Dental trauma is one of the most important reasons for emergency dental consultation. They can be of many kinds and can have many consequences on the dental organ as well as on the supporting tissues. Because of its position, the maxillary central incisor represents the tooth most often affected by these traumas. The first part of this document describes the different types of possible trauma concerning the maxillary central incisor, their epidemiology as well as their management in the emergency. The second part of this document details the different possible orthodontic treatments (and their techniques) according to the type of trauma of the maxillary central incisor.; Les traumatismes dentaires constituent un des motifs les plus importants de consultation en urgence dentaire. Ils peuvent être de plusieurs natures et peuvent avoir de nombreuses conséquences sur l’organe dentaire ainsi que sur les tissus de soutien. Du fait de sa positon, l’incisive centrale maxillaire représente la dent la plus souvent atteinte par ces traumatismes. La première partie de ce document expose les différents types de traumatismes possibles concernant l’incisive centrale maxillaire, leur épidémiologie ainsi que leur prise en charge dans l’urgence. La seconde partie de ce document détaille les différentes prises en charge orthodontiques possibles (et leurs techniques) en fonction du type de traumatisme de l’incisive centrale maxillaire.

Although infrequent, the orthodontic management of impacted second molars is essential: this malocclusion thus holds the highest score of the orthodontic treatment need index.The first part introduces the subject by explaining the risks inherent to the second impacted molars.The second part defines the subject and discusses the possible etiologies as well as the prevalence of second molars impacted.The third part details the different therapeutic alternatives, whose multidisciplinary care involves orthodontists but also dentists. Impaction of the second molar can be treated by second molar extraction and substitution by the third. The opening of the second molar after extraction of the second premolar can also be considered. Auto transplantation of the second molar also provides a solution, as does surgical exposure and orthodontic uprighting.; Bien que peu fréquente, la gestion orthodontique des deuxièmes molaires incluses ou enclavées est primordiale : cette malocclusion détient ainsi le plus haut score de l’index de besoin en traitement orthodontique.La première partie introduit le sujet en expliquant les risques inhérents aux deuxièmes molaires incluses ou enclavées.La deuxième partie définit le sujet et évoque les étiologies possibles ainsi que la prévalence des deuxièmes molaires incluses ou enclavées. La troisième partie détaille les différentes alternatives thérapeutiques, dont la prise en charge multidisciplinaire implique orthodontiste mais aussi chirurgien-dentiste. L’inclusion ou l’enclavement de la deuxième molaire peut être traité par extraction de deuxième molaire et substitution par la troisième. Le désenclavement de la deuxième molaire après extraction de la seconde prémolaire peut aussi être envisagé. L’auto transplantation de la deuxième molaire apporte aussi une solution, tout comme l’exposition chirurgicale et le redressement orthodontique.

Dental autotransplantations can be defined as the displacement of a functional dental organ from a donor site to a recipient site in the same patient. Intended to compensate an agenesis, to replace an extracted tooth, indications of autotransplantations are many and varied.The first part reminds the autotransplantations general principles, on the semantical and historical level as well as the technical one.The second part deals with the contributions of orthodontics to autotransplantations. The orthodontist has a determining position to decide that there is an indication of autotransplantation, but also works in collaboration with the dentist to ensure that autotransplantation takes place in the best possible conditions. Finally, orthodontics would have an influence on the prognosis of autotransplantations.The third part details the contributions of autotransplantations to orthodontics, opening the therapeutic alternatives field. Each autotransplantation indication is developed as well as the advantages of the technique compared to the different therapeutic alternatives.Finally, a fourth part discusses the prognosis of the technique by studying the success and survival rates modulated by risk and predictability factors, but also by developing future techniques which could improve the prognosis.; Les autotransplantations dentaires peuvent se définir comme le déplacement d’un organe dentaire fonctionnel d’un site donneur vers un site receveur chez le même patient. Destinées à suppléer une agénésie, à remplacer une dent extraite, les indications des autotransplantations sont multiples et variées. La première partie rappelle les principes généraux sur les autotransplantations, tant au niveau sémantique et historique que technique. La deuxième partie traite des apports de l’orthodontie aux autotransplantations. L’orthodontiste a une place déterminante pour poser l’indication d’autotransplantation, mais agit également en collaboration avec le chirurgien-dentiste pour que l’autotransplantation se déroule dans les meilleures conditions possibles. Enfin, l’orthodontie aurait une influence sur le pronostic des autotransplantations. La troisième partie détaille les apports des autotransplantations à l’orthodontie, ouvrant le champ des alternatives thérapeutiques. Chaque indication d’autotransplantation est développée ainsi que les avantages de la technique par rapport aux différentes alternatives thérapeutiques. Enfin, une quatrième partie évoque le pronostic de la technique en étudiant les taux de réussite et de survie modulé par des facteurs de risques et de prévisibilité, mais aussi en développant les techniques d’avenir susceptibles d’améliorer le pronostic.

