Glaucoma is a typical neurodegenerative disease that causes permanent vision deficits. Early detection is important to prevent vision loss as the patient is unaware of the damage and severity because pain is not induced during the process and progresses gradually. To predict glaucoma, fundus image-based deep learning models are being actively developed. In the traditional approach, original fundus photographs including red, green, and blue (RGB) channels were used as a learning material of deep learning models. In this paper, we propose a new pre-processing method to produce the learning images by segmenting vascular, optic nerve papillae, and optic nerve indentation images from the original fundus images, and we built a deep learning model; previous RGB channels are replaced into our three segmented images. We test the proposed method using two datasets and confirm that proposed method shows better performance than previous approaches that use original fundus photographs. [ABSTRACT FROM AUTHOR]

Among the data used for the diagnosis of calf disease, feces play an important role in disease diagnosis. In the image of calf feces, the health status can be known by the shape, color, and texture. For the fecal image that can identify the health status, data of 207 normal calves and 158 calves with diarrhea were pre-processed according to fecal status and used. In this paper, images of fecal variables are detected among the collected calf data and images are trained by applying GLCM-CNN, which combines the properties of CNN and GLCM, on a dataset containing disease symptoms using convolutional network technology. There was a significant difference between CNN's 89.9% accuracy and GLCM-CNN, which showed 91.7% accuracy, and GLCM-CNN showed a high accuracy of 1.8%. [ABSTRACT FROM AUTHOR]

With aging, there is a loss of skeletal muscle mass and function, which leads to an increased risk of falls, fractures, long-term institutional care, cardiovascular and metabolic diseases, and even mortality. Sarcopenia has been defined as a condition characterized by low muscle mass together with low muscle strength and/or low muscle performance. In 2019, the Asian Working Group for Sarcopenia (AWGS) published a consensus paper on the diagnosis and treatment of sarcopenia. The AWGS 2019 guideline, in particular, presented strategies for case-finding and assessment, which could aid in the diagnosis of possible sarcopenia in primary care settings. AWGS 2019 proposed an algorithm that recommends calf circumference measurement (cut-off <34 cm in men, <33 cm in women) or the SARC-F (strength, assistance in walking, rising from a chair, climbing stairs, falls) questionnaire (cut-off ≥4), followed by handgrip strength measurement (cut-off <28 kg in men, <18 kg in women) or the 5-time chair stand test (≥12 seconds). Finally, "possible sarcopenia" is defined by either low muscle strength (handgrip strength) or low physical performance (5-time chair stand test). Diffuse thyroid disease has ambiguous diagnostic criteria and many errors occur according to the subjective diagnosis of skilled practitioners. If image processing technology is applied to ultrasound images, quantitative data is extracted, and applied to a computer auxiliary diagnostic system, more accurate and political diagnosis is possible. In this paper, 19 parameters were extracted by applying the Gray level co-occurrence matrix (GLCM) algorithm to ultrasound images classified as normal, mild, and moderate in patients with thyroid disease. Using these parameters, an artificial neural network (ANN) was applied to analyze diffuse thyroid ultrasound images. The final classification rate using ANN was 96.9%. Using the results of the study, it is expected that errors caused by visual reading in the diagnosis of thyroid diseases can be reduced and used as a secondary means of diagnosing diffuse thyroid diseases. The electrocardiogram (ECG) signal is a good indicator for early diagnosis of heart abnormalities. The ECG signal has a different reference normal signal for each person. And it requires lots of data to diagnosis. In this paper, we propose an adaptive OpenCL-based FPGA-GPU hybrid-layer platform to efficiently accelerate ECG signal diagnosis. As a result of diagnosing 19870 number of ECG signals of MIT-BIH arrhythmia database on the platform, the FPGA accelerator takes 1.15s, that the execution time was reduced by 89.94% and the power consumption was reduced by 84.0% compared to the software execution. The GPU accelerator takes 1.87s, that the execution time was reduced by 83.56% and the power consumption was reduced by 62.3% compared to the software execution. Although the proposed FPGA-GPU hybrid platform has a slower diagnostic speed than the FPGA accelerator, it can operate a flexible algorithm according to the situation by using the GPU. Background: Inflammatory bowel disease (IBD) is a disease of unknown origin that causes chronic inflammation of the intestines and is classified as either ulcerative colitis or Crohn's disease. The incidence and prevalence of IBD in Korea have been rapidly increasing during the past three decades. This paper describes the current status of IBD in Korea. Current Concepts: Compared with western countries, genotypes and phenotypes of IBD in Korea demonstrate some distinct features, which may have an impact on the diagnosis, treatment, and prognosis of IBD. We continue to experience some