Aims: To review the experience of a single-center with RATS and to assess its outcome, feasibility and safety., Background: With the advent of minimally invasive techniquesin thyroid surgery, robot-assisted transaxillary thyroid surgery (RATS) has become one of the most popular approaches. Its main advantages are improved cosmetic outcome, thereby increasing patient satisfaction, improved visualization and range of motion. Several large-scale studies, have demonstrated its safety and feasibility., Methods: A retrospective review of all RATS consecutive cases at Rabin Medical Center., Results: A total of 38 RATS procedures were performed between July 2012 and May 2015. The mean age of patients was 39 years (22-74); all patients were females; the average BMI was 25 (18-32). Thirty-seven patients underwent partial thyroidectomy and one - total thyroidectomy. The average maximal diameter of nodules pre-operatively was 2.5 cm (0.5- 5). Seventeen patients (44%) had papillary carcinoma; 13 (33%) - hyperplastic nodules and 8 (20%) - benign thyroid adenomas or goiters. The average total operative time was 166 minutes. One case was converted to open due to a large tumor; there were 6 (15.7%) cases of vocal cord paresis, of which 1 (2.6%) was permanent; there were 8 (21%) cases of hypoesthesia - all resolved within 4-12 weeks. There were no cases of permanent brachial plexus injury., Conclusions: In skilled hands, RATS is a safe alternative to conservative thyroidectomy, and should be presented to patients with aesthetic concerns. As with any new emerging technique, careful patient selection is crucial and further evidence must be sought to confirm its indications over time.

Background: During the last decade the surgical technique of minimally invasive video-assisted thyroidectomy [MIVAT) has evolved worldwide. This approach provides Less morbidity and better cosmetic results with the same clinical outcome., Purpose: The aim of this study was to assess the feasibility and efficacy of MIVAT, a new minimally invasive technique for thyroid surgery., Methods: A retrospective analysis of patients who underwent MIVAT between 2007 and 2009 was conducted. For control, we used a demographically and clinically similar group of patients who underwent conventional open thyroidectomy., Results: During the study period 18 patients with a solitary thyroid nodule underwent MIVAT. Indication for surgery was a hyperplastic nodule in all cases. The mean nodule diameter was 1.8 cm in the final pathological examination. Mean operating time was 80.3 minutes compared to 50.27 minutes in the open approach group (p < 0.001). Hospitalization time was significantly shorter in the MIVAT group compared to the open thyroidectomy group, 3.2 versus 4.5 days (p < 0.001). There were no major complications, but 4 patients developed transient vocal cord paralysis and two had wound infection in the MIVAT group, compared to none and one respectively in the control group., Conclusions: MIVAT is a technically feasible and safe procedure that leads to an improved cosmetic result and a quicker recovery. The prolonged operation time and minor complications warrant further experience in our institution.

Contact endoscopy (C.E) of the vocal cord is a new technique for diagnosis of lesions in the vocal cords. This technique is very easy and provides immediate results of the cytological structure of the suspected lesion, assisting in decision-making regarding treatment and follow-up. In the last three years we began using C.E. This study presents our experience with six patients and a review the literature. The C.E technique is an important tool for early diagnosis of lesions of the vocal cord. We believe that in the future, with the technical development of C.E., it will be used routinely not only in the larynx but also in other areas of the body.

Background: The standard treatment for patients with early vocal cord carcinoma has been radiotherapy. In recent years, encouraging results with CO2 laser surgery has changed our treatment strategy in selective cases. During the past four years at the Rabin Medical Center we have treated patients with carcinoma of the vocal cord using CO2 laser surgery., Purpose: We conducted a retrospective study to investigate the results of the Head and Neck Surgery department with CO2 laser treatment for squamous cell carcinoma of the vocal cord during early stages (T1, T2) without radiotherapy., Materials and Methods: Twenty one patients with carcinoma of the vocal cords in early stages were treated with CO2 laser. Nineteen patients suffered from stage T1 carcinoma located in one vocal cord, three of these patients suffered from involvement of the anterior commissure and one patient also had involvement with the anterior part of the second vocal cord. Two patients with carcinoma in stage T2, one with involvement of the ventricular band and one with limited movement of the vocal cord. The follow-up period for the patients ranged from one month to four years., Results: Only one patient in stage T1 developed local recurrence after half a year, no patients died during the follow-up., Conclusion: Careful patient selection with endoscopic staging also during the biopsy, removal of the carcinoma in free border, strict follow-up and stroboscopic examination are essential to secure good results in the treatment of CO2 laser for early stages of vocal cord carcinoma.

Subglottic malignancies are rare. They constitute about 1% of laryngeal cancer. Most of these malignancies are squamous cell carcinoma. In this article were describe various definitions concerning the borders of the subglottic region. A summary of pathophysiology and behavior of cancer in this location is presented. The incidence of primary subglottic carcinoma is low. The origin of most cancers in this area is the glottic cancer that extends into the subglottic region. The tumor tends to spread through lymphatic channels to the paraglottic and preglottic (delphian) nodes and secondary to the jugular chain. The delayed diagnosis is due to the lack of symptoms in the early stage of the disease and the hidden location of the tumor. The presenting symptoms are usually dyspnea and stridor. Direct laryngoscopy is essential for early detection of the tumor and the use of CT and MRI is advocated. Therapy is usually radical. Total laryngectomy and radiation therapy have both been advocated. Between 1995 and 2000, 126 patients with laryngeal squamous cell carcinoma were presented to our department. Our experience in diagnosing and treating subglottic cancer in 4 patients is hereby described. The latest literature on the subject is reviewed.

