2. [Employees in professional European football: comparison of the personnel structure in sports medicine, sports sciences and physiotherapy between Bundesliga, La Liga, Serie A and Premier League]

Regional and league-specific differences in injury risk and time loss have been observed in professional European football. Besides time of play or different pre-season preparations, possible reasons may also include medical and sport-scientific support. A survey of what UEFA deems to be the best four football clubs has therefore been conducted to investigate the personnel status in the fields of medicine, physiotherapy, massage, sports science and athletic and "strength and conditioning" coaching in order to compare the Erste Bundesliga with the first leagues of the other countries.The study is based on a survey of all 78 teams in the highest football leagues of Germany (Bundesliga), Spain (La Liga), Italy (Serie A) and England (Premier League). The teams were contacted directly and a questionnaire concerning their personnel deployment in the different fields was handed out.The Bundesliga was found to have a significantly lower total number of employees compared with the other European leagues (6.9 vs. 11.02; p 0.0001). The number of physicians in the Bundesliga is significantly higher (2.2 vs. 1.76; p = 0.0259), but the number of physicians dedicating more than 80 % of their total medical practice to the team was significantly lower in the Bundesliga (0.2 vs. 1.45; 0.0001). In the group of physiotherapists (1.8 vs. 3.6; p 0.001), massage therapists (2.1 vs. 2.69; p = 0.0094), sports scientists (0.3 vs. 1.12; p 0.0001) and athletic and "strength and conditioning" coaches (0.5 vs. 1.83; p 0.0001), there were also significant differences between the staff structure in the Bundesliga compared with the grouped results of the other leagues.The personnel structure and the personnel employment in the Bundesliga in sports medicine and sports sciences differs significantly from La Liga, Serie A and the Premier League with the latter three leagues having more personnel and the personnel having closer ties to their teams. Further investigation is necessary to find out if this may be a reason for the differences in injury rates observed between these leagues. A special focus should be placed on country-specific differences in the professions including education and scope of work.Im europäischen Profifußball konnten in letzten Jahren Länder- und Liga-spezifisch signifikante Unterschiede von Verletzungswahrscheinlichkeiten und Ausfallzeiten beobachtet werden. Ein möglicher Grund könnte neben Faktoren wie Einsatzzeit oder unterschiedlicher Vorbereitung die medizinische und sportwissenschaftliche Betreuung sein. Aus diesem Grund wurde eine Befragung der Fußballclubs der laut UEFA 4 besten europäischen Fußballligen bezüglich ihrer personellen Aufstellung in den Bereichen Medizin, Physiotherapie, Massage, Sportwissenschaft und Athletik- sowie „StrengthConditioning“-Training durchgeführt, um den Personaleinsatz der Bundesliga mit den ersten Ligen der anderen Länder zu vergleichen. Grundlage ist eine Befragung aller 78 Mannschaften der erstklassigen Fußballligen der 4 europäischen Länder Deutschland (Bundesliga), Spanien (La Liga), Italien (Serie A) und England (Premier League). Jede Mannschaft wurde persönlich kontaktiert und der Personaleinsatz der einzelnen Fachbereiche mittels Fragebogens erfasst. In der Bundesliga zeigte sich im Vergleich zu den 3 weiteren Ligen insgesamt eine hochsignifikant geringere Mitarbeiterzahl (6,9 vs. 11,02; p 0,0001). Die Anzahl der Ärzte ist in der Bundesliga im Vergleich signifikant erhöht (2,2 vs. 1,76; p = 0,0259), die Anzahl der mehr als 80 % ihrer gesamten ärztlichen Tätigkeit für die Mannschaft tätigen Ärzte allerdings hochsignifikant niedriger (0,2 vs. 1,45; p 0,0001). Auch in der Gruppe der Physiotherapeuten (1,8 vs. 3,6; p 0,001), Masseure (2,1 vs. 2,69; p = 0,0094), Sportwissenschaftler (0,3 vs. 1,12; p 0,0001) sowie Athletiktrainer und StrenghConditioning Coaches (0,5 vs. 1,83; p 0,0001) zeigten sich signifikant niedrigere Zahlen der Personalstruktur in der Bundesliga im Vergleich mit den gruppierten Ergebnissen der 3 anderen Ligen.Es zeigten sich in dieser Untersuchung signifikante Unterschiede der Personalstruktur im Bereich Sportmedizin und Sportwissenschaft zwischen der deutschen Bundesliga im Vergleich zu La Liga, Serie A und der Premier League mit einer höheren und enger an die Mannschaft gebundenen personellen Ausstattung letzterer 3 Ligen. Ob dies ein Grund für die Unterschiede der Verletzungshäufigkeiten zwischen diesen Ligen ist, sollte weiter untersucht werden. Insbesondere sollte auf die länderspezifischen Berufsunterschiede und deren Ausbildung sowie deren Tätigkeitsfelder näher eingegangen werden.

Muscle injuries are common in professional sports, especially in football. Recent epidemiological studies showed that muscle injuries account for more than 30% of professional football injuries (1.8-2.2/1000 h exposure); however, even though there are significant differences within a European comparison, a single professional football team diagnosed on average 12 muscle injuries per season, corresponding to more than 300 availability days lost.The aim of this work is to present the diagnosis, general treatment and comprehensive management of muscle injuries in professional football.The present work is based on current scientific findings, experiences of the authors and examples from routine practice in the management of muscle injuries in a professional sports environment.The authors present a model of gradual progression for the treatment of muscular injuries and their rehabilitation. Due to the time-pressured nature of the professional sports environment, often promoted by coaches and media, this model could help lead players to recover as quickly as possible and return to competitive sports without relapse or sequel injury. This model integrates the player into the treatment plan. The progression sequences in the rehabilitation should be made clear to players and other parties involved, which are crucial for optimal healing. Even if absolute certainty cannot be achieved, i.e. the occurrence of re-injury or secondary injury, this model attempts to minimize the level of risk involved for the returning athlete. Since it is hardly possible to act strictly in line with more conservative guidelines due to the particular circumstances of the professional sport environment, the experiences of the authors are presented in the sense of best practice in order to support future decision-making processes.
