When looking at the phenomenon of occupational burnout from a broad perspective, it appears as a serious problem for many environments. However, in the case of healthcare services, it should be considered an absolute priority, especially in the conditions of the SARS-CoV-2 pandemic. First of all, it is about the huge costs of both healthcare providers, patients and their families, as well as the entire healthcare system. The purpose of this publication is to provide basic information and systematize knowledge on burnout of medical workers in the initial period of COVID-19.

In terms of legal requirements for medical personnel, education plays a key role – in each of the analyzed medical professions – it must be a higher education. In the field of post-graduate education (after obtaining the right to practice), it is also possible to complete a specialization course, as a result of which one is able to obtain the title of specialist. As a part of this form of education, medical personnel should constantly improve their professional qualifications in various forms, e.g. qualification courses and specialized training. The provisions of the analyzed acts relating to the so-called independent medical professions indicate, above all, that in order to practice each of the medical professions, it is necessary to hold (maintain) the right to practice the profession and that is subject to the obligation of continuous education, verified by medical boards. The law specifies legal requirements for people who want to practice medicine, in the first place, students of medical faculties. They establish the conditions for obtaining the prerequisite to practice the profession and provide for the need of the continuous postgraduate education after fulfilling the education requirement. This solution introduces a significant scope of legal liability for people who want to practice a given medical profession, which guarantees an appropriate level of protection for patients. 6. Anxiety disorders and mood disorders in hospital doctors: a literature review

This paper is focused on mental health among hospital doctors. This is a review of the literature dated January 1, 2005–December 31, 2019, from the MedLine and Scopus databases. The prevalence of post-traumatic stress disorder and anxiety disorders ranged 2.2–14.6% and 10.5–19.3%, respectively. Several risk factors were significant, such as having had blood exposure accidents, or the interaction between family and work life. The prevalence of mood disorders ranged 7.8–48%. Occupational constraints, such as night work or psychological demand, were related to the presence of mood disorders. This literature review showed the prevalence of disorders that can be reactive at work in hospital doctors. The risk factors studied can guide prevention policies within hospitals. The medical help is considered to be full when medical staff not only use medical and diagnostic technology properly, but also follow the ethical rules. The research showed questionnaire to be a good method of diagnostic for both medical staff ethical potential and the individual employee's one. The scale for assessing group ethical preferences is considered to be effective also. The data resulted in a high level of ethical values and behaviour for only every eleventh respondent include. The majority of respondents, more than two-thirds, showed an average level and one of five - showed a low ethical level, mainly due to the low ethics of males. The ethical learning is an urgent and indispensable task of the medical environment in public professional organizations. The ethical trainings for different social groups (as males, females doctors and the medical staff) should be distinctively different in content and occurred in each medical organisation annually. The medical ethics has become a professional competence factor of medical staff and it should be taken care of, both the public and administrators, heads of medical institutions at all levels of government.

An important problem faced by many healthcare systems is the shortage of medical staff, and in particular doctors and nurses. Their number, competences and qualifications determine the level of availability and quality of medical services. Unfortunately, the demand for medical services is increasing, along with the progressive aging of the population, as well as the increase in the incidence of chronic diseases and frequent reforms of health systems. Employee costs related to healthcare are the most burdensome for the system; therefore, based on the available resources, it is necessary to create effective teams of sector employees. This results in rationalizing employment, or providing new medical and about medical competencies to new groups of professionals, which gives rise to the skill mix phenomenon. A well-prepared and implemented skill mix contributes to improving the quality of patient care, increased patient satisfaction and better clinical outcomes. In the process of mixing of competences, the roles that have been exercised so far are being changed. While some professionals are expanding their existing roles, other employees are required to accept some aspects of the previous roles. In Poland, in order to counteract such negative trends (the shortage of doctors), changes have been introduced to increase access to medical services (e.g., nurses and midwives being vested with the right to issue prescriptions and medical ordinances, paramedics – with the right to perform medical emergency services and provide healthcare services, and physiotherapists – with the right to conduct independent physiotherapeutic visits). Introduction: Health as an irrational value requires continuous multidimensional activities to maintain it. Their implementation is based on building self-awareness in the individual through health promotion strategies consisting in shaping the desired behavior among patients. This is a chance that health promotion through implemented health education puts before the patient.Aim: The aim of the study is to present the beneficial effects of health education on the functioning of the patient.Material and methods: Literature analysis based on articles and books containing the aspect of health education.Conclusions: Daily choices create an individual lifestyle, thus not remaining neutral to health. Awareness of responsibility for one's own health shapes an individual approach to the issue of health. This awareness can be achieved through properly implemented health education assumptions. Undertaken actions are a unifying direction of modern medicine and man in the pursuit of achieving positive health indicators. Through them, it is possible to observe mutual benefits, thanks to which health protection reduces expenditures, thus saving any funds generated by the patient, and the patient himself can be healthy in complete satisfaction, striving for self-fulfillment. Only jointly implemented preventive actions controlling the potential risk are able to achieve the unifying goal, which is constantly improving the quality of life.

