Introduction: Emergency contraception is an effective and safe solution for preventing unwanted pregnancies. Many methods of emergency contraception are used, which have different mechanisms of action and time frames., Objective: Providing information to healthcare professionals and decision-makers based on the literature data about the target populations of emergency contraception, evidence-based modern methods, their effectiveness, and practical application for the purpose of reducing the incidence of unintended pregnancies., Methods: We conducted a systematic literature search in MEDLINE (PubMed), Embase and Scopus databases based on relevant keywords, for publications that were published between 1960 and 2023., Results: 23 clinical professional publications were selected that examined the effectiveness of oral and long-term usable contraceptives as emergency contraceptives. Our research results were interpreted in terms of weight, breastfeeding, time elapsed since the intercourse and future contraceptive plans, which help to select the most appropriate emergency contraceptive for healthcare professionals., Conclusion: Based on the literature data, our systematic review provides assistance for choosing between the available oral levonorgestrel, ulipristal acetate, and intrauterine contraceptive devices available in Hungary based on effectiveness, target population, and accessibility. We support the healthcare governance in creating up-to-date professional guidelines to improve the availability of emergency contraception and, consequently, enhance reproductive health. - Abstract
Uterine rupture is a rare complication during pregnancy which may even lead to the death of the mother and the fetus. In this report, the management of a herniated amniotic sac associated with uterine dehiscence is presented. A 25-year-old primigravida at 30 weeks with a history of a previous unilateral salpingectomy was transferred to our hospital with the potential diagnosis of a herniated amniotic sac, which was discovered during a routine ultrasound scan. The patient had no prior complaints or any other signs that would have indicated a serious condition during her pregnancy. She underwent an emergency cesarean section and a two-layer closure of the lesion. An adequate suture during laparoscopic salpingectomy provides prevention of uterine rupture during subsequent pregnancies. A medical history with a previous laparoscopic dehiscence should necessitate a more frequent sonographic observation during pregnancy. Orv Hetil. 2023; 164(25): 988-992.

6. [Laparoscopic treatment of deeply infiltrating colorectal endometriosis - ten years of single center experience].

- Abstract
Introduction: Deep infiltrating endometriosis penetrates the peritoneal surface beneath 5 mm. The bowel is affected in 3-37% of the cases., Objective: The aim of the authors was to analyze the results of the surgical procedures performed for bowel endometriosis., Method: Between 2009 and 2020, 675 patients underwent bowel endometriosis surgery at the Department of Obstetrics and Gyanecology of Semmelweis University. Four surgical approaches were performed: shaving, discoid, segmental and NOSE resection., Results: 182 shaving, 93 discoid, 130 NOSE and 270 segmental bowel resections were performed. Ultra-deep anastomosis was performed in 40 cases. The median operative time was 85 minutes, the shortest intervention lasted 25 minutes, the longest 585 minutes. The average operating time was 260 (± 161.3) minutes for the first, and 114 (± 47.0) minutes for the last ten operations. The average blood loss was 10 (± 20.3) mL. The average hospital stay was 6 (± 2.3) days. Serious surgical complication (Clavien-Dindo III or more severe) developed in 18 cases. In a total of 17 cases sigmoideo- or ileostomy were used. Conversion to laparotomy was necessary in 6 cases., Discussion: The same team performed all the interventions, which can show the effectiveness of the surgical techniques instead of the technique of individual surgeons. The complication rate is low in the case of an experienced surgical team, and the operating time decreases significantly in proportion to the number of operations performed., Conclusion: Bowel endometriosis can be treated safely and effectively with both conservative (shaving or discoid) and radical (segmental or NOSE resection) approach. - Abstract
