Shift work occupies precious time for family and social life. The aim of this review was to systematically assess the state of research on the impact of shift work on work-fam-ily conflict. A systematic literature search was conducted in PubMed and EBSCO to identify studies published between 1990 and 2017. In the end, 36 articles met the inclusion criteria and were considered in this review. Shift workers show higher levels of work-family conflict in comparison to workers in regular day schedules. Different shift types and a large variation of shift characteristics have been studied. Results point to a higher work-family conflict especially among night shift workers and those working in a shift schedule, including weekend work. Research testing for causality is missing.

4. « Préparer » la nuit de travail en 12 heures au début du poste pour gérer charge de travail et vigilance : le cas des infirmiers d'un service de réanimation.

12-hour shifts are increasingly common in the hospital sector. To cope with the risks induced by these schedules, caregivers use regulation strategies to manage their decrease in alertness and to ensure the quality of care. The situation studied is an intensive care unit. The data collection concerns nine 12-hour night shifts and 24 nurses. At four times during the night, they complete 3 questionnaires to assess their alertness level, to evaluate the quality of their work and to indicate their regulation strategies. Then, they are interviewed about their answers in self-confrontation interview. The results show that the alertness of nurses decreases while their quality perception remains very positive. The nurses maintain the quality of care by constructing a "working scenario" for the night. This scenario allows them to improve the reliability of care, improve care and optimize their mental and physical energy. [ABSTRACT FROM AUTHOR]

INTRODUCTION: this study has been performed to determine the influence of rotating shift work on physical working capacity of Tunisian nurses and to design recommendations to managers so that they implement effective preventive measures. METHODS: it is a cross-sectional design using a standardized questionnaire and many physical capacity tests on a representative sample of 1181 nurses and nursing assistants from two university hospital centers of the school of Medicine of Monastir located in the Tunisian Sahel. 293 participants have been recruited by stratified random sampling according to gender and departments. Maximum Grip strength, 30s sit-to-stand test, one leg test, Fingertip-to-Floor test, Saltsa test and peak expiratory flow were used to assess physical capacity. Work ability was assessed through the workability index.

The Organisational and Human Factors service of the Rail Traffic department of SNCF Network carried out a study which joined the Medical Service of SNCF to diagnose the impacts of shift and night work on the staff of signal boxes. This work combined a study of the scientific literature, a field analysis, a chronobiological study and a medical one. It resulted in a set of recommendations towards agents in shift work and their managers. A support system to the implementation of these recommendations has been established, in particular to incorporate the recommendations into the design of shift schedules and evaluate the effects of changes. Shift handovers contribute to the continuity of the work process and the reliability of information relays between two teams. However, the formalization and organization of these transmission periods are more or less devised in response to each specific work situation. Many studies show that the information content and the way this information is relayed are the result of a collective activity in which experience plays an important role. Our article approaches the relations between experience and shift handovers through three studies from different sectors (hospital, aeronautical, and railroad). In our study, experience refers to the workers’ way of working (especially night work), but also to knowledge relative to the functioning of the work teams. The results show that changes in shift handover activity occurred to cope with the needs of the department and work team and that these changes should be taken into account in efforts to improve shift handover efficiency.

