In the next two decades the aging baby boomers in Germany will gradually be leaving the work force. They are being followed by the much less numerous, "baby bust" generation who now need to finance and staff healthcare for the growing number of old people in society. In order to care for more needy persons with a smaller working population, the healthcare system must be restructured; however, despite these worrisome prospects, the awareness of the problem is still low in many areas. Here we focus on the area in the healthcare system that is growing particularly rapidly and additionally has the greatest need of personnel per patient: the care of the critically ill and functionally impaired patients. The lack of coordination of hospitals, rehabilitation centers and nursing institution is historical in origin. It promotes the tendency to discharge functionally impaired patients to nursing facilities without giving them a chance for recovery of functional autonomy. As the demographic change progresses, this tendency threatens to increase. In a first of two parts, we attempt to describe the present situation.In den nächsten zwei Jahrzehnten werden in Deutschland die Babyboomer aus dem Erwerbsleben ausscheiden. Erwerbsarbeit muss dann von der zahlenschwachen „Pillenknick“-Generation geleistet werden. Mehr ältere Personen in der Gesellschaft bedeuten trotz und teilweise wegen verbesserter medizinischer Möglichkeiten eine höhere Belastung durch Gesundheits- und Pflegeversorgung, die finanziert und personell getragen werden muss. Um mit weniger Erwerbstätigen mehr Bedürftige zu versorgen, muss das Gesundheitssystem umgebaut werden. Weil allerdings die Entwicklungen schleichend verlaufen, ist das Problembewusstsein vielerorts noch gering. Hier fokussieren wir auf den Bereich in unserem Gesundheitssystem, welcher mit am stärksten wächst und zusätzlich den größten Personalbedarf pro Betroffenem hat: die Versorgung schwerkranker und selbsthilfeeingeschränkter Menschen. Das Nebeneinander von Krankenhaus, Rehabilitationsklinik und Pflegeinstitution ist historisch bedingt und unzureichend koordiniert. Es fördert die Tendenz, selbsthilfeeingeschränkte Patient*innen in Pflegeeinrichtungen ohne Chance auf Wiederbefähigung zu entlassen, statt sie zu rehabilitieren. Mit dem weiteren demografischen Wandel droht sich diese Tendenz zu verstärken. Hier versuchen wir in einem ersten von zwei Teilen eine Beschreibung der aktuellen Situation.