Though most hemangiomas do not need treatment, a significant minority are associated with complications and external deformities that demand intervention. Steroids play an important role in therapy, but not infrequently afford only partial and temporary benefit. Thanks to improvements in the surgical approach and equipment, hemostasis control devices and laser techniques, we can now treat patients who would otherwise go untreated. Moreover, in certain cases, we can now recommend earlier intervention, saving patients from years of living with deformities and the concomitant psychosocial problems. Vascular anomalies of the head and neck include venular, venous and arteriovenous malformations. These lesions are slow growing vascular ectasia that never involute spontaneously and almost always require intervention. Treatment includes laser therapy, injection of sclerosing agents, embolization through angiography and surgery, which in many cases is the only definitive treatment. We present the current treatment approach and describe our experience in the treatment of 16 patients.

Squamous cell carcinoma of the hypopharynx is an aggressive tumor with a bad prognosis. When the tumor is resected it is necessary to reconstruct the hypopharynx and the esophagus. Today, reconstruction with a jejunal free flap is considered to be the preferred option. We present a series of five patients with squamous cell carcinoma of the hypopharynx operated on at the Department of Otolaryngology in the Rabin Medical Center. All underwent total laryngopharyngectomy and reconstruction with a jejunal free flap. Three patients resumed total enteral nutrition about two weeks post surgery. One patient suffered from a pharyngocutaneous fistula and continued parenteral nutrition via a jejunostomy. One patient suffered from necrosis of the flap, which necessitated its resection. Despite the morbidity which follows surgery of the hypopharynx and despite the low cure rates, a successful reconstruction with jejunal free flap enables the resumption of oral nutrition and improvement in the patients quality of life.

Sensory loss in the operative area after superficial parotidectomy is one of the complications which disturbs the patient. The greater auricular nerve, which stems from the cervical plexus, supplies sensation to the area. Sacrifice of the nerve during superficial parotidectomy causes sensory loss in the area. We investigated the degree of sensory loss in the operative area after superficial parotidectomy in 10 patients in whom the posterior branch of the greater auricular nerve was preserved and compared the results with those in whom it was not, a year after operation. There was more sensory loss when the greater auricular nerve was sacrificed. We therefore suggest preserving the posterior branch of the greater auricular nerve during superficial parotidectomy if at all possible.

Kimura's disease is a rare angiolymphoid proliferative disorder of soft tissue characterized by subcutaneous swelling and a predilection for the head and neck. There are usually enlarged regional lymph nodes, eosinophilia and elevated sedimentation rate and IgE levels. A 26-year-old women with subcutaneous masses in the submandibular area is reported. The diagnosis of Kimura's disease versus angiolymphoid hyperplasia is discussed.

The potential parapharyngeal space contains the great vessels of the neck, cranial nerves IX-XII, the sympathetic chain and lymph nodes. Surgical access is difficult and the infrequency of tumors in it make surgery of this part of the head and neck a challenge, 8 women and 2 men (mean age 54.7, range 31-70 years) with tumors of the parapharyngeal space operated on during the past 6 years are presented. The most common surgical approach was the cervical submandibular, used in 6 of our patients with relative ease and minimal complications. Tumors of deep lobe parotid origin should be operated through a transparotid approach. Large benign or malignant tumors have a better prognosis when the transcervical transmandibular approach is used, providing wider access.

Speech rehabilitation after total laryngectomy has often been neglected, and traditional methods of esophageal voice production are frequently unsuccessful or disappointing. The development of the tracheoesophageal puncture (TEP) technique and the valved voice prosthesis by Singer and Blom in 1979 offered patients an alternative method for laryngeal speech rehabilitation. This technique is now used successfully in an increasing number of patients after total laryngectomy. We present our experience with TEP since 1988 in 8 men and 1 woman, aged 50-86 years, who had adequate follow-up. In 5, primary TEP was performed at the same time as the total laryngectomy; in 4, secondary puncture was performed 12-18 months after primary surgery. 1 had serious operative complications, 2 had minor complications, 1 had long-term problems which necessitated discontinuing use of the prosthesis, while 8 achieved satisfactory voice use and could communicate effectively. We hope that further experience with TEP in Israel will confirm its effectiveness for speech rehabilitation after total laryngectomy.

Maxillary sinus cysts are common lesions usually found in routine x-rays of the teeth and sinuses. Although most patients are asymptomatic, there may be headache or periorbital or facial pain. Traditionally, these cysts have been treated by puncture and aspiration, or excision via intranasal antrostomy or Caldwell-Luc operation 23 patients with maxillary sinus cysts who presented with facial pain were treated by endoscopic surgery. During followup of 1 year the cyst recurred in only 1 of the patients (after 2 months). We recommend endoscopic removal of maxillary sinus cysts as the treatment of choice.

Blunt neck trauma is usually caused by auto accidents and may result in various degrees of laryngeal injury. Treatment involves careful monitoring followed by surgical intervention when there is deterioration, or by early exploration and laryngeal fixation (internal or external). 16 patients treated for blunt neck trauma, ranging in age from 10-54 years (mean 27.7), were studied. Only 1 required surgical intervention.

The frequency of penetrating neck injuries has increased in recent years. In 16 patients with such injuries treated during the past 2 years, neck exploration was performed immediately. In 4 there was destruction of the throat or trachea, in 3 a tear in the esophagus, in 1 a tear in the hypopharynx and in 1 a tear in the submandibular region penetrating the floor of the mouth. In 8 there was no damage to the neck organs. Of the 16, 1 died on the operating table from hemorrhage from multiple tears of the hypopharynx, the esophagus and the thyroid gland. 2 different approaches are used in dealing with penetrating neck injuries: immediate exploration, or the use of imaging methods to diagnose the degree of damage, followed by close supervision. In the light of our experience we advocate immediate neck exploration.