9. Travail de nuit, travail posté : facteur de risque du cancer du sein ?

Résumé Objectif L’objectif de cette revue était de déterminer le lien entre le travail de nuit/posté et le cancer du sein. Méthodes Les articles analysés ont été extraits de la base de données PUBMED de 1996 à 2015. Les mots clés utilisés ont été : « breast cancer risk », « night work » et « shift work ». Au total, 25 articles ont été retenus. Résultats Les travailleurs de nuit/postés sont plus à risque de développer un cancer du sein. Le risque relatif est faible, variant selon les méta-analyses entre 1,09 (IC95 % : 1,02–1,20) et 1,48 (IC95 % : 1,36–1,61). Cependant, cette augmentation de risque n’est pas retrouvée par certaines études de cohorte et cas-témoins. La perturbation du rythme circadien, responsable d’une dysrégulation de la synthèse de la mélatonine serait un des mécanismes impliqués dans l’augmentation de ce risque. Le statut hormonal des femmes travaillant de nuit/postées, leur origine géographique, leur mode de vie, ainsi que la carence en vitamine D apparaissent comme d’autres mécanismes potentiellement responsables de l’augmentation du risque de cancer dans cette population professionnelle. Il semble par ailleurs y avoir une relation de type dose–effet avec une majoration du risque avec le nombre d’années de travail nocturne/posté. Conclusion Le travail de nuit/posté est associé à une augmentation modérée du risque de cancer du sein, surtout chez les femmes ayant travaillé plus de 20 ans. Des recommandations concernant le suivi mammaire pour cette population professionnelle pourraient être diffusées. Le bénéfice de la supplémentation en mélatonine reste à évaluer. Objective The aim of this review was to determine the link between night/shift work and breast cancer. Methods The analysed articles were taken from the PUBMED database between 1996 and 2015. The keywords used were “breast cancer risk”, “night work” and “shift work”. In total, 25 articles were selected. Results Night/shift workers are more at risk to develop a breast cancer (relative risk (RR) between 1.09; 95 % CI: 1.02–1.20 and 1.48; 95 % CI: 1.36–1.61 in the meta-analyses). However, this risk is not found by some cohort and case-control studies. The circadian rhythm disruption, responsible of disorderliness of melatonin secretion, could be one of the mechanisms involved in the increase of that risk. Hormonal status of night/shift workers, their geographic origin, their lifestyle and their vitamin D deficiency appear as other mechanisms potentially responsible for increased risk of cancer in this professional population. Moreover, a dose–effect connection may exist, with an increase of the risk with the number of years of night/shift work. Conclusion Night/shift work is associated with a moderate increased risk of breast cancer, especially among women who worked over 20 years. Recommendations concerning the breast monitoring in this population could be diffused. The benefit of melatonin supplementation remains to be assessed. [ABSTRACT FROM AUTHOR]

13. La relève : un support pour la conception de dispositifs de retour d'expérience intégrés et dynamiques ?

Serious accidents, which occurred in France in road tunnels in the 2000's, have revealed specific risks related to tunnels, along with safety management issues. The lessons learned analyses are one resource used by tunnel operators, supported by the French Tunnel Research Centre (CETU), to improve safety management. The question raised in this paper is how these processes are linked to the actual experience sharing processes and collective safety management. Based on careful analysis of the changeover briefings of tunnel control station operators, we aim to show what makes them a means for sharing feedback, managing safety and building a collective structure. We will thus highlight the strength of communication dynamics in driving a formal lessons learned approach, in order to design innovative safety management tools within a high-risk industrial environment. especially in the service sector, the flexibility of working times is a proven means for the companies to respond to fluctuations in demand. This can lead to non-occupational strain for the employees, if they are employed in periods that could be actually used for private affairs. It turns out that the preferably privately used periods depend on the social role of individual employees. This may result in temporal conflicts that affect the balance between work and private life. Based on the data collected in employee surveys working time preferences, a cluster analysis of social roles is presented in this paper. Shift changeover periods, when one team relieves another, contribute to achieving the continuity and reliability of shift work. This article considers first-line supervisory staff shift changeovers to be an ongoing activity carried out across successive shifts. The study looks at supervisory activity in the context of a final aircraft assembly line running on a 2x8 discontinuous schedule where changeovers alternate between shift changes where the two teams meet, allowing for oral and written exchanges, and changeovers where they do not meet and only written exchanges are used. The analysis focuses on the activities of writing and using written handovers throughout shift work (before, during, and after shift changeovers). It showed, firstly, that written handovers were prepared throughout the shift no matter what type of shift changeover would follow, but were systematic whenever the upcoming changeover was without meeting, and secondly, that written handovers about what the incoming operator had to do, and what the outgoing operator had done, acted as supports for and records of collective-activity coordination. These results allow us to see shift changeovers as a task that overarches other first-line supervisory tasks and as contributing to work coordination across shifts. Some principles are proposed for adjusting and optimising shift changeovers.

