Introduction: Endoscopic sinus surgery is performed in chronic rhinosinusitis in case of unsatisfied effects of pharmacological treatment. Usually this procedure is done under general anesthesia, but it is also possible to do it in local anesthesia. Aim: To evaluate effectiveness and perioperative patient's comfort based on the developed local anesthesia protocol. Material and methods: Fifty patients were included in the survey. Local anesthesia protocol was developed, consisting of pterygopalatine ganglion blockade and lateral nasal wall infiltration. After the procedure, patients filled in a questionnaire marking pain level and comfort during and after the operation. Results: Based on the questionnaire, average pain level during the operation was at 4.2 in 10-point VAS scale. Average maximum pain during the operation was at 4.2, while average pain level during the anesthesia procedure was at 2.41. 41% and 50% of the patients felt safe and very safe during the operation, respectively. 68% of the patients were felling good level of comfort during the procedure, 26% very good. 58% of them were able to get back to normal activity in the same day. 85% of the patients would recommend the procedure in local anesthesia to the others. Conclusions: Based on the collected material, we believe that local anesthesia in endoscopic sinus surgery is a good alternative for selected group of patients. Introduction: The aim of the study was to determine the frequency of Helicobacter pylori infections in obese patients qualified for bariatric surgery and in patients with normal body weight undergoing upper gastrointestinal endoscopy. All participants gave informed consent to be included in the study. Material and methods: The research was carried out among 124 patients (60 patients with obesity and 64 patients without obesity), who underwent gastroscopy in the Laboratory of Diagnostic and Surgical Endoscopy of the University Teaching Hospital in Białystok during the period 2018-2020. The obese patients underwent gastroscopy as part of qualification for bariatric surgery. The self-constructed questionnaire contained questions about demographic data, smoking, medications taken that affect the occurrence or elimination of gastrointestinal ailments, the presence of diseases coexisting with obesity, and the presence of ailments from the gastrointestinal tract. Moreover, the description of gastroscopy and the result of histopathology were analysed***** Results: The average body mass index (BMI) in the group of obese patients was 40.42 (the lowest 30.1 - the highest 64). In the group of patients with normal body weight, the mean BMI was 22.46 (the lowest 20.1 - the highest 24.9). The urease test and histopathological examinations confirmed the comparable frequency of infections. The correlation between the occurrence of Helicobacter pylori and patient age, gender, or place of residence has not been confirmed. Conclusions: Performing gastroscopy enables the diagnosis of abnormalities as part of qualification for a bariatric procedure. A high Helicobacter pylori detection rate tested in a group of patients may be an important predictor of postoperative complications. [ABSTRACT FROM AUTHOR]

Inverted papilloma (IP) is a benign neoplasm originating in the epithelium of the nasal cavities and paranasal sinuses, characterized by local aggressiveness manifested by bone infiltration, high recurrence rate and the risk of transformation into squamous cell carcinoma. Almost 25% of it is discovered accidentally during surgery due to chronic sinusitis. The risk factors for recurrence are still not fully understood and despite the development of diagnostic tools and surgical techniques, inverted papilloma poses a considerable clinical challenge and patients require lifelong observation. Aim of the study: Assessment of the quality of life of patients after endoscopic sinus surgery. Material and methods: The research period lasted from 15 February to 31 March 2019. The study group consisted of adults aged 18 to 75 years suffering from chronic sinusitis. The method of diagnostic survey was used in this research work. Results: Impaired nasal patency causes great difficulties in everyday functioning and contributes to sleep disorders, weakness in concentration, frequent headaches, irritability, and fatigue. Conclusions: The most common and troublesome symptom of chronic sinusitis was nasal congestion, which resulted in nasal breathing difficulties. Impaired nasal patency had a large impact on the daily functioning of patients. Ailments associated with chronic sinusitis have a decisive impact on comfort and poor quality of life. It was most noticeable in the area of everyday activities, while working, studying at school, and during activities of daily living. The use of regular pharmacotherapy using nasal glucocorticosteroids did not satisfactorily affect the quality of life of patients. The research showed that pharmacological treatment only slightly improved the quality of life. Early satisfactory results of functional endoscopic sinus surgery showed a highly satisfactory result. In almost all examined patients, nasal blockage resolved, which resulted in improved patency and nasal breathing. After the procedure, the patients' quality of life improved significantly.