difficulties in making early and accurate diagnoses of IBD in Korea due to a low awareness of the disease and the high prevalence of infectious enterocolitis, such as intestinal tuberculosis, which needs to be differentiated from IBD. The treatment of IBD can be influenced by differences in genetic, environmental, and social factors. NUDT15 variants limit the use of thiopurines. Infectious diseases, including tuberculosis, hepatitis B, and cytomegalovirus colitis, are usually more common in Asia than in western countries and can adversely influence the management of IBD. Biologic agents, which have changed the treatment paradigm of IBD, are equally efficacious in Asian patients as in Western patients. However, their use is limited due to a strict Korean government health insurance reimbursement policy. The prognosis of ulcerative colitis in Korea may be better than that in western countries, as indicated by a lower colectomy rate. However, this difference is less distinct in Crohn's disease. Heat stress disorders or heat-related illnesses are a kind of physiological damage that occurs when the body cannot dissipate enough heat due to its thermoregulatory dysfunction. This paper aims to summarize the latest information on the diagnosis and treatment of heat-related illnesses. Heat stress disorders come in a variety of forms including heat edema, heat rash, heat cramps, heat syncope, heat tetany, severe heat exhaustion, and life-threatening heatstroke. Major risk factors may include excessive exercise, continuous exposure to high temperatures or humid environments, lack of acclimation, excessive clothing or protective equipment, obesity, and dehydration. Additional risk factors may include the patient's existing medical condition, environmental and personal factors, and the use of various drugs. Mild heat-related illnesses can be treated only by supportive care such as moving patients to a cool place and laying them in a supine position while elevating their legs and loosening their clothes. However, in the case of heatstroke, quickly lowering the body temperature is an essential in reducing the mortality rate. Intestinal obstruction diagnosis method based on X-ray can affect objective diagnosis because it includes subjective factors of the examiner. Therefore, in this paper, a detection method of Intestinal Obstruction from X-Ray image using Hough transform and PCM is proposed. The proposed method uses Hough transform to detect straight lines from the extracted ROI of the intestinal obstruction X-Ray image and bowel obstruction is extracted by using air fluid level’s morphological characteristic detected by the straight lines. Then, ROI is quantized by applying PCM clustering algorithm to the extracted ROI. From the quantized ROI, cluster group that includes bowel obstruction’s characteristic is selected and small bowel regions are extracted by using object search from the selected cluster group. The proposed method of using PCM is applied to 30 X-Ray images of intestinal obstruction patients and setting the initial cluster number of PCM to 4 showed excellent performance in detection and the TPR was 81.47%. Kawasaki disease, also known as mucocutaneous lymph node syndrome, is a common form of vasculitis in childhood. Among the recent trends in Kawasaki disease, it is notable that the prevalence rate is increasing, and the proportion of "incomplete" Kawasaki disease is also increasing. A delayed diagnosis with prolonged fever can cause complications, such as coronary artery aneurysm and decreased myocardial contractility. These complications might lead to significant morbidity and mortality. Kawasaki disease is diagnosed when the patient meets the principal criteria for clinical symptoms. Following the diagnostic criteria can prevent overdiagnosis but lead to missing of the incomplete form of the disease. Therefore, it is important to accurately understand Kawasaki disease. As the coronavirus disease 2019 (COVID-19) pandemic continues, concerns about a novel severe Kawasaki-like disease in children related to COVID-19 emerges. There are dozens of different types of lesions that can be diagnosed through chest X-ray images, including Atelectasis, Cardiomegaly, Mass, Pneumothorax, and Effusion. Computed tomography(CT) test is generally necessary to determine the exact diagnosis and location and size of thoracic lesions, however computed tomography has disadvantages such as expensive cost and a lot of radiation exposure. Therefore, in this paper, we propose a deep learning algorithm for judging the presence or absence of lesions in chest X-ray images as the primary screening tool for the diagnosis of thoracic lesions. The proposed algorithm was designed by comparing various configuration methods to optimize the judgment of presence of lesions from chest X-ray. Chondroblastoma is a rare benign tumor that produces giant cells and cartilage matrix. The tumor occurs in people between 10 and 25 years with slightly higher incidence in males. The condition occurs in the proximal epiphysis of the tibia and humerus, distal epiphysis of the femur, but its occurrence in the talus is relatively rare, accounting for 4% of the total number of chondroblastoma cases. Chondroblastoma is often misdiagnosed as a primary aneurysmal bone cyst, giant cell tumor, chondromyxoid, and lesion of a secondary aneurysmal bone cyst by fibrous dysplasia. The most commonly used surgical