- Abstract
The negative attitude of psychotherapists towards the treatment of older patients in the past has weakened in the recent past. Nevertheless, the question remains as to how therapists perceive older patients in comparison to younger patients, what clinical judgements they arrive at and how they perceive the relationship with them. In the present study, which was conducted as part of the ÄPP study (Older Patients in Psychotherapy), therapists were asked to assess a self-selected younger (<40 years) or an older patient (>65) with regard to various variables. A total of 527 completed questionnaires were available. Two-factor analyses of variance were used to show, among other things, that younger therapists (compared to their older colleagues) rate older patients more negatively in terms of suitability for psychotherapy, the patient's ability to establish a therapeutic working relationship and other parameters. In comparison with their older colleagues, younger therapists perceive themselves as less competent in their relationships with older patients. There are only slight differences with younger patients., Competing Interests: Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht., (Thieme. All rights reserved.)

- Abstract
Does the therapeutic style differ in age-homogeneous and age-heterogeneous therapeutic dyads?, Background and Aims: Differences between age-homogeneous and age-heterogeneous therapeutic dyads have rarely been the subject of research. The present study aimed to investigate differences in therapeutic style (Healing and Stressful Involvement)., Method: A sample of 527 questionnaires completed by therapists of different ages was available. Therapy style was compared between two patient groups (under 40 and over 65 years old) and three therapist groups (25-39, 40-59, ≥ 60)., Results: The results show in particular more stress experienced by younger therapists in the treatment of older patients, while older therapists report less stress. There were no or fewer differences in the treatment of younger patients. The regression-analytical results show that the experience of stress in the therapy of older people is associated with a greater fear of old age., Conclusion: Finally, some conclusions are discussed with regard to training and supervision of therapists in the treatment of older people.

- Abstract
The article describes how a dog can be integrated into a psychotherapeutic process. Dogs react to momentary moods and functional abilities of patients and therapists and help shape an emerging "scene" according to their assessment if they are free to express themselves and are not reduced to a function. The therapist can verbalize the patient's way of shaping the interactions and use it to promote the ability tomentalize. Central therapeutic techniques are "reflective seeing", reflection on the scene and the therapist's attitude. After the general description of the concept, the special features of therapy with children and their caregivers are presented.

- Abstract
Attachment theory is known as an important therapeutic foundation. Establishing a secure attachment to a caregiver is equally relevant for the mental health of both humans and dogs. A bond can be formed between humans and dogs that is comparable to the attachment between mother and child. Patients with adverse attachment experiences often struggle to maintain stable relationships due to internal working models. Building a therapeutic alliance can also be challenging. On the other hand, establishing a relationship with a dog seems to be successful for patients with adverse attachment experiences. Inclusion of a therapy dog provides them with the opportunity for experiences of trustful relationships. These experiences can then be transferred to the therapist. Later on, internal working models of the patients can indeed come into play in the relationship with the dog. The resulting patterns of relationship can become visible in the interaction with the dog. Within the therapeutic triade of patient, therapy dog and therapist, these patterns can be reflected and integrated into the therapy process. However, achieving this requires a targeted and professionally grounded use of the dog. If dogs are employed with a therapeutic purpose but without professional guidance, there is a risk of reestablishing insecure attachment patterns, which could potentially lead to more disadvantages than benefits for both humans and dogs.

18. Tätigkeiten ohne Patientenkontakt.

- Abstract
Interpersonal competencies of therapists in dealing with alliance ruptures is related to therapeutic success. Therefore, it is of particular importance to learn and to train these competencies. A method which was developed specially for this purpose is the Alliance-Focused Training (AFT). Seven psychotherapy trainees participated in the AFT. In semi-structured interviews pre- and post-AFT, they were asked about their expectations from and experiences with the training. The transcriptions of the interviews were analyzed using the Consensual Qualitative Research (CQR) method. Before the AFT, participants showed ambivalent attitudes and fears mainly towards self-revelation. After the training, they mainly described personal development and progress in learning to deal with alliance ruptures. Video recordings of therapy sessions, role plays and training of metacommunikation skills were experienced as being helpful and practice related. The results indicate that the AFT has a great potential for improving competencies of psychotherapists.