10. Analiza stanu psychicznego i satysfakcji z pracy personelu medycznego w okresie pandemii COVID-19.

Introduction: The COVID-19 pandemic is a threat to the physical and mental health of the global population. The healthcare workers are at high risk of mental health disorders. They are the main link to ensure the health safety for patients. Therefore, it is necessary to monitor their mental state to protect them from the negative effects of the pandemic. Material and methods: The study included 316 healthcare workers and 302 non-medical controls. The following diagnostic methods were used: Satisfaction with Job Scale, Psychache Scale, Depression, Anxiety and Stress Scale (DASS-21) and Suicidal Behaviors Questionnaire-Revised (SBQ-R). The study was conducted with online questionnaire. Results: The medical subjects reveal higher levels of depression, anxiety, stress, and psychache than the controls. The highest results are shown in the group of nursing staff. At the same time, nurses are at lower suicide risk and they disclose lower likelihood of suicidal behavior in the future than other medical professionals and have had lower frequency of suicidal ideation over the past 12 months than the control group. The COVID-19 pandemic caused negative changes in professional life in most medical subjects, especially in the group of nurses. Conclusions: The results confirmed the importance of providing psychological support to medical staff as they reveal higher level of negative mental health consequences than the general population. The line of further research is to identify the causes of lower suicidal tendencies among nurses despite the strongest deterioration in their mental state. [ABSTRACT FROM AUTHOR]

Poor psychological and social qualifications of medical personnel affect the situation and condition of patients in stationary hospices. Patients often have no possibility of telling about their non-medical ailments, and it is a constant practice to limit the information provided to patients. Content related to accomodation and death are reduced only to the technical side of care, which can not only lead to information hype and confusion, but also to the deterioration of patients’ well-being. It is important to emphasize the importance of balance between hope and honesty in providing information about the patient’s actual condition, even among people under palliative care. Our research confirms that fears in the terminal phase of cancer appear as a result of a sense of powerlessness, worry, as well as an effect of information blocking. We can talk here about adaptive disorders, because the symptoms of anxiety do not weaken, and patients in hospices often do not have the resources or sufficient support to overcome them in disease conditions and new life circumstances. The need for action to reduce anxiety rather than strengthening it becomes one of the main skills of medical personnel in palliative care. Raising qualifications in the field of social and psychological impact allows for better organization of therapy in hospice conditions and comprehensive patient care.

Introduction: Abdominal aortic aneurysm is an increasingly common anomaly. As a result of the development of medicine, the number of previously diagnosed aortic aneurysms increases, which corresponds to increased survival of patients with this disease. In connection with the above, the number of patients requiring treatment of this disease increases, and thus the implementation of rehabilitation perioperative procedures. Aim of the study: Analysis of the opinion of medical personnel on the role of perioperative rehabilitation in patients undergoing abdominal aortic aneurysm surgery and the indication of priorities in the procedure. Materials and methods: The study was conducted using the diagnostic survey method using the original questionnaire. - Abstract
The problem of SARS-CoV-2 infections is currently a major issue in healthcare due to the high risk of disease among medical personnel. The leading issues are the use of personal protective equipment (PPE) appropriate to exposure risk, rational principles for testing personnel suspected of being infected (RT-PCR tests, antigen tests) and the use of serological tests to confirm the number of convalescents. The first months of the pandemic showed that the routine use of serological tests to detect infected individuals did not bring the expected benefits. It is currently known that the use of these diagnostic methods to detect active infections is not recommended (WHO, ECDC, CDC, AOTMiT). However, by choosing a specific and sensitive test for the presence of IgG antibodies, we can determine the real number of infections in various populations on a much wider scale than RT-PCR or antigen tests would allow. This is crucial for our understanding of the extent of the pandemic since most infected people have no symptoms or only mild symptoms, which means that they are not suspected of COVID-19 and therefore are not tested. Serological tests in specific cases are also useful for confirming a past SARS-CoV-2 infection. As healthcare professionals are at an increased risk of coronavirus infection, we conducted a screening study to assess the presence of specific anti-SARS-CoV-2 IgG antibodies in selected employees of the Warsaw Children's Hospital (WSDZ) after 3 months of the pandemic in Poland. The study showed a low prevalence (2%) of specific anti-SARS--CoV-2 IgG antibodies in a group of 289 hospital employees. On one hand, this proves the low penetration of the virus in the Polish population and on the other the effectiveness of the prevention of infections in our hospital. [ABSTRACT FROM AUTHOR]

