Background: A rare but typical perioperative complication in the surgical treatment of pediatric forearm fractures is injury to the extensor pollicis longus (EPL) tendon. This article presents the long-term results after reconstruction of the EPL tendon with transposition of the extensor indicis (EI) tendon in children and adolescents after surgically treated forearm fracture., Patients and Methods: Over a period of 15 years, 22 children up to the age of 16 were treated with EI transposition analogous to adult care. In a follow-up examination, the range of motion of the finger and thumb joints, retropulsion of the thumb and index finger, abduction and opposition were examined. In addition, hand span and strength in the gross, key, and point grasp were measured. Subjective complaints were questioned and the DASH score was assessed., Results: Rupture of the EPL tendon is a rare complication, accounting for 1% of all surgically treated paediatric forearm fractures. 15 patients with an average age of 10.9 years could be followed up for an average of 66.5 months after transposition of the EI tendon. Significant differences were found in reduced mobility of the metacarpophalangeal joint of the thumb, isolated extensor strength of the index finger, a reduced span between the thumb and index finger in palmar abduction, and reduced strength in the pointed grip. Subjectively, the differences between the sides were not noticed by the patients and their parents in everyday life., Conclusions: Transposition of the EI tendon for reconstruction of a ruptured EPL tendon as a rare complication after paediatric forearm fracture does lead to a measurable and significant reduction in index finger mobility on the affected side in the long term, but without subjective limitations of the children., Competing Interests: Disclosure The authors report no conflicts of interest in this work., (Thieme. Forearm fractures are common in all age groups. Even if the adjacent joints are not directly involved, these fractures have an intra-articular character. One of the most common complications of these injuries is a painful limitation of the range of motion and especially of pronation and supination. This is often due to an underdiagnosed torsional deformity; however, in recent years new methods have been developed to make these torsional differences visible and quantifiable through the use of sectional imaging. The principle of measurement corresponds to that of the torsion measurement of the lower limbs. Computed tomography (CT) or magnetic resonance imaging (MRI) scans are created at defined heights. By searching for certain landmarks, torsional angles are measured in relation to a defined reference line. A new alternative is the use of 3D reformation models. The presence of a torsional deformity, especial of the radius, leads to an impairment of the pronation and supination of the forearm. In the presence of torsional deformities, radiological measurements can help to decide if an operation is needed or not. Unlike the lower limbs, there are still no uniform cut-off values as to when a correction is indicated. Decisions must be made together with the patient by taking the clinical and radiological results into account.

Aim: Due to the changing attitude of treating paediatric forearm fractures increasingly towards the surgical stabilisation rather than conservatively by the method of elastic stable intramedullary nailing (ESIN), we are confronted with complications which have not been described in childhood previously. Pseudarthrosis following surgically treated forearm fractures in children is only found in single reports with none in the German-speaking area. The goal of this study is to define predisposing factors which may lead to pseudarthrosis after surgery for forearm fractures., Method: From 1990 to 2011 all children having sustained a pseudarthrosis following forearm fractures treated in our institution were included. All children who did not demonstrate a complete consolidation of the forearm fractures after 6 months from injury were considered for the study. Those pseudarthroses which were caused through systemic diseases were excluded., Results: During the time period of 21 years, fourteen children were treated who fulfilled the criteria of having a pseudarthrosis. Nine of the fourteen children had primarily been treated in an outside hospital, five in our institution. The average age was 10.8 years (7-15 years). There were thirteen ulnar shaft and one radius shaft pseudarthroses. In 11 children the pseudarthrosis was located in the middle third and there was one child each with a pseudarthrosis in the proximal and distal third of the ulna. There were 13 ulnar shaft fractures and one monteggia lesion. Twelve of the fractures were primarily closed and there were two open cases. In nine cases an open reduction of the ulna was necessary, the radius was openly reduced in four patients. In five children technical mistakes of the osteosynthesis were identified to contribute to the formation of the pseudarthrosis. Five of the 14 children had experienced a re-fracture. Nine children had revision surgery. These children were treated by plate osteosynthesis or ESIN. In five patients the pseudarthrosis healed spontaneously without interference. There were 13 hypertrophic and one hypotrophic pseudarthroses., Conclusion: Pseudarthrosis of the forearm following surgical treatment of forearm fractures in children and adolescents mainly occurred in the middle third of the ulna. In primarily open fractures or in cases which needed to be openly reduced the risk of pseudarthrosis formation was higher. Inadequate osteosynthetic stabilisation is another factor to contribute to difficulties in fracture healing. Despite of the possibility of pseudarthrosis, the indication to ESIN treatment in paediatric forearm fractures is not doubted. The case is described of a 45-year-old man with fixed contracture of the proximal interphalangeal joints (digits III-IV) after a stab wound injury in the forearm with subsequent compartment syndrome and partial ulnar injury. Despite neurographic recovery there was permanent contracture during rehabilitation, which was classified as psychogenic. Differential diagnostic aspects of this assumption will be discussed on the basis of clinical criteria as well as 3-phase skeleton scintigraphy and MRT for differentiation between complex regional pain syndrome (CRPS) type II and other posttraumatic changes. History and Clinical Findings: A 80 year-old woman contracted an abrasion of her right forearm while gardening. 5 days later, dysphagia, trismus and a cramp of the right hand developed. On admission (8 days after the injury) cramps of the platysma and pharyngeal muscles were observed. A tetanus vaccination had not been carried out for years., Investigations: Electrophysiologically a pathological masseter inhibitory reflex with a missing "silent period" was observed. The level of serum anti-tetanus-toxoid IgG antibodies was 0.03 IU/ml (definitively protective above 0.1 IU/ml). Free tetanus toxin was not detectable in the serum upon mouse challenge. The diagnosis of tetanus was made based on the typical symptoms and the serological susceptibility., Treatment and Course: On admission the wound was antiseptically cleaned and a bandage was applied. On the next day a debridement was performed. After taking a serum sample the patient was immunized against tetanus (active and passive). On the following two days the patient received a further dose of tetanus-antitoxin. The patient was treated with metronidazole for 8 and with penicillin G for 10 days. Within the first days after hospital admission laryngospasms, tachycardia and tachypnoe occurred. From the 7 (th) day in hospital the trismus and the cramp of the hand improved. 4 weeks after admission the patient was discharged; the trismus had completely disappeared, but a slight cramp of the hand was still present., Conclusion: Minor superficial injuries can lead to acute tetanus, if the level of anti-tetanus toxin-antibodies is non-protective and a disinfective wound cleaning and a postexpositional vaccination are not performed.