The aim of our study is to compare the effects of three different forms of time organization (traditional 9 to 5 schedule, 6-hour shifts, and 12-hour shifts) inside French prisons, and particularly on prison guard staff. Complementary means of investigation (questionnaire, work observations, statistical treatment) were used to attempt to evaluate the difference between the effect of working hours and of the actual work on the guards’ health. Staff who worked 12-hour shifts had more positive results than staff who worked 6-hour shifts. This was mainly due to the advantages in their private lives. At the same time, some signs of fatigue prevent us from conclusively favouring this type of working rhythm. Beyond the alternation of schedules, the actual work content also has an important impact on the workers’ health.

Abstract: The purpose of the study: The aim of the present study was to investigate relationship between shift work and the cardiovascular risk factors. Methods: A cross-sectional study was included 330 subjects worked in a company of electricity production in the Centre of Tunisia. The collection of data was based on a questionnaire, a clinical exam and biomarkers. Results: A total of 290 workers was participated in our study (128 shift workers and 162 daytime workers). A raised prevalence but not statistically significant of some factors of cardiovascular risks was found in the shift workers: obesity (25.8 % versus 17.9 %), smoking (44.5 % versus 39.5 %), impaired fasting glucose (11.7 % versus 9.9 %), hypertriglyceridemia (28.1 % versus 25.9 %) and hypercholesterolemia (14.8 % versus 12.4 %). Whereas the prevalence of the alcohol consumption (25.8 % versus 16.0 %) was significantly higher in the shift workers (p =0.04). Conclusion: The high prevalence of the cardiovascular risk factors, observed in our shift workers, justify the adoption of measures to protect this category of workers. [Copyright &y& Elsevier]

Previous Canadian epidemiologic studies have identified associations between occupations and prostate cancer risk, though evidence is limited. However, there are no well-established preventable risk factors for prostate cancer, which warrants the need for further investigation into occupational factors to strengthen existing evidence. This study uses occupation and prostate cancer information from a large surveillance cohort in Ontario that linked workers' compensation claim data to administrative health databases.Occupations were examined using the Occupational Disease Surveillance System (ODSS). ODSS included 1 231 177 male workers for the 1983 to 2015 period, whose records were linked to the Ontario Cancer Registry (OCR) in order to identify and follow up on prostate cancer diagnoses. Cox proportional hazard models were used to calculate age-adjusted hazard ratios and 95% CI to estimate the risk of prostate cancer by occupation group.A total of 34 997 prostate cancer cases were diagnosed among workers in ODSS. Overall, elevated prostate cancer risk was observed for men employed in management/ administration (HR 2.17, 95% CI = 1.98-2.38), teaching (HR 1.99, 95% CI = 1.79-2.21), transportation (HR 1.20, 95% CI = 1.16-1.24), construction (HR 1.09, 95% CI = 1.06-1.12), firefighting (HR 1.62, 95% CI = 1.47-1.78), and police work (HR 1.20, 95% CI = 1.10-1.32). Inconsistent findings were observed for clerical and farming occupations.Associations observed in white collar, construction, transportation, and protective services occupations were consistent with previous Canadian studies. Findings emphasize the need to assess job-specific exposures, sedentary behaviour, psychological stress, and shift work. Understanding specific occupational risk factors can lead to better understanding of prostate cancer etiology and improve prevention strategies.Des études épidémiologiques canadiennes ont permis d’établir des liens entre certaines professions et le risque de cancer de la prostate, mais avec des données probantes limitées. Il n’y a pas de facteurs de risque évitables bien établis pour le cancer de la prostate, ce qui rend nécessaire l’examen approfondi des facteurs professionnels pour consolider les données existantes. Cette étude utilise des données sur les professions et sur le cancer de la prostate provenant d’une importante cohorte de surveillance en Ontario pour laquelle les données des demandes d’indemnisation des accidentés du travail sont jumelées aux bases de données administratives sur la santé.Nous avons examiné les professions des 1 231 177 travailleurs de sexe masculin du Système de surveillance des maladies professionnelles entre 1983 et 2015 et nous avons jumelé ces travailleurs au Registre des cas de cancer de l’Ontario afin de détecter et suivre les cas de diagnostics de cancer de la prostate. Nous avons utilisé des modèles de risques proportionnels de Cox pour calculer les rapports de risques (RR) ajustés selon l’âge et des intervalles de confiance (IC) à 95 % pour estimer le risque de cancer de la prostate par groupe professionnel.Au total, 34 997 cas de cancer de la prostate ont été diagnostiqués parmi les travailleurs inscrits dans le Système de surveillance des maladies professionnelles. Dans l’ensemble, on a observé un risque élevé de cancer de la prostate chez les hommes travaillant dans les domaines de la gestion et de l’administration (RR = 2,17; IC à 95 % = 1,98 à 2,38), de l’enseignement (RR = 1,99; IC à 95 % = 1,79 à 2,21), du transport (RR = 1,20; IC à 95 % = 1,16 à 1,24), de la construction (RR = 1,09; IC à 95 % = 1,06 à 1,12), de la lutte contre les incendies (RR = 1,62; IC à 95 % = 1,47 à 1,78) et du travail policier (RR = 1,20; IC à 95 % = 1,10 à 1,32). Des résultats incohérents ont été observés en ce qui a trait aux emplois de bureau et à ceux du secteur agricole.Les associations observées dans les professions de cols blancs, de la construction, du transport et des services de protection concordent avec les résultats d’études canadiennes antérieures. Nos résultats mettent l’accent sur la nécessité d’évaluer les expositions spécifiques aux divers emplois, le comportement sédentaire, le stress psychologique et le travail par quarts. La compréhension de certains facteurs de risque professionnels permettrait de mieux comprendre l’étiologie du cancer de la prostate et d’améliorer les stratégies de prévention.This is the first large occupational surveillance study in Ontario that links workers’ compensation claims to administrative health data. We found increased risks of prostate cancer in white collar, transportation, construction, and protective services workers and mixed findings in clerical and farming workers. Findings are consistent with recent published Canadian studies on occupation and prostate cancer. Future studies need to address job-specific exposures and examine other factors of shift work, stress, sedentary behaviour, and screening patterns.Il s’agit de la première vaste étude de surveillance professionnelle en Ontario qui établit un lien entre les demandes d’indemnisation des accidentés du travail et des données administratives sur la santé. On a observé des risques accrus de cancer de la prostate chez les cols blancs et chez les travailleurs des secteurs du transport, de la construction et des services de protection, alors que les résultats sont mitigés en ce qui concerne les employés de bureau et les agriculteurs. Les résultats concordent avec ceux d’études canadiennes récentes sur la profession et le cancer de la prostate. Les futures études doivent porter sur les expositions spécifique aux divers emplois et analyser d’autres facteurs comme le travail par quarts, le stress, le comportement sédentaire et les tendances en matière de dépistage.

