Einleitung: Laut der WHO erleiden jährlich bis 500.000 Personen eine Querschnittlähmung, wodurch die Motorik teilweise betroffen ist. Es entstehen dadurch auch Entleerungsstörungen der Blase und des Darms. Mit einer guten Patientenedukation können Patient*innen mit einer Rückenmarksverletzung durch das schrittweise Erlernen von Entleerungstechniken wieder einen Teil ihrer Autonomie zurückerhalten. Es soll in der Arbeit gezeigt werden, wie Betroffene die Probleme mit der Ausscheidung nach der Akutphase erleben und wie eine entsprechende professionelle Patientenedukation den Weg in eine weitgehende Selbstbestimmung das Darm- Blasenmanagement betreffend ebnen kann Methodik: Bei der Arbeit handelt es sich um eine Literaturarbeit, bei der es um das Auffinden relevanter Studien und Literatur das Blasen- und Darmmanagement bei Patient*innen mit Rückenmarksverletzungen betreffend, zeigen. Zusätzlich sollen andere Studien und Literatur vorzeigen, welche Möglichkeiten der Patientenedukation in diesen Bereich angeboten wird. Aus einer Grobrecherche entstanden zwei Forschungsfragen, die in einer Feinrecherche beantwortet werden sollten. Dabei wurden vor allem die Datenbanken CINAHL und Pubmed herangezogen. Letztendlich wurden acht relevante Studien identifiziert und mit Leitlinien, das Thema betreffend, ergänzt. Ergebnisse: Zunächst konnte die Bedeutung einer professionellen Patientenedukation gezeigt werden, die die Bereiche Beratung, Information, Schulung und Moderation einschließen. Verschiedene Möglichkeiten des Darm- und Blasenmanagements wurden vorgestellt und für den pflegerischen Gebrauch präsentiert. Schlussfolgerung: Anhand der Arbeiten konnte die Bedeutung einer pflegerischen Edukation demonstriert werden, wobei vor allem das Herunterbrechen komplizierter Wissensinhalte auf die Alltagssprache und ausreichende Zeit- und Personalressourcen und vor allem Empathie von Seiten der Pflege entscheidend sind. Introduction: According to the WHO, up to 500,000 people suffer from paraplegia every year, which partially affects motor function. This also leads to bladder and bowel emptying problems. With good patient education, patients with a spinal cord injury can recover some of their autonomy by gradually learning emptying techniques. The aim of this thesis is to show how patients experience problems with excretion after the acute phase and how appropriate professional patient education can pave the way for extensive self-determination in bowel and bladder management. Methodology: The thesis is a literature review, which aims to find relevant studies and literature concerning bladder and bowel management in patients with spinal cord injuries. In addition, other studies and literature should show which possibilities of patient education are offered in this area. Two research questions resulted from a general search, which were to be answered in a detailed search. The databases CINAHL and Pubmed were primarily consulted. In the end, eight relevant studies were identified and supplemented with guidelines concerning the topic. Results: First of all, the importance of professional patient education could be shown, including the areas of counselling, information, training and moderation. Different possibilities of bowel and bladder management were introduced and presented for nursing use. Conclusion: The studies demonstrated the importance of nursing education, whereby the breaking down of complicated knowledge content into everyday language and sufficient time and personnel resources and, above all, empathy on the part of the nurses are decisive.

The deviation of feces is a very old and still largely applied technique used by general surgeons. Indications divide into elective and emergency operations. Among the elective cases, rectal carcinoma, extensive perianal fistulae and neurogenic disorders of the distal colon are the most common indications. In an emergency situation causes such as extensive peritonitis (e.g. due to anastomotic leakage) and extensive trauma to the pelvis/rectum quite often result in a stoma. All segments of the intestine, from the first loop after the ligament of Treitz down to the sigmoid colon, are feasible for a deviation procedure depending on the indications and the anatomical conditions. The decision whether to create a loop ostomy or a terminal stoma is reached with respect to the underlying indications and the anatomical and pathophysiological situation. The aim of the procedure is a complete and adequate stool deviation as well as a situation where the patient is able to take care of the ostomy in a convenient and reliable fashion while guided and educated by a stoma therapist. The prevention and treatment of complications, such as stoma necrosis or retraction are absolutely crucial in order to have satisfying long-term results and an acceptable quality of life.
