Arthritis of the thumb carpometacarpal joint is a fairly common disorder of unclear etiology, affecting mostly women in the postmenopausal age. The results of the operative treatment of 21 patients, 20 women and 1 men in a mean age of 59 years with the disorder classified in 3rd and 4th grade in Eaton-Littler scale are presented. Operation consisted in resection of the trapezium followed by thumb metacarpal suspension arthroplasty with flexor carpi radialis tendon, using a slip of the palmaris longus tendon. At a mean follow-up of 14 months, patients achieved significant reduction of the pain at the movement of the thumb (VAS from a mean of 8.1 to 2.9), improvement in thumb's opposition (Kapandji test from 7.6 to 9.5), power of the hand increased form 42 to the 68% of the other side and function of the hand in DASH score improved form 58 to 12 points. Pinch strength did not improved after surgery. The results show effectiveness of the modified technique in the treatment of thumb carpometacarpal arthritis.

Changes in osteoarthritis joint appear in the articular cartilage, synovium and in subchondral bone. It is necessary to find, apart from markers of cartilage destruction, a sensitive and specific biochemical marker which would reflect the metabolism as well as degradation of subchondral bone. Bone sialoprotein is mostly synthesized in osseous tissue found directly under the surface of joint cartilage. As a result, it is being increasingly perceived as a valuable marker of the metabolism rate of this layer of bone. Bone sialoprotein seems to be of use as a marker for subchondral bone degradation rate in laboratory diagnostic work-up of osteoarthritis.

In the presented study was prooven that aCl antibodies cross-reacting with LPS (endotoxin) from arthritic synovial fluids are part of the aPL antibodies pool and correlated with presence in joints bacterial cell wall components like LPS-showing endotoxin presence in synovial fluids and they may be usefull in differential diagnosis of the different kind of arthritis and also in explanation of their aetiology at all--especially in cases of early, undifferentiated arthritis.

In this paper we described a case of 56 old patient with history of joint disease. We diagnosed chondrocalcinosis with hypomagnesemia. The patient was treated for gout during last 13 years. The frequency of chondrocalcinosis is estimated about 10% in patients after 50 years of age, but it is very rare diagnosed. A correct diagnosis is possible on the basis of simple tests and it allows to avoid the drugs which are not effective in this disease.

Factors influencing bone mineral density around the femoral stem after PM cementless total hip arthroplasty were evaluated in longitudinal study of 18 hips in 18 patients who had undergone surgery due to unilateral hip osteoarthritis. Bone mineral density in the femoral neck was determined by dualenergy X-ray absorptiometry measurement performed preoperatively and in periprosthetic femoral Gruen's zones prospectively 2 weeks, 3, 6, 12 and 24 months after surgery. The concentrations of calcium, magnesium and fluoride were measured in cortical and trabecular bone samples taken from resected femoral head and neck. At 12 and 24 months after the operation the regional bone mineral density measurement showed significant, maximum decrease but after 12 months bone mineral density appeared to be stabilized. The analysis of preoperative femoral neck density and fluoride content in trabecular bone proved that osteopenia and lower fluoride concentrations correlated significantly with greater bone density decrease after total hip arthroplasty. No other factors (age, sex, weight, calcium and magnesium concentrations in bone and fluoride concentration in cortical bone) showed significant associations.

The paper presents features and operative technique for Motta-Callea knee arthroplasty. Medium-term results in a series of 17 joints with serious osteoarthritis (stage IV and V--classification of Larsen at al) in 14 patients aged 43-75 years (mean 64 years) are presented. Patients were evaluated according to Clinical Rating System (Insall et al.) before and after surgery. The average total score tripled after operation and was 168 of 200 points available. Good results have been achieved in both younger and elderly patients with advanced osteoarthritis and osteoporosis, also with bone plasty for varus or valgus deformity. An improvement was noted in regard of pain, alignment of the knee, its sagittal stability and walking ability, stairs included.

The clinical course and therapeutic problems are presented in a case of staphylococcal septicaemia in a newborn. Generalized infection was associated with multiorgan location of pathological changes, among them, acute haematogenous osteitis and arthritis (of the temporomandibular joint, shoulder joint, hip, elbow joint, knee joint and wrist joint) and rarely occurring parotitis.