Dementias are expensive diseases: the net annual cost in European healthcare is about € 28.000 per case with a strong stage dependency, of which medical care accounts for about 19%. Diagnostic costs, on the other hand, account for only a small proportion of the total costs. With changes in the guidelines, biomarker tests are becoming increasingly important. At present, the concrete economic impact of biomarker-based diagnosis is largely unknown. To determine the actual costs of diagnostic procedures based on guidelines, we conducted a survey among the members of the German Memory Clinic Network (DNG). From 15 expert centres, the staff engagement time for all procedures was collected. Based on the individual engagement times of the different professions, the total of personnel costs for diagnostics was calculated using current gross personnel costs. The total sum of diagnostic costs (personnel plus procedures) was calculated for three different scenarios e. g. € 633,97 for diagnostics without biomarkers, € 1.214,90 for diagnostics with CSF biomarkers and € 4.740,58 € for diagnostics with FDG- plus Amyloid-PET. In addition, the actual diagnostic costs of the current practice in expert memory clinics were estimated, taking into account personnel costs, costs for the different procedures and the frequency of their use across all patients. This results in total average costs of € 1.394,43 per case as the mean across all centres (personnel costs € 351,72, costs for diagnostic procedures € 1.042,71). The results show that state-of-the-art diagnosis of dementia and pre-dementia states, such as mild cognitive impairment (MCI) requires financial resources, which are currently not fully reimbursed in Germany. The need for a biomarker-based etiological diagnosis of dementia and pre-dementia states will increase, due to availability of disease-modifying treatments. Therefore, the current gap of reimbursement must be filled by new models of compensation.Demenzen sind teure Erkrankungen: die jährlichen Kosten betragen in europäischen Versorgungssystemen etwa 28.000 €/Fall mit einer starken Stadien-Abhängigkeit, davon entfallen etwa 19% auf die medizinische Versorgung. Die ökonomischen Auswirkungen der Biomarker-basierten Diagnostik sind derzeit nicht sicher abschätzbar. Zur Erhebung der Kosten einer leitlinien-orientierten ätiologischen Erst-Diagnostik von kognitiven Störungen wurde eine Umfrage im Deutschen Netzwerk Gedächtnisambulanzen (DNG) durchgeführt. An 15 Expertenzentren des DNG wurden systematisch die Personalbindungszeiten für alle Prozeduren und alle beteiligten Berufsgruppen erhoben und die Personalkosten basierend auf den tarifvertraglichen Arbeitgeberkosten berechnet. Zusammen mit den Kosten für technische Untersuchungen wurden Gesamtkosten der Diagnostik für drei Szenarien abgeschätzt: Diagnostik ohne Biomarker € 633,97 €, Diagnostik mit Liquoruntersuchungen € 1.214,90 und Diagnostik mit FDG- plus Amyloid-PET € 4.740,58. Zusätzlich erfolgte eine Analyse der derzeitigen realen Kostensituation in Gedächtnisambulanzen, wobei die Personalbindungszeiten für einzelne Leistungen und die apparativen Kosten ins Verhältnis zur Häufigkeit ihrer Anwendung gesetzt wurden. Als Mittelwert aller Zentren ergeben sich dabei Gesamtkosten von € 1.394,43/Fall (Mittelwert der Personalkosten € 351,72, Mittelwert der Kosten für apparative Diagnostik € 1.042,71). Die Ergebnisse zeigen, dass eine ätiologische Diagnostik von kognitiven Störungen (Demenzen und leichte kognitive Störung) einen Ressourceneinsatz erfordert, welcher derzeit weder durch die Vergütungssysteme von Ambulanzen noch durch die vertragsärztliche Vergütung kostendeckend erstattet wird. Die Biomarker-gestützte Diagnostik dementieller und prädementieller Syndrome wird häufiger werden, wenn sie zur Indikationsstellung vor einer krankheits-modifizierenden Therapie erforderlich ist. Deshalb müssen neue Finanzierungsmodelle entwickelt werden, um die gegenwärtige Lücke in der Kostenerstattung für die ätiologische Diagnostik kognitiver Störungen zu schließen.

Speech and language therapy is essential in the rehabilitation of aphasic disorders following a stroke. Due to the predicted increase of aphasia and limited resources within the healthcare system, the development of efficient and sustainable treatment methods is of exceptional importance. The effectiveness of both traditional and innovative approaches needs to be evaluated against the standards of evidence-based medicine. Class I evidence has been established for high-intensity speech and language therapy in subacute and chronic stages of aphasia. Innovative training-based approaches have so far only been evaluated in small studies but promising results have been shown for computer-based naming, video-based exercises for verbalization of complex contents and approaches modeled according to "forced-use" principles with standardized contents. Adjuvant training therapies are being developed to increase and prolong the impact of training alone, most notably non-invasive brain stimulation and pharmacological modulation. Transcranial direct current stimulation has been shown to effectively enhance training in several small randomized controlled trials but several questions still remain to be answered, including the location of electrode placement as well as the length and intensity of stimulation. Mixed evidence has been collected for the effectiveness of pharmacotherapy on speech learning and further randomized controlled trials are also needed to allow more firmly based recommendations.

