The hip is a frequently affected joint in rheumatoid arthritis. Although the hip is rarely the site of primary manifestation, it is almost always affected during the course of the disease and, if left untreated, leads to progressive joint destruction with the development of secondary osteoarthritis. Despite greatly improved drug therapy options in recent years, especially with the introduction of biologic agents, total hip replacement continues to be a frequently performed procedure in rheumatoid patients. Due to the specific pathology of rheumatoid joint destruction, hip arthroplasty presents special challenges to the surgeon. This concerns the preoperative preparation, the surgical procedure as well as the postoperative treatment. However, taking these special features into account, the results of total hip replacement in rheumatic diseases are good and often lead to a significant improvement in mobility and quality of life.
