In horses, reduced insulin sensitivity, obesity or abnormal fat distribution and a predisposition to laminitis occur collectively and are summarized as equine metabolic syndrome (EMS). The incidence of the disease is increasing not only in humans but also in horses, and in addition to dietary measures for weight reduction, various drug therapy approaches from human medicine are being pursued as treatment. The aim of this work was to analyze the influence of signalment and management (exercise and feeding) as well as the severity of obesity on the EMS and to determine whether a change in management can or should improve the equine metabolic syndrome or if a reversibility of the EMS factors occurs. For this purpose, 32 horses suffering from EMS were examined and the owners were surveyed about the husbandry, exercise and feeding of the horses. This was followed by an intervention phase in which all horse owners received information on how to change/optimize posture, feeding and exercise with the aim of losing weight or reducing abnormal fat deposits. Finally, a follow-up examination was carried out after three to six months. The results showed that horse management influenced the level of insulin resistance. Horses that were exercised in a controlled manner showed significantly lower degrees of severity (p = 0.002). Feeding also had a significant impact on the severity of insulin resistance. Horses fed unrestricted were moderately or highly insulin resistant (p < 0.001). Horses with severe insulin resistance also had higher heart rates (p = 0.024), as did generally obese horses (p = 0.047). The heart rate of animals with laminitis (p = 0.020) was also significantly higher. At the follow-up after the intervention, 14 of the 32 horses could be re-examined. As a result of the intervention, insulin resistance was no longer detectable in five horses. In summary, the results show that feeding and exercise have a significant influence on the development of insulin resistance in horses with EMS and that optimization in these areas can lead to a significant improvement or even to the disappearance of insulin resistance. The human metabolic syndrome is accompanied by obesity, insulin dysregulation, arterial hypertension and cardiac dysfunction leading to an increased risk for heart diseases in the affected individuals. Insulin dysregulation can lead to an increased tone of the sympathetic nervous system. With the subsequent increased vascular tone, the perfusion in peripheral tissue might be reduced. Furthermore, increased sympathetic tone reduced the heart rate variability (HRV) and a reduction in HRV has been shown in patients suffering from metabolic syndrome even before an insulin resistance becomes clinical apparent. The equine metabolic syndrome (EMS) has several similitudes with the human metabolic syndrome. For instance, increased blood pressure has been reported in ponies with endocrine induced laminitis and this finding led to the hypothesis that endocrine induced laminitis was related to altered vascular tone in the dermal laminae within the hoof wall. Another study found an increased heart rate (HR) in ponies with EMS in comparison to a control population and a significant correlation between the left ventricular wall thickness and parameters of HRV as well as between left ventricular wall thickness and arterial blood pressure. However, no changes in the HRV could be shown in this population of ponies with EMS. Nevertheless, the aim of the presented study was to investigate further whether EMS had an effect on arterial blood pressure or the autonomic cardiac function in horses and whether the severity of general or regional adiposity was related to changes in parameters of HRV or arterial blood pressure. Therefore, the severity of general adiposity, regional adiposity and insulin resistance were assessed in horse with EMS and pooled in an EMS score. Further, heart rate variability was analyzed on recorded electrocardiograms to assess indirectly the autonomic function. Time domain parameters (mean HR; mean RR intervals; standard deviation of normal to normal RR intervals, SDNN; square root of mean squared differences between successive RR intervals, RMSSD) as well as frequency domain parameters (low frequency band, LF; high frequency band, HF; LF/HF ratio) were analyzed. Blood pressure was evaluated non-invasively at the coccygeal artery by oscillometry. 