Limited financial and infrastructure resources, but above all, lack of staff, and at the same time increasing need and patients' awareness is the leading problem that the health care sector in Poland is facing. Determining the dependence of medical staff competences and effectiveness of patients' health care in health care units might be of high importance in the process of managing medical facilities. Determining these relations will allow proper measures to be taken aiming at increasing the effectiveness of patients' health care and also improving functioning of the medical facilities. This paper looks into the issue of assessing the competences of medical staff. These competences were determined by their scope, systematization, core, and the role they might play in patient treatment and care in health care unites. The core of the analyzed competencies are knowledge and skills. Identification and reinforcement of these competences may have crucial influence on the quality and effectiveness of health care having its roots in adequate medical staff management at the medical facilities. Introduction In order to improve the situation in the area of reducing nosocomial infections, the principles of hand hygiene should be promoted and all efforts should be made to reduce the gap between recommendations and their proper application. The best solution in efforts to improve the hand hygiene situation will be to start a campaign in each institution from the assessment of the level of knowledge in the environment, to set deficits at its level and take action there. Aim The aim of the work is to get to know and assess the level of knowledge and awareness of medical personnel about hand hygiene in a hospital setting. Material and methods The study was conducted among 101 nurses in the Raciborz province, from January 2019 to March 2019. The research method were a diagnostic survey. The questionnaire technique was used. The author's questionnaire was the research tool used to assess the level of knowledge with the respondent's personal information. Results The majority of the respondents were women (87.13%), people aged 36-45 (39.6%), with a experience of 6-10 years and 11-15 years (21.2% each), working in surgical wards (53.5%), with a bachelor's degree (66.3%), living in the city (70.3%), assessing their health as good (63%). Studies have shown that a high level of knowledge was demonstrated by 24.7% of respondents, low by 33.7% and average by 41.6% of respondents. Conclusions The level of nurses' knowledge about hand hygiene is mediocre. The education of the respondents and their experience affect the level of nurses' knowledge about hand hygiene. The nursing staff is familiar with WHO rules for hand disinfection. [ABSTRACT FROM AUTHOR]

21. The way the quality of health services is perceived and treated by the managerial personnel of public hospitals

Background: Managerial knowledge and skills as well as profound understanding of goals and objectives of management determine the effectiveness and efficiency in all areas of managerial activities. In particular, this applies to the quality of healthcare services, perceived as a compliance between the effects (of medical treatment) and the assumed relevant objectives (defined/desired health condition). The aim of the research was to distinguish and define the way the health service quality is perceived by the hospital managerial personnel. Material and Methods: The questionnaire was mailed to 836 members of the managerial personnel of public hospitals in the Łódź province. Of this number 122 questionnaires were returned. Results: Only 22 (18.49%) of respondents presented the definition of quality. Attempts to meet patients’ expectations and to satisfy them were found to be the prevailing perception of the healthcare quality and 96.64% of respondents considered competences of medical staff contributory. Almost 64% of respondents disagree with the opinion that the number of medical staff does not affect the service quality. According to the respondents, a 46% increase in financial resources on average could significantly improve the quality of healthcare services. More than half (66.76%) of respondents claim that healthcare services that are available cover 82% of patients’ needs. Almost 57% (56.52%) of respondents consider that the subordinate- superior relationship influences their work involvement. According to 42.61% of respondents, the offered incentives encourage actions for the quality improvement. Conclusions: The results of the research indicate the need to develop a clear cut definition of the health service quality by the managerial personnel of public hospitals and to change their understanding, perception and treatment of the discussed issue, which provides a basis for the effective and efficient hospital management. 22. The scope of obligatory civil liability insurance of entities conducting medical activities and liability for damages resulting from violations of patients’ rights in the Polish law

