Primary torsion dystonia is a form of generalised dystonia which typi cally begins in childhood. This pharmacoresistant disease leads to severe disability due to progressive loss of movement, dysarthria, dysphagia, and pain. Deep brain stimulation (DBS) in the globus pallidus interna (GPi) is a safe and effective treatment with positive long-term clinical outcome. Various studies showed not only high functional improvement but also positive influence on quality of life, cognitive functioning, and mood. We present the first case of GPi DBS in Lithuania performed at the Hospital of Medical Academy of Lithuanian University of Health Sciences in a 16-years-old patient with 12 years history of DYT-1 negative dystonia. Maximum improvement was achieved after 6 months from the start of stimulation which resulted in maj or improvement in mobility and overall quality of life which sustained for 18 months of follow-up.