method for chondroblastoma is broad curettage with bone grafting. In general, an aneurysmal bone cyst is associated with a second degree chondroblastoma, which is approximately 20%. Chondroblastoma of the talus and secondary aneurysmal bone cysts can be misdiagnosed as primary aneurysmal bone cysts. This paper reports a case of a young male patient with chondroblastoma of the talus, which was initially misdiagnosed as an aneurysmal bone cyst with involvement of the talo-navicular joint. In emergency situations, first aid workers are the main task of trauma evaluation and care. However, the scope of first-aid treatment is small and they are mainly carried out to the emergency room of the hospital. If a specialist who is in charge of an emergency situation is connected and emergency medical treatment through remote medical direction is performed, accurate diagnosis and appropriate care can be helped. This paper has developed an smart application(app) to support remote medical direction for emergency medical services. The developed smart app allows emergency rescuers to call a doctor at a remote location and transmit real-time status of emergency patients to vital sign and video. Background: Rhinosinusitis, one of the most common conditions encountered by clinicians, affects quality of life and reduces work productivity. Despite its high prevalence and economic burden, there are considerable differences in practice regarding the management of rhinosinusitis. Based on many studies, including the updated guidelines, classifications have been subdivided, and new treatments have been added. Current Concepts: Rhinosinusitis can be classified as acute rhinosinusitis (ARS) or chronic rhinosinusitis (CRS) with a cut-off duration of 12 weeks. It is diagnosed based on subjective symptoms and objective findings on computed tomography or endoscopy. Viral infections account for the majority of the causes of ARS, with watchful waiting without antibiotics as its important initial management. Meanwhile, CRS is divided into primary and secondary CRS, which is further subdivided by anatomic distribution and endotype. Particularly, the Type 2 endotype is characterized by a high recurrence rate and high resistance to current therapies. Biologics using monoclonal antibodies could therefore be used as new therapeutic options for the treatment of primary type 2 CRS. Furthermore, given the chronicity of CRS, it is important to recognize that endoscopic sinus surgery and continuous postoperative medical treatment are important for its management. Discussion and Conclusion: Accurate diagnoses based on diagnostic criteria and subdivided classifications are necessary to determine the treatment plan and prognosis. Background: Hypertension management is challenged by the dynamic nature of blood pressure (BP) fluctuations. These fluctuations are influenced by factors such as physical activity, emotional changes, and sleep patterns. Traditional guidelines focus on static measurements taken in clinical settings. However, these readings may not accurately represent an individual's typical BP levels. This approach can lead to misdiagnoses, such as white coat or masked hypertension, resulting in either unnecessary treatment or missed diagnoses. Current Concepts: Recent global hypertension guidelines emphasize the importance of home BP monitoring and 24-hour ambulatory BP measurements for more accurate diagnoses and treatment plans. However, several barriers exist, such as the high cost of ambulatory BP devices and practical challenges associated with home BP monitoring, which limit their widespread utilization. Despite these challenges, integrating BP measurements into mobile devices offers a promising approach to monitoring BP outside the clinical setting. Various innovative approaches have been developed, including smartwatches utilizing photoplethysmography sensors and machine learning algorithms. However, their accuracy and the need for periodic calibration are areas of concern. Discussion and Conclusion: Mobile device BP monitoring represents a significant advancement in hypertension management, offering the potential for continuous, convenient, and less intrusive monitoring. Despite these potential benefits, challenges such as device accuracy verification, large-scale data interpretation, and calibration must be addressed. Metatarsalgia is one of the most common causes of patients complaining of pain in their feet. This pain is the plantar forefoot, including the second to fourth metatarsal heads and arises from either mechanical or iatrogenic causes. On the other hand, it is frequently accompanied by a deformity of the toes as well as of the first and fifth rays. The pain has a variety of causes, and sometimes the cause is difficult to distinguish. The variability of possible causative factors necessitates an individualized approach to treatment. Epiphora is one of the most common problems in ophthalmological practice, and is caused by dysfunction of the lacrimal pathway. The lacrimal drainage system is a continuous anatomical structure consisting of the lacrimal punctum, inferior and superior canaliculi, common canaliculus, lacrimal sac, and nasolacrimal duct. Lacrimal disease can be medically treated in case of acute inflammation or partial obstruction at the beginning of treatment, but surgical treatment is necessary in most cases. This paper discusses the etiology, clinical