Transcranial direct current stimulation (tDCS) has been successfully used in neuroscientific research to modulate cognitive functions. Recent studies suggested that improvement of behavioral performance is associated with tDCS-induced modulation of neuronal activity and connectivity. Thus, tDCS may also represent a promising tool for reconstitution of cognitive functions in the context of memory decline related to Alzheimer's disease or aphasia following stroke; however, evidence from randomized sham-controlled clinical trials is still scarce. Initial results of tDCS-induced behavioral improvement in patients with Alzheimer's dementia and its precursors indicated that an intense memory training combined with tDCS may be effective. Early interventions in the stage of mild cognitive impairment could be crucial but further evidence is needed to substantiate this. In patients with aphasia following stroke tDCS was applied to the left and right hemispheres, with varying results depending on the severity of the symptoms and polarity of the stimulation. Patients with mild aphasia can benefit from tDCS of the language dominant hemisphere while in patients with severe aphasia tDCS of right hemispheric homologous brain language areas may be particularly relevant. Moreover, recent studies suggested that an intervention in the subacute phase of aphasia could be most promising. In summary, tDCS could provide the exciting possibility to reconstitute cognitive functions in patients with neurological disorders. Future studies have to elucidate whether tDCS can be used in the clinical routine to prevent further cognitive decline in neurodegenerative diseases and whether beneficial effects from experimental studies translate into long-term improvement in activities of daily life.

To examine the attitude of patients with mild cognitive impairment to diagnostics under different healthcare settings.A comparative survey was carried out of 38 patients at a university outpatient clinic and 91 patients at a specialist practice with regard to their attitudes towards early diagnosis of dementia and willingness to undergo CSF analysis.Willingness to undergo CSF analysis was higher among the patients at the university outpatient unit than those at the specialist practice (p = 0.040), and willingness to undergo early diagnosis was comparable high in both groups.Different attitudes of patients with mild cognitive impairment should be reflected in different healthcare settings.

Epidemiological studies demonstrated positive effects of continuous physical activity and balanced diet on cardiovascular fitness. In chronic neurodegenerative disorders, e.g. Parkinson's disease and Alzheimer's disease, physical activity has become a successful supportive symptomatic therapy. However, it has become evident that physical activity not only improves motor symptoms but also has high impact on cognition in both (elderly) healthy brain and neurodegenerative alterations in the CNS. Nutrition also has been reported to exert positive effects on brain function.Animal studies indicate an increased endogenous plasticity as the underlying mechanism in terms of activation of neuronal precursor cells in different brain areas, leading to improved brain function.First experimental studies in humans also show that physical activity and balanced nutrition increase the release of neurotrophic factors in the brain, increase the volume of grey matter in learning- and memory-associated brain regions and improve cognitive function. This phenomenon opens up noninvasive causal therapeutic options in neurodegenerative disorders and during aging-associated cognitive decline by inducing changes in lifestyle. This option could provide a socioeconomically and ethically reasonable treatment for neurodegenerative disorders.The presented article summarizes the current knowledge from animal experiments and studies in humans. It provides an overview of potential cellular and molecular candidate mechanisms and discusses novel translational clinical studies and first clinical applications.

Amyotrophic lateral sclerosis (ALS) has traditionally been assumed to be a purely motor disturbance. It is now recognized that 2-5% of patients with ALS develop frontotemporal dementia. Additionally, neuropsychological analysis and functional imaging suggest that a proportion of patients with classical ALS also have neuropsychological impairment. On postmortem examination, ALS patients classified as not demented showed atrophy of the frontotemporal cortex. Conversely, in patients with frontotemporal dementia without known motor impairment, atrophy of spinal and bulbar neurons were found on postmortem examination. It is still not known whether patients with ALS and neuropsychological impairment form a distinct subgroup or if they are part of a continuous spectrum that runs from pure motor impairment to pure neuropsychological impairment.

