Abstract

Après transplantation intestinale ou multiviscérale (TIMV) pédiatrique, la disparité de taille entre l’abdomen du receveur et le greffon expose aux risques de syndrome compartimental abdominal (SCA). Nous rapportons notre expérience de la fermeture pariétale différée progressive (FD) chez ces enfants. Entre 04/1993 et 03/2009, 62 TIMV ont été réalisées chez 57 enfants. En cas de tension pariétale abdominale excessive pour une fermeture primaire (FP), une FD a été réalisée avec prothèse de Silastic® et pansement occlusif aspiratif. Nous avons comparé les transplantations avec FD (23) et avec FP (39). Les indications de transplantation, l’état pré-opératoire, l’âge à la greffe (médian 2,1 ans), le rapport de poids donneur / receveur, le taux de réduction de l’intestin et/ou du foie, et l’incidence des complications pariétales, étaient similaires dans les 2 groupes. Intubation post-opératoire, séjour aux soins intensifs, et hospitalisation, étaient plus longs après FD. Deux décès par SCA sont survenus après FP, aucun après FD. Depuis 2000, la survie des patients à un an est de 75 % après TI, et 57 % vs 75 % après TC avec FP vs FD (p = 0,06). La fermeture pariétale différée progressive après TIMV est une technique sûre, et utile surtout pour les enfants dont la cavité abdominale est hypoplasique. [ABSTRACT FROM AUTHOR]
