have appeared that require an early and accurate diagnosis of pulmonary fibrosis.