Introduction: Shift work can cause health problems and sleep disorders and therefore affect mental health. These psycho-affective disorders can, in return, worsen sleep disorders and thus establish a vicious circle. The aims of our study were to assess the frequency of sleep and psycho-affective disorders among paramedical personnel doing shift work and to screen them for obstructive sleep apnoea-hypopnoea syndrome (OSA)., Methods: It was a cross-sectional study carried out among paramedical staff at the University Hospital Center Mongi Slim La Marsa in Tunis. Three questionnaires translated into Arabic (the Berlin questionnaire, the Spiegel questionnaire and the Hospital Anxiety and Depression Scale [HAD]) were presented by the same investigator during the survey., Results: One hundred and fifty-eight paramedics were interviewed (46.2% were nurses, 23.4% were blue-collar workers, 19% were senior technicians and 11.4% were health care aides, midwives and physiotherapists). The average duration of shift work was 10.27 years, the average age was 36.48 years and there was a female prevalence of 70.9%. Sleep disorders were detected in 40.5% of the cases, OSA in 24%, anxiety in 53.2% and depression in 17.1%., Conclusion: Sleep and psycho-affective disorders are frequent among paramedical personnel undertaking shift work in hospitals. Este estudo teve como propósitoclassificar o cronótipo dos funcionários da equipe de enfermagem e identificar a presença de stress correlacionado ao cronótipo e turno de trabalho. Foi realizado em hospital privado da cidade de Limeira, estado de São Paulo, com 87 sujeitos, dos diferentes setores e turnos de trabalho. Os instrumentos utilizados foram o Inventário de Sintomas de Stress de Lipp e o Questionário de Indivíduos Matutinos e Vespertinos. Verificou-se que 74,7% dos sujeitos estavam adequados ao turno de trabalho; no entanto, 55,4% apresentavam sintomas de stress, demonstrando correlação estatisticamente significativa (Teste Exato de Fisher p=0,035). Verificou-se também stress nos sujeitos com maior tempo de serviço na instituição, com diferença significativa (Teste Exato de Fisher p=0,003). Concluiu-se que a maioria dos sujeitos estava adequada ao turno de trabalho, de acordo com seu cronótipo, porém mesmo nestes indivíduos o stress foi observado, levando à confirmação de dados obtidos em outras pesquisas quanto ao caráter estressante da profissão.

