Recent geodetic observations detect recurrent slow slip events (SSE), which occurred beneath the Bungo Channel and southwest Shikoku Island, with interval of approximately 6 years (e.g. GSI, 2010). In order to reveal a three-dimensional resistivity structure around SSE region, we are carrying out wideband magnetotelluric (MT) surveys. We also plan to establish a permanent long-term MT monitoring network that aims to detect temporal changes of resistivity structure during SSE cycle. As of June, 2016, MT surveys were performed at 31 sites by using Phoenix wideband MT instruments. In the most of sites, high quality MT responses were obtained using the BIRRP code (Chave and Thomson, 2004) for the period range 0.01 to 10, 000 sec. The spatial distributions of the phase tensor ellipses and the induction vectors suggest that resistivity contrasts are located around SSEs.

We have investigated to know the limit where the life on the earth can be resistant against various kinds of the extreme environmental factor in the space. Spores and leaves of a moss may be extremely resistant against them, so we have chosen three kinds of moss, and investigated the effect of three kinds of extreme environmental factors: vacuum (1 x 10(exp -3) Pa), nitrogen-exchanged air (1 Pa), and ultrahigh pressure (7.5 GPa). Ratio of budding in the spores and leaves exposed to vacuum for two months was not different from that of the control groups not exposed to vacuum. Budding of the group exposed to nitrogen-exchanged air for one month did not show any difference from that of control groups. Furthermore, 20 percent of spores were resistant against ultrahigh pressure exposed for one week. These results indicate that spores and leaves of the moss have much stronger resistibility against extreme environmental factors that the 'tun' of Milnesium tardigradum we have studied up to now. Criptobiosis is a feature of Minesium tardigradum (Phylum Tardigrata). When this animal is exposed to environmental conditions that are not good for their activity, they are transferred to a special shape called 'tun'. We have investigated the resistibility of the tun against super high pressure. Twenty specimens of M. tardigradum were transferred to tun in dry conditions, and were kept in desiccator for a week. Tuns were placed in 7.5 GPa for 20 min, 3 hrs, 6 hrs, 12 hrs, and 24 hrs, respectively. While almost of tuns exposed up to 6 hrs recovered, but all of the no specimens exposed for 24 hrs did recover. Twenty-five percent of the specimens exposed to 12 hrs were alive, but they couldn't walk and were died within a week. We supposed that resistibility of M. tardigradum should be further enhanced if their tun is formed in complete vacuum and low temperature., 資料番号: AA0063349115

63歳男性.患者は表在性膀胱癌に対する経尿道的膀胱腫瘍切除術後8年目に外尿道口部発赤が出現し, 精査加療目的に入院となった.入院時, 自然尿細胞診はclass IVで, 画像所見では異常はみられなかった.膀胱および尿道ランダム生検の結果, 膀胱癌の尿道舟状窩および亀頭部皮膚への限局性の再発と診断し, 陰茎部分切除術, 両側浅鼠径リンパ節郭清術を施行した.病理組織学的に転移性尿路上皮癌と診断され, 術後の尿細胞診は陰性化した, A 63-year-old man underwent transurethral resection of a bladder tumor to treat superficial bladder cancer in 1995. Histological examination showed urothelial carcinoma, G2>G3 with carcinoma in situ (CIS) at the bladder neck. He underwent postoperative intravesical bacillus Calmette-Guerin (BCG) therapy. In June, 2003, he complained of rubor of his external urethral meatus and visited our clinic. Biopsies at the external urethral meatus and the fossa navicularis of the urethra showed CIS. Radiological examinations, cystourethroscopy and multiple biopsies from other sites of urothelium, including bladder and urethral mucosa, did not reveal any other malignancies. Thereafter, partial penectomy and bilateral inguinal lymphadenectomy were performed. Histological examinations showed CIS at the urethral mucosa of the fossa navicularis and the skin of the glans penis. Postoperative urine cytologies were negative.