32 horses with EMS could be recruited for the study. The comparison between groups based on t tests or one-way ANOVAs showed that the HR (p=0.048) and pulse frequency (p=0.007) were increased in horses with more severe EMS score. Similarly, horses with severe insulin resistance (p=0.024) as well as obese ones (p=0.047) had increased HR. No difference could be documented in the analyzed parameters of HRV. Nevertheless, in our population older horses showed a shift of autonomic nervous tone towards sympathetic predominance (LF/HF, LF, HF each with p=0.014). In conclusion, we could show that the presence of severe obesity, the degree of insulin resistance and the EMS score had an influence on the HR. Horses with more severe degree of EMS had higher HR in our population. However, it was not possible to show a decrease in the parameters of HRV due to altered autonomous tone or an increased arterial blood pressure in the examined horses with EMS. [ABSTRACT FROM AUTHOR]

Objective: To explore the characteristics of obesity and glycolipid metabolism in premature ovarian insufficiency (POI) patients. Methods: 102 POI patients with age 30.34±4.73 years and 68 healthy age-matched women (control group) with age 30.76±4.69 years, who came to Department of Gynecological Endocrinology in Beijing Obstetrics and Gynecology Hospital between January 2018 and June 2019, were recruited. Age and menstruation of these patients were recorded. Body height, body weight, waist circumference, hip circumference, basal follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), fasting plasma glucose (FPG), fasting insulin (FINS) and blood lipids (total cholesterol, triglyceride, high density lipoprotein-cholesterol and low density lipoprotein-cholesterol) were measured. The indices between POI patients and control group were compared. Results: ①The hip circumference of POI patients was higher than control group (P =0.016). But there were no significant differences in other anthropometric indices between the two groups (P>0.05). ②The comparison of the prevalence of overall obesity(27.4% vs. 23.5%) and abdominal obesity (33.3% vs. 33.8%) between POI patients and control group had no significant differences (P>0.05). ③The level of TG, LDL-c and FINS in POI patients were higher than control group (P<0.05). While there were no significant differences in TC, HDL-c, FPG and HOMA-IR between the two groups (P>0.05). Conclusions: The prevalence of obesity in POI patients was similar to control group. While some of the glycolipid metabolism indices are different between POI patients and control group. So it is necessary to give a comprehensive assessment of POI patients in clinical work. - Abstract
With a prevalence of 15%, polycystic ovary syndrome (PCOS) is the most common endocrinopathy in fertile-aged women. Insulin resistance and obesity play a pivotal role in the pathophysiology of PCOS, modulate the severity of symptoms and are associated with an increased risk for cardiometabolic sequelae such as diabetes, non-alcoholic fatty liver disease and atherosclerotic cardiovascular disease. PCOS should be considered as a gender-specific cardiovascular risk factor. Therefore, if traits indicative for PCOS are present, affected women should undergo PCOS diagnostics as a first step, thereby making it possible to initiate cardiovascular primary prevention strategies in this population of young women at high cardiometabolic risk. In women with known PCOS, screening and treatment of cardiometabolic risk factors and/or diseases should be routinely integrated into the concept of PCOS care. The close link between insulin resistance/obesity and PCOS can be used to improve PCOS-specific symptoms and enhance cardiometabolic health., (© 2023. - Abstract
The term prediabetes describes a fasting blood glucose level that is elevated but not yet in the diabetic range, a blood glucose level that is elevated after 120 min in a standard 75‑g oral glucose tolerance test, or both. The American Diabetes Association definition also includes glycated hemoglobin A (HbA1c). The incidence of prediabetes is rapidly increasing. Progression from normal glucose tolerance to diabetes is a continuous process. Insulin resistance and insulin secretory dysfunction, the simultaneous presence of which characterizes manifest diabetes, are already present in the prediabetic stage. Prediabetes is associated with an increased risk of diabetes; however, by no means all people with prediabetes go on to develop diabetes. Nevertheless, the identification of an increased risk of diabetes is still relevant insofar as it requires the adoption of diabetes prevention measures. Structured lifestyle intervention has been shown to be the most effective strategy for treating prediabetes. To increase its efficiency, it should, as far as possible, be made exclusively available to