features, diagnosis, and surgical treatment of various lacrimal diseases through a selective review of the relevant literature. Advances in lacrimal duct surgery can now be performed in such a way that the structural integrity and normal physiological function of the entire efferent lacrimal pathway is preserved. An isolated dislocation of the intermediate cuneiform bone is a rare midfoot injury. This paper reports a case of a 60 year old man who fell from a height with his foot in the plantar flexed position. An isolated dorsal dislocation of the intermediated cuneiform was confirmed. Objectives: Pulse is a method of Korean medicine diagnosis and is an important clue to detect the organs, nature, and progress of the disease. Pulse examination is included in the basic examination of Korean medicine doctors, but there is no standardized method for diagnosing pulse although the types and methods of the pulse taking are briefly described in the literature, making it difficult to spread the examination method. In this regard, I would like to propose an objective evaluation method.Methods: Although the importance of pulse examination and the method of pulse examination are known in the literature, it is difficult for undergraduate students or inexperienced Korean medicine doctors to access it, so in this paper a method of marking the size of the pulse power in the blank space for objective evaluation was devised and presented.Results: The size of the pulse power should be indicated using the 1-cell, 3-cell, or 5-cell method according to the left and right wrists and the cun, guan and chi on both sides.Conclusion: The method of pulse diagnosis is an important diagnostic method as a verification process for making a Korean medical diagnosis. The remaining Korean medicine diagnostic methods, including pulse diagnosis, also need to undergo objectification. It is believed that the objectification of these diagnostic methods will lead to an improvement in the treatment rate of Korean medicine.

The human mind is a self-evolving system that develops along a multidimensional hierarchical pathway in response to traumatic stimulus. In absence of trauma, a mind integrated in conflict-free state is called monistic. When the monistic mind responses to a traumatic stimulus, a response polarity forms toward stimulus polarity within the mind, turning it into a bipartite structure. Dialectical interaction between the two opposites, originating from their incompatibility, creates a new third polarity in the upper dimension. Thereby, the mind turns into a trinity structure. When the interaction among the three polarities becomes optimized, the plasticity of the mind gets maximized into the “far-from-equilibrium state,” and the function of three polarities is synchronized. Through this recalibration, the mind returns back to its monistic structure. If the mind with the recurred monistic structure responds to another traumatic stimulus, this cycle of hierarchical transformation repeats itself in this cyclical and fractal growth process through synchronization of basic trinity system. Applying this concept to the process of post-traumatic growth (PTG), this paper explores how the mind transforms traumatic experiences into PTG and proposes a ‘PTG Clock' that shows a fundamental sequence in the development of the human mind. The PTG Clock consists of seven hierarchical phases, and each of the first six phases has two opposite sub-phases: shocked/numbed, feared/intrusive, paranoid/avoidant, obsessional/explosive, dependent/depressive, and meaningless/searching for meaning. The seventh, the synchronization phase, completes one cycle of the mind's transformation, realizing a grand trinity system, where the mind synchronizes its biological, social, and existential dimensions. At that point, the mind becomes more susceptible to not only the stimulus of its own traumatic experience but also the pain of others. Thereby, the PTG Clock sets out on a journey to another cycle of transformation in higher dimensions. The outbreak of Middle East respiratory syndrome coronavirus(MERS-CoV) infection in South Korea has become a public health threat. There are many confirmed cases of MERS in healthcare workers. Understanding the nature of the infection and the mechanismof transmission will be a useful lesson. This paper gathers data from the press records in KCDC from May 20th to June 26th 2015 to identify the age, sex, occupation and etiologic exposure of exposed healthcare workers in order to come up with a response plan. By June 26th, 2015, there were 181 confirmed cases of MERS-CoV infection in Korea. 36 (19.9%) of them were healthcare workers. These healthcare workers were exposed to MERS-CoV across 12 healthcare facilities, including Samsung Medical Center and Dae-Chung Hospital;three were infected inside ambulances. Their occupational categories are as follows: 7 doctors (19.4%), 12 nurses (33.3%), 9 caregivers or geriatric care assistants(25.0%), and 8 others (22.2%). These healthcare workers were infected by 12 super-spreaders. 