41歳男.患者は頻尿および排尿時痛を主訴とし, 膀胱頸部を含む膀胱内に広範な膀胱上皮内癌(CIS)が認められた.BCG膀胱内注入療法が施行されたが, 尿細胞診ではclass IVが検出された.更にこの尿細胞が診陽性を持続するため, 経直腸的前立腺針生検を行った.その結果, 前立腺組織の先端部に尿路上皮癌(UC), G3が検出された.膀胱癌の前立腺浸潤と診断し, 尿度摘除を含めた膀胱前立腺全摘除術, 回腸導管造設術が施行された.病理組織像ではUC, G3が前立腺内に胞巣状に広がっており, 前立腺間質への浸潤が認められた.pT4aN0M0との診断で, 術後は補助化学療法が3コース施行された.1年8ヵ月現在, 再発の徴候は認められていない, We report a case of carcinoma in situ (CIS) of the bladder involving the prostate with an unusual invasive pattern following Bacillus Calmette Guerin (BCG) therapy. A 41-year-old man achieved complete response after a course of intravesical instillation of BCG for diffuse CIS of the bladder. Two years later, urine cytology became positive. We performed random biopsy of the bladder and urethra three times and examined the bilateral upper urinary tract with retrograde pyelography and split urine cytology. However, none of these examinations revealed any malignant features, leading to a suspicion that the prostate was the recurrent site. Transrectal needle biopsy of the prostate revealed urothelial carcinoma (UC) at the transition between bladder and prostate. Transurethral biopsy of the prostatic urethra also detected UC in a core of the bladder neck only. Under a diagnosis of UC involving the prostate, we performed total cystectomy with ileal conduit diversion. Histopathological findings of the surgical specimen showed prostatic stromal invasion of the tumor. In this case, CIS at the bladder neck might directly and silently invade the prostatic stroma, thus transurethral biopsy contributed little to the diganosis. We recommend transrectal needle biopsy of the prostate as well as TUR biopsy in such rare cases.

過去10年間に膀胱癌と診断され, 根治的膀胱全摘除術を受けた43例を, 40～64歳(12例), 65～69歳(12例), 70～74歳(12例), 75歳以上(7例)の4群に分け, 治療成績を臨床的に比較検討した.1)術前合併症は高血圧, 糖尿病, 冠動脈疾患, 不整脈が多く, 70歳を過ぎると増加傾向がみられたが, 各群間に有意差はなかった.2)2つ以上の合併症を有する割合は, 加齢とともに増加する傾向がみられた.高齢者では他の3群よりも出血量, 輸血量が有意に多かった.3)術後合併症の発生には有意差はなかったが, 術後死亡は75歳以上群で多臓器不全を合併した1例であった.術後補助療法は75歳以上群には1例しか施行されていなかった.4)癌死は75歳以上群ではみられなかったが, 他因死は3例であった.75歳以上群の3年生存率は100%であった.したがって高齢者に根治手術を行うかの最終的な決定は患者の医学的評価に基づいて判定されるべきであり, 歴年齢のみによって決定すべきではないと考えられた, Between 1995 and 2004, 43 patients underwent radical cystectomy and urinary diversion for the treatment of invasive bladder cancer at our institution. Of these patients, seven who were 75 years old or older, were considered elderly. Survival and treatment outcome of these patients were compared to younger counterparts stratified into three groups by age at diagnosis (12 patients younger than 64, 12 patients 65 to 69 years, and 12 patients 70 to 74 years). Preoperative morbidity was encountered in 57% of the elderly patients, and 42% of the elderly patients had two or more complications. There was one operative death (14%) among the elderly patients but no such deaths in the 3 younger groups. The postoperative complication rate for patients age 75 years or older was 86%, compared to 75% for patients younger than 64, 75% for those age 65 to 69 and 83% for those age 70-74. The prevalence did not differ significantly between the older and youger patients. There were no cancer deaths among the elderly patients, but 8 of the 36 younger patients died of cancer. The cancer-specific 5-year survival rate was 100% at 34 months in the elderly population. These findings suggest that radical cystectomy and urinary diversion is a relatively safe procedure and a curative operation is worth attempting in elderly patients with invasive bladder cancer, if they are in generally good health.