Compared to traditional models of care, midwife-led models of care provide better opportunities for midwives to make use of their full scope of practice. This has consequences for their professional satisfaction. The aim of this survey was to gain an overview of the occupational situation of midwives in the maternity units of a Swiss canton and to compare the situations between institutions with and without midwife-led care.An online-survey was developed based on a literature search. All 17 institutions with maternity units in the canton of Zurich were eligible to participate. Data was analysed descriptively using Stata 15.16 maternity units (94.1%) participated in the survey: 12 public hospitals, 2 private hospitals, and 2 midwifery-led birthing centres. A total of 5 units (31.3%) provided midwife-led births or were birthing centres. In institutions providing midwife-led care, women and midwives were more likely to know each other before admission to birth (60.0 vs. 9.1%, p=0.063), fewer routine interventions were carried out (e. g. venous access: 20.0 vs. 81.8%, p=0.036), midwives had more decision-making competences as well as responsibility (e. g. autonomous discharge check-up: 60.0 vs. 9.1%, p=0.063) and used supervisions more often (60.0 vs. 9.1%, p=0.013).Fostering midwife-led models of care increases continuity of care and decreases the application of routine interventions. It also fosters the independent assumption of tasks.

The therapeutic alliance is considered to be a significant and empirically well-documented determinant of therapeutical success. The aim of the present study was to replicate this effect using a large daily clinical sample and to consider different aspects of the therapeutic relationship in an extreme group of particularly low relationship satisfaction separately.A longitudinal examination of a sample of n=809 patients (M=34,32; SD=10,7; 72,6% female) in a day care hospital setting was carried out. Using multiple regression analysis, the link between therapeutic alliance (Helping Alliance Questionnaire; HAQ-S) in the third week and therapeutic outcome (Brief Symptom Inventory-18; BSI-18) was investigated. Analyses have been conducted for the overall sample as well as for the extreme group representing the lowest decile regarding relationship satisfaction and the remaining 90%. A distinction was also made between the 2 subscales of the HAQ, satisfaction with the relationship and satisfaction with the outcome. The therapeutic alliance after 3 weeks was a significant predictor for therapeutic outcome. Within the extreme group with initially low relationship satisfaction, relationship satisfaction proved to be a significant and strong predictor for therapeutic outcome, but was not useable for the prediction of individual cases due to wide confidence interval (β=0,622; 95% CI [0,051; 1,095]). In contrast, the relationship satisfaction in the remaining 90 percent explained no additional variance of therapeutic outcome beyond the explanation by satisfaction with the outcome (β=0,244; 95% CI [0,176; 0,391]).The results emphasize the importance of therapeutic alliance also in a day care hospital setting. Relationship satisfaction plays a central, other factors exceeding role in the prediction of therapeutic outcome only in the extreme group of particularly dissatisfied patients.Ensuring at least a sufficiently good therapeutic relationship is of great importance and therefore requires early identification and, if necessary, intervention.

The label cancer in itself leads to complex psychological reactions in patients that are closely related to previous experiences and self-image. A cancer diagnosis can lead to increased distress and subsequent mental comorbidity such as depression or anxiety disorders. The responsibility of the pathologist, as the person who first uses the label cancer, is high. The pathologist must be conscious and focus not only on the accuracy of the anatomical pathology, but also on the potential influences the wording may have on the mental health of the patient. The word "carcinoma" should only be used in the case of tumors with the respective biological behavior. The example of non-invasive follicular thyroid neoplasm with papillary-like nuclear features shows that changing a label can lead to an improvement in distress and quality of life.

Alliance Focused Training (AFT) 1 aims at enhancing therapists' competences in resolving ruptures in the therapeutic alliance using video recordings and role-plays. This pilot study funded by the Heigl Foundation aimed at presenting initial results and clinical experiences with AFT in Germany, and to prepare a subsequent RCT.7 trainee therapists participated. Therapies of 15 patients with depressive disorder were analyzed.Trainees experienced AFT as very helpful for their professional development and for dealing with alliance ruptures. The therapeutic competence significantly improved both in self and in observer ratings.

Treatment Satisfaction of Mothers with Postpartum Depression Concerning Circle of Security Intervention Treatment satisfaction of mothers with postpartum depression who undergo mother-infant treatment is rarely examined, albeit seen as one aspect of treatment success. This study deals with maternal treatment satisfaction concerning the Circle of Security (COS) group intervention, compared to standard-mother-infant treatment (TAU), within a RCT trial. Treatment satisfaction was captured by the Fragebogen zur Beurteilung der Behandlung (FBB-E) of 52 mothers assigned to COS or TAU at children's age between 16 to 18 months. Process quality as well as outcome quality was assessed. Process quality included the evaluation of treatment procedure, information supply, evidence and justification of the measures undertaken, therapist's competence and coordination ability as well as the quality of relationship between therapist and mother. Results showed overall good to high and marginally different treatment satisfaction between both groups. COS-mothers declared themselves significantly more satisfied with the child (FBB-E-subscale: Success concerning the infant) than TAU-mothers. Both groups showed higher treatment satisfaction with the process quality than with the outcome quality of intervention. Overall, COS seem to meaningfully foster the acknowledgment of the child. In mother-infant treatment, particular attention is warranted to aspects of the process quality that attain the mother's treatment satisfaction.