30 of the workers(83.3%) were infected without being aware of their contact with MERS patients, while 6 (16.7%) were aware of this contact at the time of infection. The high number of confirmed cases of MERS-CoV in healthcare workers is direct proof of the failure of crisis communication in South Korea, and the delay in the diagnosis of the index case was due to the lack of risk communication regarding the threat of a MERS outbreak. Diabetic retinopathy(DR) is the leading cause of new onset blindness among working-aged groups in industrialized countries, and its incidence is expected to increase along with the rising incidence of diabetes mellitus. Primary interventions such as strict glycemic control, tight blood pressure regulation, and lipid-lowering therapy can significantly reduce the risk of DR occurrence and progression. Currently, laser photocoagulation is the mainstay of treatment of proliferative DR and some cases of diabetic macular edema (DME). However, a considerable number of DR patients still suffer from severe visual impairment in spite of the application of laser photocoagulation and even of pars plana vitrectomy. Considering the limitations of current DR treatments, ongoing efforts have been devoted to the development of new therapeutic strategies, and it has become necessary to focus on pharmacologic treatment. Since inflammation has been identified as playing a substantial role in the pathogenesis of DR, corticosteroids with an anti-inflammatory effect can be included in the treatment of DR, though this may cause cataract and intraocular pressure elevation. The recent discovery of inhibitors of vascular endothelial growth factor is a revolutionary event in the management of DR, specifically DME. Some new agents aiming at the process of angiogenesis and increased vascular permeability are still under investigation, offering hope for a more effective future treatment of this sight-threatening disease. Atopic dermatitis (AD) is one of the common forms of eczema in children, manifesting with chronic, relapsing attacks of skin eruptions with severe pruritus. Unfortunately, there is no consensus on the definition of AD or diagnostic criteria for AD. Therefore, various diagnostic criteria for AD have been proposed so far, which can be divided into a hospital- or community-setting. This paper reviews representative diagnostic criteria for AD among the proposed criteria. Hanfiin and Rajka's criteria is regarded as a 'gold standard' for AD diagnosis in a hospital-setting. AD diagnosis is made by combining the positive findings from skin symptoms, past history, and laboratory results. For epidemiological surveys in a community-setting, the International Study of Asthma and Allergies in Childhood (ISAAC), is commonly used worldwide as a questionnaire-based diagnostic criterion. For a community-setting, however, new diagnostic criteria with a good sensitivity and specificity need to be developed for epidemiological research as well as clinical consultations with patients. This paper evaluates the current state of work-related musculoskeletal disorders of Korean farmers. Korean farmers are exposed to a multitude of ergonomic risk factors, and have high rates of musculoskeletal disorders such as backpain and knee osteoarthritis, while hip osteoarthritis is relatively low. As of 2010, the average age of Korean farmers is 62.3, and the average period of their work in agriculture is 33.3 years. Therefore, it is difficult to distinguish the influence of natural aging and farm work on the development of musculoskeletal disorders. It is necessary to conduct research on a correlation between specific diagnosis such as rotator cuff syndrome, and farm work in general because farmers complain of a great deal of pain. Background: Periodontal disease, also known as gum disease, is a major dental inflammatory disease with a very high prevalence; it is the main cause of tooth loss. Therefore, diagnostic biomarkers that can monitor gum inflammation are important for oral healthcare. Since the gingival crevicular fluid (GCF) adequately reflects changes in the periodontal environment, they have become a target for the development of effective diagnostic biomarkers for periodontitis. In the present study, the level of the target molecules suggested as diagnostic biomarkers for periodontitis were analyzed in GCF samples collected from healthy individuals and periodontitis patients. In addition, useful targets for the diagnosis of periodontitis were evaluated.Methods: GCF samples were collected from healthy individuals and periodontitis patients using absorbent paper points. SDS-PAGE and Coomassie staining were performed for protein analysis. The protein concentrations of GCF specimens were determined using the Bradford method. The levels of the target molecules appropriate for diagnosing periodontal disease were measured by ELISA, according to the manufacturer’s protocol.Results: The protein concentration of GCF collected from periodontitis patients was 3.72 fold higher than that in an equal volume of GCF collected from healthy individuals. ELISA analysis showed that the level of interukin-6 (IL-6), IL-8, metalloproteinases 2 (MMP-2), MMP-9, tumor necrosis factor-alpha (TNF-α), azurocidin, and odontogenic ameloblast-associated protein (ODAM) were higher in the GCF samples from the periodontitis patients than in those from the healthy individuals. However, the level of IL-6 and TNF-α were relatively low (＞ 5 pg/ml). The prostaglandin E2 (PGE2) levels were not significantly different between the two GCF samples.Conclusion: These results indicate that IL-8, MMP-2, MMP-9, azurocidin, and ODAM are potentially useful diagnostic biomarkers for periodontitis; combining multiple biomarkers will improve the diagnostic accuracy of periodontitis.