77歳男性.患者は限局性前立腺癌に対する外照射および高線量率組織内照射による放射線療法後の経過観察中, 陰茎痛および発熱が出現した.陰茎背部に発赤, 硬結を認め, レボフロキサシンの経口投与を行ったが, 同部の自壊, 排膿を認め, 緊急入院となった.入院時, 陰茎背側は腫大, 硬化し, 潰瘍形成を伴っており, 細菌感染による炎症性病変と判断し, 病変部のデブリドメンおよびメロペネム三水和物, メシル酸パズフロキサシンの点滴静注, ガンマグロブリン製剤の投与を行ったが, 疼痛の増強, 病巣の拡大がみられた.膿汁の細菌培養の結果, 陰性であることが判明し, 非細菌性病変を考え, ステロイド投与を開始したところ, 疼痛および潰瘍性病変は改善した.以上より, 最終的に陰茎壊疽性膿皮症の診断に至った, We report a case of pyoderma gangrenosum of the penis presenting as Fournier's gangrene. A 77-year-old man who had undergone radiotherapy for localized prostate cancer 16 month earlier, presented with penile pain and fever. Symptoms began with erythema and induration on the dorsal surface of the penile shaft followed by spontaneous purulent drainage with severe pain. Magnetic resonance imaging was unremarkable except for swelling of the penile skin. Biopsy of the ulcerative penile lesion demonstrated a nonspecific inflammation without vasculitis or malignancy. Despite broad-spectrum antibiotics and debridement, the penile lesion extended and new satellite lesions developed as pustules on the glans. Since cultures were negative for aerobic and anaerobic bacteria, a course of intravenous prednisolone was then initiated at 100 mg/day. Within 24 hours the temperature normalized, progression of the penile lesions stopped and became convalescent. The steroid was then tapered and discontinued. The penile lesions healed slowly during the subsequent 1-month period. Based on the clinical course and histopathological findings as well as exclusion of other ulcerative conditions, a diagnosis of pyoderma gangrenosum was made. Penile involvement of this non-infectious ulcerating skin disease has rarely been reported. Pyoderma gangrenosum affecting the penile skin, such as that in present case, may show a similar presentation as Fournier's gangrene. Prompt differential diagnosis is mandatory since effective management for each processes is markedly different.

Changes in mineral components during the processing of umeboshi was investigated using ripened Nanko mume fruit as raw material. Calcium and Magnesium contents were increased or almost equal by pickling in salt, and potassium was decreased, Among the trace elements, iron showed remarkable increase by salting, Furthermore, mineral component of salt used for pickling was recognized to reflect in the content of these elements in salted mume fruits. Sodium content in edible part of Umeboshi was decreased with desalting and following pickling in seasoning solution, but those of calcium and magnesium did not changed. Potassium was also decreased but large part of potassium remained in edible part, suggesting that a function of Umebosshi as an alkaline food remained after hypochloric treatment. Contents of all minerals investigated showed smaller change in kernel than those in pulp. Many parts of organic acid in edible part remained after Umeboshi processing, and it was speculated that resulting low pH condition effected on the mineral composition in Umeboshi.

61歳男.左腎腫瘤性病変の精査加療目的に当科入院した.HE染色像で特徴的所見を有し, 更に近位尿細管のマーカーであるビメンチン, EMA(epithelial membrane antigen)及び高分子サイトケラチンに関しては, 陽性或いは強陽性であったことから腎実質性腫瘍と診断し, 経腹膜的に根治的左腎摘除術及び腎門部リンパ節郭清術を施行した.その後, 外来にて姑息的治療のみで経過観察を行ったが, 転移巣は増大し術後約20ヵ月後に死亡した, We report a case of collecting duct carcinoma (Bellini duct carcinoma) producing carcinoembryonic antigen (CEA). A 61-year-old man visited our hospital because of a left renal mass detected by ultrasonography in an other hospital. Computed tomography showed a low density tumor measuring about 3 cm in the left kidney. Angiography demonstrated a hypovascular tumor. The serum level of CEA was increased to 20 ng/ml. (normal < 7 ng/ml). Left radical nephrectomy was performed. Histological examination revealed collecting duct carcinoma with papillary growth (T1aN1M0). Cancer cells showed a positive immunohistochemical staining for CEA. Under a diagnosis of CEA-producing collecting duct carcinoma of the left kidney, the patient underwent systemic chemotherapy (M-VAC). The serum level of CEA decreased to the normal level after the nephrectomy, but six months postoperatively, metastatic bone tumor at the left pelvic bone was revealed on the plain film and at the same time, the CEA level was increased again.

