Vaginal stenosis or gynatresia is a congenital disease and it is part of Mayer-Rokitansky-Kustner-Hauser syndrome. Acquired vaginal stenosis is a rare complication of vaginal delivery and may be caused by an infection, charlatans, birth injury or postpartum hypoestrogeny. We report a case of postpartum complete vaginal stenosis secondary to vaginal injuries as a result of medical negligence. The study involved a 19 year old patient, who had had dead child born vaginally at home, presenting with a two-year history of secondary amenorrhea associated with chronic pelvic pain. The patient reported the occurrence of multiple not sutured vaginal tears. Pelvic MRI showed complete extended vaginal stenosis of about 25mm with upstream haematological retention and bilateral hematosalpinx. The patient underwent release of vaginal adhesions followed by regular vaginal dilation. Only two cases have been reported in the literature. Pain and dyspareunia were the most common symptoms. Objective: To describe the management of patients with an erosive complication after tension free vaginal tape. To establish a diagnostic approach, describe the results on symptoms and continence after a conservative surgery., Methods: Retrospective descriptive study of 38 patients supported for complications after suburethral synthetic sling. We have diagnosed 12 erosions vaginal and five bladder erosions. Patients have been treated from a recovery surgery according to a conservative approach (partial resection or endoscopic section)., Results: Of the 12 patients with a vaginal erosion, 10 expressed symptoms in relation with their erosion. After partial resection, eight patients (80% of patients with symptomatic erosion) did not have more complain. Of these, 50% were continent, the other with mostly a slight recurrence accessible to a second suburethral sling. First endoscopic section was a minimally invasive option for bladder erosion. However, an only cystoscopic approach seemed to be not sufficient to cut the sling as far as possible. After section, two patients have been treated from a laparotomy with bladder dissection, one for immediate outcome unsatisfactory, the other for reccurent symptoms. The results for continence were excellent as the suburethral portion was not resected., Conclusion: Our study showed the importance of regular clinical surveillance, feasibility and the relative success of this conservative approach. The technical feasability and the increasing number of patients must not forget to respect good indication and rigorous technique. These complications invite us to moderate our indications especially for young patients.

This article discusses all of the urethral diseases observed in patients with neurogenic bladder. The frequency of these disorders has decreased considerably with progress in intermittent catheterization and bladder retraining. However, urologists may occasionally be required to manage an urethral diverticulum, or an ulcer of the urethra over an indwelling catheter and must be familiar with the specific features in this setting.

The authors report a case of perineal Crohn disease with three anal fissures. Because of a persistant fever without any clinical aspect of abscess they practiced MRI examination that discovered an abscess of the recto-vaginal wall. The patient was operated under general anaesthesia. This clinical case shows the interest of radiologic exams, particularly pelvic MRI for the precise lesional diagnosis of anoperineal lesions of Crohn's disease, that is still complex and difficult to treat.

Objectives: Surgical treatment in which a vaginal mucosa island is buried leads to a risk of epithelial inclusion cyst formation. The aim of this study is to describe this complication, assess incidence, precise facilitating factors and discuss treatment., Patients and Method: This study concerned 84 patients operated on between January 1996 and December 1998. They were treated with modified vaginal wall sling procedure. Fifty women were post-menopausal and 22 had estrogenotherapy. All patients had post-operative surveillance. The mean post-operative follow-up was 19 months (range: 1-68 months). Epithelial inclusion cyst formation diagnosis reposed exclusively on clinical assessment.Results. - Seven out of the 84 patients (8.3%) were diagnosed with epithelial inclusion cyst formation within 19 months of their operation (range: 3-34 months). Out of the seven patients, four were post-menopausal and three had received estrogenotherapy for many years. In six cases, epithelial inclusion cyst was symptomatically revealed by perineal pain or dysuria. These cases were successfully treated by cyst marsupialisation without recurrent incontinence., Discussion and Conclusion: The results of this short study show that epithelial inclusion cyst formation is a specific complication of surgical procedures burying a full thickness of vaginal mucosa and that estrogen impregnation seems to be the main facilitating factor. Successful treatment of symptomatic cases of epithelial inclusion cyst can be achieved by marsupialisation.

