Subarachnoid hemorrhage (SAH) is a rare but very serious condition, mainly affecting young women. In addition to the neurological complications well described in the literature, other medical complications affecting other organs may arise, increasing the vulnerability of these patients. These extra-neurological manifestations, the result of catecholaminergic stress and the systemic inflammatory response, may be cardiovascular, pulmonary or metabolic in nature. Indeed, with advances in the endovascular and surgical management of intracranial aneurysms, these extra-neurological complications will have an increasingly important impact on the outcome of patients with SAH. Giant intracranial aneurysms (GIAs) are rare and heterogeneous lesions with complex vascular anatomy and represent almost 5% of intracranial aneurysms.

La découverte d'anévrysmes intracrâniens non rompus (ANR) est de plus en plus fréquente, du fait de l'évolution des techniques d'imageries non invasives comme l'angioscanner ou l'angio-IRM. L'ANR est une vasculopathie acquise, dont la prévalence est estimée à 3 %, et qui peut être favorisée par certaines pathologies (syndrome de Marfan, polykystose rénale, etc.). Sa découverte est, dans la majorité des cas, fortuite. Le risque hémorragique est faible (< 1 %/an) et le choix de traitement (conservateur, chirurgical ou endovasculaire) dépendra de la confrontation entre l'histoire naturelle de l'ANR, propre à chaque patient (autrement dit le risque de rupture), et le risque du traitement prophylactique, qu'il soit endovasculaire ou microchirurgical. Abstract: We report a case of polyarteritis nodosa revealed by intracranial haemorrhage. A 40-year-old woman presented two episodes of cerebral haemorrhage twelve days apart, the second due to an aneurysm rupture. The diagnosis of polyarteritis nodosa (PAN) was based on the following criteria: histological aneurysm examination, angiography suggesting PAN with cerebral, renal and splenic localizations, loss of weight and cutaneous nodules. Cerebral haemorrhage in PAN is rare and exceptionally the presenting feature of the disease. [Copyright &y& Elsevier]

The diagnosis of nontraumatic intracranial hemorrhage is currently made by computed tomography and is rarely problematic. The causes of bleeding are very numerous. It is important to determine the cause of the hemorrhage promptly, because there may be a recurrence of bleeding. Guidelines for radiologists are proposed and discussed here. [ABSTRACT FROM AUTHOR]

Abstract: Cerebral hemorrhage is usually associated to many cardiac disorders, mimicking acute coronary syndrome. We relate a case of a postmenopausal woman presenting at emergency room for acute coronary syndrome and whose evaluation revealed a subarachnoidal hemorrhage, normal coronary arteries, and a typical Tako Tsubo aspect on echocardiography with apical ballooning and hyperkinesis with basal hypokinesis. Abstract: Although the pathogenesis of cerebral aneurysms has been studied intensively, it is yet poorly understood. However, a genetic predisposition to this pathology has been often suspected. We describe a patient with both intracranial aneurysm and Angelman syndrome. Angelman syndrome is characterized by severe mental retardation, inappropriate laughter, absent speech, dysmorphic facial features and seizures. It is due to genetic abnormalities of chromosome 15. Cerebral aneurysms are sometimes associated with inherited diseases like autosomal dominant polycystic kidney disease. Moreover several candidate genes have been analysed, to search for genetic variants which might be associated with the occurence of intracranial aneurysms. Our question is: is the association described in our observation fortuitous or do these diseases share a same genetic predisposition? Our observation also supports the hypothesis of a genetic participation in the genesis of cerebral aneurysms. [Copyright &y& Elsevier]

Objectives: To measure the variability in choosing treatment options in different clinical situations involving intracranial aneurysms., Materials and Methods: A questionnaire regarding 25 clinical cases was presented via visual projection, to attendees of the Congrès de la Société française de neurochirurgie et de la Société de langue française de neurochirurgie held in Toulouse from May 9th to May 12th, 2012. The audience (n=59) consisted of 58 neurosurgeons and one neuroradiologist. A minority of them (29.2%) stated that they specialized in vascular neurosurgery. The questionnaire dealt with 25 illustrative clinical cases, in which age, gender, and clinical context were provided along with the corresponding angiographic image of the aneurysm. The questionnaire asked whether the physician would have proposed treatment, and if so, which type of treatment (clip, coil or other). In addition, the physician had to indicate, on a scale of one to ten, the degree of confidence they had in their decision., Results: For one-third of the cases, there was at least 10% of the respondents who opted for a decision opposite to the one of the majority. For 41.7% of the cases, the proportion of respondents who opted for the clip was not significantly different from the proportion of those who opted for the coil., Conclusion: Even in a relatively homogenous group of physicians, there were significantly diverging opinions regarding the management of cerebral aneurysms.