ESWLが施行された161例を対象とし, low risk群(133例)とhigh risk群(28例)に分類し, さらに術後感染予防としての薬剤投与の有無と投与方法によって無作為に細分類した。1) Low risk群では, 全体の術後発熱頻度は1.5%と低く, 術後7日目の尿細菌培養陽性率は10.0%であった。術後に抗菌剤OFLXを投与したA群(66例)と無投与であったB群(67例)の間で発熱頻度および尿細菌培養陽性率に差はなかった。2) High risk群全体での術後発熱頻度は21.4%とlow risk群より有意に高く, 術後7日目の尿細菌培養陽性率は24.0%であった。発熱例と患者背景の関係を見たとき, 腎盂腎炎発作の既往とD-Jステント留置例においてとくに発熱頻度が高かった, One hundred and sixty one patients with upper urinary stones were examined for antimicrobial prophylaxis following extracorporeal shock wave lithotripsy (ESWL). They were divided into two groups, the low-risk group (n = 133) and high-risk group (n = 28), according to the risk factors of urinary tract infection. The patients in the low-risk group were further randomized into two groups which were orally given ofloxacin for 7 days after ESWL (Group A, n = 66), no antimicrobial (Group B, n = 67). The patients in the high-risk group were randomly subdivided into three groups which were given flomoxef intravenously for 2 or 3 days and ofloxacin for 4 or 5 days thereafter (Group C, n = 10), flomoxef only for 2 or 3 days and no drugs later (Group D, n = 10), ofloxacin for 7 days (Group E, n = 8). In all of the patients in the low-risk group, during the 7 days after ESWL, fever elevation was observed in only 1.5% of patients, and bacteriuria in 10.0% on the 7th day. There was no difference in frequency of fever elevation and bacteriuria following ESWL between Group A and Group B. These findings indicate that prophylactic antimicrobial after ESWL treatment is not necessary for low risk patients with urinary tract infections. In the high-risk group, the over-all rates of fever elevation and bacteriuria were 21.4% and 24.0% respectively. The difference of effectiveness among the prophylactic regimens of the three groups (Group C, D, E) was not shown.(ABSTRACT TRUNCATED AT 250 WORDS)

28歳女.左腰痛で左腎腫瘍を指摘された.CTで腫瘍は4.7×5.3cmで, 内部に石灰化と共に低吸収域を認めた.同部位のCT値はマイナス38で, 脂肪成分の存在が示唆された.造影CTでは早期相で不均一に濃染を受け, 後期相では腎実質の方が造影効果は強かった.左腎血管造影で腎下極に不均一に濃染されるhyper-vascular tumorを認め, 腫瘍血管はmicroaneurysm様の変化を伴っていた.angiomyolipomaの可能性も否定できなかったが, 石灰化を伴うことより腎細胞癌を強く疑い, 左腎摘除術を行った.手術所見で周辺臓器への浸潤は認めず, 腎門部リンパ節の腫大もなかった.摘出標本で, 腫瘍は腎被膜に限局し, 重量は310gであった.病理所見は腫瘍被膜を有する腫瘍で, 淡明な細胞質を持つ腫瘍細胞が胞巣状, 一部に嚢胞状, 管状構造を形成するように増殖し, 硝子化, 出血を伴っていた.画像上fat densityを示した部位は骨稜を形成しており, 内部に脂肪髄を認めた, We report a case of renal cell carcinoma with heterotopic bone formation in a 28-year old woman. The patient was referred to our hospital with a complaint of left lumbargo. Laboratory data were within normal limits. Radiography (KUB) suggested a calcification in the left kidney and abdominal computed tomographic (CT) scan confirmed the presence of a renal mass which contained a calcification. Selective renal angiography revealed a hypervascular (microaneurysm-like change) tumor at the lower part of the kidney. Left nephrectomy was performed. Histopathological diagnosis was renal cell carcinoma with heterotopic bone formation (clear cell carcinoma, G1 > G2, pT1b). There has been neither metastasis nor any recurrence during the 7 months since her operation.

