Kontaktallergie (Sensibilisierung) und daraus resultierende allergische Kontaktdermatitis (ACD) sind von erheblicher Tragweite aus Sicht des Gesundheitswesens. Für die vorliegende Übersichtsarbeit hierzu wurden alle passenden Artikel aus dem Zeitraum 2018–2019 per systematischer Suche in Medline und Web of ScienceTM ermittelt; außerdem wurden alle verfügbaren Ausgaben der Fachzeitschriften „Contact Dermatitis" und „Dermatitis" manuell durchsucht und so eine frühere, ähnliche Übersichtsarbeit fortgeschrieben und neu fokussiert. Neue Allergene ebenso wie bekannte Allergene, die in neuem Expositionskontext gefunden wurden oder deren Bedeutung in sonstiger Hinsicht neu bewertet wurde, werden in Abschnitten nach Substanzklasse gegliedert beschrieben (z. B. Metalle, Konservierungsmittel, Duftstoffe). Als gemeinsames Ergebnis zahlreicher Untersuchungen fällt sowohl ein Mangel an Informationen über die Produktzusammensetzung auf, z. B. in Bezug auf ein neu beschriebenes Allergen in Leinenturnschuhen (Dimethylthiocarbamylbenzothiazolsulfid), als auch vor allem die mangelnde Kooperativität von Herstellern von Blutzuckermessgeräten bzw. Insulinpumpen. Zu den letztgenannten Geräten hat eine internationale Zusammenarbeit zwischen Dermatologen und Chemikern gezeigt, dass sie bei zahlreichen Diabetikern schwere ACD verursachen, indem sie Isobornylacrylat bzw. N,N'-Dimethylacrylamid freisetzen. Eine verbesserte und vollständige Deklaration der Inhaltsstoffe für Produkte alle Art – und nicht nur, wie wir es derzeit in Europa haben, bei Kosmetika (abgesehen von der vollständigen Auflistung der Duftstoffe) – muss auf die legislative Agenda gesetzt werden. Background: Contact allergy (CA) to topical corticosteroids (CS) is relatively rare; however, current data from Germany are not available. Furthermore, valid risk assessment needs to take into account of the actual exposure to CS in the population., Patients and Methods: The reaction profile of 9 CS included in a "CS test battery" of the German Contact Dermatitis Research Group (DKG) in patients seen in German clinics belonging to the Information Network of Departments of Dermatology (IVDK; http://www.ivdk.org) between 01/1995 and 12/2004 was analyzed. Applying the "Clinical Epidemiology/Drug Utilization Research" approach, annual incidences of CA to CS in Germany were extrapolated. These estimates were used as numerator for a relative incidence (RI) estimate which used exposure in terms of "defined daily doses" (DDD) to the respective CS as denominator, the latter information collected by the AOK Research Institute (WIdO)., Results: On average, 7.4% of all patients were patch tested with the CS series, mostly yielding < 1% positive reactions. Exceptions included hydrocortisone 17-butyrate (1.5%), amcinonide (1.6%) and budesonide (2.6%). With a RI of 10.7, 23.6 and 4.9 per 100,000 DDD, respectively, the three CS mentioned classify as moderate topical drug allergens. Clobetasol 17-propionate and triamcinolone acetonide both yielded a RI of 1.4/100,000 DDD, indicating low sensitization risk. For hydrocortisone, betamethasone, prednisolone and dexamethasone (RI < 1/100,000 DDD) the risk of sensitization appears minute., Conclusions: The results support and extend previous evidence on the CA risk of CS, adding to a therapeutic index and risk assessment.

