A 76-year-old man presented to our hospital with asymptomatic bleeding of the urethra. Endoscopic examination showed multiple urethral papillary tumors in the pendulous urethra, and the tumors were surgically resected. Histopathological examination indicated urethral condyloma acuminata, and the results of a polymerase chain reaction-based invader assay using urethral swabs taken after surgery suggested lowrisk human papilloma virus infection. This is a relatively rare case because urethral condyloma acuminata has been reported in only a few elderly patients so far. No obvious recurrence of condyloma acuminata has been observed for 18 months after surgery.

In the tetraploid cyclamen cultivars×C. purpurascens Mill. crosses, few fruits and no interspecific hybrids were obtained. Although more than half of cross combinations in the diploid cyclamen cultivars × C. purpurascens Mill. crosses produced interspecific hybrids, efficiency of the hybrid production depended on the genotypes of seed parents in the crosses., The results of the present study indicated that the ploidy level and genotypes of seed parents influenced hybrid production in the cyclamen cultivars × C. purpurascens Mill. crosses. application/pdf, The effects of temperature on the formation and growth of shoot and tuber in the micropropagation of Gloriosa superba L. were investigated. The explants formed the greatest number of shoots at 30℃, and the shoot formation was inhibited at 15℃. At 30℃, a lot of tubers, produced in vitro, sprouted and formed secondary shoots. The optimal temperature for tuber growth in vitro was 20℃. These results suggested that the optimal temperature for shoot formation differed from the temperature for the tuber growth in vitro in the micropropagation of G. superba L., グロリオーサのシュート基部切片を外植体としたマイクロプロパゲーションにおけるシュートおよび塊茎の形成と生長に及ぼす温度の影響を調査した。その結果、シュート基部切片からのシュート形成は低温区で抑制される傾向が認められ、外植体から形成されたシュート数は、30℃区で最大の値を示した。なお、高温区、特に30℃区では、in vitroで形成された塊茎が萌芽して形成された二次シュートが多く認められた。また、シュートの基部に形成された塊茎は20℃で最も大きく肥大した。これらの結果から、グロリオーサのマイクロプロパゲーションにおいては、シュート形成と塊茎肥大に適した温度が異なる可能性が示唆された。

極細硬性尿管鏡Micro 6及びパルス波ダイレーザー砕石装置Paltholithを用いてTULを施行した. 1)全対象例138例における成功率は82.6%であった. 2)TULを第一選択とした症例における成功率, 残石率はESWL後補助療法で施行された症例よりも良好な傾向にあり, efficiency quotientは0.75であった. 3)U2, U3の下部尿管における成功率はそれぞれ96.0%, 86.2%と良好な結果が得られた. 4)以上より, TULは, U2及びU3の下部尿管結石に対して第一選択治療となりうることが示唆された, Between September 1993 and December 1996, 138 patients underwent transurethral ureterolithotripsy (TUL) either as primary treatment or as a second-line therapy after extracorporeal shock wave lithotripsy. In all patients, a semirigid 6.0 F ureteroscope was used. Lithotripsy was performed using a pulsed-dye laser. The overall success rate was 82.6%. The success rates according to the location of stones were as follows, 76.9% for stones in the upper ureter (U1), 96.0% for those in the midureter (U2), and 86.2% for those in the distal ureter (U3). In 68 patients treated with TUL as primary therapy, the success rate was 88.2% and efficiency quotient, which was modified for TUL was 0.75. Complications were rare: no ureteral perforations and no major bleeding occurred, but urosepsis developed in 2 patients. In conclusion, transurethral ureterolithotripsy using a small caliber ureteroscope with pulsed-dye laser is recommended as the primary treatment for mid- and distal ureteral stones, because of its superior success rate. In addition, for upper ureteral stones, laser tripsy is recommended as a helpful auxiliary procedure.