Hydatidiform mole is an extremely rare gestational trophoblastic disease. The pathogenesis is unique in that the maternal tumor arises from pregnancy tissue. In terms of occurrence, it mainly affects women of reproductive age. In our case report, a 53-year-old female patient was diagnosed with molar pregnancy as the background of her perimenopausal bleeding disorder. Mola is a high-risk disease because the symptoms can be deceiving and may be very similar to those of an incomplete miscarriage, ectopic pregnancy or perimenopausal bleeding. Appropriate diagnosis is key to terminating the pregnancy as soon as possible. The purpose of our case report is to draw attention to the atypical appearance of molar pregnancy; in relation to our perimenopausal female patient, we review the basic principles of treatment of hydatidiform mole and present the diagnostic and therapeutic management of a successfully completed case. - Abstract
In women, primary sclerotising cholangitis (PSC) associated with ulcerative colitis and intrahepatic cholestasis is a rare disease. To date, there are no data from Hungary on the fertility and pregnancy outcome of women with this chronic liver disease. Our aim is to present the favorable pregnancy outcome of a woman with PSC associated with ulcerative colitis, intrahepatic cholestasis and postpartum colectomy, and review of the literature. A young nulligravida was first diagnosed with ulcerative colitis. Five years later, PSC developed with progressive fibrosis and cholestasis necessitating liver transplantation. While on waiting list, spontaneous conception occurred. Except for pregnancy-induced hypertension, pregnancy uneventfully progressed until the third trimester when 8 g oral cholestyramine/day was administered to lower high maternal (over 100 μmol/L) total bile acid (TBA) level. In the 36th week of gestation acute exacerbation of ulcerative colitis resulted in maternal fever and chorioamnionitis leading to fetal distress. Elective delivery of the eutrophic neonate followed by emergency cesarean section. In the early puerperium, colitis progressed to septic pancolitis resistant to medical treatment. 12 days after laparoscopic subtotal colectomy, the patient was discharged in good health condition. 3 months later, ileostomy was closed and she got back on the transplantation waiting list. Our data correspond with previous reports and suggest that women with PSC with underlying ulcerative colitis and cholestasis have a good chance for favorable pregnancy outcome. However, both PSC and underlying colitis might progress during pregnancy and puerperium. Oral cholestyramin is an effective and safe treatment for high maternal TBA levels. Összefoglaló. Az "akut has", mint kórkép, a várandósság során mind diagnosztikailag, mind terápiásan nagy kihívást jelent a szülész számára a megváltozott anatómiai viszonyok és a képalkotó eljárások korlátozott használhatósága miatt. Közleményünkben bemutatjuk egy harmadik trimeszterbeli gravida hasi vérömlenyének az esetét, melynek vérzésforrása irodalmi ritkaság, a vérzés típusát a felkereshető irodalomban még nem írták le terheseknél. Az esettanulmány megírása a páciens orvosi dokumentációi és az ellátásában résztvevő klinikusok, patológusok leletei és beszámolói alapján történt. A várandósságának 33. hetében járó gravida akut hasi tünetegyüttessel jelentkezett klinikánkon, anamnéziséből trauma kizárható volt. Az anya romló kardiorespiratorikus állapota miatt sürgősségi császármetszéssel élő, koraszülött magzatot segítettünk világra. A műtét során kb. 1500 ml vért találtunk a hasüregben, melynek okát általános sebész bevonásával kerestük. Aktív vérzésforrásként a lép ereinek területéről vénás jellegű szivárgó vérzést találtunk, ezért splenectomia mellett döntöttünk. Az eltávolított szervkomplexum teljes szövettani feldolgozása helyenként a kisebb léphilusi vénaágak izomfalának felrostozódását és fokozatos eltűnését igazolta, a patológus a vérzés okát a meggyengült érfal endothelen keresztüli vérszivárgásának véleményezte, definitív érfalszakadásra utaló elváltozás nem volt azonosítható. A haemoperitoneum várandósság alatt kifejezetten ritka kórkép, azonban mortalitása mind az anyára, mind a magzatra nézve rendkívül magas. Irodalmi adatok alapján a vérzésforrás leggyakrabban a kismedencei szervek területén jelentkezik. Ritkán a lépartéria aneurysmájának rupturája is előfordul terhességben, a lépvénás eredetű vérzés viszont irodalmi ritkaságnak számít, és az összes feldolgozott és közölt esetben az ér szakadásos sérülése volt a fő ok, az esetünkben leírt, ismeretlen etiológiájú, érfalvesztéses hemorrhágiát sehol nem említik. A várandósunk ellátásánál ellátásánál kulcsfontosságú volt a gyorsan elvégzett exploratív laparotomia, mely a kórkép okát felderítette és megszüntette, lehetővé téve az anya és gyermeke túlélését és felépülését. Esetünk rámutat arra, hogy a várandóssági hasűri vérömlenyek kóroktana akár irodalmi ritkaságokat is rejthet, azaz a pontos ok kiderítésével, lumenes vérzésforrás keresésével nem szabad szükségtelen időt veszteni az édesanya és gyermeke életmentő ellátásának rovására. Orv Hetil. 