Background: Hair cosmetics, in particular hair dyes, bleaches, and permanent wave solutions, contain a number of potential contact allergens. The pattern of exposure to these in hairdressers, clients and home-users may change over time., Patients and Methods: Data on female patients who consulted centers of the Information Network of Departments of Dermatology (IVDK, http://www.ivdk.org) between 01/2003 and 12/2006 was analyzed. Patients were divided into a) hair-dressers, given they worked in this occupation and suffered from occupational skin disease (n = 480),and b) "clients",who never worked as hairdressers, but in whom hair cosmetics were considered a potential cause of contact dermatitis (n = 780)., Results: In hairdressers, the most important allergens were ammonium persulfate (21.7% positive),p-toluenediamine (PTD,19.6% positive),p-phenylenedi-amine (PPD,18.1% positive) and, with decreasing time trend, glyceryl monothioglycolate (2005/2006 still 7.5% positive). Biocides must be regarded as important allergens in hairdressers, namely,(chloro) methylisothiazolinone and methyldibromoglutaronitrile. Clients, who were considerably older on average, had possibly been tested in a more aimed fashion, yielding (strikingly) higher sensitization prevalences to a number of allergens of oxidative hair dyes such as PPD or PTD., Conclusions: The continual analysis of sensitization prevalences in patients patch tested can identify areas needing in-depth research and preventive intervention, such as avoiding the use of certain biocides in shampoo intended for professional use by hairdressers or further reducing the concentrations in oxidative hair dyes.

Background: Anogenital dermatoses (AGD) are common and often very distressing. Clinically it is often unclear if allergic contact dermatitis or irritant dermatitis is involved. In order to optimize therapy and prophylaxis, it is essential to identify relevant allergens or irritants., Patients and Methods: Data of the Information Network of Departments of Dermatology (IVDK, data center in Göttingen) collected between 1999 and 2003 were analyzed. The anogenital area was involved in 1 168 patients with suspected allergic contact dermatitis. Clinical variables and patch test results were statistically compared with the remaining IVDK patch test population, the latter standardized for age and sex., Results: Allergic contact dermatitis had been suspected prior to patch testing in 39.5 %, while in 24.6 % this diagnosis was eventually confirmed. Irritant contact dermatitis was diagnosed in 11.8 %. Other diagnoses, included balanitis, lichen sclerosus et atrophicus and herpes genitalis. Positive reactions to cinchocaine (6.6 %), bufexamac (3.5 %) and benzocaine (2.4 %) were observed significantly more often among patients with anogenital dermatitis. Among those in whom co-factors were considered important (n = 422), wetness (38.4 %), occlusion (30.3 %), mechanical strain (4.7 %) and heat (3.6 %) were mentioned as irritation factors., Conclusion: Because of the significantly higher frequency of sensitization to cinchocaine, benzocaine and bufexamac in patients with anogenital dermatitis, these ingredients should be used only with caution. According to the literature, ingredients of toiletries, cosmetics and contraceptives of any kind seem to cause allergic contact dermatitis rarely although there are several case reports. Comprehensive patch test including the standard series plus major sensitizers such as cinchocaine, benzocaine and bufexamac, and in particular patients' own skin care products, is recommended.

Introduction: Shot peening is widely used for surface treatment of hip implants. Shot peening with steel balls followed by a cleaning process with glass beads is used for introduction of negative stress in the production of morse taper junctions of the MRP-Titan stem. An increasing number of publications in maxillofacial surgery and orthopaedic surgery show that there is a significant contamination of Alumina or glass blasted surfaces. Latest research suggested an association between contaminant particles with early loosening of endoprostheses (third body wear). The aim of this study is to evaluate the amount and the effects of surface contamination with glass particles on morse taper junctions of implants and explants of the MRP-Titan stem., Material and Method: The surface of morse taper junctions of the MRP-Titan stem (5 original-package implants and explants each) are analysed for glass particle contamination. A field emission scanning electron microscopy (LEO 1525) is used for the detection of the glass-particles on the implant surface with a backscattered electron detector. The relative surface area covered by particles was calculated by means of an image analyzing software (analySIS, Soft Imaging System GmbH)., Results: The surface of the implants showed a considerable contamination with glass particles with a mean of 6.67 +/- 0.82% compared to 2.06 +/- 0.74% on the surface of the explants. The difference was statistically significant (p<0.0001)., Discussion: The results of this study show that there is a relative high percentage of contamination with glass particles on shot peened morse taper junctions of the MRP-Titan stem. This contamination is significantly lower on the surface of the explants. With respect to third body wear and osteolysis in total hip arthroplasty further studies are necessary to minimize contamination while maintaining adequate surface quality.