Abstract: In the elaboration, the objective scope of obligatory civil liability insurance of entities conducting medical activities in the context of protection from damages resulting from violations of patients’ rights was presented. Based on art. 25 sec. 1 of the Act on Medical Activity, insurance protection covers damages that are the result of the provision of medical services or an illegal omission to provide them. It concerns consequences of erroneous actions related to the provision of medical services as well as damages occurring as a result of an unjustified refusal to provide a medical service or premature cessation of the provision of services if there was an objective prerequisite to continue them driven by medical grounds. The objective scope of insurance protection resulting from obligatory civil liability insurance of an entity conducting medical activities does not apply – as a rule – to damages resulting from violations of patients’ rights. It cannot be considered that a damage related to violation of a patient’s right constitutes a consequence of the provision of medical services or an illegal omission of the provisions of medical services. Such damage is a consequence of a violation of the patient’s right. Financial consequences of patients’ claims resulting from violations of patients’ rights will be borne by entities conducting medical activities. If a patient requests a financial redress, its payment will not be made from the obligatory civil liability insurance policy. The violation of patient’s right to medical services constitutes the only exception.

Introduction: The subject of the work is the evaluation of the occupational satisfaction level of physiotherapist. In the available literature, there are few scientific reports assessing the degree of satisfaction of persons performing this profession. The results of the study show what factors affect the level of occupational satisfaction among physiotherapists. Thanks to the analysis it is possible to eliminate negative factors, which contribute to the increase of dissatisfaction with the work performed. Aim: The main aim was to evaluate the professional satisfaction of physiotherapists in the Zachodniopomorskie voivodship. In addition, the relationship between: the physiotherapist's workplace, their remuneration and the ability to perform new rehabilitation techniques and the level of job satisfaction, and whether or not there are factors influencing the level of job satisfaction. Materials and Methods: The study group consisted of 100 people (74 women, 26 men) aged 22 to 47 years. The respondents are physically active physiotherapists, inhabitants of the West Pomeranian Voivodeship. The study was conducted using the diagnostic survey method and the MSQ Satisfaction Questionnaire (MSQ) was used. Results:The level of professional satisfaction was at a satisfactory level. The main determinants of work were self-fulfillment, self-reliance, ambition and pay. Respondents were less satisfied with working conditions (lack of opportunity for senior management positions and lack of promotion opportunities). The respondents said they were very happy to help others. Conclusions: Professional satisfaction is a key element in the development of a person's attitude towards work. Satisfaction with business and life can only be increased by pointing out and then resolving negative factors.

Background: Protection of nuclear medicine unit employees from hazards of the ionizing radiation is a crucial issue of radiation protection services. We aimed to assess the severity of the occupational radiation exposure of technicians performing scintigraphic examinations at the Nuclear Medicine Department, Central Teaching Hospital of Medical University in Łódź, where thousands of different diagnostic procedures are performed yearly. Materials and Methods: In 2013 the studied diagnostic unit has employed 10 technicians, whose exposure is permanently monitored by individual dosimetry. We analyzed retrospective data of quarterly doses in terms of Hp(10) dose equivalents over the years 2001-2010. Also annual and five-year doses were determined to relate the results to current regulations. Moreover, for a selected period of one year, we collected data on the total activity of radiopharmaceuticals used for diagnostics, to analyze potential relationship with doses recorded in technicians performing the examinations. Results: In a 10-year period under study, the highest annual dose recorded in a technician was 2 mSv, which represented 10% of the annual dose limit of 20 mSv. The highest total dose for a 5-year period was 7.1 mSv, less than 10% of a 5-year dose limit for occupational exposure. Positive linear correlation was observed between total activity of radiopharmaceuticals used for diagnostics in the period of three months and respective quarterly doses received by technicians performing examinations. Conclusions: Doses received by nuclear medicine technicians performing diagnostic procedures in compliance with principles of radiation protection are low, which is confirmed by recognizing the technicians of this unit as B category employees. Cel pracy. Analiza częstości i przyczyn występowania bólu głowy wśród personelu medycznego i jego wpływ na funkcjonowanie w pracy i wypełnianie obowiązków zawodowych. Materiał i metody. Badaniami objęto 145 osób, pracujących w szpitalach w Lublinie oraz Warszawie. Narzędziem badawczym był anonimowy kwestionariusz ankiety. Wyniki i wnioski. Stwierdzono, że średnio połowa badanego personelu medycznego jest zdania, że praca zawodowa, którą wykonują jest przyczyną bólu głowy. Wynika to z sytuacji stresowych mających związek ze specyfiką pracy oraz szkodliwych warunków pracy. Główna przyczyna bólu głowy wśród personelu medycznego to kontakt ze środkami do dezynfekcji powierzchni i skóry oraz kontakt z różnorodnymi środkami medycznymi i lekami. Headache is a common condition that may result from an ongoing disease process in the body as well as be the result of negative health behaviors, improper lifestyle or harmful working conditions. Purpose. The aim of the work was the analysis of the frequency and the causes of headache among medical staff and its impact on the functioning of the working and professional duties. Material and methodology. The study group consisted of 145 people, chosen randomly, working in hospitals in Lublin and Warsaw. The study was based on questionnaire test. Results and conclusions. It was found that, on average, half of the medical staff is of the opinion that the work they perform is the cause of headache pain. This due to the stressful situations in connection with specificity of work and harmful working conditions. The main cause of headaches among medical personnel is contact with measures to surface and skin disinfectant and contact with a variety of medical measures and drugs. - Abstract
Background. Euthanasia is nowadays perceived in a different ways by the society. For some people is a way of painless ending of one's life, for the others is murder of the human being. Belonging to the different society groups may be one of the reasons of the evaluation of the euthanasia and its legalization. Objectives. The aim of the study was to determine differences in attitudes of euthanasia legalization between medical staff and people not working in the medicine area. Material and methods. The diagnostic survey was used and the examined group was comprised of 230 persons; 115 medical staff and 115 people not working in the medicine area from different cities of Poland. Results. Most people of medical staff and people not working in the medicine area recognized legalization of euthanasia in Poland. Religion plays an important role in supporting euthanasia legalization. Among medical staff who admits to be a catholic support the idea of euthanasia legalization but in a lower percentage than unbelievers. Neither the education level nor abode has an impact of supporting of euthanasia legalization in these two groups. For the respondents the disease connected with continuous pain is the major reason influence to the decision of euthanasia. According to two groups of subjects the patient is the person who decides of euthanasia. Conclusions. The contact with the incurable ill and suffering patients may impact the supporting of the euthanasia legalization by medical staff even among catholic believers. [ABSTRACT FROM AUTHOR]