those people on whom it is most likely to confer a benefit. This would make it necessary to stratify people with prediabetes according to their risk profile. In a population of people at increased risk of diabetes (Tübingen Diabetes Family Study), a cluster analysis was performed, resulting in six clusters/subgroups. Within these, three high-risk subgroups were identified: Two of these risk groups show predominant insulin secretory dysfunction or predominant insulin resistance and high diabetes and cardiovascular risk. The third group shows a high risk of nephropathy and high mortality, but a comparatively lower diabetes risk. In general, prediabetes cannot yet be treated in a targeted pathophysiologically oriented manner. The new classification of prediabetes-based on pathophysiology-is now opening up new avenues for diabetes prevention. Current and future studies should confirm the assumption that the effectiveness of established, or not yet established, preventive measures depends on the respective subgroup., (© 2023. Im Verlauf der SARS-CoV-2-Pandemie gab es jedoch auch immer wieder Berichte über das Erstauftreten von Diabetes mellitus im Zusammenhang mit COVID-19: Immerhin bis zu jede*r sechste Hospitalisierte soll zumindest vorübergehend unter Hyperglykämien leiden, die teilweise auch im post-akuten Setting bestehen bleiben. Die Autorinnen dieser Arbeit stellten sich daher die Frage, wie auf eine neu aufgetretene hyperglykämische Stoffwechsellage im Zusammenhang mit COVID-19 ernährungstherapeutisch zu reagieren ist. Ziel der Arbeit ist es, den aktuellen Forschungs- und Wissensstand zur Ernährungstherapie bei COVID-19-assoziierten Hyperglykämien und Diabetes systematisch zu erfassen und abzubilden. Methoden: Im Rahmen eines Scoping Reviews wurde eine umfangreiche Literaturrecherche in fünf Datenbanken, zahlreichen Fachjournalen sowie auf Websites von Ernährungsfachgesellschaften durchgeführt. 22 Volltexte wurden in das Scoping Review aufgenommen und im Hinblick auf relevante Daten und Aussagen zur Ernährungstherapie bei CO-VID-19-assoziierten Hyperglykämien und Diabetes mellitus ausgewertet. Die extrahierten Ergebnisse wurden sowohl tabellarisch als auch narrativ synthetisiert. Ergebnisse: Die extrahierten Informationen ließen sich grob in sechs Themenfelder gliedern: die glykämische Kontrolle unter enteraler und parenteraler Ernährung; die Abstimmung von Ernährungs- und Insulintherapie bei Hyperglykämien und Diabetes mellitus im Rahmen von COVID-19; kohlenhydratreduzierte Ernährung und ketogene Ernährung; Nährstoffsupplementation; Ernährungstherapie bei Patient*innen mit COVID-19, Hyperglykämie oder Diabetes in Kombination mit Adipositas und Mangelernährung; sowie Ernährungstherapie bei Hyperglykämie und Diabetes im Rahmen von COVID-19 nach der Akutphase. Diskussion: Es konnten einige ernährungstherapeutische Ansätze identifiziert werden, die in Zukunft bei COVID-19 und Hyperglykämien unterstützend eingesetzt werden könnten. Diese beruhen jedoch zum überwiegenden Teil auf klinisch nicht erprobten Hypothesen. In some patients, high glucose levels persist beyond the acute infection. The aim of this review, therefore, is to gather information on possible nutritional interventions for COVID-19 related hyperglycemia and diabetes mellitus and to present the current state of knowledge and research regarding this topic. Methods: For this scoping review, an extensive literature search was performed in five different medical databases as well as additional scientific journals and websites of international nutritional societies. Information sources were systematically screened. 22 sources of evidence were consensually included and relevant data on nutritional interventions for COVID-19 related hyperglycemia or diabetes were extracted. Results were extracted into tables and outlined in a narrative synthesis. Results: Six main topics were identified: glycemic control during enteral and parenteral nutrition; pairing of nutritional therapy and insulin regimen; carbohydrate restriction and ketogenic diet; supplementation of certain nutrients; nutritional therapy for obese or malnourished COVID-19 patients with hyperglycemia/diabetes mellitus; and nutritional glycemic management in the post-acute phase. Discussion: We were able to identify some promising dietetic interventions that could support pharmacological hyperglycemia management in the future. That being said, most of these strategies were exclusively based on hyoptheses, with no clinical data available yet. Therefore clinical trials testing the efficacy and safety of these nutritional interventions are urgently needed.

