- Abstract
Mud fields located In the inner part of shelves in front of large estuaries are preferential environments for the investigation of the mechanisms and evolution in time of the supply of such sedimentary bodies. The study of different constituents of the particulate organic matter trapped in the sediments of the West Gironde mud patch contributes to the knowledge of the input origins. This study was carried out with different techniques, including pyrolysis-gas chromatography-mass spectrometry and high performance liquid chromatography, on samples from a core collected in the centre of the mud field. At the bottom of the core, the increase of sugars, amino-sugars and some aromatic hydrocarbons may indicate a more marine character of organic components. The same appears to be the case at the top of the core, where, despite a finer sedimentation, biochemical characteristics could show a more pronounced marine influence than in the middle part. In the middle part of the core, on the other hand, the increase in phenols could result from an increase in lignin-derived. terrestrial inputs. The increase in pyrolysis-derived cyclopentenone may indicate the same terrestrial influence, because this component has its origin in aliphatic polycarboxylic acids resulting from sail leaching. The continental character of the sedimentation is also reflected in the increase of the silt-clay fractions representative of suspended matter originating in the Gironde estuary, associated with a slight carbonate decrease. The taxonomic significance of some phenols is noteworthy: this is especially the case of the cinnamyls which, in the middle part of the core, emphasize the input of herbaceous debris and of the hydroxybenzyls which, at the bottom, provide evidence of the algal origin of part of the organic material. Thus, by means of pyrolysis results and HPLC phenol analysis, it is possible to establish the more or less continental signature of organic components trapped in the ''Ouest-Gironde'' mud patch. This study of organic matter, which confirms earlier sedimentological and micropaleontological data, constitutes a new approach, adapted to the identification of the origin of carbon sources.

- Abstract
The southern Biscay shelf is a typical example of a continental margin submitted to a macrotidal regime and the heavy sedimentary fluxes of continental provenance. The aim of this study was to determine the manner in which clay sedimentation is organized on the margin and to evaluate the impact of both river-borne inputs and marine hydrodynamic activity. It was established that the distribution of clay minerals (in the order of frequence: micas, chlorites, illites and smectites) is upset by the interconnection of several sedimentary processes: discontinuous river-borne discharges (richer in smectites); the mixture of newly-arrived material with fine fractions (rich in micas and chlorites) resulting from the reworking of remnant material that represents the major part of the sea-floor and the ultimate stockage occurring at depths greater than 50 m. It appears that physical processes control to a large extent the distribution of clay phases.

- Abstract
The mud-patch located in the western part of the Gironde Estuary system comprises a lenticular sedimentary body 420 km super(2) in area, oriented NE-SW at a depth ranging between 30 and 75 m. Its thickness does not exceed 4 m; it fills a light depression in a substrate composed of palimpsest shelly pebbles to find sand deposits. The deposition of this mud-patch from the original estuarine suspended matter began less than 2000 years ago, when the sea reached its present-day level. The sediments are silty-clayey, organized in several centimetrical to decimetrical fining-up sequences, more silty at the base of the mud-patch. Such rythmic deposits are characteristics of a reworking by storm or strong wave activity. These sequences show a seaward evolution. The lithological characteristics of the mud suggest that deposition occurred in 2 main stages: 1) infilling of the depressions in sandy-gravelly substrate with interbedded sand and mud; and 2) vertical and lateral development of the muddy lens.

