目的：急性期医療を受ける患者の地域での生活を視野に入れた看護を展開するために患者の情報と看護の視点について明らかにすることを目的とした． 方法：特定機能病院に勤務する中堅以上の看護師３３名を対象に，地域での生活を視野に入れた患者の情報と看護の視点についてフォーカス・グループ・インタビューを行い分析した． 結果及び考察：急性期医療を受ける患者の地域での生活に必要な情報と看護の視点として，コアカテゴリー《地域での生活を可能にするニーズ》が抽出された．さらに地域での生活を可能にするための状態とニーズとして【身体・生理的な状態とニーズ】，【生活の自立と安全の状態とニーズ】，【病気の受け入れと心理的反応の状態とニーズ】，【社会的環境の状態とニーズ】，【医療・療養への自己決定の状態とニーズ】の５つのカテゴリーに分類された．これらより，患者の暮らしの希望，療養の目標，５つの視点の状態からニーズを導き看護を展開する看護の過程として，“Nursing Care for Patient Goals”（NCPG）を構造化した． 結論：地域包括ケアシステムの中において急性期医療を受ける患者の情報と看護の視点として地域での生活を可能にするための５つの状態とそのニーズが重視されていた．, Objective : The aim of this study was to identify information and care perspectives of nurses for patients leaving an acute care hospital for life in the community, and to consider appropriate nursing care in the community-based integrated care system. Method : Focus group interviews were conducted with 33 nurses working in an acute care hospital. The data were analyzed using qualitative inductive analysis. Results & Discussion : The core category “Needs to enable patients to live in the community” was extracted as the information and care perspective necessary for patients receiving acute care to leave hospital for life in the community. The information and care perspectives were classified into five conditions : physical/physiological condition and needs, life independence and safety status and needs, acceptance of/emotions about illness and needs, social environment and needs, and decision-making and needs. “Nursing Care for Patient Goals” was structured as a nursing process. Conclusion : We identified five conditions and their needs that would enable patients receiving acute care to leave hospital for life in the community.

本研究は、急性期脳卒中患者の家族を対象とした研究を概観し、研究の現状および家族をどのような側面から捉えようとしているのかを明らかにすることを目的とした。医学中央雑誌Web を用いて2005年から2019年12月までの過去15年間の国内文献を検索し、12件を対象文献とした。対象文献は質的研究が多く、文献数に増加傾向は見られなかった。対象となった家族は、患者との続柄は様々で年代も幅広く混在し、患者の後遺症の種類や程度も様々であった。研究結果の分析から、家族を捉える側面は【療養の場の移行】【代理意思決定】【ケアニーズ】【ストレス】【対処】の５つに分類された。以上を考察した結果、対象の特徴を特定した質的研究の蓄積の必要性とともに、量的研究では、家族支援およびその評価指標として移行理論や、ニーズ、ストレス、意思決定などの概念および尺度の活用可能性が示唆された。

本研究の目的は，急性期作業療法でBADL とIADL のから成る応用的活動を活用しやすくするための介入ツールを作成することである．日本作業療法士協会作業療法事例報告集などから急性期作業療法に利用される119 の活動項目を抽出し，試作版院内活動表を作成した．これを用いて作業療法士8 名が入院患者16 名に介入し，実施された活動を記録した．また，作業療法士を対象に有用性を質問紙にて調査した．試作版院内活動表の119 項目中91 項目が実施されており，難易度の易しい活動から次第に複雑な活動が実施されていた．また，質問紙調査からは，経験年数の少ない作業療法士ほど臨床の助けになるという結果が得られるとともに，作業療法士は対象者の入院中の活動の増加を感じていた．実施結果を踏まえ，実施回数の少ない活動を削除する等の修正を行い，院内活動表ver.1 を作成した．応用的活動をリスト化し介入に用いる方法は，急性期で経験年数の少ない作業療法士が応用的活動を介入に活用するための有効な方法となり得ることが示唆された．

