Patients with advanced heart failure suffer from frequent hospitalizations. Noninvasive hemodynamic telemonitoring for assessment of pulmonary filling pressure has been shown to reduce hospitalizations. In this article, our experience with possible control intervals and the standardization of the follow-up care of hemodynamic telemonitoring is reported.A literature search and our own experience in the follow-up care concerning the implantable pulmonary artery pressure sensor for noninvasive hemodynamic telemonitoring in patients with advanced heart failure are presented.For standardized follow-up care of heart failure patients with hemodynamic monitoring a specialized team consisting of a heart failure nurse and heart failure physician is essential. These teams should ideally work based on a unique standard operating procedure (SOP) to ensure standardized control intervals and a standardized approach to classical hemodynamic changes. However, all therapeutic recommendations have to be prescribed by a physician and must be modified if individually appropriate.Optimized follow-up care for hemodynamically guided heart failure management requires the implementation of novel structures in the German health care system in order to transfer the clinical benefit from clinical trials into daily routine.

We report on the right ventricular (RVP), the pulmonary artery (PAP) and the left atrial pressure (LAP) sensor for non-invasive assessment of the ventricular filling pressure.A literature search concerning the available implantable pressure sensors for noninvasive haemodynamic telemonitoring in patients with advanced heart failure was performed.Until now, only implantation of the PAP-sensor was able to reduce hospitalizations for cardiac decompensation and to improve quality of life. The right ventricular pressure sensor missed the primary endpoint of a significant reduction of hospitalizations, clinical data using the left atrial pressure sensor are still pending.The implantation of a pressure sensor for assessment of pulmonary artery filling pressure is suitable for reducing hospitalizations for heart failure and for improving quality of life in patients with advanced heart failure.

The time constant of left ventricular pressure fall, tau, has frequently been used as a measure of myocardial relaxation in the blood-perfused, ejecting heart. The aim of the present study was to characterise tau in relation to beta-adrenergic activation, coronary perfusion pressure and flow as well as cardiac oxygen supply and demand in the isolated, isovolumically beating heart. Therefore, tau was analysed from digitised left ventricular pressure data in a total of 23 guinea pig hearts perfused with saline at constant pressure (60 cmH2O). The coronary venous adenosine concentration ([ADO]) served as an index of myocardial oxygenation. In the present study, a right heart catheterisation was carried out on three consecutive days in 17 healthy horses to evaluate the day-to-day variability of cardiac pressure values. Cardiac pressure values were measured in the right atrium, the right ventricle, the pulmonary artery, and the pulmonary capillaries (pulmonary artery wedge pressure). Additionally it was examined wether the cardiac pressure variability was influenced by the heart rate and if there are differences between trained and untrained horses. Beside the coefficient of variances, statistical analysis with assessment of variance components were carried out. Therefore the variation between repeated measurements at one day and within one horse, and the variation between repeated measurements over three days and within the horses (day to day intraindividual variation) as well as the variation between the horses (interindividual variation) were taken into account. The absolute interindividual variance of the pressure values and the heart rate was between 0.4 and 19.6 at all day and for all horses. Between trained and untrained horses the variation of the pressure values and the heart rate was lower than in the trained horses. A significant difference between trained and untrained horses was seen in right atrial mean pressure (p0.001) and right atrial diastolic pressure (p0.005). The coefficient of variances of the mean heart rate was 11.8%. The coefficient of variances of the different pressure values ranged between 13.1 (mean pulmonary artery pressure) and 46.4% (right ventricular pressure). By splitting the variances, 54.4-78.9% of the variation was caused by the effect of days, 6.2-30.7% of the variation was caused by the effect of horses and 12.5-23% of the variation was caused by the repeated measurements at the different days. In sumary the results shows that nearly all of the pressure values had a low effect of days on the variation, thus cardiac pressure measurements at one day seem to be sufficient.
