Les traitements actuels de l'uvéite associée à l'arthrite juvénile idiopathique (U-AJI) et de l'uvéite à anticorps antinucléaires (ANA) positifs incluent les collyres corticoïdes, le méthotrexate (MTX) ou les antirhumatismaux biologiques modificateurs de la maladie (bDMARDs). Cependant, la rémission durable est un objectif difficile à atteindre. Le baricitinib, qui inhibe la signalisation JAK-STAT, pourrait cibler plusieurs voies cytokiniques associées à l'U-AJI et à l'uvéite à ANA positifs. Évaluer l'efficacité et la sécurité du baricitinib chez des patients pédiatriques atteints d'U-AJI active ou d'uvéite à ANA positifs ayant une réponse inadéquate au MTX ou aux bDMARDs et recevant une dose stable de corticoïdes sous forme de collyre. Cet essai de phase 3 en ouvert, contrôlé par un bras actif, a été mené dans 23 centres auprès d'enfants âgés de 2 à < 18 ans. Les patients ont reçu une dose quotidienne de baricitinib par voie orale (2 à < 9 ans : 2 mg, 9 à < 18 ans : 4 mg). Un groupe de référence a reçu de l'adalimumab (20 ou 40 mg selon le poids) administré par voie sous-cutanée toutes les 2 semaines. Le critère d'efficacité principal était la proportion de patients répondant au traitement à la semaine 24, défini selon les critères du SUN (Standardization of Uveitis Nomenclature) comme une diminution en 2 étapes du niveau d'inflammation (cellules de la chambre antérieure) ou une diminution atteignant zéro jusqu'à la semaine 24 dans l'œil le plus sévèrement atteint à l'inclusion. Une analyse bayésienne a été effectuée pour le critère principal, selon des critères de réussite prédéfinis (probabilité postérieure que le taux de réponse au traitement dépasse 57 % est d'au moins 80 %). Les évaluations de sécurité du baricitinib chez les patients pédiatriques atteints d'U-AJI ou d'uvéite à ANA positifs incluaient les événements indésirables apparus sous traitement (EIAT). Sur les 24 patients ayant reçu le baricitinib, 14 étaient des filles ; l'âge moyen était de 11,6 ans. Cinq patients (toutes des filles) ont reçu de l'adalimumab ; l'âge moyen était de 6,6 ans. Le critère principal n'a pas été atteint. Dans le groupe du baricitinib, 33 % des patients ont obtenu une réponse à la semaine 24, soit une probabilité postérieure de 1,03 % d'un taux de réponse > 57 %. Les données de sécurité étaient cohérentes avec le profil de sécurité établi dans le cadre d'autres indications du baricitinib chez des patients pédiatriques et adultes. Dans le groupe du baricitinib, 83,3 % des patients ont présenté au moins 1 EIAT, dont 41,7 % d'intensité légère, 29,2 % d'intensité modérée et 12,5 % d'intensité sévère. Le critère principal de l'étude n'a pas été atteint. Ces données fournissent des informations supplémentaires concernant le traitement des enfants atteints d'U-AJI ne répondant pas au MTX ni aux bDMARDs. Peripheral arterial disease of the lower limbs (PAD) is a widespread disease whose diagnosis and treatment are still inadequate, despite several available current national and international recommendations. Screening for PAD is recommended in patients presenting with lower limb symptoms (claudication) and in those at risk. For all patients with PAD, initial management measures include treatment of cardiovascular risk factors, lifestyle modification, exercise training and antithrombotic therapy, at least for any symptomatic PAD. Revascularization is discussed on a case-by-case basis, depending on the stage of the disease, in a multidisciplinary setting. Pelvic congestion syndrome is a debilitating condition that is often under-diagnosed and under-treated, defined by chronic pelvic pain in the presence of pelvic varicose veins in women or, more rarely, in men. The differential diagnosis of chronic abdominal pain is vast and often leads to lengthy and costly diagnostic procedures. Conservative treatment is often insufficient, and embolization of pelvic varices is the treatment of choice to improve symptoms. Identification of subjects at increased cardiovascular risk (CV) using traditional risk calculators is established.