Introduction: Patients with diabetes mellitus type 2 and also pre-diabetic patients with impaired glucose tolerance have a high risk of cardiovascular diseases, because of common major risk factors. These risk factors also contribute to arterial stiffness. It has been previously described that chronic insulin resistance, positively correlates with arterial stiffness. This study aimed to investigate arterial stiffness as a consequence of acute lipid induced insulin resistance. Subjects and Methods: 18 healthy men were enrolled in a two day study with either a venous lipid infusion or a saline infusion followed by a hyperinsulinemic euglycemic clamp. After the infusion vascular functions were measured using applanation tonometry. Results: The hyperinsulinemic euglycemic clamp showed a significant decrease in glucose uptake as a parameter of insulin resistance after lipid infusion (p, Hintergrund und Fragestellung: Patienten mit Diabetes mellitus Typ 2 und prädiabetische Patienten mit verminderter Glukosetoleranz haben ein erhöhtes Risiko eines kardiovaskulären Ereignisses durch gemeinsame Risikofaktoren. Probanden und Methoden: Bei 18 gesunden männlichen Probanden wurde mittels intravenöser Lipidinfusion eine akute Insulinresistenz induziert, die mittels hyperinsulinämischem euglykämischem Clamp verifiziert wurde. Währenddessen erfolgte die Messung der Parameter der arteriellen Steifigkeit mittels Applanationstonometrie. Zum Vergleich wurde der gesamte Versuchsaufbau ebenfalls mit einer Kochsalzinfusion durchgeführt. - Abstract
Visceral obesity as a component of the metabolic syndrome is characterized by systemic and local inflammation, which can be quantified in organs (metaflammation). This process can be regarded as a chronic, sterile, and low-grade state of inflammation without infection, trauma, tumor or autoimmunity. It is caused by an inflammation of the visceral adipose tissue (adipose inflammation or adipoflammation) due to adipocyte hypertrophy and hyperplasia with increased infiltration by monocytes and macrophages. Important is the presence of proinflammatory, so-called polarized M1 macrophages that are induced by interferon gamma (IFN-γ) and lipopolysaccharides (LPS) with secretion of interleukin (IL)-6, tumor necrosis factor (TNF) and IL‑1. In contrast, the anti-inflammatory, so-called polarized M2 macrophages induced by IL‑4 and IL-13 with secretion of IL‑8 and IL-10 decrease. In addition, the secreted adipokine pattern changes from anti-inflammatory to proinflammatory. Adipocyte necrosis, local hypoxia, dysregulated autophagy, activation of inflammasomes, modulation of toll-like receptors, and epigenetic factors play a complex role. This mechanism results in local insulin resistance and subsequently a systemic insulin resistance of peripheral organs as well as a spillover of local mediators of inflammation into the systemic circulation (measured as obesity C‑reactive protein, CRP). The activation of inflammatory signal transduction cascades leads to inhibitory phosphorylation of the insulin signaling pathway and a weakening of the effect of insulin. In parallel, ectopic lipid accumulation occurs in the liver, musculature, pancreas, pericardium and lungs. Diacylglycerol (DAG) activates specific isoforms of protein kinase C (ε in the liver and τ in the musculature), which in turn lead to inhibition of the insulin signaling pathway. Insulin resistance in obesity and type 2 diabetes mellitus is an inflammatory disease.