2022; 163(8): 328-332. Summary. Acute abdomen during pregnancy is a major challenge for physicians, due to the altered anatomical conditions and the limited use of medical imaging. We present a pregnant woman with a rare form of abdominal bleeding. This case study is based on the patient's medical documentations and interviews with her physicians. A woman in the 33rd week of her pregnancy was admitted to our department with acute abdomen, trauma was ruled out. Due to the deteriorating cardiorespiratory status we performed an emergency caesarean section delivering a living premature newborn. A general surgeon was present, and 1500 ml of blood was evacuated from the abdomen. Active venous type bleeding was present from the splenic vessels, therefore we performed a splenectomy. The histopathological analysis of the whole organ complex showed that the media layer segmentally and progressively disappeared in the splenic hilar veins, giving rise to the high probability that the bleeding was caused by a diathesis-type leakage through the endothelium and to the weakened media layer in the absence of a luminal-type vessel rupture. Hemoperitoneum during pregnancy is an exceptionally rare condition, however its mortality rate is high both for the mother and the fetus. The bleeding usually originates from the pelvis. On rare occasions the bleeding occurs from the splenic vessels although most of them are arterial bleedings from an aneurysm.In our case the emergency laparotomy was essential for the correct diagnosis and the treatment, which resulted in saving and recovering both the mother and the fetus. 11. Creating conditions for male partner involvement in birth in the Hungarian obstetric wards during the COVID-19 pandemic

Összefoglaló. Bevezetés: Az Egészségügyi Világszervezet adatai alapján 2020 őszére a világ minden régióját elérte az új koronavírus (SARS-CoV-2) okozta fertőzés. Magyarországon 2020 márciusában az első esetek - több más betegjogi kérdés mellett - ráirányították a figyelmet a betegtájékoztatás kiemelt fontosságára, annak jogszabály szerinti betartására, valamint az alkalmazás során fellépő nehézségekre. Tekintve, hogy az együttszülés lehetősége az utóbbi évtizedekben egyre népszerűbb a családtervezés és a közös életkezdés folyamatában, az apák felkészítése a szülésre - a járványhelyzet miatt megváltozott tájékoztatási szigorítások összehangolásával - a szülészeti osztályok jelentős feladata lett. A betegtájékoztatás teljes körű alkalmazása során számos szempontot kellett figyelembe venni ahhoz, hogy a szülés az anya és az apa számára pozitív emlék maradjon, még olyan helyzetben is, mint a COVID-19-járvány. Célkitűzés: Elemezni kívántuk, hogy a magyarországi szülészeti osztályokon hogyan lehetett a világjárvány idején is betartani a hatályos jogszabályokat és eljárásrendeket, milyen intézkedések váltak szükségessé az együttszülés gyakorlatának fenntartásához, és egyidejűleg biztosítani, hogy a szülőnőnek és férfi partnerének a tájékoztatáshoz való jogai ne sérüljenek. Módszer: Online 14 feleletválogatós kérdőívet küldtünk valamennyi magyarországi, állami fenntartású kórház és klinika szülészeti osztálya vezetőjének. A beérkezett kérdőívek adatait összevetettük az intézmények honlapján az együttszüléssel kapcsolatos információkkal is. Eredmények: A 2020. március 20. és szeptember 20. közötti fél évben a várandós párok tájékoztatása során a szolgáltatók minden esetben a hatályos eljárásrendeket követték, ez azonban több esetben többlet anyagi ráfordítást és szervezést jelentett számukra. Következtetés: Adataink rámutatnak arra, hogy az együttszüléssel kapcsolatosan a szülésre érkezők igyekeznek a lehető legjobban tájékozódni, és ugyanolyan elvárásaik vannak a szülés körülményeit illetően, mint a járványmentes időszakban. Amennyiben a tájékoztatás kielégítő és részletes számukra, az esetleges hátrányos változtatásokat is jobban tolerálják. A járványra tekintettel valamennyi osztályon megmaradt az együttszülés lehetősége, a megfelelő járványügyi intézkedések megtartása mellett. Orv Hetil. 