Abstract Introduction The Brazilian psychiatric reform has revolutionized the way that mental health care is provided all over the country, introducing the Psychosocial Care Centers (CAPS) and encouraging care at liberty. The CAPS have been assigned many objectives, such as prevention of hospitalizations and intervention in crises or suicide. This paper aims to describe the correlation between the implementation of CAPS and the rates of psychiatric hospitalizations and suicides from 2008 to 2018. Methods This study has an ecological time series design and included residents of the city of Porto Alegre, RS, Brazil, who were hospitalized through the Sistema Único de Saúde (SUS). The data were obtained from official databases (DATASUS, CNES, and IBGE) and indicators were calculated (CAPS coverage, hospitalization rate, and suicide rate). Associations between the indicators were tested using Pearson’s correlation coefficients. Results We found a negative correlation between provision of CAPS and psychiatric hospitalizations (r = -0.607 p = 0.048). Conclusion These results support the hypothesis that there is a negative correlation between implementation of the CAPS and psychiatric hospitalizations. This reinforces the importance of implementing policies related to improving psychiatric reform.

OBJECTIVE: Developed by the Ministry of Education, the Qualis evaluation criteria have strongly impacted the scientific production of Post-Graduation Programs. A new set of more stringent criteria has been proposed for Qualis. Our aim was to evaluate the impact of the new Qualis criteria on the scientific production of Post-Graduation Programs in psychiatry over the last 10 years. METHOD:We extracted data from annual reports published between 1998 and 2008, and compared performance measured in terms of the old Qualis rating criteria and the new set of criteria. RESULTS: There was a 25% increase in the number of Information Science Institute-indexed articles in the second five-year period, which rose from 1,213 to 1,518. While, according to the old Qualis criteria, 84% of the Information Science Institute production would have been classified as highly-rated (IF > 1), only 17% of the papers were classified as highly-rated (A1) according to the new Qualis rating criteria. Most papers (65%) were assigned to intermediate categories (B1 and B2) with an IF < 2.29. DISCUSSION:All psychiatric Post-Graduation Programs have increased their production, but by favoring quality over quantity, the new rules have proved to be more useful for discriminating among the scientific production.OBJETIVO: Nosso objetivo foi analisar o impacto dos novos critérios de avaliação de publicações científicas (Qualis) sobre a produção científica de Psiquiatria ao longo dos últimos 10 anos. Os critérios de avaliação do Ministério da Educação têm refletido em forte impacto na produção científica dos Programas de Pós-Graduação. MÉTODO: Extraímos dados dos relatórios anuais publicados entre 1998 e 2008 e comparamos os desempenhos medidos em termos dos antigos critérios de avaliação Qualis e dos novos conjuntos de critérios. RESULTADOS: Houve um aumento de 25% no número de artigos indexados no Information Science Institute no segundo período de cinco anos, que passou de 1.213 para 1.518. Embora, de acordo com os critérios de anteriores do Qualis, 84% da produção Information Science Institute tenha sido classificada como de alta qualificação (IF > 1), apenas 17% dos trabalhos foram classificados como de alta qualificação (A1), de acordo com os novos critérios de avaliação Qualis. A maioria dos trabalhos (65%) foi atribuída a categorias intermediárias (B1 e B2), com IF < 2,29. DISCUSSÃO:Todos os Programas de Pós-Graduação de psiquiatria aumentaram sua produção, porém, favorecendo a qualidade ao invés da quantidade, as novas regras provaram ser de mais utilidade para discriminar a produção científica.

11. Leading countries in mental health research in Latin America and the Caribbean Os países líderes em pesquisa em saúde mental na América Latina e Caribe

OBJECTIVE: The prevalence and burden of mental disorders have been growing in Latin-American and the Caribbean countries and research is an important tool for changing this scenario. The objective of this paper is to describe the development of mental health research in Latin American and the Caribbean countries from 1995 to 2005. METHOD: The indicators of productivity were based on the ISI Essential Science Indicators database. We compared the number of papers and citations, as well as the number of citations per paper between 1995 and 2005 for each country ranked in the Essential Science Indicators. RESULT: Eleven Latin-American countries were ranked in the ISI database and six of them demonstrated a higher level of development in mental health research: Argentina, Brazil, Chile, Colombia, Mexico, and Venezuela. Mexico produced the largest number of papers, while Brazil showed a larger number of citations per paper. CONCLUSION: Mental health research is still incipient in Latin American and the Caribbean countries, and many challenges remain to be overcome. Also, it is necessary to establish the research priorities, to allocate more funding, and to improve researchers training in research method and design.OBJETIVO: A prevalência e a carga dos transtornos mentais vêm crescendo nos países latino-americanos e a pesquisa tem sido considerada uma importante ferramenta para alterar este cenário. Este estudo descreve o desenvolvimento da pesquisa em saúde mental nos países latino-americanos e Caribe no período de 1995 a 2005. MÉTODO: Foram utilizados os indicadores de produtividade baseados no banco de dados "Essential Science Indicators" do ISI. Foram comparados o número total de artigos e citações e também o número de citações por artigo para cada um dos países classificados no Essential Science Indicators. RESULTADOS: Foram encontrados 11 países latino-americanos e Caribe no ISI, e seis destes apresentaram um maior desenvolvimento em pesquisa em saúde mental: Argentina, Brasil, Chile, Colômbia, México e Venezuela. O México foi o que apresentou o maior número de artigos, enquanto o Brasil apresentou maior número de citações por artigo. CONCLUSÃO: A pesquisa em saúde mental nos países latino-americanos e Caribe ainda é incipiente e muitos desafios necessitam ser superados, como o estabelecimento de prioridades, maior alocação de fundos e aprimoramento do treinamento dos pesquisadores em metodologia e desenhos de pesquisa.

