Nerve palsy as a complication of hematoma following total hip arthroplasty (THA) is a rare development. Although rare, this complication can cause permanent nerve palsy. The authors experienced a case of recovery from sciatic nerve palsy after emergency evacuation of a hematoma. The expanding thigh hematoma was due to anticoagulation treatment for prevention of venous thromboembolism after total hip arthroplasty. After 10 months from the operation, the patient regained complete motor power and leads an ordinary life. Purpose: The purpose of this study is to evaluate the usefulness of a modular total hip system for combined anteversion in cementless modular total hip arthroplasty. Materials and Methods: We performed twenty cementless modular total hip arthroplasty procedures. The patients lay in lateral position and 15°internal rotation of the thigh. The surgeon inserted an acetabular cup component first, followed by a femoral component. He selected an appropriate modular neck component using an intra-operative manual combined anteversion test. All patients underwent post-operative computed tomography. Results: The average measurement of anteversion of the acetabular cup was 15.9±2.9°and anteversion of the femoral stem was 15.2±11.9°. Then, the combined anteversion was calculated to 24.0±8.2°. Significant difference was observed between combined anteversion and theoretical combined anteversion. Conclusion: The method using an intra-operative manual combined anteversion test cannot realize the theoretical combined anteversion in cementless modular hip arthroplasty. [ABSTRACT FROM AUTHOR]

The various implants used in total hip arthroplasty can be classified according to the design and the fixation type. In general, they can be divided into two groups; cemented and cementless types. The surgeon's decision regarding which type of implant to use should be based on the goal of the arthroplasty operation, the bony deformity of the patient, the function of the involved hip joint, and the experience of the operator. When using cementless implants, primary fixation, survival rate, and successful fixation on long-term follow-up depend on the material of the implant, the bone quality, and, ultimately, the interaction between the implant and the bone. Cementless implants have shown a high success rate in primary total hip arthroplasty and relatively fine outcomes on long-term follow-up. In comparison of the two implant types, superiority has yet to be determined, however, a choice made based on the objective of the surgery, the function of the joint, the quality of the bone, and the surgeon's experience will result in a good outcome. Purpose: Long-term follow-up results of concurrent bilateral or unilateral cementless hip arthroplasty were compared. Materials and Methods: Forty eight and 49 patients with concurrent bilateral and unilateral hip artrhoplasties who had more than a 17-year follow-up time were selected. At final follow-up, mean ages were 69.1 and 66.7 years old in the bilateral and unilateral groups, respectively. Mean follow-up times were 21.5 and 22.4 years in the bilateral and unilateral groups, respectively. Clinical results were attained using Harris hip score and standardized anteroposterior and lateral view X-rays. Results: The bilateral group had mean Harris scores of 44.3(standard deviation 3.78) preoperatively, and 81.2 (10.75) at final follow-up. For the unilateral group, the mean scores were 46.5(3.27) preoperatively, and 80.1 (12.05) at final follow-up. At final follow-up, an acetabular cup was securely located in 78.9% and 82.8% of the bilateral and unilateral groups, respectively, and all cases showed firm fixation of the femoral stem in both groups. Osteolysis around the cup and stem were found in 26.3% and 31.4% of the bilateral group and 16.6% and 17.1% of the unilateral group, respectively. Polyethylene wear rate was 0.17 mm/yr and 0.16 mm/yr in the unilateral and bilateral groups, respectively. Using the Kaplan-Meier method, 17-year survival rates for cup and stem were 78% and 89% in the bilateral group, respectively, and 78% and 86% in the unilateral group, respectively. Conclusion: In 17-year long-term follow-up, concurrent bilateral hip arthroplasty was insignificantly different clinically and radiologically from the result of unilateral hip arthroplasty. Vascular complications related to hip arthroplasty are common, however, total occlusion of a critical artery that threatens survival of an extremity is extremely rare. Total hip arthroplasty (THA) is rarely performed in below-knee amputee patients. To the best of the authors' knowledge, periprosthetic femoral fracture in such patients has not been previously reported. Such devastating complication can occur even with minor trauma. To prevent occurrence of such events, there may be several important factors to be considered in performance of THA surgery in below-knee amputee patients and during the course of rehabilitation. Purpose: The aim of this study was to evaluate the usefulness of a method for positioning the femoral stem in neutral position refer to the measured value of "lateral distance" in pre-operative templating and the femoral stem position post-operatively. Materials and Methods: A total of 81 hips in 76 patients underwent cementless total hip arthroplasty between March 2008 and April 2012 using the C2 stem (Lima Corporate S.P.A, Udine, Italy). We measured the perpendicular distance from the lateral outermost part of the greater trochanter to the lateral outermost part of the femoral stem (lateral distance, LD) in pre-operative templating. We aimed to insert the femoral stem in neutral position by comparing LD measured intra-operatively. We evaluated the position of the femoral stem on postoperative plain radiography. We compared the result of post-operative re-measurement of the LD with that of pre-operative measurement. Results: In all of the 81 cases, the femoral stem was inserted in neutral position, defined as the femoral stem position within 3°valgus or varus. The average angle of the inserted femoral stem was 1.20°(0.003-2.85°). LD measured in preoperative templating was 15.6 mm(10-21.5 mm), and it showed no correlation ratio with LD; LD re-measured post-operatively was 15.9 mm(10.3-23.5 mm) (P=0.781). Conclusion: Measuring LD pre-operatively and using the value intra-operatively is an effective method for insertion of the femoral stem in neutral position. [ABSTRACT FROM AUTHOR]