2021; 162(21): 824-829., Introduction: According to data collected by the World Health Organization, by the autumn of 2020, all regions of the world were affected by SARS-CoV-2 viral infection. In Hungary, the first cases were detected in March 2020, and the epidemic, among several other patient rights issues, drew attention to the paramount importance of patient information, its compliance with the law, and the difficulties encountered in its application. During the last decades, the possibility of male partner involvement in birth has become increasingly popular in the process of family planning and starting a life together. Preparing fathers for childbirth and coordinating information restrictions that have changed due to the epidemic is a priority for obstetric departments. The full application of patient information requires a number of considerations to ensure that childbirth remains a positive memory for mother and father, even in a situation such as the COVID-19 pandemic., Objective: We wanted to analyze how the obstetric wards in Hungary were able to comply with the laws and procedures in force at the time of the pandemic, what measures became necessary for creating male partner involvement in births, and at the same time to ensure that the rights of the mother and the father are not violated., Method: We sent an online questionnaire to the head of the obstetrics department of all Hungarian state hospitals and clinics, using 14 multiple-choice questions. The data of the received questionnaires were also compared with the information related to male partner involvement in births published on the websites of the institutions., Results: In the period from 20 March to 20 September 2020, in terms of informing pregnant couples, the service providers always followed the procedures in force, but in several cases this meant more financial expenditure and organization for them., Conclusion: Our data showed that those who give birth insist on the best possible information about conditions of male partner involvement in births, have the same expectations about the conditions of childbirth as during the epidemic-free period. If the information is satisfactory and detailed for them, they are more tolerant of adverse changes. In view of the epidemic, the possibility of father involvement in birth remained in all departments available, while maintaining appropriate epidemiological measures. - Abstract
Introduction: Genitourinary syndrome of menopause (GSM) affects up to 40-57% of postmenopausal women. Intravaginal microablative fractional CO
2 laser is a new proposal for the management of GSM, although the evidence of safety and efficacy of the procedure appears to be insufficient. Aim: The aim of the study was to assess the efficacy of fractional CO2 laser for the treatment of GSM at the Department of Obstetrics and Gynecology of the University of Debrecen. Method: Postmenopausal women with symptoms of GSM underwent three sessions of microablative fractional rejuvenation CO2 laser therapy at 4-6 weeks intervals. Vaginal health index (VHI) scores were completed before each treatment and at 6 weeks follow-up as an objective measurement and visual analog scale was used to assess subjective complaints. Statistical analysis included Student's paired two-sampling t-test for the measure of statistical significance using the standard cutoff for significance p<0.05. Results: 51 women participated (mean age 57.0 ± 9.9 y). Average VHI score was 14.0 ± 4.9 before treatment, 15.0 ± 4.7 after the first session, 18.2 ± 4.6 after the second treatment and 19.5 ± 4.9 at follow-up. The improvement of VHI score was statistically significant between all sessions. Average VAS score was 15.6 ± 14.1 before treatment, 9.0 ± 10.8 after the first session, 5.9 ± 9.2 after the second treatment and 3.4 ± 7.5 at follow-up. The improvement of VAS score was statistically also significant between all sessions. Conclusions: Our study suggests that the fractional CO2 laser is an effective and safe treatment of symptoms associated with GSM. Orv Hetil. 2019; 160(41): 1617-1622.- Published

13. [The effect of surgical treatment of bowel endometriosis on fertility].