- Abstract
Medical staff are exposed to blood and other potentially infectious biological patients' material, it is a serious problem in healthcare facilities. The aim of presented study was to identify epidemiological situation among occupational exposure of workers in the population of the District Railway Hospital in Katowice in the years 2007-2012. Conducted retrospective epidemiological study allowed to identify risk factors and circumstances of the registered occupational exposure cases. Descriptive and analytical statistics method were used for statistical analysis of data, all statistical analyses were performed using Statistica 9.0 software. Interpretation of statistical significance was based on the criterion p<0.05. The occupational exposure occurred most frequently in hospital wards and operating rooms in morning hours, when medical procedures were performed. Young employees with less experience were the most exposed group of workers. Nurses and midwives were frequently exposed to patients' blood and the other staff were rather exposed to other potentially infectious biological material. The most common mechanism of occupational exposure was stabbing a needle with light in a finger. Cannulas implementation in hospital effectively eliminated stab cases. Despite of vaccination part of exposed workers (14,7%) had actually no protection to HBS in exposure day. Patient's serologic status decided that the post-exposure procedures was implemented. None of the exposed workers in 2007-2012 had occupational infection with HBV, HCV or HIV. [ABSTRACT FROM AUTHOR]

It may involve both reduction of the workforce, job connect, extend driving range, elimination of positions, processes, retraining, training, introduction of new forms of employment, the implementation of incentive pay systems staff, and create new jobs. Introduction: Changes of the health care system caused that patients became subject to the heath care. Aim of the study: The assessment of expectations of the patients with dermatologic disorders towards a therapeutic team and care in the heath care institutions. Material and methods: The study included 281 patients. We used an anonymous survey consisted of a self-administered questionnaire and DLQI -- Dermatology Quality Life Index survey. Results: In all, 53.5% of patients had a contact with the dermatologist. Of 41.3% patients had other consultations then due to skin problems. Most 60.6% patients had the internist and surgery/orthopedic (15.8%) consultations. Of 35% patients were hospitalized due to dermatologic disorders, due to surgery/orthopedic (34.3%) and internist (19.6%) disorders. In all, 75.8% responders declared that medical staff had the same attitude toward patients with skin lesions as toward other patients. Almost 35.3% of respondents declared that (patients with healthy skin, were treated with the better way by nurses). They explained it a fear of infection (20.7%) or a lower professionalism of the therapeutic team (17.6%). Dermatologist should be polite (15.7%), competence (7.5%) and sympathy (6%), and nurses should be polite (24.2%), patience (23.5%) and sensitive (19.2%) in the opinion of respondents. Almost 53.4% of respondents could not give any suggestions on better care of the patients with dermatologic disorders. The patients with a lower quality of life than patients had negative attitudes toward them than to patients with non- dermatologic disorders. Conclusions: Most patients declared that medical staff had the same attitudes toward other patients. The worse attitude to patients with skin lesions was observed among nurses. Most the surveyed were convinced that none procedure for the patient with skin problems should not be embarrassing for a member of the therapeutic team. The dermatologic patients more often expected from doctors positive attitude, polite and competence, and from nurses/midwifes positive attitude, polite, empathy and care. Patients who had a lower quality of life, experienced more negative doctor attitudes than patients with high DLQI index. [ABSTRACT FROM AUTHOR]