症例1, 45歳男, 排尿終末時痛, 頻尿を主訴とする.超音波診断とCT scanで前立腺腫瘍を認め, 組織学検査では前立腺平滑筋肉腫であった.前立腺全摘除術及び膀胱部分切除術を施行した.術後補助療法を施行せず, 術後1年再発, 転移の徴候はなく, 経過観察中である.症例2, 63歳男, 肺癌と大腸ポリープで内科に入院した.CT scanで大きな前立腺腫瘍と肺, 肝, 両側腎に多発性腫瘍を認めた.前立腺腫瘍精査のため当科を受診した.経直腸的前立腺針生検を実施し, 病理学的検査により前立腺平滑筋肉腫と診断した.治療は行わず3ヵ月後に死亡した, We herein present two cases prostatic leiomyosarcoma. The first case was in a 45-year-old man who presented at our department with the chief complaints of pain on voiding and pollakisuria on November 13, 1996. Ultrasonography and computed tomographic (CT) scan revealed a prostatic tumor. A histological examination of biopsy specimens revealed leiomyosarcoma of the prostate. Total prostatectomy and partial cystectomy were performed. No adjuvant therapy was performed. He is still alive without disease 12 months after operation. The second case was in a 63-year-old man who was admitted to our hospital for treatment of a lung tumor and colon polyp on February 28, 1997. CT scans showed a large prostatic tumor and multiple tumors in the lung, liver and bilateral kidneys. He was referred to our department for evaluation of the prostatic tumor. A transrectal needle biopsy of the prostate for histological diagnosis revealed leiomyosarcoma. No treatment was performed and he died 3 months later. In addition, 57 cases of prostatic leiomyosarcoma collected from the Japanese literature are also reviewed.

ESWL治療を行い, 結石成分の分析に加え結石内細菌の有無も検索しえた26例の, いわゆる"非感染結石"を有する上部尿路結石症患者のうち, 5例で結石内細菌が陽性を示した.結石内細菌陽性のために, ESWL治療後重篤な感染症の合併をみた症例はなかったが, 結石内細菌陽性群では治療後1日目に体温の有意な上昇が認められたことなどより, 尿路粘膜の浮腫やSteinstrasseなどにより, ESWL治療後に尿路の閉塞が持続した場合などには特に, 結石内細菌がESWL治療後の感染症合併の危険因子となりうる可能性がある, On 26 patients with upper urinary stones treated by extracorporeal shock wave lithotripsy (ESWL), the influences of the bacteria within the stones on the inflammatory complications, especially on urinary tract infection, after the ESWL treatment, were studied. The constituents of the stones obtained from these 26 patients consisted of the mixed stone of calcium oxalate and calcium phosphate (10 patients), calcium oxalate alone (2 patients), calcium phosphate alone (1 patient) and uric acid (1 patient). Of these 26 stones including no infection stones such as struvite and carbonate apatite, 5 stones (19.2%) had bacteria within the stone. Although no patients had severe inflammatory complications after ESWL treatments, the fact that the patient group having bacteria within the stones had a significantly elevated body temperature at one day after ESWL treatment compared to that on the preoperative day (P < 0.01) suggests that we should take into consideration the bacteria within non-infection stones as one of the risk factors of inflammatory complications after ESWL treatment.

