In all developed countries there is the possibility to protect oneself from vaccine-preventable diseases. However, not all individuals make use of this option. It is precisely in highly developed countries where a trend to vaccination hesitancy is noticeable, i. e. reluctance to get oneself or one's children vaccinated. The reasons why this is so are many, but the most important reason is the fear of postvaccinal complications, especially of those that imply sequelae or those with fatal outcomes.Whereas there are some proven associations between vaccination and adverse drug reaction, for example febrile seizures after the measles-mumps-rubella (MMR) vaccination, other hypotheses can be refuted, for example autism after the MMR vaccination. On one hand, this article gives an overview of known postvaccinal complications with indication of a causal association with vaccination and on the other hand addresses hypotheses of potential adverse drug reactions that have been refuted by pharmacoepidemiological studies.Only the scientific debate of these hypotheses, which are repeatedly discussed, especially on social media, can contribute to corroborating or refuting a potential causal association. If evidence for a causal association grows, e. g. intussusception, the relevant authorities (e.g. Paul Ehrlich Institute, European Medicines Agency) will take risk-minimizing measures. If studies and meta-analyses do not reveal any evidence of a causal association, a targeted information strategy will be required in order to prevent myths from circulating, vaccination coverages from declining, and infectious diseases from spreading.

Pregnant women and neonates are prone to a variety of infections and often suffer from complicated courses of the disease. The growing knowledge on immunology and vaccinations during pregnancy has led to further development of already known strategies in protecting pregnant women and neonates in the most vulnerable moments of their lives. Vaccination during pregnancy against pertussis and influenza is safe and induces high levels of protecting antibodies in the mother. In addition, these specific antibodies are actively transferred to the fetus, protecting the newborn during the first weeks of life, when infections are often severe and associated with high morbidity and mortality. Viral infections like measles, rubella and varicella also may have devastating effects on mother and child when occurring during pregnancy. Available vaccinations (MMR, VZV) are inducing a stable and durable immunity, but are contraindicated during pregnancy, because of being live-vaccines. Therefore, every woman should be immune either by past infection or by vaccination before becoming pregnant. If susceptibility is only detected during pregnancy, vaccination should be started as soon as possible after birth. In conclusion, vaccinations during pregnancy offer a safe and efficient protection against dangerous infections for mother, fetus and newborn.

Asthma and allergies are among the most frequent chronic diseases in childhood. Their prevalence has increased during the second half of the twentieth century but seem to stabilize in recent years in many Western European countries such as Switzerland. Although the genetic background is known to have a strong impact on an individual's risk to develop asthma or allergy it seems likely that environmental factors are mostly responsible for the increase in prevalence over time. Much research has been devoted to environmental risk factors promoting the development of asthma and allergies. Yet, there is increasing interest in environmental factors which might protect from the development of these diseases and which might have been reduced along with the development of a "Western lifestyle". The role of environmental allergens, the hygiene hypothesis and the role of the farm environment on the development of asthma and allergies are discussed. An emphasis is put on the microbial environment as encountered in the farming environment which has been shown to confer protection from the development of asthma and allergies. It is likely that among other factors the development of a "Western lifestyle" went along with a marked change of the microbial environment children are exposed to.

Preterm infants are at increased risk of suffering from vaccine preventable diseases. Therefore it is recommended to vaccinate them in time at the same chronological age as full term infants. Several reports have shown that vaccinations of very and extreme low birth weight infants are often delayed but till now there are no data for Germany. Our study analysed the vaccination rates and suffered complications of 318 preterm infants with birth weights under 1 500 g at the age of 2 years from all over the country copying their vaccination-record and asking the parents to answer a questionnaire. The preterm infants received the first 3 doses of the pentavalent or hexavalent vaccines consequently (1st dose: 100%, 2nd dose: 100%, 3rd dose: 97.5%), whereas the booster dose was accomplished to a lesser extent (89.0%) during their second year of life. The majority of the vaccinations were administered delayed. Moreover, the preterm infants received the first doses of the pneumococcal conjugate vaccine and the measles-mumps-rubella vaccine quite consequently, but the booster dose was again injected less frequent. According to their parents 17.8% of these preterm infants suffered from non-severe adverse reactions and none of them had an episode of apnoea following the immunisation. In conclusion preterm infants in Germany receive their basic immunisations consequently, but often too late. There is still the need to improve the frequency of the booster dose application especially in the second year of life and to prevent delays of vaccinations., ((c) Georg Thieme Verlag KG Stuttgart-New York.)

Most allergic reactions after vaccination occur in patients sensitive to egg protein. Therefore this subject is well investigated, and the majority of common vaccines today contain only traces of egg protein. In contrast, there is little knowledge of hypersensitivities to other substances frequently contained in vaccines, e. g. antibiotics, phenol, gelatin and different preservatives. Here we report the case of a boy who had an anaphylactic reaction after being vaccinated against measles, mumps, rubella (MMR), and tick-born encephalitis (TBE) simultaneously. Different tests finally revealed a hypersensitivity to gelatin. This should be kept in mind especially during emergency care, since gelatin containing products like Haemaccel, Gelifundol or Gelofusin are widely used as colloid for resuscitation. If type 1 reactions after vaccination occur, gelatin should be taken into account as the causative agent. A medical alert card is recommended for such patients.