Artificial intelligence is a concept that includes machine learning and artificial neural networks and is characterized by the rapid analysis of large-scale data based on fast computation of computers. Various medical studies have been reported to estimate the diagnosis and prognosis of specific diseases using artificial intelligence technology. Recently, many medical studies using a convolutional neural network, which is particularly useful for imaging recognition and analysis, have been reported. For various diseases in orthopedic surgery, including spinal diseases, imaging data, such as simple radiographs, computed tomography, and magnetic resonance imaging, are essential for determining the diagnosis and treatment of patients. Therefore, compared to other fields of medicine, there are many disease entities where artificial intelligence can be applied to the diagnosis and treatment of orthopedic diseases. With the increasing importance of medical informatics, the introduction of artificial intelligence by orthopedic surgeons will positively contribute to the development of medicine. Purpose: This article provides an update and overview of a nursing research program focused on understanding the pathophysiology and management of irritable bowel syndrome (IBS). Methods: This review includes English language papers from the United States, Europe, and Asia (e.g., South Korea) from 1999 to 2013. We addressed IBS as a health problem, emerging etiologies, diagnostic and treatment approaches and the importance of a biopsychosocial model. Results: IBS is a chronic, functional gastrointestinal disorder characterized by recurrent episodes of abdominal pain and alterations in bowel habit (diarrhea, constipation, mixed). It is a condition for which adults, particularly women ages 20-45, seek health care services in both the United States and South Korea. Clinically, nurses play key roles in symptom prevention and management including designing and implementing approaches to enhance the patients’ self-management strategies. Multiple mechanisms are believed to participate in the development and maintenance of IBS symptoms including autonomic nervous system dysregulation, intestinal inflammation, intestinal dysbiosis, dietary intolerances, alterations in emotion regulation, heightened visceral pain sensitivity, hypothalamic-pituitary-adrenal dysregulation, and dysmotility. Because IBS tends to occur in families, genetic factors may also contribute to the pathophysiology. Patients with IBS often report a number of co-morbid disorders and/or symptoms including poor sleep. Conclusion: The key to planning effective management strategies is to understand the heterogeneity of this disorder. Purpose: This study aimed to examine the factors influencing the degree of harm caused by fall incidents in hospitals. Methods: This cross-sectional descriptive study used secondary data from the "Korean Patient Safety Incident Report 2019" and was based on the International Classification for Patient Safety conceptual framework. We analyzed a total of 4,176 fall incidents between January 1 and December 31, 2019, in Korea. Multinomial logistic regression analyses were conducted to identify the factors that influence the degree of harm from these incidents. Results: Among the fall incidents, 443 (10.6%) were sentinel events, 2,514 (60.2%) were adverse events, and 1,219 (29.2%) were near misses. The factors associated with sentinel events were old age (≥60 years), gender, patient diagnosis, medical department, early detection after falls, long-term care hospital, and reporters. Adverse events were significantly associated with diagnosis, medical department, nurses' night shift time, large bed size, location (i.e., operating room, recovery room, and intensive care unit), and reporters. Conclusion: Both patient and incident characteristics are significantly associated with the degree of harm in hospitals. Therefore, multiple factors should be considered to establish healthcare policies and thus prevent fall risks and minimize damage following falls in hospitals. [ABSTRACT FROM AUTHOR]

Background: Adolescent idiopathic scoliosis (AIS) has a diagnosis rate of 3% to 5% per year, but the number of cases requiring surgical treatments is very small, accounting for only 0.17% to 1.75% of all AIS patients. Most patients with AIS are diagnosed, treated, and managed in outpatient clinics. Current Concepts: AIS is a disease that occurs more frequently, and progresses faster, in females than in males. Scoliosis deformity can occur due to various causes. To differentially diagnose AIS, it is necessary to perform careful physical examinations, investigate family history, and check for neurological, growth, and developmental abnormalities. Definitive diagnosis of AIS can be performed through radiographic imaging. In the case of atypical curvature and symptoms, examinations such as magnetic resonance imaging could be required in addition to radiographic imaging. Treatment of AIS patients in outpatient clinics can be performed through observation, exercise, and orthosis. The selection and application of treatment methods and the termination period of the treatments are complexly affected by the age at the time of diagnosis; pattern, location and size of the curve; and growth potential. Discussion and Conclusion: AIS is a disease in which good results can be obtained with conservative treatments such as exercise and orthosis, which is generally applied in an outpatient setting. Vascular anomalies comprise a heterogeneous group of disorders characterized by abnormal growth or development of blood vessels. Diagnosis of vascular anomalies is often challenging due to the large variety of conditions, which exhibit phenotypic heterogeneity and a wide range of symptomology and severity. Accurate diagnosis is crucial for appropriate evaluation and management, often requiring multidisciplinary specialists. Recent interdisciplinary collaboration has led to collaborative studies and their outcomes are being prospectively evaluated. While there is still a role for surgical intervention in various vascular anomalies, discoveries of pharmacologic agents effective in treating vascular anomalies have broadened our medical therapeutic options. This paper focuses on vascular anomaly issues often seen by the pediatricians and reviews the clinical pearls on infantile hemangiomas, lymphatic malformations, venous malformations, and arteriovenous malformations.