We describe the triple operation for prolapse with prosthesis in patients with pelvic organ prolapse using a vesicovaginal mesh for the cystocele, a rectovaginal mesh for the rectocele and a posterior retro-and trans levatory vault suspension sling. Preliminary results in a consecutive series of 92 patients who underwent surgery between June 2001 and December 2002 showed three cases of vaginal erosion in contact with the prosthetic material, and one hematoma of the pararectal fossa with secondary abscess formation requiring ablation of the implant. There was one immediate anatomic failure. Function was good with no reports of dyspareunia or dyschesia.

HPV related pathologies are the commonest reasons for consulting a gynaecologist. Genital warts are associated with infection by HPV type 6 or 11. Etiopathogenetic factors and treatment modalities are reviewed.

The authors report 8 cases of iatrogenic ureteric lesions, induced by gynaecological or obstetric operations. The diagnosis was based on a recent history of surgery, clinical features, abdominal ultrasonography and IVU. Treatment consisted of ureterovesical reimplantation.

Ureteric trauma constitutes a serious disease which can induce renal complications. Ureteric trauma is often iatrogenic, mainly due to gynaecological surgery. The authors report a series of 42 cases of ureteric trauma observed over a 20-year period. These patients consisted of 33 females (78%) and 9 males (21%) with a mean age of 38 years (range: 20 to 65 years). 24 ureteric lesions (57%) were secondary to obstetric procedures and operations, 9(21%) were due to gynaecological operations (hysterectomy), ureteroscopy was responsible in 3 cases, 2 lesions were due to lumbar sympathectomy, one case was secondary to resection of the rectum and 2 cases were secondary to abdominal trauma. The most frequent lesion was ureteric section (21 cases), usually affecting the pelvic ureter. Iatrogenic lesions were associated with iliac vessel ligation (1 case) and vesicovaginal fistulas (8 cases). The clinical features were dominated by vaginal urinary fistula (33 cases) and low back pain (15 cases). The diagnosis was essentially based on IVU, which showed ureterohydronephrosis in 18 cases and a silent kidney in 2 cases. Eight ureteric lesions were diagnosed at operation, 31 after an interval of 3 to 20 days and 2 after an interval of 3 months. Treatment consisted of ureterovesical reimplantation in 17 cases, Boari-Kuss bladder flap in 3 cases, psoas bladder in 6 cases, end-to-end ureterorraphy in 5 cases, ileoureteroplasty in 3 cases, nephrectomy in 2 cases, appendicoplasty in 1 case, double J stent in 2 cases. The course was marked by the development of hydronephrosis (4 cases), ureteric stenosis (2 cases), vesico-ureteric reflux (1 case), urinary fistula (2 case) and lengthening of the ileal graft (1 case).

Objectives: The aim of this study is to evaluate the infectious risk of the hysteroscopy., Methods: A prospective study was carried for nine months in the Belle-de-Mai maternity, in Marseille. We performed 81 operative or diagnostic hysteroscopies. A vaginal sample was achieved before the intervention and an endo-uterine sample after the procedure. The statistical analysis used the Chi 2 test., Results: Fifty per cent of the patients of whom vaginal sample was contaminated present an endo-uterine sample infected at the end of an operative hysteroscopy. In 25% of cases, whereas the vaginal sample was normal, the endo-uterine prelevment was infected. The relative risk of the endo-uterine bacterial contamination, in case of vaginal contamination was 8.75 during this procedure. Within diagnostic hysteroscopy, no patient with a normal vaginal sample, was contaminated. No patent infection was observed., Conclusion: There is a risk of endo-uterine contamination during hysteroscopy. This risk is higher when the vaginal sample is contaminated before the intervention. This risk is smaller than the infectious risk of hysterectomy.

