In prospective randomized trials differences in population based means can be considered as estimates of mean causal exposure or treatment effects. Nevertheless, occurence of intermediate events in the course of a trial may lead to biased estimates of treatment effects. Particularly this will be the case, if the probability of such an event is not independent of initial treatment allocation and the intermediate event is both a risk factor for the main outcome parameter (e.g. survival) and a predictor of subsequent treatment. This situation was referred as 'treatment by indication problem' by Robins (1992) and is common in epidemiological trials. Robins demonstrated that the usual approach of an adjusted estimation of treatment effect using a time-dependent proportional hazards model may be biased in this situation, whether or not one further adjusts for past confounder history in the analysis. In this thesis a novel inference procedure for randomized trials is introduced which is based on the idea of Robin's G-Estimation principle but particularly consideres specifics of randomized trials. The suggested procedure allows for an unbiased and consistent estimation of a treatment effect parameter (or paremeter vector) which is connected by a real-valued function to the parameters of an underlying distribution of survival times. In fulfilment of the requirements of a causal individual-level based model, a link of a subject's observed and counterfactual survival time is directly achieved by the inverse distribution function of survival times in a reference treatment arm. In this term, causal inference is feasible based on the likelihood function and its corresponding test statistics, even under appropriate consideration of time-dependent confounders.

4. Prognostische Wertigkeit des Hämoglobins bei Patientinnen mit metastasiertem Mammakarzinom

Ziel der Arbeit war es bei Patientinnen mit metastasiertem Mammakarzinom, die im Rahmen der AGO-Mamma-3 Studie mit Epirubicin/Paclitaxel oder Capecitabin/Paclitaxel behandelt wurden, den prädiktiven sowie den prognostischen Wert des Hämoglobins und weiterer klinischer Parameter zu evaluieren. In der Auswertung zeigte sich eine signifikante Risikoreduktion für das Eintreten einer Tumorprogression und/oder Tod mit steigenden Hb-Werten zu Beginn des ersten Chemotherapiezyklus (Hazard Ratio je Hb-Anstieg um 1 g/dl: 0.81, 95%KI: 0.70-0.93, p = 0.003). Darüber hinaus ließen sich Her2-Status, Östrogen- und Progesteronrezeptorstatus sowie das Patientenalter als wichtige Prognosefaktoren für das metastasierte Mammakarzinom bestätigen. It was the aim of the thesis to evaluate the predicative and prognostic value of hemoglobin and further clinical parameters for patients with metastatic breast cancer who were treated during the AGO-Mamma-3 study with Epirubicin/Paclitaxel or Capecitabin/Paclitaxel. The analysis showed a significant reduction in the risk of progression of the tumor and / or death with rising levels of hemoglobin at the beginning of the first cycle of the chemotherapy (hazard ratio per rising hemoglobin quantity by 1 g/dl: 0.81, 95%KI: 0.70-0.93, p = 0.003). Furthermore Her2-Status, estrogen- and progesterone receptor status as well as the age of the patient were confirmed as important factors for the prognosis of metastatic breast cancer.

Ziel der vorliegenden Dissertation war es, Zusammenhänge zwischen Ausprägungsgraden atopischer Erkrankungen und Lebensqualität bei Patienten in der Rehabilitation zu ermitteln. Das Datenmaterial wurde im Rahmen der AURA-Studie gewonnen und umfasst Anamnesebögen (123 Erwachsene und 156 Kinder und Jugendliche), objektive (SCORAD, Asthmaschweregrad, Pricktest, FEV1%Soll, FEV1%FVC und FE(NO)) und subjektive Parameter (BSI und KINDL-Fragebögen). Es zeigten sich eindeutige Interdependenzen zwischen Umwelt, Alltagsleben und Erkrankung. Weiterhin sank mit zunehmender Krankheitsschwere die Lebensqualität. Es bestand jedoch kein zwangsläufiger Zusammenhang zwischen klassischen objektiven Parametern und subjektiven Lebensqualitätsparametern. The aim of this thesis was to determine correlations between severity of atopic diseases and quality of life in in-patient rehabilitation. This study used data of the AURA study and included questionnaires of 123 adults and 156 children and adolescents, objective parameters (SCORAD, asthma severity, skin prick tests, FEV1%, FEV1%FVC and FE(NO)) and subjective parameters (BSI and KINDL questionnaires). There was a clear interdependence between environment, everyday life and atopic disease. Furthermore, the quality of life decreased with increasing disease severity. However, there was no inevitable association between conventional objective parameters and subjective parameters of quality of life.

- Abstract
The fetal stress responses in sows euthanized by electrical current during their second and last trimester of pregnancy (G1 and G2) were evaluated. Three methods of euthanasia of pregnant sows generally applicable to cases of epizootic or emergency slaughter were investigated: 1. conventional application of electrical current to the head and heart (HH); 2. application of electrical current to the head, heart and the uterus (HHU); 3. application of electrical current to the head, heart and from the upper body to the vagina (HHV). Fetuses were delivered by cesarean section at intervals of 3 to 4 minutes and remained attached to the sow by the umbilical cord. Fetal vitality, reflexes, heart rate, blood pressure, rectal body temperature, intracardial arteriovenous pCO2, pH and lactic acid were monitored for a period of 30 minutes. No method was found to kill the fetal pigs immediately. In fetuses at G1 there were no significant differences between the HH and HHU and HHV methods. Fetuses at G2 showed a significantly faster decrease in heart rate and blood pressure as well as a shorter period of time for the absence of fetal body movements and reflexes for the HHT method, compared to the other methods. Since it is not yet known to what extent the fetal pig experiences pain and suffering, the prolonged process of dying for the in utero fetus due to hypoxia which includes struggling and gasps is inconsistent with criteria for humane euthanasia and animal welfare.
