This paper examines the formation of the concept of evidence-based medicine（EBM） in physician culture by analyzing articles on EBM published in medical journals between November 1992 and January 1996. The analysis focused on contestation over EBM. The results revealed that articles supporting and criticizing EBM, although seemingly at odds with each other, share a common view. In other words, together, the papers defending and denying EBM constituted a rhetoric that promoted the “probabilization of medicine”.

Clinical guidelines have an increasing role in the delivery of standardized health care and appropriate treatments to patients. Japanese Circulation Society (JCS) and Japanese Heart Rhythm Society (JHRS) regularly issue guidelines relating to cardiac arrhythmias. In this article, author described practical steps of guideline publication as a steering committee member of the JCS/JHRS 2020 Guideline on Pharmacotherapy of Cardiac Arrhythmias. Nowadays, guidelines are developed according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE). GRADE system has been adopted by many working groups producing guidelines. GRADE approach is important to guarantee the transparency in working group communication of how the guideline was developed and how the recommendation was reached. Patient and public involvement is important in setting clinical question and subsequent development steps in clinical practice. GRADE is important also in distinction between the level of the evidence and the strength of recommendation to the individual clinical question. To determine the strength of recommendation, GRADE takes the quality of evidence and the balance of benefit and harm into account. Japanese Counsil for Quality Health Care has started Medical Information Distribution Service (so-called ‘Minds’) to disseminate and implement the evidence-based medicine (EBM) and developed a protocol and schedule of evidence-based guideline publication. The current Japanese guidelines contain Minds grade of recommendation and Minds classification of evidence. In conclusion, the frequency of guideline publication is increasing. Understanding GRADE and Minds approaches enable practitioners to make reasonable informed clinical choice.

システマティック・レビューにおいては,網羅的・系統的な文献検索が不可欠な作業として求められる。本稿では,システマティック・レビューの理解に必要となるキーワード,EBMとコクランについて紹介したのち,システマティック・レビュー,特にシステマティック・レビューにおけるデータベースの選択・検索について概説する。システマティック・レビューの文献検索は,データベースごとに特徴と機能,注意点を理解したうえで,検索式を設定することが重要である。また,システマティック・レビューやシステマティック・レビューに利用でき得るリソースの情報収集とアップデートを継続することも必要である。

近年，ユーザーのニーズに合わせてカスタムメイドが可能である金属3Dプリンタを用いた積層造形（Additive Ｍanufacturing: ＡＭ）が注目を集めている.航空機や自動車のマイナーチェンジによる部品の変更において金型を作り直しての製造は負担となるが,積層造形により部品を製作することで金型の新規作製に依らずとも改修などに対応ができる．また，航空宇宙分野においては高価な金属材料が多量に使用されるため,歩留まりの向上および構造部材の軽量化か求められており，接合を要せずに一体で製造が可能であり，軽量化が可能な金属3Dプリンタに大きな期待が寄せられている．しかし現状で実用化されている製品は，外観を重視する装飾品や大きな耐荷重を必要としない部品，また破損しても重大な問題や事故に繋がらない用途に限られ,広く従来の塑性加工品や鋳造品の代替部品には適用には至っていない.その一方で積層造形材の強度や延性,機能性などが従来の鋳造品に比べて優れる場合も多く，積層造形特有の組織形成を活用することでより優れた材料を製作することが期待できる．この積層造形特有の組織の形成要因を，造形条件（出力，走査速度，積層厚さ等），組織，特性の相関から知見を得ることで解明し，再現性が良く安定した特性を有する積層造形材を製作することで,広範にわたり従来鋳造品の代替とすることが期待される．しかし，積層造形技術により製作した材料特有の組織や強化機構，強度との関係を体系的に示した研究例は少ない． そこで，本研究では固溶強化合金で析出物を有しない無変態（単相）オーステナイト系ステンレス鋼SUS316Lを供試材とし，従来塑性加工（Plastic Forming: PF）材との機械的性質および微視組織の比較，ならびに積層方向依存性（異方性）を明らかにすることにより，積層造形材がもつ独白の強化機構や微視組織の特徴を明らかにする．また，航空機用に使用される高強度ステンレス鋼SUS630において，溶融凝固を繰り返す熱履歴を受ける積層造形法におけるδフェライト出現の有無やそれによる影響を調べ，ＳＬＭ法特有の組織および強度に及ぼす影響を調べることを本研究の目的とした． 以下に本研究の結論を述べる．まず，SUS316Lについて，（i）選択的レーザ溶融（Selective Laser Melting:SLM）材,電子ビーム溶融（Electron Beam Melting:EBM)材ともに積層方向に柱状粒が形成すること（ii）ＳＬＭ材では加熱・冷却が繰り返されるため，柱状粒の粒界がジグザグに折れ曲がり，ひずみの大きい結晶組織となること（iii）ＥＢＭ材では造形中チャンバ内粉末床温度が700～1000゜Ｃと高温に保持され冷却速度が小さいため，積層方向に｛001｝面が配向し，ひずみの少ない結晶組織となり，SUS316凵こおいて〈001＞方位のクリープ強度が強いため，0deg材（積層方向と引張負荷軸が同一の試験片）の方が90deg材（積層方向と引張負荷軸が垂直の試験片）に比べてクリープ抵抗が強いこと（iｖ）90deg材の方が0deg材に比べて機械的性質（0.2％耐力，引張強さ，延性）に優れ，特に24°ＣにおけるＳＬＭ材においては顕著であるのは溶融凝固界面（Molten Pool Boundaries: ＭＰＢs）に沿った欠陥が0deg材において開口き裂発生点となるためであること（ｖ）ＥＢＭ材は従来の溶体化処理を施した熱間加工材のPF材と同等の機械的性質を示すことを明らかにした． 次に,SUS630について,（i）積層造形まま（as-built）材および直接時効（Direct Aged: ＤＡ）材では溶体化処理をしないため，ひずみの少ない粗大δフェライト相が存在し溶体化時効処理（Solution Treatment and Aged: STA）材よりも高延性となり，このδフェライト相は急熱・急冷という熱履歴を受けるＳＬＭ積層造形プロセス中において形成され，凝固割れを防止すること（ii）積層造形材では強度は従来材と同程度であるが，ＭＰＢsに沿った欠陥が存在するため，従来材よりも延性が低下し，また90deg材の方が0deg材に比べて強度が優れること（iii）ＤＡ材では，逆変態オーステナイト相が増加し，0deg積層材においてＭＰＢsに沿った開口き裂発生を緩和するため，延性が向上すること（iｖ）造形中に未溶融部と接するＭＰＢs部分では,冷却速度が大きいため微細粒となる一方でレーザ走査中心部では積層方向に向かって〈001＞方位を有する結晶が粗大成長することを明らかにした., 首都大学東京, 2018-03-25, 修士（工学）