経験年数の少ない作業療法士に対して，介入のための視座を与える急性期作業療法の介入戦略の構築を目指し，事例文献の評価項目，介入方針，実施内容について整理，分析することで，現在の急性期作業療法の実態を調査した．調査対象とした日本作業療法士協会事例報告集には583 事例が掲載されており，そのうち身体障害領域の回復状態が急性期の事例は132 事例であったが，介入期間が1 ヵ月以内の事例は19 事例であり，長期的な介入が多かった．19 事例からは，機能改善，早期離床，ADL の改善を基本方針とし，座位訓練や，本人・家族指導，他職種と連携したADL 訓練を行い，その結果，上肢機能やバランス，起居・移乗動作の改善をしているという標準的な急性期作業療法の状態が示されたが，同時に，心身機能だけでなく，入院生活に関わる様々な活動の何ができ，何ができないかという事例に応じた評価・介入がなされていた．急性期作業療法の介入は身体機能に限らず，病院内でできる様々な活動の数を増やすという介入戦略の可能性についての示唆を得た．

Aim Reducing the use of physical restraint in intensive care units is challenging, and little is known about the influence of culture on physical restraint use in this setting. The present study aims to verify the hypothesis that mutual support and a culture of blame among staff are associated with higher physical restraint use for mechanically ventilated patients. Methods We undertook a survey of nurses in intensive care units caring for mechanically ventilated patients in acute care units. The perceived frequency of physical restraint, mutual support, and culture of blame were measured. We predefined a high frequency physical restraint use group and compared the institutional characteristics, human resources, mutual support, and culture of blame between this group and the others (the control). Results Three hundred and thirty‐three responses were analyzed. The mean number of beds per nurse was not significantly different between the groups; the mean and percentage of positive responses about mutual support and a culture of blame were significantly lower in the high frequency physical restraint use group. After adjusting variables in a multivariable regression analysis, a less positive response about the culture of blame was the only independent factor to predict high frequency physical restraint use. Conclusion The study suggests that changing the culture of blame, rather than increasing the number of nurses, is important for reducing physical restraint use., Perceived frequency of physical restraint for mechanically ventilated patients in Japan.

The present study was designed to identify factors likely to elevate the incidence of walking dependence upon discharge of elderly patients from a hospital where they received treatment of acute internal disease. The study involved 68 patients aged 65 and over who had been able to walk unassisted before hospitalization. These patients were divided into the groups of walking dependence (patients unable to walk) and the groups of walking independence (patients able to walk unassisted) at the discharge period. The author analyzed using a model of multiple logistic regression analysis with a category framework involving 10 variables. Results were as follows : 1) The author picked out 4 factors, i.e. "period of bedredden status", "period of symptoms", "period of hospital stay" and "period of physical immobilization". 2) It shown that the risk of walking dependence became higher as the period of each of these variables increased by another day. 3) Using this analysis, the author could suppose the walking dependence at discharge period in 54% of the patients. It is therefore desirable that patients at elevated risk for walking dependence are identified by evaluation of these four factors and to provide active support to these patients so as to shorten the period of these four factors as soon as possible during the hospital stay.本研究の目的は、内科の急性疾患で入院した高齢患者の退院時の歩行能力低下にどのような要因が関与しているかを後ろ向き調査から検討した。対象は、入院前、自ら歩行可能であった６５歳以上の患者６８名である。退院時に自立した歩行が不可の患者を歩行能力低下群、自立した歩行が可能の患者を歩行自立群とした。要因の検討には、１０変数からなる概念枠組みから多重ロジスティック回帰分析を用いモデル化した。結果、１）「ベッド上生活日数」、「症状持続日数」、「在院日数」、「身体拘束日数」の４因子が抽出された。２）各変数の日数が１日増すごとに危険率が高まることが示された。３）４つの要因の検討から、退院時に歩行能力を低下させる高齢患者の５４％を予測できることが説明された。以上を通して退院時に歩行能力低下をさせないためには、４つの要因からリスクの高い患者を予測し、早期にこれらの日数を減らす援助を入院時より意図的に実践することが必要であると示唆された。