When infection of implants is suspected, optimal management requires accurate confirmation or exclusion of infection. However, in spite of demonstrative clinical signs cultures of smears or chemical parameters of inflammation frequently are ambiguous. Scintigraphy with indium-labeled white blood cells (WBC) has been reported to be sensitive and specific in the diagnosis of low-grade sepsis of the musculoskeletal system. Twenty-eight patients with possible infection were prospectively studied. Infection was suspected in 19 cases with total hip joint prosthesis, 14 cases with knee joint prosthesis and 1 case with shoulder joint prosthesis. All of them underwent scanning with indium-111-labeled WBC and subsequently underwent surgery. At surgery infections were determined by means of culture or histologic results. When correlated with culture and histologic results sensitivity of 111-indium-WBC imaging was 89% with a specificity of 67% and a predictive accuracy of 77%. In patients with rheumatoid arthritis, however, predictive accuracy of 111-indium-labeled WBC imaging was higher than with standard diagnostic methods. The difference was statistically significant (P0.05, chi(2)-test). In the patients examined as a whole, predictive accuracy of 111-indium-labeled WBC imaging does not differ from that of standard diagnostic methods. That is why expensive and time-consuming 111-indium-WBC imaging is not justified generally in diagnosis of infection of implants. 111-Indium-WBC imaging is well suited to supplement standard diagnostic methods in patients with rheumatoid arthritis.

Insbesondere bildgebende Verfahren wie die funktionelle Magnetresonanztomografie (fMRT), haben innerhalb der letzten Jahre verdeutlicht, dass verschiedene Sprachkomponenten in weit verzweigten kortikalen Netzwerken im menschlichen Gehirn repräsentiert sind. Die in dieser Arbeit durchgeführte Studie kombinierte transkranielle Magnetstimulation (TMS) und fMRT mit dem Ziel phonologische und semantische Sprachnetzwerke detaillierter zu charakterisieren. Bisherige Studien identifizierten u. a. den linken Gyrus supramarginalis (SMG) und posterioren Anteil des Gyrus frontalis inferior (pIFG) als Kernregionen der phonologischen Entscheidungsaufgabe. Semantische Entscheidungsaufgaben wurden dahingegen u. a. mit dem linken Gyrus Angularis (ANG) und dem anterioren Anteil des Gyrus frontalis inferior (aIFG) in Verbindung gebracht. In der vorliegenden Arbeit wurde ein Laborexperiment an 17 gesunden, rechtshändigen und deutschsprachigen Probanden in zwei Sitzungen durchgeführt. Mithilfe eines hemmenden TMS Protokolls, der „continuous Theta Burst Stimulation“ (cTBS) sollte die kortikale Erregbarkeit über den Stimulationszeitraum hinaus reversibel moduliert werden. Eine neuronavigierte cTBS wurde entweder über dem linken SMG oder dem linken ANG appliziert. Der cTBS folgte, direkt im Anschluss, eine ereigniskorrelierte fMRT Untersuchung, während derer die Probanden auditiv präsentierte semantische und phonologische (Einzel ) Wortentscheidungsaufgaben bearbeiteten. Mit der fMRT sollten Veränderungen kortikaler Aktivierungsmuster durch die konditionierende TMS während der Bearbeitung der Aufgaben abgebildet werden. Die Ergebnisse der Studie tragen zum Verständnis der Wirkungsweise der cTBS über nicht motorischen Kortexarealen bei. Die Stimulation führte zu einer signifikanten Abnahme des „blood oxygenation level dependent“ (BOLD) Kontrasts am Stimulationsort. Weiterhin zeigten die Ergebnisse, dass die cTBS nicht nur am Ort der Stimulation zu einer Änderung der BOLD Kontrasts führt, sondern ein gesamtes Netzwerk spezifisch zu modulieren scheint. Eine cTBS über dem linken SMG führt u. a. zu einer verminderten BOLD Antwort im linken pIFG. Die Stimulation des linken ANG resultierte u. a. in einer erniedrigten BOLD Reaktion des aIFG. Eine Aufgabenspezifität nach Stimulation des SMG bzw. ANG wurde jedoch nicht evident. Dennoch unterstützen die Ergebnisse die Annahme, dass mögliche Verhaltenseffekte nach einer cTBS nicht zwingend auf den Ort der Stimulation zurückgeführt werden können. Weiterhin verdeutlichen die Stimulationseffekte, dass der SMG und der ANG in zwei unterschiedliche Netzwerke eingebunden sind.