35. Patients and medical staff safety in the magnetic resonance department

W praktyce klinicznej badania metodą rezonansu magnetycznego (magnetic resonance - MR) znajdują coraz więcej zastosowań. Dynamiczny rozwój tej metody obrazowania wiąże się z ciągłym wzrostem liczby placówek wyposażonych w skanery MR. Zwiększa się zarówno liczba osób zatrudnianych w pracowniach rezonansu magnetycznego, jak i liczba osób poddawanych tego typu badaniom, a co za tym idzie - liczba osób potencjalnie narażonych na zagrożenia związane z przebywaniem w polu magnetycznym. Niezbędna zatem staje się wiedza dotycząca zasad bezpieczeństwa pracy w pracowni MR i przestrzegania właściwych procedur. Ma to na celu zapewnienie ochrony pacjentom i osobom pracującym w środowisku pola magnetycznego (technicy, elektroradiolodzy, pielęgniarki, lekarze radiolodzy) przed potencjalnym ryzykiem związanym z możliwością wystąpienia zdarzeń niepożądanych. Dla ich zrozumienia konieczna jest podstawowa znajomość zasad organizacji i funkcjonowania pracowni MR, właściwej kwalifikacji i przygotowania pacjenta do badania oraz zasad przeprowadzenia badania MR i opieki nad pacjentem po badaniu. Celem artykułu jest przybliżenie tych aspektów. Omówiono w nim rodzaje stref zagrożenia występujących w środowisku pola magnetycznego oraz stosowanych oznaczeń kompatybilności sprzętów medycznych używanych w pracowniach MR, a także bezwzględne i względne przeciwwskazania do obrazowania metodą MR. Ponadto w artykule zostały zawarte informacje dotyczące specyfiki badań MR u kobiet ciężarnych, w połogu, w trakcie terapii hormonalnej. Osobny fragment pracy przedstawia skrótowo środki kontrastowe stosowane w MR i możliwe powikłania po ich podaniu. Magnetic resonance imaging finds more and more applications in clinical practice. It is associated with a significant increase in the number of laboratory equipped with MR scanners. Both the number of people employed in magnetic resonance departments and the number of patients increases, therefore also the number of people potentially exposed to risk related with the magnetic field. Therefore, knowledge about safety at work in the MR laboratory and compliance with relevant procedures are necessary. This is to ensure protection of patients and people working in the magnetic field environment against potential risks associated with the possibility of adverse events. To understand them, it is necessary to have basic knowledge of the physical and technical aspects of this imaging method, knowledge of the principles of functioning of the MR department, rules of proper qualification, patient preparation for the examination and the principles of proper MR and post-test examination patient care. The aim of the following article is to bring all these aspects closer. The types of hazard zones occurring in the magnetic field environment, the applied signs of compatibility of medical devices used in MR laboratories and absolute and relative contraindications to magnetic resonance imaging will be discussed. In addition, the article contains information about specificity of MR research in pregnant women, in puerperium, during hormonal therapy. A separate part of the article briefly discusses the contrast agents used in magnetic resonance and possible complications after their administration.

The characteristic fea- ture of the psychiatric ward is the so-called. “paternalism” manifested by legitimate interference in the patient’s personal space, for his good. In health care, the notion of “coercion measures” is most often associ- ated with a psychiatric patient, in which case direct coercive measures are required.The aim of the study was to analyse the perception of coercive measures by two groups of patients and the medical staff. The study involved 307 persons, 136 staff members and 171 patients who com- pleted the MAVAS questionnaire, to assess attitudes towards aggres- sion and coercive measures at the ward.Research shows that the differences exist in the perception of ag- gression and legitimacy of coercion between patients and medical staff. Patients emphasize that their aggression in the ward is closely related to the conditions in the hospital, perceived by them as difficult, while the team members highlight that the assault is highly related to pa- tients sickness. Pacjen- ci podkreślają, że ich agresja na oddziale ściśle wiąże się z warunkami w szpitalu, postrzeganymi przez nich jako trudne, podczas gdy perso- nel uwydatnia chorobowe przyczyny agresji pacjentów. Wskazane są zmiany systemowe, minimalizujące istniejące na oddziałach rozbież- ności, które mają znaczny wpływ na jakość opieki zdrowotnej.

