Abstract

Tubulointerstitial nephritis is a common cause of acute renal failure, in two thirds of cases it is associated with drugs (mostly antimicrobials and NSAIDs), in 5-10% of cases it is associated with infections (bacterial/viral/parasitic), in 5-10% of cases it is idiopathic (this is the case of the TINU syndrome characterized by interstitial nephritis and bilateral uveitis, and the anti-glomerular basal membrane antibody syndrome), and finally in 10% of cases it is associated with systemic diseases (sarcoidosis, by Sjogren, LES). The pathogenesis is based on a cell-mediated immune response and in most cases removing the causative agent is the gold standard of therapy. However, a percentage of patients, in a variable range from 30% to 70% of cases, do not fully recover renal function, due to the rapid transformation of the interstitial cell infiltrate into vast areas of fibrosis. Clozapine is a second generation atypical antipsycothic usually used for the treatment of schizophrenia resistant to other types of treatment; it can cause severe adverse effects among which the best known is a severe and potentially fatal neutropenia, furthermore a series of uncommon adverse events are recognized including hepatitis, pancreatitis, vasculitis. Cases of acute interstitial tubular nephritis associated with the use of clozapine have been described in the literature, although this complication is rare. Medical personnel using this drug need to be aware of this potential and serious side effect. Abstract

Sarcoidosis is a granulomatous disorder with multiorgan involvement which may appear in an isolated form but more often as a systemic disease. We report the case of a 53-year-old woman presenting with acute renal failure, hypercalcemia, elevated 1.25 dihydroxycholecalciferol, and a history of fatigue, weight loss and arthralgia of several months. Kidney biopsy had revealed interstitial noncaseating granulomas, so sarcoidosis was considered as a potential diagnosis after exclusion of other granulomatous disorders. Granulomatous tubulo-interstitial nephritis (GIN) is an uncommon disease with a low, but perhaps underestimated incidence: only about 100 cases have been described in the literature. In these cases it was found that the disease may lead to deterioration of renal function and irreversible progress to end-stage renal disease. The treatment of choice is the administration of steroids.

Abstract

The discovery of glycosuria with normal glycemia is generally interpreted as a consequence of a congenital tubular defect. Nevertheless it may be also the result of an acute interstitial nephritis. This conclusion is supported by the clinical and biochemical picture observed in two children, presenting with apparently unexplained fever and significant increase of inflammatory blood indices, in whom euglycemic glycosuria represented the critical finding for diagnosis of acute interstitial nephritis.

Abstract

Hypereosinophilic syndrome is a pathological condition whose etiology is still unclear. Having been introduced for the first time Hardy and Anderson in 1968, the term is used to indicate hypereosinophilia which does not accompany those pathologies with which it is more commonly associated. This paper reports the case of a patient affected by hypereosinophilic syndrome associated with kidney insufficiency. This association with a clinical picture of eosinophilic interstitial nephritis has rarely been described in the past. The Authors discuss the possibility of a possible evolution towards renal insufficiency, probably caused by the successive appearance of arteriolar glomerulitis with vasculitis, leading to the dysfunction of a growing number of nephrons.