12. Religiousness and mental health: a review

OBJECTIVE: The relationship between religiosity and mental health has been a perennial source of controversy. This paper reviews the scientific evidence available for the relationship between religion and mental health. METHOD: The authors present the main studies and conclusions of a larger systematic review of 850 studies on the religion-mental health relationship published during the 20th Century identified through several databases. The present paper also includes an update on the papers published since 2000, including researches performed in Brazil and a brief historical and methodological background. DISCUSSION: The majority of well-conducted studies found that higher levels of religious involvement are positively associated with indicators of psychological well-being (life satisfaction, happiness, positive affect, and higher morale) and with less depression, suicidal thoughts and behavior, drug/alcohol use/abuse. Usually the positive impact of religious involvement on mental health is more robust among people under stressful circumstances (the elderly, and those with disability and medical illness). Theoretical pathways of the religiousness-mental health connection and clinical implications of these findings are also discussed. CONCLUSIONS: There is evidence that religious involvement is usually associated with better mental health. We need to improve our understanding of the mediating factors of this association and its use in clinical practice.

The gap in Mental Health Care is a worldwide problem. Primary Health Care (PHC) is the framework basis for providing integrated care. Integrating Psychiatry into PHC is fundamental to solve this problem. Based on the issue of how integration of PHC into Mental Health Care is being developed with matrix support from psychiatrists, this paper investigates the role of Psychiatry in the organization of PHC, within a health comprehensive model approach. Problem-based analysis was used to systematize the knowledge and experiences gained by the authors during the execution of several research projects developed at the Laboratory of Interdisciplinary Research in Primary Health Care (LIPAPS) of Rio de Janeiro State University. Conceptual references such as Comprehensive Care, Collaborative Care and the Brazilian Model of Matrix Support Teams, as well as the most frequent Mental Health demands in PHC are reviewed, while discussing their influence on mental health care, medical education and changes imposed on Psychiatry. In conclusion, PHC may be considered as a new scenario for Psychiatry that raises important challenges related to mental health education and services. [ABSTRACT FROM AUTHOR]

Attention deficit hyperactivity disorder is characterized by severe and persistent pattern of hyperactivity and/or inattention, which is harmful. The correct assessment may be influenced by comorbidity and difficult of characterization. This paper evaluated the contribution of psychologist and psychiatrists on a correct avaliation and treatment elaboration. Ten psychologist and eight psychiatrists take part on semi-structured interview. The analysis was descriptive and qualitative. After comparing them we found that the psychologies and psychiatries of this sample contribute to the diagnostic in their specific ways and that the junctions of knowledge throw interdisciplinary work allows a better disorder comprehension and the development of a more efficient intervention plan. OBJECTIVE: This paper reports the first population estimates of prevalence and correlates of personality disorders in the Mexican population. METHOD: Personality disorders screening questions from the International Personality Disorder Examination were administered to a representative sample of the Mexican urban adult population (n = 2,362) as part of the Mexican National Comorbidity Survey, validated with clinical evaluations conducted in the United States. A multiple imputation method was then implemented to estimate prevalence and correlates of personality disorder in the Mexican sample. RESULTS: Multiple imputation method prevalence estimates were 4.6% Cluster A, 1.6% Cluster B, 2.4% Cluster C, and 6.1% any personality disorder. All personality disorders clusters were significantly comorbid with DSM-IV Axis I disorders. One in every five persons with an Axis I disorder in Mexico is likely to have a comorbid personality disorder, and almost half of those with a personality disorder are likely to have an Axis I disorder. CONCLUSIONS: Modest associations of personality disorders with impairment and strong associations with treatment utilization were largely accounted for by Axis I comorbidity suggesting that the public health significance of personality disorders lies in their comorbidity with, and perhaps effects upon, Axis I disorders rather than their direct effects on functioning and help seeking.OBJETIVO: Este trabajo presenta las primeras estimaciones poblacionales de la prevalencia de los trastornos de personalidad y sus correlatos en la población mexicana. MÉTODO: Se aplicó un tamizaje con base en el International Personality Disorder Examination a una muestra representativa de la población adulta mexicana en áreas urbanas (n = 2362) como parte de la Encuesta Mexicana Nacional de Epidemiología Psiquiátrica, validada con evaluaciones clínicas realizadas en los Estados Unidos. RESULTADOS: Se implementó un método de imputación múltiple para estimar la prevalencia y los correlatos de los trastornos de personalidad en la muestra mexicana proporcionando una prevalencia de 4.6% Grupo A, 1.6% Grupo B, 2.4% Grupo C, y 6.1% cualquier trastorno de personalidad. Todos los grupos de trastornos de personalidad fueron significativamente comórbidos con los trastornos del Eje I del DSM-IV. Una de cada cinco personas con un trastorno de Eje-I en México presenta un trastorno de personalidad comórbido y casi la mitad de aquellos con un trastorno de personalidad presenta un trastorno del Eje I. CONCLUSIONES: Asociaciones modestas de trastornos de personalidad con discapacidad y asociaciones mayores con la utilización de servicios se debe a la comorbidad con el Eje-I. El impacto de los trastornos de personalidad en la salud pública reside en su comorbilidad con los trastornos del Eje-I y no en su impacto directo sobre el funcionamiento o la búsqueda de ayuda.