Medical personnel, including nursing staff, can become a source of influenza virus infections for their patients and co-workers. However, patients can infect the personnel, too. Therefore, influenza vaccinations are recommended for medical staff as an effective and safe form of preventing influenza and its complications., Aim: The aim of the study was to evaluate the current influenza vaccine coverage rate in nursing personnel and identify of incentives and disincentives for the performance of influenza vaccinations in nursing staff., Materials and Methods: The cross sectional study was carried out a group of 770 nurses, using the research questionnaire, previously validated., Results: The influenza vaccine coverage rate in the study group was 15%. The probability of having a vaccination this season and in the previous seasons was higher in medical staff working in out-patient health care (p <0.05, OR 5.15, 95% CI 3.33-7.96), with work experience >40 years (p <0 , 05, OR 6.93, 95% CI 2.04-23.78), dealing with chronic diseases (p <0.05, OR 1.57, 95% CI 1.00-2.46). The most important encouraging factor for influenza vaccination was that the employer paid for the vaccine available in the workplace, the desire to protect oneself from the disease, the belief in the safety and efficacy of the vaccine. The most important factor discouraging vaccination against influenza was the lack of reimbursement of the vaccine and the inconvenience of its implementation in the workplace, as well as doubts related to the safety and effectiveness of the vaccine., Conclusions: The influenza vaccine coverage rate is very low and certain activities should be undertaken to increase it. Measures taken to increase the influenza vaccine coverage rate for nursing staff should be targeted and include identified disincentives and incentives for vaccination., (© 2019 MEDPRESS.)

Anaphylaxis is a severe potentially life-threatening systemic hypersensitivity reaction, with rapid onset, involving respiratory and/or cardiovascular symptoms and in vast majority of patients, also dermal symptoms. Currently anaphylaxis rate in Europe equals 1.5–7.9 cases per 100 000 yearly. In epidemiologic studies constant increase of anaphylaxis frequency is observed, while on the other hand it still remains “underdiagnosed”. What is more, anaphylaxis may occur everywhere, it's course is unpredictable, and triggers of reaction are widely present in everyday life, as also do the co-factors, which may increase the risk of occurrence and severity. Since full prevention before anaphylaxis event is impossible, the key issue is to keep updated basic knowledge of symptoms and first line intervention, including adrenaline, for self-medication. Unfortunately, the results of large cohort multicenter studies indicate that adrenaline in anaphylaxis remains underused, both by medical staff and patients at risk. These observations clearly urge to perform multilevel constant education. Above-mentioned problems initiated the worldwide anaphylaxis campaign, which has started in 2013-2014 years. Poland actively takes part in it (campaign “Be prepared for shock”). Its aim is broad education addressed to medical staff of all levels (GPs, allergists, other medical staff), as well as to wide community (potential patients) and other non-medical professionals in whose every day work, this knowledge is substantial.

Niniejsza praca ma na celu identyfikację systemu motywowania w podmiocie leczniczym oraz diagnozę głównych czynników motywujących personel medyczny. Cel ten został zrealizowany poprzez dokonanie charakterystyki najważniejszych pojęć i definicji odnoszących się do motywowania pracowników jak również w oparciu o wyniki badań przeprowadzonych w wybranym podmiocie leczniczym, zlokalizowanym w Opatowie. Badania te pozwoliły na potwierdzenie głównej hipotezy badawczej sformułowanej na potrzeby niniejszej pracy, a mianowicie, iż najważniejsze znaczenie w motywowaniu personelu medycznego mają czynniki finansowe. The aim of this thesis is to identify the motivation system of medical entity and to diagnose the main factors that motivate the medical staff. This aim has been achieved not only through the characterization of the most important concepts and definitions relating to employee motivation, but also through the results of research that has been carried out in one of the medical entities which is located in Opatów. These studies led to confirmation of the main hypothesis that has been formulated for the purpose of the thesis, namely that the financial factors play a major role in medical staff reinforcement.

