In this paper we analyze the opening hours of 50 randomly selected university libraries (UL) in Austria, Germany, Canada and the USA. The aim of this analysis is to identify possible influences on the extent of the opening hours. Potential influencing factors that we consider are library size and country (state) or linguistic area in which a library is located. The results of the analysis show that larger UL and UL that are located in the United States tend to be opened longer. [ABSTRACT FROM AUTHOR]

In recent years, and particularly since 9/11/2001, more and more delinquent Haitian youth have been deported from Canada back to Haiti. In this paper I analyze the generation-specific reactions and statements within the Haitian migrant community in Montreal, Canada. Elder migrants perceive of the deportation as a threat to their home country and as an assault upon their personal migration projects. For the younger generation, in contrast, the issue is rather one of negotiating ones role within the migrant community and of coming to terms with their own life prospects in Canada. Both kinds of reactions become comprehensible if set in relation to one another. Furthermore, this article suggests to examine present-day state regulative practices such as deportations not only in relation to the receiving states, but to also include the consequences for the diaspora communities. This journal of educational research contains five papers written by international authors. The initial paper presents the evaluation of a French immersion program for students from the second through the fourth grade in Toronto. The aim of the second paper, written in French with an English summary, was to find the attitude dimensions of French Canadian primary school teachers and to discover the contents of those dimensions. The third paper, also in French, describes an experiment of computer assisted instruction for diabetic, insulin dependent patients. The fourth paper identifies problems experienced by first year university students of various ethnic backgrounds at the University of Fort Hare, South Africa. The final paper investigates relationships among locus of control, difficulties with social groups, and psycho-social difficulties in social situations. Current educational research being done by various international educators is chronicled, and information about current events of the World Association for Educational Research is presented. (JN)

A guide to the use of radio and telecommunications to promote international student communication describes a project in which teenagers from Cologne (West Germany) and Edmonton (Alberta) discuss their concerns via radio as well as provide descriptions of life in West Germany and Canada. Transcripts from two radio programs--one in English and one in German--illustrate the exchange of ideas on how to solve international problems such as pollution, international warfare, and starvation in the Third World, and discuss topics such as lifestyles, school, leisure time, and hopes for the future. Other materials included are photographs of the students and production staff, teaching tips, worksheets, and newspaper articles about the project. The materials are suggested for use in German language teaching, social studies instruction, and promotion of German language study. (MSE)

An article is presented on the history and human geography of transatlantic telegraph networks in Heart's Content, Newfoundland and Labrador. The author offers information on social factors influencing the lives of the operators of the Heart's Content telegraph station as well as the divide which separated the operators of these remote telegraph stations from the globalizing effects of early telecommunications technology. The history of the economy of Heart's Content is described and the effect of the introduction of telegraphy on the literacy of rural Canadians is examined.

Background: Although psychotherapy is an important pillar in the treatment of bipolar disorders, alongside pharmacotherapy, non-drug and complementary procedures, there is no up to date evidence synthesis for inpatient psychotherapeutic treatment and work with caregivers., Objective: To review and evaluate the current study situation on evidence-based inpatient psychotherapy for bipolar disorders., Material and Methods: 1.Summary of the evidence for inpatient psychotherapy in adolescents and adults with bipolar disorders from current review articles and guidelines (German S3 guidelines, Australian, Canadian, and British NICE guidelines). 2. Systematic literature search (PRISMA) in Cochrane trials and Medline (via PubMed). 2a. Identification of original articles using the following search term: "bipolar fft" OR "bipolar ipsrt" OR "bipolar cbt" OR "bipolar cognitive remediation" OR "bipolar psychotherapy inpatient". 2b. Screening of n = 942 publications on the following inclusion criteria: randomized controlled efficacy trials, inpatient treatment/recruitment in the inpatient setting, adolescent or adult patients with bipolar disorder or caregivers., Results: The guidelines recommend a combination of pharmacotherapy and psychotherapy for the treatment of patients with bipolar disorders (so far no evidence-based presentation of inpatient psychotherapy). The results from reviews and original papers are heterogeneous. Recently described evidence-based psychotherapeutic approaches for inpatient treatment are family focused therapy (FFT), interpersonal and social rhythm therapy (IPSRT) and psychoeducation., Conclusion: Although the current evidence is heterogeneous and further systematic studies are necessary, the results indicate that psychotherapy should be started or initiated in the inpatient setting with inclusion of caregivers., (© 2023. 20. [High-cost patients in Germany: General description of utilization and costs].

Background: Studies from different countries have shown that a small number of insured persons (high-cost patients) are responsible for a large portion of health care spending. At the same time, it is assumed that some of these costs could be saved by a better management of this group of people. The aim of this article is to analyze the performance and cost profiles of high-cost patients, to put them in an international comparison, and to derive a better management approach., Methods: Retrospective observation study based on statutory health insurance data from two statutory health insurances for the year 2013., Study Population: top 5 %, as well as top 1 % of the most expensive insured persons. Identification of characteristics of high-cost patients and international comparison with the Netherlands, the USA, Canada, Spain, England and Japan., Results: 5 % of insured persons account for almost half of the total costs and the most expensive 1 % of 22 %. These high-cost patients in Germany are, on average, 20 years older than the general population. Almost every person of the high-cost population was prescribed at least one medication during the study period (99.2 %), and 85.8 % had at least one hospital stay. Hospital care accounts for the biggest part of total costs: 75 % together with drugs. The average per capita costs caused by one of the 5 % most expensive insured persons in the year under review are 20 times higher than that of the other 95 % of insured persons. High-cost patients are generally more multimorbid and have higher mortality rates. The most common diagnoses of these patients are hypertension, lipoprotein metabolism disorder and back pain., Conclusion: Similar to other developed countries, Germany faces the challenge to develop and implement adequate intervention approaches addressing the special requirements of high-cost insured persons. This paper provides a first basis. The analogies of high-cost patients in Germany and other countries illustrate the need for transnational research and intervention approaches on this specific issue. Background: This paper analyses schemes to promote the authorisation of and reimbursement for orphan drugs., Methods: 8 countries - Australia, Canada, Germany, Great Britain, France, Netherlands, Switzerland, USA - were studied to compare specific regulations for orphan drugs regarding drug admission, health technology assessment (HTA), decision-making for reimbursement, and off-label and compassionate use. Information was obtained by reviewing published and grey literature. Expert interviews were also conducted., Results: The comparison of orphan drug legislation reveals that the EU and the USA offer the greatest incentives for the development of orphan drugs, whereas there is a tendency for Australia and Switzerland to profit from incentives in other countries. Although not explicitly stated, economic evaluation of orphan drugs takes the special circumstances for orphan drugs into account. In addition to common reimbursement practices, special schemes or programmes for the reimbursement of high-priced orphan drugs exist in all countries that were analysed. Therefore access to orphan drugs seems to be warranted. However, due to co-payments of 5%, the USA may form an exception., Conclusion: On the one hand, the use of special criteria for drug admission, HTA, and reimbursement promotes R&D for orphan drugs. On the other hand, high opportunity costs arise, because huge efforts are made for a minority of patients. Prevention in nephrology is only possible with the cooperation of patients and their families. The nurse plays a considerable role in working with patients and is a major player in the team, responsible for follow-up of the patient, where the earliest interventions can help delay and sometimes avoid dialysis. The hypertension clinic is the beginning of a continuum until dialysis. This paper describes three clinics that are managed in the renal service and indicates how they contribute to offering optimal care to a renal population.
