Purpose: There has been little research regarding the effectiveness of oseltamivir for influenza B infections. We sought to identify the different clinical manifestations between patients treated with and without oseltamivir. Methods: We retrospectively studied the medical records of 72 inpatients or outpatients from two medical centers diagnosed with influenza B infections by either a rapid antigen test or multiplex reverse transcriptase PCR between January 2012 and July 2012. We compared gender, age, past medical history, admission period, total fever duration, fever duration after hospitalization, post-oseltamivir medication peak temperature, laboratory test, chest X-ray, antibiotic medication, and the presence of concomitant viral or bacterial infections. Results: The number of subjects in our study was 72 who were diagnosed with influenza B pneumonia, acute bronchitis, acute bronchiolitis, croup, and mean age was 3.6±2.8 year old. The demographic characteristics and clinical manifestations of oseltamivir and the non-oseltamivir groups, including hospitalization period (4.18±2.10 vs 4.79±1.49 days, P=.17) and total fever duration (5.32±2.07 vs 6.41±3.25 days, P=.09), demonstrated no significant differences. Notably, the oseltamivir group did have significantly reduced usage of antibiotic treatment than the non-oseltamivir group (P=.04). When we limited our patient group to patients under the age of three, similar results were seen. The group prescribed oseltamivir within 48 hours of fever onset had less antibiotic usage, in addition to a shorter fever duration. Conclusion: Oseltamivir appeared to have no benefit in improving the clinical course. However, if it is prescribed within the first 48 hours of symptoms, it may be more effective. This study aims to investigate the status of agricultural injuries and to suggest measures to prevent and to manage them. To determine the scale of agricultural injuries in Korea, fatal injury data were analyzed by securing from the Annual Report on the Cause of Death Statistics and moderate to severe injuries were analyzed from the Emergency Department-based Injury Surveillance System (EDBISS). Nationwide data on minor injuries were analyzed from the Korea National Health and Nutrition Examination Survey (KNHANES), the Korean Working Condition Survey (KWCS), and the Farmers Survey of Occupational Injuries and Illness. The SMR of farmers was significantly higher than that of other occupations. When the distribution of severity of injury was investigated from the EDBISS data, the injury severity of farmers was higher than that of other occupations. The rate of industrial injuries from the KWCS data was 4.51% in skilled agricultural and fishery workers, which was around twice as high as those of craft and related trade workers (2.27%) and plant and machine operators and assemblers (2.13%). The occupational injury reporting rate from KNHANES was the second highest, or 4.3%, in skilled agricultural and fishery workers, following that of plant and machine operators (6.2%), while the rates of manual workers, sales and service workers, clerks and professionals, and managers and administrators were 3.5%, 3.0%, 2.7%, and 2.1%, respectively (P <0.001). Social concern over this issue, a reporting system for occupational injuries, preventive measures such as safety education programs, and an accident compensation system are needed to prevent and manage agricultural injuries. Spontaneous esophageal perforation (also known as Boerhaave syndrome) is associated with sudden increase in intra-esophageal pressure by vomiting, coughing or straining at stool. This case describes a 13-year-old girl who developed the perforation after consuming 270 mg/kg of caffeine tablets for a suicide attempt. Presumably, the perforation was caused primarily by caffeine-induced severe vomiting, and secondarily by caffeine-induced gastric acid secretion with smooth muscle relaxation and by the large amount of caffeine-induced sudden increase in intra-esophageal pressure.

We report a case of a 14-year-old Korean boy presenting with pain in the left lower quadrant, persisting for over 10 days. Except for the persistent pain, no other symptoms, such as vomiting and diarrhea, were observed. In addition, laboratory findings were normal. He was eventually diagnosed with epiploic appendagitis, a rare disease in children. This case highlights the need for imaging study for early diagnosis of epiploic appendagitis or surgical emergencies in children who visit emergency departments with pain in the left lower quadrant without specific clinical findings.