The bacteria in 37 stones obtained from 37 patients with urinary stone diseases, that is, 11 renal stones (containing 2 staghorn calculi), 21 ureteral stones, 4 bladder stones and 1 urethral stone, were studied, according to the Nemoy & Stamey's method. The stones were collected by partial nephrectomy (1 case), nephrolithotomy (1 case), pyelolithotomy (1 case), percutaneous nephrolithotripsy (PNL) (6 cases), 12 ureterolithotomies, transurethral ureterolithotripsy (2 cases), cystolithotripsies (4 cases) and spontaneous deliveries (10 cases). According to a stone analysis by infrared spectrophotometer revealed 30 were noninfection stones (81.1%) and 7 infection stones (18.9%). Of these 37 stones, 5 stones (13.5% of 11 stones) had bacteria within them. These stones consisted of 4 infection stones (57.1% of all infection stones) and 1 noninfection stone (3.3% of all noninfection stones). Out of 4 patients having bacteria within their stones, urine culture of whom were carried out before stones were collected, only one patient (25%) had the same species of bacterium (E. coli) both within the stone and in urine. The E. coli within the stone and that in urine, however, showed quite different reactions to some antibiotics. The fact that all organisms within stones cannot be detected by urine culture before collecting stones of the patients in our present study, suggests that patients undergoing endourological surgery, such as extracorporeal shock wave lithotripsy and PNL, may have a risk of complications, such as severe urinary tract infection and urosepsis, if the possibility of the presence of organisms within stones is not taken into consideration.

application/pdf, Rooting ability was investigated of the softwood cuttings taken from different positions of the current basal shoots at the early stage of growth in Spiraea thunbergii SIEB. The results were as follows. 1. Cuttings were taken from the shoots 50-60 cm in length on May 10, 1979. After the cutting base was dipped into 20 ppm NAA solution for 20 hours, they were inserted in the rooting medium (coarse vermiculite) on the bed covered tightly with vinyl film. The medium temperature was kept at 20-22℃ but the air temperature under the film varied from 20 to 30℃ during the experiment. The data was taken after three weeks. The cuttings from the middle and upper position (except the shoot tip) of the shoots survived more and rooted earlier than those from the basal position. The number of roots per cutting was more in the former than in the latter. 2. When cuttings were taken from 35-45 cm shoots and handled in the same way as mentioned above, the basal cuttings survived much more than the terminal ones. Rooting was promoted in the former but retarded in the latter. The cuttings of which proximal ends were not cut with knife rooted similarly to those with adjusted proximal cut surface. 3. Dipping the base of basal and terminal cuttings taken from 35-45 cm shoots into 20 ppm NAA solution resulted in early and heavy rooting. However, the treatment decreased the number of survived cuttings to some extent. 4. The basal cuttings taken from 20-25 cm shoots rooted similarly to those taken from the same position of 35-45 cm shoots., ユキヤナギのさし木繁殖に最も適するさし穂を得るため, 1979年5月10日に, 母株の地際から発生した成熟度の異なる新梢から採穂し, バーミキュライト床に密閉ざしを行った. 3週間後の調査結果は, 以下の通りであった. (1) 50-60cmの新梢から採穂した時, 頂端を除き, 中・上部からのさし穂は多くの根を早く発根し, 下部からのさし穂の発根は劣った. (2) これと異なり, 35-45cmの新梢から採穂した時は, 基部からのさし穂が, 早く, しかも多く発根し, 頂部からのさし穂の発根が劣った. (3) 20-25cmの新梢の基部から採穂する時, さし穂基部はそのままでも, あるいは切り返されても, 発根状態に差がなかった. (4) NAA処理は, 発根までの所要時期を短縮し, 斉一な発根を促がし, さし穂あたりの発根数を増加させたが, さし穂の枯死を促進する傾向があった.

Clinical studies on PC-904, a new broad spectrum semisynthetic penicillin with high antipseudomonas activity, were carried out as follows. PC-904 was administered to 20 cases of chronic urinary tract infections with drip infusion at the dosage of 1 to 4 g twice a day for a period of 4 to 8 days. Clinical effects were evaluated with "criteria for clinical evaluation of antimicrobial agents on U. T. I.". Clinical efficacy rate was 20% in small dosage group (1 g/time), 60% in large dosage group (more than 2 g/time), and totally 40%. In these cases, PC-904 was effective for the cases to resist against other various antibiotic therapies. But it was not effective to patients with an indwelling catheter or low vital activities. As to the relation between the MIC and the efficacy, it seems to be more effective to be treated with more than 2 g of the PC-904. No significant adverse reaction was noted, but transient elevations of GOT or GPT were observed in 2 cases.