Digital media, online programs, and electronic health systems are available and easily accessible for diagnostic, prevention, and intervention of somatic and psychiatric disorders. These modern tools can assess objective as well as subjective information about acute symptoms, wellbeing, life quality, sleep, physiological indicators, etc. Wearables and apps collect data over days and weeks in the real world of subjects. This information can be used to document baselines as well as changes over time influenced by events or interventions. Online treatment programs provide information for education about symptoms, course, origin, and treatment options of a disorder. They also support a patient's self-help via self-management, exercises, and techniques. We illustrate and discuss modern diagnostic and therapeutic eHealth options. We also review the empirical evidence for online interventions and refer to typical examples. Most studies have been conducted with subjects suffering from depression and anxiety. Electronic health systems do not work just by recommending a program or providing access to an online platform. Patients need to be motivated to lock in and work with a program. They need support and guidance through online programs. Therefore, we claim that therapists need to become experts in digital media and electronic health systems to support patients and to integrate apps and online programs into their treatment.

Characteristics of art therapists in rehabilitative therapy Objectives: This study examines the sociodemographic, qualification- and activity-related characteristics of art therapists working in the field of rehabilitation. In 2013, an analysis of occupational groups was carried out in Germany, with the objective of describing the art therapists working there.A total of 2,303 complete datasets were submitted. From this group, those therapists mainly working in the field of rehabilitation/follow-up care/participation of disabled persons (according to Social Security Code VI and IX, n = 302) were selected and described. Most art therapists are female (average age 45 years) and largelywork part-time. Music and art therapy are the most common venues.More than 80% have a graduate degree. Methods of quality management are used.More than half of the therapists working in rehabilitation hospitals are employed in the field of psychosomatic medicine. Both individual and group therapy (each patient attending 1-2 times a week) are common. The results provide an overview of art therapy in the field of rehabilitation and show the spread in rehabilitation. Further research is indicated.

This study was part of a double-blind randomised controlled trial aimed to evaluate the effects of culture-sensitive patient information materials (PIM) compared with standard translated material. The study aimed to obtain the data for the development of culture sensitive PIM about unipolar depression for the 4 largest migrant groups in Germany (Turkish, Polish, Russian and Italian migration background).A qualitative study using 4 manual-based focus groups (FG), one for each migrant group, with 29 participants (9 with a Turkish (TüG), 8 with a Polish (PoG), 5 with a Russian (RuG) and 7 with an Italian (ItG) migration background) was conducted. The discussions were recorded, transcribed and analyzed using qualitative content analysis.7 categories were identified. For the (1.) development of a good culture-sensitive PIM an easy language, a clear structure, an assessable extent of information and the avoidance of stereotypes were highlighted cross-culturally in all four FG. RuG and PoG had the largest (2.) lack of information about the German health care system. Concerning the (3.) illness perception RuG named problems with recognizing and understanding depression. PoG, RuG and TüG thematized (4.) feared consequences of the illness and of professional helpseeking. ItG, PoG, RuG had fears concerning (5.) psychotropic drugs as a result from insufficient knowledge about medication. For (6.) doctor-patient relationship cultural specifics were identified in RuG and TüG and for (7.) migration or culture specific reasons for depression in RuG, ItG and TüG.Although the identified categories were relevant for all or for the majority of migrant groups, for most categories specific cultural aspects were discovered.

The Psychoanalytic-interactional Method (PIM): A Psychodynamic Treatment for Adolescents with Severe Disorders of Personality Functioning The psychoanalytic-interactional method (PIM) was developed as a psychodynamic treatment for adult patients with severe disorders of personality functioning (Streeck u. Leichsenring, 2015). However, it is also well suited for the treatment of adolescent patients because its techniques fit with specific conditions of adolescence. A modified version of the PIM for adolescents (Streeck-Fischer, Cropp, Streeck, Salzer, 2016) has proven to be efficacious. The paper describes the basic principles of the PIM and names aspects that have to be taken into account in the treatment of adolescents with severe disorders of personality functioning. Finally, previous empirical results regarding the PIM treatment in adolescence are presented.