We present a case of a 11-deoxycorticosterone (DOC)-producing adrenocortical tumor. A 55-year-old female was admitted to our hospital with the chief complaints of sustained hypertension and weakness of lower extremities. A laboratory study revealed a decrease in the serum potassium level and plasma renin activity, a normal level of plasma aldosterone and a significant elevation of plasma DOC and 18-OH-DOC levels. The plasma DOC level was increased by ACTH stimulation and was not suppressed by dexamethasone. The tumor appeared at a low intensity of the T1 weighted image of magnetic resonance imaging (MRI) and at a high intensity on the T2 weighted image. Left adrenalectomy was performed and histological examination revealed a benign adrenal adenoma. Postoperatively, the abnormal blood pressure, serum potassium level and plasma level of DOC and 18-OH-DOC became normal.

A case of spontaneous peripelvic extravasation associated with primary ureteral tumor is reported. A 56-year-old woman presented with left flank pain. Excretory urogram and abdominal computed tomographic (CT) scan demonstrated left hydronephrosis with extravasation of contrast materials around the renal pelvis. Retrograde pyelogram showed the filling defect in the left upper ureter. Under diagnosis of ureteral tumor, total nephroureterectomy was performed. Histological findings revealed transitional cell carcinoma. 80 cases of spontaneous peripelvic extravasation in Japan were reviewed and discussed briefly.

第22回宇宙利用シンポジウム(2006年1月17日-19日, 日本学術会議6階会議室 六本木、東京), The Twenty-second Space Utilization Symposium (January 17-19, 2006: Science Council of Japan, Roppongi, Tokyo, Japan), Several animals show resistance to excess environmental stresses such as desiccation for a long time, tremendous level of pressure and temperature, in cryptobiotic stages. Milnesium tardigradum (Phylum Tardigrada) is one of these animals. When the specimens encounters desiccation, their body soon changes into a criptobiotic condition called 'tun'. The body weight of 'tun' decreases to 3% in comparison with normal condition, and the metabolism decreases to 0.01%. The criptobiotic stage of this animals shows a tremendous resistance to excess environmental conditions. For example, it is tolerant to -253C to 50C, vacuum to 600 MP, up to 5700 gray, and a number of chemicals. We have focused on phyciological and molecular mechanisms of the criptobiotic stage of M. tardigradum. We have reported life cycle of this species in this symposium, and proposed a research project on this phenomenon. Furthermore, we have discussed utilization of this animal for investigation of space research. We are organizing a joint study between Japan and Russia. To get more information concerning zeolite synthesis in the presence of 1-butanol (n-BuOH), influences of the alkali metal cation and the synthesis conditions on the zeolite synthesis were investigated. It was found that highly crystalline TON (Theta-1), MER (Merlinoite) and LTL (L) type zeolites were synthesized when KOH was employed as an alkali source, whereas MFI (ZSM-5) and MOR (Mordenite)type zeolites for NaOH. Characterization of zeolites obtained was conducted using X-ray diffraction, scanning electron microscopy, FT-IR, thermal analysis, 13C and 27Al MAS NMR. From the results of FT-IR and 13C CP MAS NMR measurements of as-made zeolites, it was concluded that n-BuOH molecules were occluded in the zeolitic pores of TON and MFI type zeolites, while no n-BuOH molecules for MOR, MER and LTL type zeolites.