22. The impact of different shift work schedules on the levels of anxiety and stress in workers in a petrochemicals company

This study evaluated anxiety and stress in workers under different shift work conditions. The sample comprised 239 workers, with an average age of 42.6, standard deviation = 5.7 years, divided into fixed daytime working (n=52) and different working shifts (n=187). Documentation: Free and informed consent form; ID's; State-Trait Anxiety Inventory; Lipp's Stress Symptom Inventory for Adults. We used the t-test for independent samples, ANOVA, Pearson's correlation and the two-sample Comparison of proportions Test. Results showed that shift workers had higher State-Trait Anxiety scores than fixed daytime workers (t=-4.994; p=0.0001; t=-2.816; p=0.005, respectively). Both samples exhibited stress, but there were no statistically significant differences between the groups (t=-1.052; p=0.294). Shift work schedules caused more situational and dispositional anxiety, but did not significantly increase stress levels when compared to fixed daytime working.

Serious accidents, which occurred in France in road tunnels in the 2000’s, have revealed specific risks related to tunnels, along with safety management issues. The lessons learned analyses are one resource used by tunnel operators, supported by the French Tunnel Research Centre (CETU), to improve safety management. The question raised in this paper is how these processes are linked to the actual experience sharing processes and collective safety management. Based on careful analysis of the changeover briefings of tunnel control station operators, we aim to show what makes them a means for sharing feedback, managing safety and building a collective structure. We will thus highlight the strength of communication dynamics in driving a formal lessons learned approach, in order to design innovative safety management tools within a high-risk industrial environment., Les accidents graves survenus en France dans les tunnels routiers dans les années 2000 ont mis en évidence des risques spécifiques liés au contexte « tunnel » et des enjeux concernant la gestion de la sécurité. Les dispositifs de retour d’expérience sont un des moyens mobilisés par les exploitants de tunnel, accompagnés par le Centre d’Etude des Tunnels (CETU), pour améliorer leur gestion de la sécurité. En quoi ces dispositifs sont en lien avec les pratiques réelles de partage d’expérience et de gestion collective de la sécurité, telle est la question soulevée dans cette communication. Partant d’une analyse des communications de relève des opérateurs de supervision d’un poste de commande de tunnel, il s’agira de montrer comment les relèves sont un lieu de partage d’expérience, de gestion de la sécurité, et de construction du collectif. Il s’agira ainsi de mettre en relief comment les dynamiques de la relève peuvent alimenter les démarches de retour d’expérience formelles pour concevoir des dispositifs de gestion de la sécurité innovants dans des contextes de risques industriels importants.