From June 1990 to June 1996, 18 patients with ureteric injuries were treated in our institution. All ureteric lesions were diagnosed post-operatively and repaired early (2 cases) or after a delay (16 cases). 4 patients underwent exclusive endourological treatment, 10 patients underwent repair by open surgery without preliminary endourological procedure, and in the remaining 4 patients, the two methods were combined. In this group, ureteroscopy, which facilitates intraoperative identification of the ureteric segment below the lesion, allowed end-to-end ureteric anastomosis, which, in the light of our results, constitutes the reference surgical technique.

Two cases of neonatal hydrocolpos due to distal vaginal atresia is reported where hydro-vaginal distension produced respiratory distress and bowel obstruction radiologic and ultrasonic studies established the diagnostic of hydrocolpos surgery combining abdomino-perineal approaches was successfully done. In this occasion the authors emphasize the characteristics of this abnormality with review of literature and insist on the major interest of prenatal diagnostic in taking early infants.

In some couples, an absence of pregnancy may be related to sexual difficulties. They should always be considered if the couples do not mention this subject. It is also necessary to keep in mind that sterility may result in sexual problems. In all cases, it is the physician's responsibility to listen to, reassure and comfort the couples who look to him/her for the realisation of their dream: having a child.

Gynaecological types of pain in high-powered sportswomen can be of traumatic origin (bony, muscular or cartilaginous) in the region around the pubis. A case of vaginal pain which proved to be due to adductor tendinitis in the hip in a long distance walker is described. A review of the differential diagnosis of gynaecological pain that can occur in sportswomen is carried out. These consist of instability in the symphysis of the pubis as well as muscular lesions of the abdominal muscles of the hip adductors and of the ilio-psoas muscles. In addition, stress fractures of the pubic rami as well as inflammation of the symphysis due to stress, and pubic osteomyelitis are described.

There are numerous etiologies of benign vulvo-vaginal tumefactions in children. However, the etiological diagnosis can usually be based on clinical data after careful examination. Para-urethral cysts are the main cause of labial and vestibular tumefactions; they are generally cured by simple puncture. Solid vaginal tumors are uncommon, but they always require surgical resection in order to exclude the exceptional and severe malignant tumours. Neonatal hydrocolpos is frequent in our experience and must always be treated surgically; associated malformations are frequent and must be systematically searched for. Ectopic prolapsed ureterocele and circular prolapse of the urethral mucosa are easily recognizable and need surgical treatment.

A child aged 4 1/2 years was found to have secondarily infected retained secretions in the vagina from a vaginal membrane. The authors underline the value of intravenous urography coupled with rectal insufflation, which is essential in order to confirm the diagnosis of colpos and to eliminate any associated urinary malformation.

The three main types of infections affecting the lower genital tract are due to: Gardnerella, Gonococcus and Chlamydia. The author's text and tables summarize the epidemiology and the management of such infections.

A large vaginal ulcerated area which came about because super-absorbant tampons had been worn for three years is reported. Cure was obtained when the area was excised and sutured. There was no recurrence after the patient stopped using the tampons. There are 36 cases reported in the literature. Ulceration occurs in young women (25 years), nulliparous or primiparous (79%), who use tampons abnormally during the periods and between the periods (75%). The ulceration is always characteristic in appearance: it is a punched-out area, round or oval in shape, and is situated in the upper third of the vaginal barrel near where the tampon presses on to the vagina. Spontaneous cure occurs in 75% of cases when the tampon is no longer used. Physio-pathological hypotheses as to the causation are given. A suggestion is made as to the relationship between this syndrome and the staphylococcal toxic shock syndrome. The discovery of vaginal ulceration is not usually due to use of menstrual tampons. One has to think of adenosis of the vagina, herpetic ulceration, a syphilitic chancre or cancer, the diagnosis depending a little on the age of the patient. In our case that we report, continuous usage of super-absorbant tampons was accompanied by a large vaginal ulcer. Because of this case we have reviewed the literature about the cases that have been reported and have collected 36 cases.