We are reporting a case of complete ulnar nerve paralysis due to compression from a false aneurysm located in the distal forearm. The aneurysm developed after the patient stabbed himself with a meat hook. The ulnar paralysis developed after a 14-day period of latency. The diagnosis was made using detailed patient history, clinical examination, and a sonogram. Early surgical decompression of the nerve led to complete restitution of all functions.

In a retrospective study from 1986 to 1995, 64 forearm fractures were treated with brace. Clinical and roentgenographic follow-up data were available for 49 patients (76.6%). There were 49.0% ulna shaft, 38.9% radius and 12.2% forearm fractures. The average time to healing was 10.2 weeks. Functional results were excellent in 57.1%, good in 34.7% and poor in 8.2% of cases. Two (4.1%) fractures (radius, forearm shaft) were not considered as healed and were re-operated with plate osteosynthesis. The ideal indication for fracture bracing is ulna shaft fracture. Radius and forearm shaft fractures can also be treated, but patients must be informed about the long time to healing and operative alternatives.

Tendon transpositions serve to restore functions which were lost through an injury of nerves or muscles. At the Department of Hand Surgery and Plastic Surgery at the Berufsgenossenschaftliches Unfallkrankenhaus Hamburg, 116 tendon transpositions were carried out for radial paralysis between 1960 and 1989. The follow-up for 85 patients is presented and the evaluation scheme developed by Haas (1991) further modified and our scoring system compared to others such as Zachary (1946) modified by Neumann and Partecke (1980). The results were good, also when evaluated according to Riordan (1959) and Merle d'Aubigné (1953).

This article describes the differential diagnosis between hereditary Madelung's deformity and acquired posttraumatic Pseudo-Madelung's deformity. Three cases of posttraumatic Pseudo-Madelung's deformity were reported. Aspects of aetiology, clinical radiological appearance of hereditary Madelung's deformity and acquired Pseudo-Madelung's deformity are discussed. Differential diagnostic criteria for the discrimination between both forms are also discussed. A rare case of acquired posttraumatic "reversed" Madelung's deformity is also reported.

Results of long-term follow up of 66 patients with ischemic contracture of the forearm and hand, all surgically treated in the "Unfallkrankenhaus Hamburg)) between 1961 and 1982, are presented. Whereas ischemic contracture of the forearm flexors resulted mainly from fractures, ischemic contracture of the intrinsic muscles of the hand was most often seen after pressure injuries. All patients in this study presented to us with fully established ischemic contractures. Muscle and nerve damage was retrospectively evaluated according to operative notes, and the degree of damage could be classified into four groups. Most frequently, neurolysis, scar excision and muscle-sliding operations were performed; furthermore, tendon lengthening, tendon transpositions, wrist arthrodesis and nerve grafting were indicated. Results were judged according to twelve separately measured functions, each measurement giving a possible score of three to six points. Muscle-sliding operations result in an improved score regardless of ischemic contracture stage. For a stage 2 contracture, a 20 point improvement can be expected. In stage 1 contracture--presenting with extension deficiency of four or more points--, complete recovery can be expected following a muscle-sliding operation. For isolated muscle injuries, tendon lengthening is recommended. For stage 2 contracture, the transposition of superficial to deep flexor tendons results in the same score as a muscle-sliding operation; however, the transposition procedure should be reserved for special indications. In stage 3 contracture, the muscle-sliding operation is the treatment of choice, with secondary procedures such as tendon transpositions and nerve grafts often being necessary. In stage 4 ischemic contracture, muscle-sliding operations may improve extension deficiency; however, wrist arthrodesis, especially in combination with extensor tendon transpositions, may be beneficial. No experience with free muscle transplantation was made during the study period.

137 patients with forearm fractures and internal plate fixation were investigated for postoperative radio-ulnar synostosis. 10 patients (7.3%) developed such a complication, 7 after complete, 3 after either ulnar or radial lesion. Of 73 patients with complete forearm fractures, 50 had separate incisions, and developed radio-ulnar synostosis only secondary to severe soft tissue injury. Of 12 other patients, by contrast, who were operated on for radial and ulnar plate fixation via one common incision (Boyd's approach), synostosis occurred in 5. After isolated ulnar fractures in 32 cases, synostosis was seen in two cases - in one after severe soft tissue injury, in the other after a modified Boyd's approach. Only one patient with isolated and open fracture of the radius presented later with radio-ulnar synostosis.

From 1970 to 1978 61 patients with forearm fractures were treated by delayed osteosynthesis after primary conservative therapy. The reasons for this procedure are given. Therapy results of 96 adults with forearm fractures and primary osteosynthesis are better than those with a delayed operation, i.e. after four weeks or even longer.