PubMedのような文献データベースには多くの論文が収録されているが、その中から、エビデンスレベルの高い論文を選び出すためには、臨床研究の進め方や、その研究デザインについての知識が欠かせない。エビデンスを視野に入れたPubMed検索に必要な基本的な知識を概説した。

＜point＞血液浄化法の役割は、著しく低下した肝機能を補助するとともに昏睡起因物質などを除去することにより、肝臓が十分に再生するまでまたは肝移植までの間、体内の代謝環境を維持することである。80%以上の施設においてPEと何らかのHDFが行われている。しかしながら、これらの治療法の有効性に関する質の高い、客観的評価はいまだ行われていないのも現状である。最近、水溶性毒素のさらなる除去を目指しon-line HDFも応用されている。また、アルブミン結合毒素を廃棄・除去しながら同時に拡散による溶質除去を行うPDFが開発され、有効性が報告されている。(著者抄録)

For busy physicians，the need to obtain highquality，evidence-based information rapidly is increasing as the importance of evidence-based medicine(EBM) becomes more widely accepted. Various information sources(point of care information tools) have been provided to respond to this need. Some reports have attempted to evaluate these tools. As these reports, the frequency of updates was highest for DynaMed and UpToDate had the largest number of topics. We set 3 clinical questions and compared the results of searches using 3 representative international point-of-care information tools (UpToDate, DynaMed, ACP PIER) and 1 Japanese tool (Konnichi-no Rinsho-Support). The survey showed that 3 international tools provided almost the same core references in their contents. The frequency of updates was not much difference between UpToDate and DynaMed. UpToDate provided recommendations based on not only references but also the opinions of experts. It could be said that UpToDate was the best tool, while it had some difficulties such as the length of sentences and high price. Although topic cover rate was less than UpToDate, DynaMed reached the level of practical use. The structure of ACP PIER is similar to DynaMed. Konnichi-no-Rinsho-Support is expected to be further developed as a point-of-care information tool for ]apanese researchers.

厚生労働省難治性疾患克服研究事業「難治性の肝・胆道疾患に関する調査研究」班として、「原発性胆汁性肝硬変(PBC)の診療ガイドライン(2012年)」が作成された。研究班で策定された診断基準や治療指針が掲載されるとともに、具体的な診断・治療の指針が推奨度とともに記載されている。本疾患は稀少疾患であるため国内外においてエビデンスレベルの高い資料は少ないが、わが国における実情を踏まえ、よりエビデンスレベルが高い文献を参考に研究班員間でコンセンサスを得る方法で作成された。既に日本肝臓学会雑誌「肝臓」に掲載され、難病情報センターおよび医療情報サービスMindsのホームページ上で閲覧可能である。(著者抄録)

In order to perform scientific health guidance in school, teachers need to have “health information literacy.”Teachers with health information literacy can collect and analyze health information, examine the information critically, and apply useful findings from the information to health guidance. The most important word for understanding health information literacy is “Evidence,” and the method of the study guarantees the scientific rationality of “Evidence” in the study. In classifying evidence with a focus on the method of the study, experimental studies that establish a control group are more valuable than other studies；however, few such studies are seen in school. If teachers learn “health information literacy”,more worthy studies and new evidence will be produced.