Interstitial lung disease (ILD) is a group of diseases, involving the inflammation and fibrosis of the interstitium of the lung. ILD is classified according to whether or not the cause is known. Known causes of ILDs include inhalation of environmental substances, drugs, infection, and related connective tissue disease. ILD of unknown cause is called idiopathic ILD. The most common form of idiopathic ILD is idiopathic pulmonary fibrosis (IPF). IPF is a chronic progressive fibrosing ILD that results in the decline of lung function with exertional dyspnea, cough, bibasilar inspiratory crackles, and digital clubbing. The incidence of IPF increases with age, and is predominant in men. The most characteristic feature of IPF is a usual interstitial pneumonia (UIP) pattern detected on high-resolution computed tomography (HRCT). The typical HRCT pattern in case of UIP is honeycombing, with or without traction bronchiectasis or bronchiolectasis; this may be superimposed with fine reticulation. The typical distribution of UIP is subpleural, and there is basal predominance with heterogeneity. A definitive diagnosis of IPF in patients with clinically suspected IPF is made when there is presence of a UIP pattern on HRCT. Bronchoalveolar lavage or surgical lung biopsy is not recommended if a UIP pattern is detected on HRCT. However, bronchoalveolar lavage and surgical lung biopsy are required if probable UIP pattern, indeterminate UIP pattern, or an alternative diagnosis pattern are found on HRCT in order to diagnose IPF. A specific combination of HRCT patterns and histopathological patterns requiring multidisciplinary discussion is necessary to rule in IPF or rule it out. [ABSTRACT FROM AUTHOR]

Purpose: Chronic ankle instability is a very common abnormality of the ankle, but there is still controversy regarding its evaluation criteria. The stress view has difficulties in reflecting the patient's symptoms and treatment progress. Therefore, this study examined the relationship between the center of pressure (COP) measured by a pedobarograph and the symptoms of the patient. Materials and Methods: Thirty patients with chronic ankle instability from February to August 2018 were included. Each patient was surveyed with the foot and ankle outcome score (FAOS). The COP was measured with a foot pressure scanner, and the travel distance and ellipse area of the COP were calculated. Each patient was measured on one foot and on two feet with his or her eyes closed and open. The relationship between the COP measurement and FAOS score was analyzed using the Pearson correlation coefficient. Results: The participants were consisted of 21 male and nine female, with a mean age of 30 years, mean weight of 72 kg, and mean foot size of 259 mm. With the eyes open, the correlation coefficient between the FAOS and travel distance of the affected side was -0.394 (p<0.05) and that between the FAOS and the ellipse area of the affected side was -0.425 (p<0.05). On the other hand, no significant correlations were found between the travel distance and ellipse area of the affected side when patients closed their eyes. Conclusion: Measurement of the COP using foot pressure scanner could evaluate objectively patients with chronic ankle instability, with measurements in patients with their eyes open being more significant. Based on the findings of this study, an analysis of the COP with the patients with their eyes open and standing on one foot may help determine the management strategy and assess the progress of the patients. A hypertrophied peroneal tubercle can present as a bony prominence at the lateral aspect of the foot and a peroneal tenosynovitis or tear. We report a case of a 52-year-old man complaining of lateral foot tingling pain and numbness. The sural nerve entrapment and peroneus longus tenosynovitis by hypertrophied peroneal tubercle were confirmed. The enhanced primary care demonstration (EPD) was launched in November 2014 to provide high-quality care for people with chronic illnesses. In the EPD, comprehensive assessment and care planning (CAP) is a critical component, along with behavior modification and case management services. In this study, we measured CAP duration and calculated the fee for CAP sessions performed with patients with hypertension and/or diabetes mellitus. In 5 primary care clinics participating in the EPD, the duration of CAP sessions and usual consultations was