- Abstract
Background: In interventional radiology the highest radiation doses are usually recorded for both the medical staff and the patients. Interventional procedures with X-rays are implemented in a number of medical specializations. This paper concerns the exposure of interventional teams performing orthopedic procedures under X-rays control., Material and Methods: Doses for interventional teams were measured in the 3 Łódź hospitals. Thermoluminescent dosemeters were applied to measure the following dose equivalents: Hp(3) for eye lens, Hp(0.07) for palm skin, Hp(10) at the level of the neck without a protective shield (i.e., collar) and Hp(10) for the whole body on the front surface of the trunk (measured under the protective apron at the level of the chest)., Results: Doses for the operator who performs surgery, assisting physicians and scrub nurse were measured during 95 procedures. The highest doses were received by the operator the dose for eyes per 1 procedure did not exceed 0.1 mSv, the highest dose for hands was 1.6 mSv and the highest recorded effective dose was 0.02 mSv., Conclusions: On the basis of the results of measurements and their comparison with the values reported in the literature it may be concluded that the obtained results fall within the published reference range (for non-vascular procedures). This proves the compatibility of practice in the monitored Łódź hospitals with routine methodology applied in other interventional departments. The measurement results confirm that the usage of thermoluminescent dosimetry is fully adequate for the evaluation of exposure in interventional radiology and that the usage of at least 2 dosemeters for that staff is necessary. - Abstract
Background: Extended control of staff exposure in interventional radiology has been legally required over the last few years. This is determined by a number of factors, including the type of procedure, technical conditions and methodology. In orthopedic procedures fluoroscopy is used to control surgical reconstructions. An influence of particular factors on the registered values of doses received by the members of medical team performing osteosynthesis for limb fractures is presented in this paper., Material and Methods: Doses received by individual interventional team members performing specific functions, operator, assisting physicians and scrub nurse, during a series of the procedures were measured. Each person was equipped with 4 dosimetric tools, containing thermoluminescent dosimeters, to measure the equivalent doses for the eyes, hand skin and the neck (outside the shield) and to evaluate effective doses. The investigations were performed in operational theatres of 3 hospitals in Łódź., Results: The equivalent doses per one procedure for the eyes and hand skin of the operator were 0.029-0.073 mSv and 0.366-1.604 mSv, respectively. Significantly higher doses were noted during the procedures of intramedullary osteosynthesis, especially for the operator. An average age and body mass index (BMI) of patients treated in the monitored hospitals did not differ statistically., Conclusions: Based on the dosimetric measurements the following conclusions can be drawn: in orthopedic procedures of interventional radiology (IR) the exposure of the staff is mostly determined by the type of procedure and more precisely by its complexity and by the optimized use of X-ray unit, including pulsed fluoroscopy. It is also revealed that the operator is the most exposed person in the interventional team. - Abstract
The risk of postoperative infection is determined by a number of factors including the underlying health of the patient, the type of surgery and the environment in which the procedure take place. The predominant infective organisms are Staphylococci. Coagulase-negative staphylococci are regarded as emerging pathogens, especially in clean surgery. The operating team is the major source of the contaminated bacteria. Bacteria dispersed from the skin is of greater importance than bacteria from the respiratory tract, so masks are not always necessary in the theatre during an operation except by those at the table. Masks that function as protective barriers are another emerging issue. Due to a greater awareness of HIV and other blood-borne viruses, masks are taking on a greater role in protection health care workers from potentially infectious blood and body fluids. Bloodborne infection pose a large risk to healthcare workers. The incidence of occupational HBV infection has decline as a result of vaccine-induced immunity. Future vaccines against HIV and HCV would be a boon to healthcare workers.

- Abstract
In seventeen patients who suffered of corneal ulcer, and were negative in bacteriological and mycological examinations, parasitological tests were performed between 1990-1992. Acanthamoeba keratitis was diagnosed in one patient with characteristic corneal changes of right eye following trauma. The patient was not a contact lens wearer. The amoeba found in direct smears of scraping from corneal ulcer and isolated and cloned in in vitro conditions (isolate-clone-No. 2961) belonged to Acanthamoeba polyphaga species. The trophozoites showed a low degree of invasiveness and high virulence when tested in BALB/c mice. The authors discussed the epidemiology of Acanthamoeba keratitis and pointed out to the necessity of accurate laboratory diagnosis and discerning interpretation of the results to prevent an Acanthamoeba-phobia among the potential patients and most of all among the medical staff.