- Abstract
Introduction: Bowel endometriosis is when endometrial-like tissue penetrates the bowel serosa, or it reaches the subserous neurovascular plexus. The effect of surgery for colorectal endometriosis on infertility and pregnancy is not fully proven. Aim: The aim of the present study was to analyse the pregnancy outcome and mode of delivery of patients who underwent 'nerve sparing' anterior resection of the colon. Method: Between 2009 and 2017, we operated 121 patients with bowel endometriosis, and built up a prospective database where we assessed their wish of pregnancy, the way of the conception, pathologies during pregnancy and mode of delivery. Statistical analysis: The relationship between endometriosis and pregnancy pathologies was tested by a χ
2 probe and Fisher's exact test, additionally the odds ratio (OR) and 95% confidence interval (CI) were determined. For p<0.05, the result was considered significant. Results: Out of 121 bowel endometriosis patients, 48 (39.6%) women got pregnant, 37 (30.5%) of them with in vitro fertilisation. The control group was built from patients who underwent in vitro fertilisation because of andrological factors. We found that women with endometriosis have a significantly higher risk for praeeclampsia (p = 0.023) and placenta praevia (p = 0.045) during pregnancy. Conclusions: Our study is a unique description of pregnancy outcome and mode of delivery after surgery for bowel endometriosis, which, despite the small number of cases, has yielded similar results to the previous multicentric studies. - Abstract
The basic legal conditions for pursuing medical activities in Hungary require that psychologists be employed at certain somatic departments. In the Hungarian medical practice, however, attitudes recognizing the potential role of psychological factors in the course of diseases are not widely present. The modern psychosomatic approach to healing includes biological, psychological and social factors that may predispose one to, precipitate or perpetuate a medical condition. This case study of a 35-year-old woman reports on a therapy in which the symptoms were indicative of both premature ovarian failure and anxiety, influencing each other in a bidirectional way. Therapy also included interventions on physical (hormonal therapy, Jacobson's progressive relaxation technique) and psychological levels (cognitive behavioral interventions). - Abstract
Introduction: At present, there is no obligatory guideline for the prevention of early-onset neonatal group B streptococcal disease in Hungary., Aim: The aim of the present study was to gain insight into the spontaneously developed preventive strategy of the domestic obstetric divisions and departments in Hungary., Method: Standardized questionnaire was sent out to each of the 71 obstetric divisions and departments in Hungary., Results: Overall, 20 (27.4%) of the chairpersons replied, and thus, 39.9% of the total number of live births in Hungary were included in the study. Despite missing public health guidelines, each of the divisions and departments developed their own strategy to prevent neonatal group B streptococcal disease. In 95% of cases, bacterial culture of the lower vagina was the method of identifying pregnant women at risk. In 5% of the cases intrapartum antibiotic prophylaxis was based on risk assessment only. Of the departments using culture-based prophylaxis, 58% departments sampled women after completion of 36th gestational weeks. Antibiotic of choice was penicillin or ampicillin in 100% of cases. Of the study participants, 80% reported on multiple administration of colonized pregnant women after onset of labor or rupture of the membranes., Conclusions: The authors concluded that the rate of participation in the study was low. However, prevention of early-onset neonatal group B streptococcal infection is a priority of obstetric care in Hungary. Lack of a nation-wide public health policy did not prevent obstetric institutions in this country to develop their own prevention strategy. In the majority of cases and institutions, the policy is consistent with the widely accepted international standards.