診療ガイドラインは現在多くの学会が学会主導で改訂や新規作成に取り組んでいる。そうした中、図書館員に文献検索を依頼したいという需要が大変増えている。本稿では診療ガイドラインの作成方法、作成における図書館員の役割と必要な知識、実際の活動につき報告する。診療ガイドライン作成の文献検索を図書館員が担当することで社会的にも認知され，医学図書館員の専門性の向上につながると考える。

多忙な臨床家が、根拠に基づく医療を実践しようとしても、次々と報告される膨大なエビデンスを実臨床にもれなく応用することは、容易ではない。このような場合、診療ガイドラインは極めて有力な資料となる。近年、ガイドライン作成が盛んに行われ、臨床における効果が期待されているが、作成および改訂方法、利用方法、評価方法等今後の課題も少なくない。本来、根拠に基づく医療は、(1)Evidence:根拠(Meta-analysis、RCT)のみでなく、(2)Expertise:医師の技量、専門性や、(3)Patient's preference:患者の意見・嗜好(患者の経済的、性格的価値観、既往歴、年齢、他)を統合して用いることを推奨しており、診療ガイドラインには、これらをコンセンサスで総合して、臨床医療に役立つように、エビデンスのレベルや推奨度(臨床でのお勧め具合)が設定され、提示されている。(著者抄録)

情報検索に当たっては、まず疑問を整理することが重要である。背景疑問と前景疑問を区別し、背景疑問を、PECO（Patient、Exposure、Comparison、Outcome）で定式化することから情報検索が始まる。検索に使用する情報源については様々なものがあるが、教科書は最新のエビデンスについていけないという欠点があり、原著論文でいちいち勉強するというのは手間がかかりすぎて現実的でない。そこで、エビデンスに基づいて作成された二次資料から始めて、原著論文へと向かう検索戦略を推奨している。また、臨床上の疑問の種類により、それに適したデータベースを選んで検索するというのも重要である。

筆者は臨床工学科の医学概論を担当しており,この講義では,わが国特有の医療に関する問題点を取り上げ,これらから現在の医療はどの様なものであるかを講義し,学生諸君が本来医療はどうあるべきか,を考える資料を提供した.その内容の一部を川崎医療福祉大学公関セミナーで紹介した.ここで取り上げる医療に関する話題は医学書には記載されていないものに限った.江戸時代に,最初にわが国へ西洋医学が持ち込まれた経過から,わが国の医療がどの様な変遷を維て現在に至ったかを述べ,それに伴って生じた問題点を現在の視点から検討した.世界保健機構(WHO)は,わが国の医療水準を世界最高と評価しているが,評価の内容や,その世界最高の医療がどの様にして維持されているか,世界各国と比較検討した.その結果,わが国が世界に誇る高い医療水準は医療従事者の過重労働で維持されていることが示された.

The trend of clinical use of herbal medicine is discussed. Recently, some of these substances have been prescribed for children. However, it has only been 20 years since herbal medicine has been widely used in the field of pediatric surgery. Some of the principal herbal medicines used for daily treatment in pediatric surgery today are Rikkunshi-to for gastric motility disorders ; Daikenchu-to for ileus and constipation ; Inchinko-to for biliary atresia. The results of treatment using these medicines have been better with EBM than the results without any prescription.

この20 年間に保健医療・健康科学の領域にパラダイムシフトをもたらし，現在も影響し続けている二つの出来事はEBM（Evidence-based Medicine）／臨床疫学の急速な普及と，ヒトゲノム計画の推進／オーダーメイド医療の実現構想である．このような保健医療・健康科学上の趨勢と将来像を概説した上で，特に疫学概念の変遷と発展を中心に看護学の大学院教育でどのような接点を持ち得るかを，欧米の看護系雑誌の記述を踏まえて考察した． 健康科学と看護学との関係は「人の健康・生活を考える」と言う点においては，歴史的発展過程や方法論は異なっていても裾野を共有する学問分野である．幅広い視点を持つことで看護学の研究領域がより豊かなものとなり，また客観的に位置付けできると思われる．看護学の大学院教育を考える場合に，健康科学領域は看護学と異なる素材を多数提供できる分野である．

足浴の睡眠への効果の科学的根拠に関する知見を検討することを目的に,1984〜2003年9月を対象とした文献検索を,医学中央雑誌(Web版)及びPub Med(Web版),Chinahl(Web版)により行った.得られた32文献のうち,適当と判断された19件について分析した結果,足浴の睡眠への効果について研究が行われるようになったのは1992年からであり,その多くが足浴は睡眠に効果があると結論づけていることが分かった.しかし,睡眠への効果を直接評価した研究は少なく,足欲の効果に関する科学的根拠を構築するためには,さらなる研究が必要であると考えた

In addition, unprecedented complex structures such as a mesh shape can be fabricated with the layer build process. To evaluate the applicability of the new manufacturing process to bi-propellant thruster parts, especially to injectors and nozzles, tensile tests and neutron radiography were conducted using the specimens which was fabricated by EBM (Electron Beam Melting). In addition,injectors and several types of nozzles imitated for an actual thruster were also fabricated to confirm the configuration accuracy and the residual stress in the structure. The titanium alloy was selected for the material because well characterized powder of the alloy can be readily available.