measured. The duration of CAP sessions was measured on 2 separate occasions because CAP involves 2 separate consultations, including an initial consultation for comprehensive patient assessment and laboratory testing and a follow-up consultation for creating a care plan based on the assessment and test results. The CAP fee was calculated as the ratio of CAP time to the usual consultation time. The median (interquartile range) and the mean ± standard deviation of CAP duration were 15.4 (7.1) minutes and 15.6 ± 4.2 minutes, respectively. The first and second CAP sessions lasted for 5.3 and 4.6 times longer than usual consultations, respectively. The calculated CAP fees were 76,299 won (median) and 65,766 won (mean). The length of CAP sessions for patients with hypertension and/or diabetes mellitus was approximately 5 times longer than that of usual consultations. If consultation lengths were measured in a representative patient sample, it would be possible to improve the external validity of the measurements. [ABSTRACT FROM AUTHOR]

Abdominal aortic aneurysm (AAA) is a disease in which the abdominal aorta is swollen to a diameter as large as 3.0cm or even more. Although the symptoms are usually not prominent, it is fatal once it bursts. If AAA is diagnosed early and treated, the majority of deaths from ruptured AAA can be prevented. It is screened for with ultrasonography, which is a fast, cheap and safe diagnosis tool. Most patients can also tolerate the screening well. Several countries are operating national AAA screening programs in efforts to reduce AAA-related deaths. For instance, the Screening Abdominal Aortic Aneurysms Very Efficiently program in the United States and the National Health Service Abdominal Aortic Aneurysm Screening Programme in the United Kingdom are some of the prominent programs currently available. If Korea conducts AAA screening programs for a target population similar to these cases, the cost is estimated to be 25.7 billion Korean won (23.4 million US dollars [USD]). If the target population is extended to males 65 years of age and above with experience smoking more than 5 pack-years of cigarettes, the cost is estimated to be 36.4 billion Korean won (33.1 million US dollars [USD]). However, considering the actual screening rate of the current national cancer screening program, the realistic costs are estimated to be 10.6 billion Korean won (9.7 million US dollars [USD]) and 15.0 billion Korean won (13.7 million US dollars [USD]), respectively. Nasal provocation test (NPT) is a procedure used to evaluate the hyper-responsiveness of nasal mucosa after exposure to a provocative allergen. We aimed to identify the clinical indication and contra-indication for the use of NPT, and to introduce the actual NPT laboratory protocol in our clinical practice. We also provide clinical information which could be useful in conducting NPT. Finally, we discuss the current limitations of NPT and present a plan to overcome these difficulties. Background/Aims: Double balloon enteroscopy (DBE) allows both diagnosis and therapeutic maneuvers in the small bowel. Its use was pioneered in Europe and Asia but there remains a relative paucity of literature from North America. Our aim in this study was to determine diagnostic and therapeutic yield in a North American setting. Methods: A five-year retrospective analysis of all patients undergoing DBE at a single tertiary care North American hospital was performed. Results: Four-hundred fifty-seven procedures, 265 anterograde and 192 retrograde, were performed on 335 patients. The most common indications were obscure gastrointestinal bleeding, small bowel obstruction, and suspected masses and mucosal abnormalities. Total enteroscopy was achieved in 19 of the 89 patients who had both anterograde and retrograde procedures. Overall diagnostic yield in the determination of cause of symptoms or previous imaging was 52%. The most common causes of obscure bleeding were small bowel ulcers (10%), vascular lesions (25%) and neoplasms (10%). The most common causes of small bowel obstruction were strictures, some of which underwent dilation. Other therapeutic interventions included polypectomy, retrieval of retained capsules, stent retrievals and percutaneous enteral jejunostomy tube placement. Overall complication rates were very low (0.6%) and included medication reaction (n=1), scope dysfunction (n=1) and perforation (n=1).