10. La maladie de Mondor pénienne : une observation.

Résumé Introduction La maladie de Mondor est une thrombophlébite superficielle rare, touchant historiquement le système veineux thoracique de la femme. Elle peut toutefois s’exprimer chez les deux sexes et intéresser toute la surface cutanée. Observation Nous rapportons l’observation d’un homme de 40 ans, diabétique de type 1, souffrant d’une thrombose de la veine dorsale superficielle de la verge qu’il traitait comme une mycose. Un traitement anti-inflammatoire non stéroïdien complété par une héparine de bas poids moléculaire permettait une évolution favorable en 2 semaines. Conclusion L’étiopathogénie et le traitement de la maladie de Mondor sont encore incomplètement dominés. Certains facteurs prédisposants ont été toutefois soulignés dans la littérature, auxquels il nous paraîtrait intéressant d’intégrer le terrain diabétique, susceptible par ses fréquentes problématiques pelviennes, d’être un irritant inflammatoire veineux local. This article provides an overview of 10 important articles published in year 2013 in the field of ambulatory general internal medicine. The newest guidelines about glaucoma screening are detailed. In the midst of the lung cancer screening controversy, an article summarizes new guidelines for the management of solitary pulmonary nodules. Cohort studies are detailed, which have shown association between high calcium intake and cardiovascular mortality, grade 2 or 3 obesity and mortality, aspirin use and lower melanoma incidence, and a reduction of the prevalence of dementia in last 20 years. Finally, 2 publications clarify the optimal length of corticoid treatment for COPD exacerbations and the most appropriate treatment regimen for Helicobacter Pylori eradication.

As a consequence of growing global migration, physicians in French speaking Switzerland often face communicational difficulties with allophone patients. This paper first discusses advantages and shortcomings of various ways of dealing with this kind of situations. The indication of using professional interpreters will be addressed, as well as some specific therapeutic, linguistic and relational features of triadic consultations involving a physician, a patient and an interpreter. Finally, useful practical information and advices are provided to clinicians in order to help them optimize their consultations with allophone patients.

- Abstract
Verbal language is a major tool of medical communication. However, its use can be problematic, namely because the speakers of a given language do not necessarily agree on the meaning of the words they exchange. This phenomenon is usually called linguistic variability. Based on a famous political and legal case and medical examples, we will show how variability is a critical source of misunderstandings and other communicational breakdowns. In addition, we will suggest some strategies which are likely to limit the impact of variability on clinician/patient interaction.

- Abstract
Action research is a useful instrument for the organization health care and the clinical governance of psychiatric institutions. What this type of research offers can be illustrated by the cohort study of migrant patients without health insurance who consulted the Department of Psychiatry of the Vaudois university medical center (CHUV) in 2008. While giving greater visibilty to the psychological suffering and social distress of these patients, the study also enabled the authors to determine which clinical procedures were actually offered to these patients and the amount these procedures cost the department. The small number of cases that were identified as well as their uneven distribution amongst the different services of the department suggest that considerable efforts must still be made to improve access for this population to public psychiatric services.

Metastatic testicular germ cell tumours with complete involution of the primary site, or "burned-out tumour", constitute a classical but rare entity, whether the initial cell type is seminomatous or non-seminomatous. The authors report one case of this type of tumour. The diagnosis is primarily based on ultrasonography, indicating the need for orchidectomy in the presence of an anomaly, followed by histology, revealing a fibrous scar with no tumour cells. Aetiopathogenic hypotheses are divided between ischaemic and immunological theories. Scrotal ultrasonography and tumour marker assays should be performed systematically in the presence of retroperitoneal adenopathy with normal testicular clinical examination.

- Abstract
Distortion of the penis can be an obstacle to normal sexual intercourse and can lead to psychological problems. Nesbit first described an operation for the correction of three cases of congenital incurvation of the penis. Several authors later proposed modifications of the original procedure, but the principle technique relies on folding the corpus cavernosum on the side opposite the deviation. We have treated 26 cases with distorted penis: 14 congenital, 3 postsurgical, 1 post-trauma and 8 due to la Peyronie's disease. Our functional results have been excellent. The risk of postoperative retraction is considerably reduced when the fold is created without removing the albugin patches. The quality of the results is directly related to rigorous patient selection.

The authors report about one case of retroperitoneal benign schwannoma located near the right kidney and causing renovascular arterial hypertension. The diagnosis of benign schwannoma, which is unfrequent in the absence of Recklinghausen's disease, was established after the pathological study of the complete specimen including the right kidney.