A ocorrência de abandono de tratamento psiquiátrico de nível secundário é uma relevante questão clínica e econômica. Taxas de abandono em psiquiatria são mais altas que em outras especialidades médicas. Estudos publicados nos últimos 15 anos têm identificado diversos fatores estatisticamente associados ao não-comparecimento às consultas e/ou ao abandono de tratamento em saúde mental. OBJETIVO: Avaliar variáveis demográficas, psicopatológicas, de diagnóstico e tratamento enquanto possíveis preditores de abandono de tratamento em serviço especializado de saúde mental. MÉTODO: Estudo observacional, avaliando 896 pacientes encaminhados, no período de abril de 2004 a março de 2006, por Unidades Básicas de Saúde da área de abrangência do serviço especializado. RESULTADOS: Pacientes solteiros, com idade abaixo da média do grupo (39,2 anos) e desempregados abandonaram significantemente mais o tratamento. Duas variáveis relativas ao exame psicopatológico se associaram significantemente com abandono de tratamento (memória quanto ao passado recente e relação do humor com fatos reais, atuais). Pacientes adultos que receberam diagnóstico psiquiátrico de oligofrenia abandonaram significantemente menos e pacientes com diagnóstico relativo aos transtornos da infância e adolescência (F80-F98 da CID-10) também abandonaram significantemente mais o tratamento. O registro de comorbidade psiquiátrica e tratamento exclusivamente farmacológico se associaram a não abandono do tratamento. CONCLUSÃO: Os resultados confirmam achados de diferentes autores da evidência de características associadas a abandono ao tratamento. Tais achados sugerem que determinados subgrupos de pacientes necessitem de abordagens customizadas, a fim de influenciar positivamente sua adesão final ao tratamento indicado.Psychiatric out-treatment drop-out is a relevant clinical and economical issue. Drop-out rates in psychiatry are higher than in any other medical specialties. Articles published in the last 15 years have identified several factors statistically associated to no-compliance and treatment dropping-out. OBJETIVE: The objective of this paper is to evaluate demographical, psychopathological, diagnose and treatment variables as possible predictors of attrition from a mental health out-service. METHOD: Natural experiment, evaluating 896 patients referred by primary care units from April 2004 till March 2006. RESULTS: Single patients, unemployed people, and those whose age was below group-average (39.2 years) dropped-out significantly more than others. Two variables related to psychopathological examination were associated to treatment drop-out (recent memory and relation of affective disposition to current circumstances). Adult patients that received a diagnosis of Mental Retardation and those with a diagnosis related to groups F80-F98 (CID-10) also dropped-out significantly more. The identification of a psychiatric comorbidity and exclusively pharmacological treatment were associated to adherence. CONCLUSION: Results support the findings of different researchers as to the evidence of characteristics associated to treatment dropping-out. These findings suggest that certain group of patients need a more customized care, in order to positively influence their final adherence to their treatment.

In this paper we make a study of related articles about the Brazilian psychiatric reform published inthe Saúde em Debate Magazine from february/1985 to june/1995. 17 texts were examined in two blocks: 1989-1991 and 1992-1994, according to their main axis: assistance model, citizenship and paw. It was observed that thistheme is relatively recent in the magazine, in consonance with the history of the psychiatric reform process. Theheterogeneity of the reform proposes, that is comprehensible due the Brazilian regional diversity and severalinterests at take, express it in the publications. The analyzed texts make theoretical reflections from concreteexperiences guided to the reform process.

This paper analyzes the clinical files from two inmates in the General Insane Asylum La Castañeda (Mexico City) during the Revolution (1910-1920). It exposes three aspects: First, the methodological problem that this kind of files present. Secondly, it analyzes the social relations and the cultural context of both cases. Third, it exposes the value of this kind of clinical files for the history of psychiatry and culture.