The prosthetic project represents the essential element of successful treatment. Nevertheless, some clinical situations can not be solved without changing the dental positions. Thus, orthodontic treatment becomes essential to realize the prosthetic project. It fits totally in the minimal invasive dentistry by improving the dental position even if it leads to longer treatment. It also makes integration of the prosthesis better and longer. Reciprocally, some cases need firstly prosthetic treatment before considering orthodontic treatment. This work aims to understand the complementary of the two disciplines through the contribution of each other. The first part details the role of orthodontics in preprosthetic therapy. Finally, the second part defines the place of the prosthesis treatment and its interest in orthodontic treatment.; Le succès du projet prothétique constitue l’élément indispensable de la réussite du traitement. Néanmoins, certaines situations cliniques ne peuvent se résoudre sans modification des positions dentaires. Le traitement orthodontique s’avère alors indispensable pour mener à bien la prise en charge prothétique. Il s'inscrit dans le concept d'une dentisterie conservatrice en améliorant la position et la répartition des piliers dentaires même s’il augmente la durée globale du traitement. Il permet également d'optimiser l'intégration de la prothèse ainsi que son pronostic. Réciproquement, la mise en œuvre du traitement orthodontique dans certains cas ne peut s’envisager qu’après passage par une thérapeutique prothétique. Ce travail a pour objectif de comprendre la complémentarité des 2 disciplines que sont l’orthodontie et la prothèse à travers l’apport de chacune au service de l’autre. La 1re partie détaille le rôle de l’orthodontie dans la prise en charge préprothétique. Enfin, la 2e partie aborde la place de la prothèse et son intérêt dans le traitement l’orthodontique.

Impacted canine is frequently observed in an orthodontic practice and makes the treatment harder to apply. Despite a frequently discreet symptomatology, an early diagnosis should be made. Its purpose is to allow the canine to evolve on the dental arcade by using the right therapeutics at the right time. Therefore, it is recommended to consult a practitioner by the age of 10 to perform an examination and begin, if needed, a preventive treatment in the mixed dentition. If the preventive therapeutics was not effective or delayed, it will need a multidisciplinary approach. With permanent dentition, only surgery could be used or it could be associated with prosthesis and/or orthodontia. Surgery remains the preferential method and is supported by periodontics. Nevertheless, even with anticipation, technological and knowledge development, failures and complications still happen.; La canine incluse est fréquemment rencontrée au cabinet d’orthodontie ; elle complique le traitement. Malgré une symptomatologie souvent discrète, le diagnostic doit être précoce. L’objectif est de permettre l’évolution de la canine sur l’arcade en mettant en place au moment opportun des thérapeutiques adaptées. Pour pallier cette problématique, il est recommandé de consulter vers 10 ans un orthodontiste pour faire un bilan et débuter si besoin un traitement préventif en denture mixte. Si la thérapeutique préventive n’a pas été suffisante ou si la prise en charge est trop tardive, on aura recours à une approche pluridisciplinaire. En denture permanente, les méthodes pourront être exclusivement chirurgicales ou associées à la prothèse et/ou l’orthodontie. La thérapeutique ortho-chirurgicale est l’option préférentielle, elle se fait en collaboration avec la parodontie. Malgré une anticipation, un développement des nouvelles technologies et des connaissances, les échecs et les complications restent encore présents.