Les préoccupations en santé publique révèlent le peu Notre étude vise à comparer les effets sur la santé de trois formes d’organisation temporelle (horaires traditionnels, factions de 6 heures et de 12 heures) au sein d’établissements pénitentiaires français, et plus particulièrement auprès des personnels de surveillance. Des moyens d’investigation complémentaires (questionnaire, observations du travail, traitement statistique) ont été mis en œuvre pour tenter de faire la part entre le poids des horaires et le poids du travail sur l’état de santé des agents. Si les personnels en factions de 12 heures présentent des résultats meilleurs que les personnels en 6 heures, c’est en particulier dû à des avantages dans la vie extra-professionnelle, bien que des signes certains de fatigue ne permettent pas de conclure significativement en faveur de telles modalités. Au-delà de l’alternance des horaires, le contenu du travail pèse de manière importante sur la santé des agents. The aim of our study is to compare the effects of three different forms of time organization (traditional 9 to 5 schedule, 6-hour shifts, and 12-hour shifts) inside French prisons, and particularly on prison guard staff. Complementary means of investigation (questionnaire, work observations, statistical treatment) were used to attempt to evaluate the difference between the effect of working hours and of the actual work on the guards’ health. Staff who worked 12-hour shifts had more positive results than staff who worked 6-hour shifts. This was mainly due to the advantages in their private lives. At the same time, some signs of fatigue prevent us from conclusively favouring this type of working rhythm. Beyond the alternation of schedules, the actual work content also has an important impact on the workers’ health. Nuestro estudio pretende comparar los efectos sobre la salud de tres formas de organización temporal (horarios tradicionales, turnos de seis horas y de doce horas) en establecimientos penitenciarios franceses, y particularmente en el personal de guardia. Se emplearon métodos de investigación complementarios (cuestionarios, observaciones del trabajo, análisis estadístico) para intentar diferenciar qué peso tienen respectivamente los horarios y el trabajo propiamente dicho, sobre el estado de salud de los guardianes de cárceles. El personal que trabaja en turnos de 12 horas presenta mejores resultados que el personal que trabaja en turnos de 6 horas. Este resultado se debe particularmente a las ventajas de este horario en la vida extra-profesional. Sin embargo, los signos claros de fatiga nos impiden estar a favor de tal modalidad. Más allá de la alternancia de turnos, el contenido del trabajo pesa de manera significativa sobre la salud de los guardianes de cárceles.

Introduction: While some studies have suggested associations between shift work and obesity, few have been population-based or considered multiple shift schedules. Since obesity is linked with several chronic health conditions, understanding which types of shift work influence obesity is important and additional work with more detailed exposure assessment of shift work is warranted., Methods: Using multivariate polytomous logistic regression, we investigated the associations between shift work (evening/night, rotating and other shift schedules) and overweight and obesity as measured by body mass index cross-sectionally among 1561 men. These men had previously participated as population controls in a prostate cancer case-control study conducted in northeastern Ontario from 1995 to 1999. We obtained information on work history (including shift work), height and weight from the existing self-reported questionnaire data., Results: We observed an association for ever (vs. never) having been employed in rotating shift work for both the overweight (OR [odds ratio] = 1.34; 95% CI [confidence interval]: 1.05-1.73) and obese (OR = 1.57; 95% CI: 1.12-2.21) groups. We also observed nonsignificant associations for ever (vs. never) having been employed in permanent evening/night shifts. In addition, we found a significant trend of increased risk for both overweight and obesity with increasing duration of rotating shift work., Conclusion: Both the positive association between rotating shift work and obesity and the suggested positive association for permanent evening/night shift work in this study are consistent with previous findings. Future population-based research that is able to build on our results while examining additional shift work characteristics will further clarify whether some shift patterns have a greater impact on obesity than others., Competing Interests: The authors have no conflicts of interest to declare.

- Abstract
Objective: The aim of this review was to determine the link between night/shift work and breast cancer., Methods: The analysed articles were taken from the PUBMED database between 1996 and 2015. The keywords used were "breast cancer risk", "night work" and "shift work". In total, 25 articles were selected., Results: Night/shift workers are more at risk to develop a breast cancer (relative risk (RR) between 1.09; 95% CI: 1.02-1.20 and 1.48; 95% CI: 1.36-1.61 in the meta-analyses). However, this risk is not found by some cohort and case-control studies. The circadian rhythm disruption, responsible of disorderliness of melatonin secretion, could be one of the mechanisms involved in the increase of that risk. Hormonal status of night/shift workers, their geographic origin, their lifestyle and their vitamin D deficiency appear as other mechanisms potentially responsible for increased risk of cancer in this professional population. Moreover, a dose-effect connection may exist, with an increase of the risk with the number of years of night/shift work., Conclusion: Night/shift work is associated with a moderate increased risk of breast cancer, especially among women who worked over 20 years. Recommendations concerning the breast monitoring in this population could be diffused. The benefit of melatonin supplementation remains to be assessed., (Copyright © 2015. Published by Elsevier SAS.)
